Comparison of the efficacy and safety of once-daily dosing and on-demand use of udenafil for type 2 diabetic patients with erectile dysfunction

We compared the efficacy and safety between once-daily dosing and on-demand use of udenafil for type 2 diabetic patients with erectile dysfunction （ED）. A multi-center, randomized, open-label, parallel-group, 12-week study was conducted. 161 patients who improved with on-demand 200 mg of udenafil according to Sexual Encounter Profile （SEP） diary Question 2 and 3 （Q2 and Q3） were randomized into 200 mg on-demand （n = 80） or 50 mg once-daily （n = 81） dosing groups for 8 weeks. The dosing period was followed by a 4-week treatment-free period. The primary efficacy endpoint was the change of the International Index of Erectile Function （IieF） erectile function domain （EFD） score. The secondary efficacy endpoints included changes to the SEP diary Q2, Q3, IieF Q3, Q4, other domains of IieF, Global Assessment Question, and shift to the normal rate （EFD 〉 26）. Vascular endothelial markers were also assessed. The IieF-EFD score of both groups improved after 8 weeks of treatment （P 〈 0.0001）. There was no statistically significant difference between two groups. Improvement was not maintained after the treatment-free follow-up period. Similar results were observed in the secondary efficacy endpoints. There was also no significant difference in vascular endothelial markers. Daily udenafil was well-tolerated, and there was no significant difference in the adverse drug reactions and adverse events between the two groups. Flushing and headache were the most frequent adverse events. Both regimens improved ED in diabetic patients and were well-tolerated. Further studies are needed to assess the effect of daily udenafil treatment in diabetic patients.

Predictive value of ^(18)F-fluorodeoxyglucose PET/CT for transarterial chemolipiodolization of hepatocellular carcinoma

AIM： To investigate the correlation of 18F-fluorodeoxy- glucose （18F-FDG） positron emission tomography （PET） with clinical features and the prediction of treatment response. METHODS： A total of 83 hepatocellular carcinoma （HCC） patients undergoing 18F-FDG PET before transar- terial chemolipiodolization with systemic chemo-infusion between October, 2006 and May, 2009 were retrospec-tively enrolled. The patients included 68 men and 15 women （mean age, 60 ~ 10.7 years）. The effect of 18F- FDG-monitored PET uptake on clinical features and on the evaluated treatment response was ascertained with modified Response Evaluation Criteria in Solid Tumors. The PET parameters of maximal standardized uptake value of the tumor （Tsuvmax）, the ratio of the tumor maximal standardized uptake value （SUV） to the liver maximal SUV （Tsuvmax/Lsuwax） and the ratio of tumor maximal SUV to the liver mean SUV （msuvmax/LSUVrnean） were tested as predictive factors. RESULTS： Among the 3 SUV parameters, the TSUV- =maxdLsuvmean ratio （cutoff value of 1.90） was significantly associated with tumor burden including tumor size, tu- mor number, α-fetoprotein levels and tumor stage （P 〈 0.001, P = 0.008, P = 0.011, P 〈 0.001, respectively）. The objective response rates in patients with a high SUV ratio （≥ 1.90） were significantly better than those with a low SUV ratio （〈 1.90）（P = 0.020）. The overall survival rates of patients exhibiting a low Tsuvmax/Lsu- Vmean ratio （〈 1.90） and those with a high SUV ratio （≥1.90） was 38.2 and 10.3 mo, respectively （P 〈 0.01）. However, the time to progression showed no significant difference between the groups （P = 0.15）. CONCLUSION： 18F-FDG PET can be an important predictor of HCC treatment. In particular, the Tsuvmax/ Lsuwean ratio （cutoff value of 1.90） can provide useful information in treatment prognosis for HCC patients treated with Iocoregional therapy.

