Duodenal obstruction following acute pancreatitis caused by a large duodenal diverticular bezoar

Bezoars are concretions of indigestible materials in the gastrointestinal tract. It generally develops in patients with previous gastric surgery or patients with delayed gastric emptying. Cases of periampullary duodenal divericular bezoar are rare. Clinical manifestations by a bezoar vary from no symptom to acute abdominal syndrome depending on the location of the bezoar. Biliary obstruction or acute pancreatitis caused by a bezoar has been rarely reported. Small bowel obstruction by a bezoar is also rare, but it is a complication that requires surgery. This is a case of acute pancreatitis and subsequent duodenal obstruction caused by a large duodenal bezoar migrating from a periampullary diverticulum to the duodenal lumen, which mimicked pancreatic abscess or microperforation on abdominal computerized tomography. The patient underwent surgical removal of the bezoar and recovered completely.

Newly developed autoimmune cholangitis without relapse of autoimmune pancreatitis after discontinuing prednisolone

A 57-year-old man presented with a 2-wk history of painless jaundice and weight Iossl He had a large ill- defined enhancing mass-like lesion in the uncinate pro- cess of the pancreas with stricture of the distal common bile duct. Aspiration cytology of the pancreatic mass demonstrated inflammatory cells without evidence of malignancy. Total serum immunoglobulin G level was slightly elevated, but IgG4 level was normal. After the 2-wk 40 mg prednisolone trial, the patient＇s symptoms and bilirubin level improved significantly. A follow-up computed tomography （CT） scan showed a dramatic resolution of the pancreatic lesion. A low dose steroid was continued. After six months he self-discontinued prednisolone for 3 wk, and was presented with jaundice again. ACT scan showed newly developed intrahepatic biliary dilatation and marked concentric wall thickening of the common hepatic duct and the proximal common bile duct without pancreatic aggravation. The patient＇ s IgG4 level was elevated to 2.51 g/L. Prednisolone was started again, after which his serum bilirubin level became normal and the thickening of the bile duct was resolved. This case suggests that autoimmune pancre- atitis can progress to other organs that are not involved at the initial diagnosis, even with sustained pancreatic remission.

A dose-dependent bimodal switch by homologous Aux/IAA transcriptional repressors

Combinatorial interactions between different regulators diversify and enrich the chance of transcriptional regulation in eukaryotic ***,a dose-dependent functional switch of homologous transcriptional repressors has rarely been ***,we show that SHY2,an auxin/indole-3-acetic acid(Aux/IAA)repressor,exhibits a dose-dependent bimodal role in auxin-sensitive root-hair growth and gene transcrip-tion in Arabidopsis,whereas other Aux/lAA homologs consistently repress the auxin *** co-repressor(TOPLESS[TPL])-binding affinity of a bimodal Aux/IAA was lower than that of a consistently re-pressing Aux/*** switch of a single amino acid residue in the TPL-binding motif between the bimodal form and the consistently repressing form switched their TPL-binding affinity and transcriptional and bio-logical roles in auxin *** on these data,we propose a model whereby competition between homologous repressors with different co-repressor-binding affinities could generate a bimodal output at thetranscriptional anddevelopmental levels.

通过分析植物组合发酵物发酵前后的氨基酸、有机酸含量变化和对胶原蛋白的合成提高率的影响,并对一款含有该植物组合发酵物的精华露采用测定受试志愿者使用前后皮肤含水量、皮肤平滑度和光泽度等客观指标,结合问卷调查收集受试者使用感受等主观指标进行了人体功效评价。结果表明,植物组合发酵物与发酵前比较,氨基酸和有机酸的含量增加,同时胶原蛋白合成率提高。含有该植物组合发酵物的精华露能显著提高皮肤含水量,改善皮肤平滑度和光泽度。使用4周后97.2%的受试者对该产品整体效果表示满意,实验过程中无受试者出现不良反应。研究结果显示,该款产品具有一定的皮肤改善效果。
