Bacterial infections post-living-donor liver transplantation in Egyptian hepatitis C virus-cirrhotic patients: A singlecenter study

AIM To determine risk factors, causative organisms and antimicrobial resistance of bacterial infections following living-donor liver transplantation(LDLT) in cirrhotic *** This prospective study included 45 patients with hepatitis C virus-related end-stage liver disease who underwent LDLT at Ain Shams Center for Organ Transplant, Cairo, Egypt from January 2014 to November 2015. Patients were followed-up for the first 3 mo after LDLT for detection of bacterial infections. All patients were examined for the possible risk factors suggestive of acquiring infection pre-, intra-and post-operatively. Positive cultures based on clinical suspicion and patterns of antimicrobial resistance were identified. RESULTS Thirty-three patients(73.3%) suffered from bacterial infections; 21 of them had a single infection episode, and 12 had repeated infection episodes. Bile was the most common site for both single and repeated episodes of infection(28.6% and 27.8%, respectively). The most common isolated organisms were gramnegative bacteria. Acinetobacter baumannii was the most common organism isolated from both single and repeated infection episodes(19% and 33.3%, respectively), followed by Escherichia coli for repeated infections(11.1%), and Pseudomonas aeruginosa for single infections(19%). Levofloxacin showed high sensitivity against repeated infection episodes(P = 0.03). Klebsiella, Acinetobacter and Pseudomonas were multi-drug resistant(MDR). Pre-transplant hepatocellular carcinoma(HCC) and duration of drain insertion(in days) were independent risk factors for the occurrence of repeated infection episodes(P = 0.024).CONCLUSION MDR gram-negative bacterial infections are common post-LDLT. Pre-transplant HCC and duration of drain insertion were independent risk factors for the occurrence of repeated infection episodes.

Annexin A2 as a biomarker for hepatocellular carcinoma in Egyptian patients

AIM To investigate the clinical utility of serum annexin A2(ANXA2) as a diagnostic marker for early hepatocellular carcinoma(HCC).METHODS This study was performed in HCC Clinic of Ain Shams University Hospitals, Cairo, Egypt and included: Group 1: fifty patients with early stage HCC(Barcelona Clinic Liver Cancer stage A); Group 2: Twenty five patients with chronic liver disease; and Control Group: fifteen healthy, age-and sex-matched subjects who were seronegative for viral hepatitis markers. The followinglaboratory investigations were done: Viral hepatitis markers [hepatitis B surface antigen and hepatitis C virus(HCV) antibodies], HCV RNA in HCV antibody-positive patients, serum alpha fetoprotein(AfP), and serum ANXA2 *** In this study, 88% of HCC patients(n = 44) were HCVpositive, while HBV infection represented only 8% of all HCC patients(n = 4); and two patients were negative for both viral markers. A highly significant difference was found between patients with HCC and chronic liver disease as well as controls with regard to serum ANXA2 levels(130, IQR 15-240; 15, IQR 15-17; and 17, IQR 15-30 ng/m L, respectively). The area under the curve of ANXA2 was 0.865; the cut-off value was established to be 18 ng/mL with a diagnostic sensitivity of 74% and a specificity of 88%, while the sensitivity and specificity of AfP at the cut-off value of 200 ng/dL were 20% and 100%, *** Serum ANXA2 may serve as a biomarker for the early detection of HCC.

Prospective Assessment of the Rockall Risk Scoring System in Egyptian Patients with Upper Gastrointestinal Bleeding

Background: One of the major challenges in managing acute upper gastrointestinal bleeding involves the identification of patients who are at high risk of rebleeding and death;conversely, the identification of patients who are suitable for early discharge and outpatient endoscopy is also important for effective resource. The use of Rockall system has been shown to represent accurate and valid predictor of rebleeding and death. The aim of this prospective study was to evaluate validity of Rockall score for prediction of rebleeding and mortality in Egyptian patients with upper gastrointestinal bleeding. Patients and Methods: 50 patients presented with upper GI bleeding were subjected to detailed clinical, laboratory, sonographic and endoscopic examination together with Rockall score calculation. Results: According to the findings of upper GI endoscopy patients were further subdivided into two subgroups as follows: Group 1 (n = 32): Patients with portal hypertension manifested by variceal bleeding and/or portal hypertensive gastropathy;and Group 2 (n = 18): other causes of acute upper gastrointestinal bleeding (peptic ulcer disease, Mallory Weiss syndrome…). Clinical condition of the patients was assessed after 48 hours and mortality was recorded. Rockall score was the highest in mortality group, and higher in rebleeding group than stable group (P ff value of Rockall score = 3.5 was the best for prediction of rebleeding with sensitivity 100%, specificity 53%, while the cutoff value = 6.5 was the best for prediction of mortality with sensitivity 100% and 93.5% specificity. Conclusion: Rockall score is clinically useful in prediction of rebleeding and mortality in patients with upper gastrointestinal bleeding. Enhanced axonal regeneration and functional recovery of the injured sciatic nerve in a rat model by lithium-loaded electrospun nanofibrous scaffolds

Improvement of Surface Mechanical and Tribological Characteristics of L-PBf Processed Commercially Pure Titanium through Ultrasonic Impact Treatment

