Clinical outcomes of patterned laser trabeculoplasty as adjuvant therapy in open angle glaucoma and ocular hypertension

AIM： To assess the efficacy and safety of patterned laser trabeculoplasty（PLT） as an adjunctive treatment in open angle glaucoma（OAG） or ocular hypertension（OHT） patients who were under antiglaucoma medical ***： This study was a retrospective review of primary or secondary OAG patients and OHT patients with medically uncontrolled（≥18 mm Hg） intraocular pressure（IOP） who underwent 360o PLT from June 2016 to August 2016. Follow-up visits at week 1, and 1, 3 and 6 mo were performed. IOP, best corrected visual acuity（BCVA）, complications and eye drop glaucoma medication were recorded at each follow-up visit. Success was defined as IOP reduction ≥20% from baseline. RESULTS： Forty-one eyes of 25 patients were included in this study. Pre-treatment mean IOP was 20.2±1.6 mm Hg. After PLT, IOP was 19.3±5.2, 16.1±2.7, 17.1±3.7 and 16.3±3.5 mm Hg,at 1 wk, 1, 3 and 6 mo, respectively. IOP reduction from baseline was statistically significant from the first month, remaining stable at 6 mo（P〈0.001）. PLT success at 6 mo of follow-up was 48.78%. The number of glaucoma medication per eye（P=0.10） and the mean BCVA both remained constant（P=0.37）. Complications included transient IOP spikes in 4 eyes（9.8%） and peripheral anterior synechiae in 7 eyes（17.1%）. CONCLUSION： PLT is an effective and safe method for the management of patients with OHT or OAG as an adjunctive therapy. Additional larger studies should be designed to verify the long-term stability of IOP reduction with this laser technology.

AIM:To describe the safety and efficacy of patterned laser trabeculoplasty(PLT)as an adjunctive treatment in primary open angle glaucoma(POAG)and ocular hypertension(OHT)after 18-month follow-up in Hispanic ***:A single-center,retrospective study was *** patients with OHT or POAG undergoing PLT from June 2016 to August 2016 were included in the *** parameters were intraocular pressure(IOP),the number of IOP-lowering medications,best corrected visual acuity(BCVA),laser parameters and postoperative adverse *** efficacy outcome measures were the proportion of eyes achieving an IOP reduction≥20%at 18 mo versus baseline medicated IOP or a reduction in the number of medications while maintaining IOP ***:From 40 PLT-treated eyes(mean baseline IOP 20.3±1.7 mm Hg),24 patients were analyzed(age 63.4±7.3 y).The mean IOP reductions from baseline across visits(months 1,3,6,9,12,and 18)ranged from 14.1%to 20.8%.Success rate after 18-month follow-up was 61.7%with a mean IOP of 16±3.2 mm Hg(P<0.001).The number of glaucoma IOP-lowering medications per eye(preoperative 2.1±1.1 and postoperative 2.3±1.1,P=0.86)and the mean BCVA(preoperative 0.10±0.22 and postoperative 0.11±0.22 logMAR,P=0.42)remained *** events comprised transitory IOP spikes in 4 eyes(10%)and peripheral anterior synechiae in 7 eyes(17.5%).CONCLUSION:Mid-term results of PLT show that this procedure may be an efficacious and safe technique to approach medically uncontrolled OHT or POAG patients.

Generating native user interfaces for multiple devices by means of model transformation

In the last years, the types of devices used to access information systems have notably increased using different operating systems, screen sizes, interaction mechanisms, and software features. This device fragmentation is an important issue to tackle when developing native mobile service front-end applications. To address this issue,we propose the generation of native user interfaces(UIs) by means of model transformations, following the modelbased user interface(MBUI) paradigm. The resulting MBUI framework, called LIZARD, generates applications for multiple target platforms. LIZARD allows the definition of applications at a high level of abstraction, and applies model transformations to generate the target native UI considering the specific features of target platforms. The generated applications follow the UI design guidelines and the architectural and design patterns specified by the corresponding operating system manufacturer. The objective is not to generate generic applications following the lowest-common-denominator approach, but to follow the particular guidelines specified for each target device. We present an example application modeled in LIZARD, generating different UIs for Windows Phone and two types of Android devices(smartphones and tablets).

