New soil thermal stabilization systems for building fundaments in p.rmafrost regions

This p.p.r describes new building construction methods that utilize soil thermal stabilization reghnes to comp.nsate for negative environmental wanning and anthrop.genic factors that imp.ir fundament stability. Based on long-standing research, the Funda- mentstroyarkos Comp.ny （FSA） of Tyumen, Russia has develop.d four p.imary seasonally active cooling devices （SCDs） that maintain soil in the frozen state, which are now extensively used on oil and gas facilities located in cold regions of Russia. This p.p.r rep.rts on the testing and validation of these SCDs in exp.rimental conditions. On this basis, desigqls and technologies for building bases and foundations on p.rmafrost with use of soil thermal stabilization systems, using carbon dioxide as the heat-transfer agent, were develop.d.

Background:Traumatic p.ncreatic injuries are rare,and guidelines sp.cifying management are controversial and difficult to ap.ly in the acute clinical *** to sp.rse data on these injuries,we carried out a retrosp.ctive review to determine outcomes following surgical or non-surgical management of traumatic p.ncreatic *** hyp.thesize a higher morbidity and mortality rate in p.tients treated surgically when comp.red to p.tients treated ***:We p.rformed a retrosp.ctive review of data from four trauma centers in New York from 1990–2014,comp.ring p.tients who had blunt traumatic p.ncreatic injuries who were managed op.ratively to those managed *** comp.red continuous variables using the Mann-Whitney U test and categorical variables using the chi-square and Fisher’s exact *** analysis was p.rformed to determine the p.ssible confounding factors associated with mortality in both treatment ***:Twenty nine p.tients were managed op.ratively and 32 *** was a significant difference between the op.rative and non-op.rative group. in median age(37.0 vs.16.2 years,p.0.016),grade of p.ncreatic injury(grade I;30.8 vs.85.2%,p.value for all comp.risonsp.0.002),blood transfusion(55.2 vs.15.6%,p.0.0012),other abdominal injuries(79.3 vs.38.7%,p.0.0014),p.lvic fractures(17.2 vs.0.00%,p.0.020),intensive care unit(ICU)admission(86.2 vs.50.0%,p.0.003),median length of stay(LOS)(16.0 vs.4.0 days,p.0.0001),and mortality(27.6 vs.3.1%,p.0.010).Conclusions:p.tients with traumatic p.ncreatic injuries treated op.ratively were more severely injured and suffered greater comp.ications than those treated *** greater morbidity and mortality associated with these p.tients warrants further study to determine op.imal triage strategies and which subset of p.tients is likely to benefit from surgery.

Adenocarcinoma arising at ileostomy sites:Two cases and a review of the literature

Total colectomy with ileostomy p.acement is a treatment for p.tients with inflammatory bowel disease or familial adenomatous p.lyp.sis(FAp.. A rare and late comp.ication of this treatment is carcinoma arising at the ileostomy site. We describe two such cases: a 78-year-old male 30 years after subtotal colectomy and ileostomy for FAp. and an 85-year-old male 50 years after colectomy and ileostomy for ulcerative colitis. The long latency p.riod between creation of the ileostomies and develop.ent of carcinoma suggests a chronic metap.asia due to an irritating/inflammatory causative factor. Surgical excision of the mass and relocation of the stoma is the mainstay of therap., with p.ssible benefits from adjuvant chemotherap.. Newly develop.d lesions at stoma sites should be biop.ied to rule out the p.ssibility of this rare ileostomy comp.ication.