AIM: To assess the clinical significance of incidental focal colorectal 18 F-fluorodeoxyglucose ( 18 F-FDG) uptake on 18 F-FDG-positron emission tomography/computed tomography (PET/CT). METHODS: The records of all the cases which had undergone colonoscopy after PET/CT within a two weeks interval were reviewed. Adenomas were considered advanced when they were villous, ≥ 10 mm in size, or had high-grade dysplasia. Colorectal cancers and advanced adenomas are collectively referred to as advanced colorectal neoplasms. Receiver-operating characteristic curve analysis was used to determine thesignificant predictive maximum standardized uptake value (SUVmax) cutoff value for advanced colorectal neoplasms and cancer. RESULTS: Ninety-five colorectal lesions matched the site of incidental focal colorectal 18 F-FDG uptake on PET/CT and 146 did not. Colonoscopy showed advanced colorectal neoplasms corresponding to the site of 18 F-FDG uptake in 49 of the 95 (51.5%) lesions with incidental uptake. Of the lesions without incidental uptake, only 6 of 146 (4.1%) had advanced colorectal neoplasms on colonoscopy, indicating a statistically significant difference between the two groups (P < 0.001). The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), and accuracy of incidental focal 18 F-FDG uptake in identifying advanced colorectal neoplasms were 89.1%, 75.3%, 51.6%, 95.9%, and 78.4%, respectively. In detecting only CRC, these values were 89.2%, 69.6%, 34.7%, 97.3%, and 72.6%, respectively. The significant SUVmax cutoff value for advanced colorectal neoplasms (area under the curve 0.755, P < 0.001) was 4.35, with a sensitivity, specificity, PPV, NPV, and accuracy of 75.5%, 65.2%, 69.8%, 71.4% and 70.5%, respectively. For CRC, 5.05 was the significant SUVmax cutoff value (area under the curve 0.817, P < 0.001), with a sensitivity, specificity, PPV, NPV, and accuracy of 84.8%, 71.0%, 80.9%, 89.8%, and 75.8%, respectively. Hepatocellular carcinoma(HCC) is one of the most common causes of cancer-related death worldwide. Chronic infection of hepatitis B virus(HBV) and/or hepatitis C virus(HCV) is a major risk factor in the development of the HCC, independently from excessive alcohol abuse and metabolic disease. Since the biology of HBV and HCV is different, their oncogenic effect may go through different mechanisms, direct and/or indirect. Viral hepatitis infection is associated with cellular inflammation, oxidative stress, DNA damage, that may lead to subsequent hepatic injuries such as chronic hepatitis, fibrosis, cirrhosis, and finally HCC. Direct oncogenic properties of these viruses are related with their genotypic characteristics and the ability of viral proteins to interact with host proteins, thus altering the molecular pathways balance of the cells. In addition, the integration of HBV DNA, especially the gene S and X, in a particular site of the host genome can disrupt chromosomal stability and may activate various oncogenic mechanisms, including those in hematopoietic cells. Recently, several studies also had demonstrated that viral hepatitis could trigger the population of hepatic cancer stem cells. This review summarize available pre-clinical and clinical data in literature regarding oncogenic properties of HBV and HCV in the early initiation of HCC.

既有浅埋隧道上方填土可能导致其衬砌变形、安全系数降低,甚至发生破坏,危及行车安全。以长枪岭隧道上方某拟建公园填土工程为例,选取若干典型断面,对既有浅埋隧道受不同厚度的填土影响进行数值分析,并确定容许填土厚度。结果表明:既有隧道上方填土将导致衬砌整体下沉,左右两侧沉降基本对称,位移值自上而下逐渐减小;填土后的安全系数降低,但幅度不大,拱脚及仰拱处安全系数偏小,可能出现开裂;既有隧道拱顶位移值随着填土厚度的增加而近似线性增大,为满足位移控制要求,容许填土厚度应为0.3 H^0.4 H(H为原覆土厚度)。根据若干断面的分析结果可得整个填土区域的容许填土线,在此范围内进行填土施工,并采取相应的工程措施,既有隧道的运营安全可得到保证。本分析方法简单易行,满足工程精度,可用于指导工程实践,对类似工程有一定的借鉴意义。

Interstitial tissue-specific gene expression in mouse testis by intra-tunica albuguineal injection of recombinant baculovirus