T follicular helper expansion and humoral-mediated rejection are independent of the HVEM/BTLA pathway

The molecular pathways contributing to humoral-mediated allograft rejection are poorly defined. In this study, we assessed the role of the herpesvirus entry mediator/B- and T-lymphocyte attenuator （HVEM/BTLA） signalling pathway in the context of antibody-mediated allograft rejection. An experimental setting was designed to elucidate whether the blockade of HVEM/BTLA interactions could modulate de novo induction of host antidonor-specific antibodies during the course of graft rejection. To test this hypothesis, fully allogeneic major histocompatibility complex-mismatched skin grafts were transplanted onto the right flank of recipient mice that were treated with isotype control, anti-CD40L or modulatory antibodies of the HVEM/BTLA signalling pathway. The frequencies of CD4 T follicular helper （Tfh） cells （B220-, CD4＋ CXCR5＋ PD-lhigh）, extrafollicular helper cells （B220-, CD4＋ CXCR5- PD-1＋ and PD-1-） and germinal centre （GC） B cells （B220＋Fas＋ GL7＋） were analysed by flow cytometry in draining and non-draining lymph nodes at day 10 post transplantation during the acute phase of graft rejection. The host antidonor isotype-specific humoral immune response was also assessed. Whereas blockade of the CD40/CD40L pathway was highly effective in preventing the allogeneic humoral immune response, antibody-mediated blockade of the HVEM/BTLA-interacting pathway affected neither the expansion of Tfh cells nor the expansion of GC B cells. Consequently, the course of the host antidonor antibody-mediated response proceeded normally, without detectable evidence of impaired development. In summary, these data indicate that HVEM/BTLA interactions are dispensable for the formation of de novo host antidonor isotype-specific antibodies in transplantation.

The diagnosis of cystadenoma is rare, even more so when located in the extrahepatic bile duct. Unspecific clinical signs may lead this pathology to be misdiagnosed. The need for pathological anatomy in order to distinguish cystadenomas from simple biliary cysts is crucial. The most usual treatment nowadays is resection of the bile duct, together with cholecystectomy and Roux-en-Y reconstruction.

A Comparative Analysis of Vessels Detained under the PSC Agreements of Paris, Tokyo and Vina del Mar

Twenty years ago, in 1992, the International Maritime Organization set the objective of extending the regional Port State Control agreements to the world scale. This would be achieved, it was hoped, by ＂exporting＂ the model of the Paris MoU （Memorandum of Understanding） agreement of 1982 to other regional areas. Now would seem to be a good time to consider the prospects for a future scenario where all the countries involved form a truly global network with uniform procedures. The authors of this article put forward an earlier objective that could be the beginning of that global policy, fi＇om a detailed look at the three most important regional agreements （Paris, Tokyo and Vifia del Mar） that encompass geographically the following areas, respectively： Europe plus Canada, the Asia-Pacific, and South America. To this end, we first make a theoretical comparison based on the legal standards of application, and then present a study made of all the vessels detained under the three agreements, with a view to determining whether or not there are different kinds of behaviours, with regard to the flags and ＂recognised organizations＂ used by the vessels to certify their levels of safety. The numerical variables we have taken as basic with respect to compliance with the international standards are： the date that the vessel＇s keel was laid （age of the vessel） and as significant categorical variables, the type of vessel, the flag, the typology of that flag, the corresponding classification society, and whether or not that society is a member of the IACS （International Association of Classification Societies）. The results show a similar trend between the Paris and Tokyo agreements, but in the case of Vifia del Mar, the level of efficacy in the control appears to be insufficient in comparison with the standard of the other two.

Computational Aspects of Dam Risk Analysis： Findings and Challenges

In recent years, risk analysis techniques have proved to be a useful tool to inform dam safety manage- ment. This paper summarizes the outcomes of three themes related to dam risk analysis discussed in the Benchmark Workshops organized by the International Commission on Large Dams Technical Com- mittee on ＂Computational Aspects of Analysis and Design of Dams.＂ In the 2011 Benchmark Workshop, estimation of the probability of failure of a gravity dam for the sliding failure mode was discussed. Next, in 2013, the discussion focused on the computational challenges of the estimation of consequences in dam risk analysis. Finally, in 2015, the probability of sliding and overtopping in an embankment was an- alyzed. These Benchmark Workshops have allowed a complete review of numerical aspects for dam risk analysis, showing that risk analysis methods are a very useful tool to analyze the risk of dam systems, including downstream consequence assessments and the uncertainty of structural models.
