本论文包括三章:第1章:引言本章主要介绍了天然产物的涵义,及其来源和人类生活中的重要性。第2章:牛角瓜化学成分的研究利用传统的分离技术,从采自海南的药用植物Calotropis gigantea中分离得到了7个化合物,并通过现代波谱分析方法,包括1D,2D-NMR,MS,IR等,鉴定了这九个化合物的结构。分别是:蒲公英甾醇乙酸酯(C),α-香树乙酸酯(C),α-香树辛酸酯(C),11-酮-α-香树乙酸酯(C)11,12-环氧蒲公英甾醇乙酸酯(C),日尔曼醇乙酸酯(C),Calotropisis(C).其中,化合物C7为新化合物。第3章:苏丹药用植物Ziziphus spina-christi化学成分研究从采自苏丹的药用植物Ziziphus spina-christi中分离得到了3个化合物,并通过现代波谱分析方法,鉴定了它们的结构。分别是:羽扇豆醇,羽扇酮和β-胡萝卜甙。

AIM: To compare the response of standard hepatitis B virus (HBV) vaccination between patients with chronic hepatitis C virus (HCV) infection and healthy individuals. METHODS: This is a prospective case-control study. A total of 38 patients with chronic HCV infection and 40 healthy controls were included. Vaccination was performed by injection of 20 μg recombinant HBsAg into the deltoid muscle at mo 0, 1 and 6. Anti-HBs concentration was determined 3 mo after the last dose and compared between the two groups. The response pattern was characterized as (1) high-response when the anti-HBs antibody titer was > 100 IU/L, (2) low-response when the titer was 10-100 IU/L and (3) no-response when the titer was < 10 IU/L. RESULTS: In the patient group, there were 10/38 (26.3%) non-responders, 8/38 (21.1%) low-responders and 20/38 (52.6%) high-responders. The corresponding values in the control group were 2/40 (5.0%), 7/40 (17.5%) and 31/40 (77.5%), respectively. The response pattern was statistically different between the two groups. In multivariate analysis, smoking was a significant confounder, while HCV infection lost its significant correlation with lower antibody response. CONCLUSION: Patients with chronic HCV infection tend to respond weakly to HBV vaccination compared to healthy individuals, though this correlation is not independent according to multivariate analysis.

Background: Whether on-call gastroenterologists should perform emergency endoscopic treatment or delay endoscopy the next day has not been investigated. This study aims to investigate the effect of timing of endoscopy on patients with acute esophageal variceal bleeding. Patients and Methods: We included 167 patients with acute variceal bleeding in the gas-troenterology unit of the air force hospital, Cairo, Egypt, from 15 January till 15 July 2010. Endoscopy was scheduled once the patient’s hemodynamics permitted. Time from admission to endoscopy was recorded. Patients were stratified into 2 groups: Early endoscopy: when done ≤ 12 hours and late endoscopy: when done > 12 hours. Patients were followed up for 42 days. Results: The study included 102 males (61.1%) and 65 females (38.9%) (mean age 54 ± 11 years). 151 patients received endoscopic treatment. Early endoscopy group included 85 patients (56.3%), and the late endoscopy group included 66 patients (43.7%) (p = 0.14). Early endoscopy patients were transfused a mean of 2.9 (±1.8) units of blood versus 4.3 (±0.14) units in the late endoscopy patients p = 0.001. Mean hospital stay was 7.1 (±3.3) days versus 9.3 (±4.6) days in the early and late groups respectively p: 0.001. 5-day mortality was reported in 6 (±7) cases in the early endoscopy group and 23 (±34.8) cases in the late endoscopy group p: 0.001. Conclusion: Early endoscopic intervention for patients with acute variceal bleeding is associated with a more favorable outcome in terms of morbidity and mortality. On call gastroenterologists should comply with the 12-hour period rather than delaying cirrhotic patients with acute variceal bleeding to the following day.

Background: The optimal TRT dose/fraction for LD-SCLC remains debatable, and due to increasing number of population in Egypt and number of patients as well, so reducing the duration of radiation therapy is favored. The idea of application from DNA information organization for providing the next generation super-computers has been proposed by lessons learned from nature. Thus, molecular environment of reserving information was considered as succeeding generation of a mother-board project containing some subcategories with various applications. MERI, constructed based on the binary coding system using DNA synthesizer machine, enables retrievable reading of molecular structures using DNA sequencer machine. It has high capacity for reservation of information about 1.212 × 1011 Kb/cm2 with long-term stability. It can be used in various fields of science and technology regarding its high capacity, small size and safety measures.

The emerging fields of silicon(Si) photonic micro–electromechanical systems(MEMS) and optomechanics enable a wide range of novel high-performance photonic devices with ultra-low power consumption, such as integrated optical MEMS phase shifters, tunable couplers, switches, and optomechanical resonators. In contrast to conventional SiO;-clad Si photonics, photonic MEMS and optomechanics have suspended and movable parts that need to be protected from environmental influence and contamination during operation. Wafer-level hermetic sealing can be a cost-efficient solution, but Si photonic MEMS that are hermetically sealed inside cavities with optical and electrical feedthroughs have not been demonstrated to date, to our knowledge. Here, we demonstrate wafer-level vacuum sealing of Si photonic MEMS inside cavities with ultra-thin caps featuring optical and electrical feedthroughs that connect the photonic MEMS on the inside to optical grating couplers and electrical bond pads on the outside. We used Si photonic MEMS devices built on foundry wafers from the iSiPP50G Si photonics platform of IMEC, Belgium. Vacuum confinement inside the sealed cavities was confirmed by an observed increase of the cutoff frequency of the electro-mechanical response of the encapsulated photonic MEMS phase shifters, due to reduction of air damping. The sealing caps are extremely thin, have a small footprint, and are compatible with subsequent flip-chip bonding onto interposers or printed circuit boards. Thus, our approach for sealing of integrated Si photonic MEMS clears a significant hurdle for their application in high-performance Si photonic circuits.

The spatial and temporal slip distribution of the Lushan earthquake was estimated using teleseismic body wave data. To perform a stable inversion, we applied smoothing constraints and determined their optimal relative weights on the observed data using an optimized Akaike＇ s Bayesian Information Criterion （ABIC）. The inversion generated the source parameters. Strike, dip and slip were 218°, 39° and 100. 8° ,respectively. A seismic moment （M0） was 2. 1 × 10^20 Nm with a moment magnitude （Mw） of 6. 8, and a source duration was approximately 30 second. The rupture propagated along the dip direction, and the maximum slip occurred at the hypocenter. The maximum slip was approximately 2. 1 m, although this earthquake did not cause an apparent surface rupture. The energy was mainly released within 10 second. In addition, the Lushan earthquake was apparently related to the 2008 Wenchuan earthquake. However, the question of whether it was an aftershock of the Wenchuan earthquake requires further study.