The purpose of this study is to establish a gene delivery system for interstitial tissue-specific protein expression in mice testes using modified recombinant baculovirus. Green fluorescent protein （GFP）-expressing recombinant baculovirus （GFP-baculovirus）, in which the insect cell-specific polyhedron promoter was replaced by the cytomegalovirus （CMV）-ie promoter, was used to transfect testicular cells in vitro, and for intra-tunica albuguineal injection of the interstitial tissue of the testis. GFP expression was monitored in frozen testes sections by fluorescence microscopy. Expression of GFP in testicular tissues was also assessed by reverse transcription polymerase chain reaction （RT-PCR）, and protein expression was assessed by Western blot. Testicular cells in vitro were infected efficiently by modified recombinant GFP-baculovirus. lntra-tunica albuguineal injection of GFP- baculovirus into the mouse testis resulted in a high level of GFP expression in the interstitial tissues. RT-PCR analysis clearly showed GFP gene expression in the testis, particularly interstitial tissues. Intra-tunica albuguineal injection of a modified baculovirus that encoded recombinant rat insulin-like growth factor binding protein （IGFBP）-5 resulted in an increase in IGFBP-5 in testis and semen. In conclusion, we have developed an efficient delivery system for gene expression in vivo in testicular cells, particularly cells of the interstitial tissue using intratunica albuguineal injection of a modified recombinant baculovirus. This method will be particularly relevant for application that requires gene delivery and protein expression in the testicular cells of the outer seminiferous tubule of the testis.

The performance of our method is qualitatively and quantitatively compared with some existing fusion approaches. Experimental results show that the proposed method can preserve more useful information and with higher spatial resolution.

目的评价卵巢恶性肿瘤患者治疗后;F-FDG PET/CT显像的诊断和预后价值。方法回顾性分析卵巢恶性肿瘤患者治疗后的FDG PET/CT显像结果,计算PET/CT诊断肿瘤复发或第二原发肿瘤的准确性,并评价其对患者诊疗过程的影响。用Cox回归分析PET/CT显像结果与患者总体生存率的关系。结果 268例卵巢恶性肿瘤患者[平均年龄（50±13）岁]共行416次PET/CT显像,其中87次（21%）结果阳性,329次结果阴性;灵敏度、特异性、准确性、阳性预测值和阴性预测值分别为98.8%、98.2%、98.3%、93.1%和99.7%。362次随访显像中,38次（11%）的显像结果使患者治疗决策发生了改变;54次因临床有可疑征象而进行的PET/CT显像中,9次（17%）显像排除了肿瘤复发。PET/CT显像后进行了中位时间26.7个月的随访,24例（9%）患者在随访期内死亡。单因素分析结果发现,患者年龄、国际妇产科联盟（FIGO）分期、复发病史和PET/CT显像结果均与患者的总体生存率相关。多因素分析结果显示,PET/CT显像结果与患者的总体生存率相关（P<0.05）。结论卵巢恶性肿瘤治疗后FDG PET/CT是诊断复发和第二原发肿瘤的有效方法,其便于对患者采用更合适的治疗手段,并能帮助预测预后。

以甘肃河西走廊金塔县沙漠森林公园作为典型胡杨林地的研究区域,通过野外实地采样和植被生长状况的调查,研究不同土层土壤水盐动态变化以及对胡杨生长状况的影响.结果表明：在距离地表60cm以内,随着土层的加深,土层中水分的含量随之增高;而在60cm以下,随着土层的加深,土层水分的含量在不断减少,但变化平缓.土壤盐分的变化总体趋势为随着土层的不断加深,土层中盐分的含量在不断的减少,其中以0～10cm层土壤的盐分最高.在不同的水分和盐分条件下,胡杨林的生长状况为,随着水分的增加,不管是树高、胸径还是地径都呈现出增加的趋势,植被的生长状况也趋向良好;随着土壤含盐量的增加,植被的生长状况趋势于不良,区域植被趋于衰落.在一定条件下,土壤水盐运动是相伴而行的,土壤水分运移的过程带动了土壤盐分的运动,所以在水分和盐分共同作用下,每块样地土壤全盐量和含水量表现出相矛盾的现象.