Multi-pass ultrasonic impact treatment(UIT)was applied to modify the microstructure and improve the mechanical and tribological characteristics at the near-surface region of commercially pure Ti(CP-Ti)specimens produced by the laser powder bed fusion(L-PBf)*** considerably refined the L-PBf process-related acicular martensites(α′-M)and produced a well-homogenized and dense surface microstructure,where the porosity content of 1-,3-,and 5-pass UITed samples was reduced by 43,60,and 67%,*** UITed samples showed an enhancement in their near-surface mechanical properties up to a depth of about 300μ*** nanoindentation results for the 3-pass UITed sample revealed an increase of about 53,45,and 220%in its nanohardness,H/E_(r),and H_(3)/E_(r)^(2)indices,*** stylus profilometry results showed that performing the UIT removed the L-PBf-related features/defects and offered a smooth *** roughness average(R_(a))and the skewness(R_(sk))of the 3-pass UITed sample were found to be lower than those of the L-PBfed sample by 95 and 223%,*** the UIT also enhanced the material ratio,where the maximum load-bearing capacity(~100%)in as-L-PBfed(as-built)and 3-pass UITed samples was obtained at 60-and 10-µm depths,*** tribological investigations showed that applying the UIT resulted in a significant reduction of wear rate and average coefficient of friction(COf)of *** instance,under the normal pressures of 0.05 and 0.2 MPa,the wear rate and COf of the 3-pass UITed sample were lower than those of the L-PBfed sample by 65 and 58%,and 20 and 17%,respectively.

One-pot glycerol conversion to acrylic acid catalyzed by Cu modified HY zeolite synthesized from natural resources and pro-analytical precursor

The conversion of glycerol to acrylic acid was carried out by the one-pot process *** catalyst used in this conversion is Cu-modified HY zeolite synthesized using pro-analytical precursors and alternative precursors from natural resources i.e.,Indonesian natural zeolite and *** crystal structure and physicochemical properties of catalysts were determined by various characterization techniques such as XRD,fTIR,SEM-EDX,TEM,SAA,and TPD-NH_(3).Based on XRD analysis,it shows that HY and CuHY,both from synthetic and alternative precursors,are confirmed to have typical peaks of Y *** mapping images show a uniform distribution of Cu on the HY *** addition,the TEM analysis also shows uniform particle size *** results of catalytic glycerol conversion show that Cu-modified HY catalysts give a higher acrylic acid yield than *** results also show that the highest yield of acrylic acid was obtained in 27.5%and 25.8%at a dehydration time of 3 h using pro-analytical and natural resources CuHY catalyst,*** Cu-modified HY catalysts having weak acid sites show the best catalytic activity in the conversion of glycerol to acrylic acid.

Efficacy of tadalafil on improvement of men with erectile dysfunction caused by COVID-19:A randomized placebo-controlled trial

Objective: According to the high prevalence of COVID-19 and the subsequent risk of men's sexual health, we decided to investigate the efficacy of tadalafil on improvement of men with erectile dysfunction caused by ***: In this study, 70 outpatients who were recovered from COVID-19 without acute respiratory distress syndrome with negative polymerase chain reaction test and a complaint of erectile dysfunction were divided into two groups: 35 patients who received tadalafil 5 mg daily and 35 who received placebo. for each patient, basic assessment of sexual function was performed using the 5-item version of the International Index of Erectile function (IIEf-5) questionnaire. Then, treatment was started from 2 months after complete recovery of COVID-19 with negative polymerase chain reaction test for 3 months. At the end of the treatments, the patients were re-evaluated for sexual function using the complete version of IIEf questionnaire. finally, the results before and after treatment in the intervention group were compared with those of the control ***: Treatment with both tadalafil and placebo improved the patients' sexual function criteria compared to the baseline. However, this improvement was significantly higher in the intervention group with tadalafil than the control group with placebo (pf tadalafil 5 mg seems to be effective and safe for improvement of erectile dysfunction caused by COVID-19.

Automated Extraction and Analysis of CBC Test from Scanned Images

Health care is an important part of human life and is a right for everyone. One of the most basic human rights is to receive health care whenever they need it. However, this is simply not an option for everyone due to the social conditions in which some communities live and not everyone has access to it. This paper aims to serve as a reference point and guide for users who are interested in monitoring their health, particularly their blood analysis to be aware of their health condition in an easy way. This study introduces an algorithmic approach for extracting and analyzing Complete Blood Count (CBC) parameters from scanned images. The algorithm employs Optical Character Recognition (OCR) technology to process images containing tabular data, specifically targeting CBC parameter tables. Upon image processing, the algorithm extracts data and identifies CBC parameters and their corresponding values. It evaluates the status (High, Low, or Normal) of each parameter and subsequently presents evaluations, and any potential diagnoses. The primary objective is to automate the extraction and evaluation of CBC parameters, aiding healthcare professionals in swiftly assessing blood analysis results. The algorithmic framework aims to streamline the interpretation of CBC tests, potentially improving efficiency and accuracy in clinical diagnostics.

An efficient room-temperature self-powered,broadband(300 nm–1100 nm)photodetector based on a CuO–TiO_(2)/TiO_(2)/p-Si(100)heterostructure is *** CuO–TiO_(2)nanocomposites were grown in a two-zone horizontal tube furnace on a 40 nm TiO_(2)thin film deposited on a p-type Si(100)*** CuO–TiO_(2)/TiO_(2)/p-Si(100)devices exhibited excellent rectification characteristics under dark and individual photoillumination *** devices showed remarkable photo-response under broadband(300–1100 nm)light illumination at zero bias voltage,indicating the achievement of highly sensitive self-powered photodetectors at visible and near-infrared light *** maximum response of the devices is observed at 300 nm for an illumination power of 10 *** response and recovery times were calculated as 86 ms and 78 ms,***,under a small bias,the devices showed a prompt binary response by altering the current from positive to negative under illumination *** main reason behind this binary response is the low turn-on voltage and photovoltaic characteristics of the *** illumination conditions,the generation of photocurrent is due to the separation of photogenerated electron-hole pairs within the built-in electric field at the CuO–TiO_(2)/TiO_(2)*** characteristics make the CuO–TiO_(2)/TiO_(2)broadband photodetectors suitable for applications that require high response speeds and self-sufficient functionality.
