Challenging aspects of treatment strategies in heart failure with preserved ejection fraction: “Why did recent clinical trials fail?”

Heart failure(HF) is the leading cause of hospitalization among older adults and the prevalence is growing with the aging populations in the Western countries. Epidemiologic reports suggest that approximately 50% of patients who have signs or symptoms of HF have preserved left ventricular ejection fraction. This HF type predominantly affects women and the elderly with other co-morbidities, such as diabetes, hypertension, and overt volume status. Most of the current treatment strategies are based on morbidity benefits such as quality of life and reduction of clinical HF symptoms. Treatment of patients with HF with preserved ejection fraction displayed disappointing results from several large randomized controlled trials. The heterogeneity of HF with preserved ejection fraction, understood as complex syndrome, seems to be one of the primary reasons. Here, we present an overview of the current management strategies with available evidence and new therapeutic approach from drugs currently in clinical trials, which target diastolic dysfunction, chronotropic incompetence, and risk factor management. We provide an outline and interpretation of recent clinical trials that failed to improve outcome and survival in patients with HF with preserved ejection fraction.

The study is focused on the extraction of valuable metals from automotive shredder residue(ASR)by different leaching ***,ASR samples were roasted at 600°C to simulate a thermal treatment *** water,citric and sulphuric acid were preliminarily investigated,thus two further full factorial systems entailing H2SO4–H2O2and H2SO4–H2O2–Fe3+were *** preliminary experimental results showed that0.1 mol·L-1H2SO4solution extracted 100%of Cu,Fe and Zn,whereas citric acid leached 100%of Zn and Pb,59%of Fe and 62%of Cu;whereas,H2SO4–H2O2and H2SO4–H2O2–Fe3+(Fenton's)leaching media showed that Cu,Fe and Zn can be extracted simultaneously and completely from the ASR ashes before final disposal.

Effect of False Windows on Light Exposure and Sleep Quality in Hospitalized Patients

Purpose: We aimed to investigate the effects of installing false windows next to hospital beds without windows on the amount of light received by patients and their sleep quality. Methods: The study included patients admitted to the Department of Neurology at our hospital between September 2020 and August 2021. An Actigraph device was fitted to patients’ wrist and their beds to measure the amount of light received and sleep quality. Patients were divided into three groups: bed with a window, aisle bed with a false window, and aisle bed without a window. Mean sleep efficiency (%), mean steps (per day), and the amount of light (lux) received by the patients and beds were measured. Results: Valid data were obtained for 48 participants (median age, 66.5 years). There were 23 patients in beds with a window, 13 patients in aisle beds without a false window, and 12 in aisle beds with a false window. No statistically significant differences were found in terms of mean sleep efficiency, number of steps taken, and the amount of light received by the patients (P > 0.05);however, difference in the mean amount of light received by the beds at the location of the bed was statistically significant (P Conclusion: The amount of light that the patient receives is not necessarily affected by the location of the bed or the presence of a false window.

The impact of the composition of blends comprising low‐density polyethylene(LDPE),isotactic polypropylene(PP)and a styrenic copolymer additive on the thermomechanical properties as well as the direct‐current(DC)electrical and thermal conductivity is *** presence of 5 weight percent(wt%)of the styrenic copolymer strongly reduces the amount of PP that is needed to enhance the storage modulus above the melting temperature of LDPE from 40 to 24 wt%.At the same time,the copolymer improves the consistency of the thermomechanical properties of the resulting ternary *** both the DC electrical and thermal conductivity strongly decrease with PP content,the addition of the styrenic copolymer appears to have little influence on either ***,PP in combination with small amounts of a styrenic copolymer not only allows to reinforce LDPE at elevated temperatures but also functions as an electrical conductivity‐reducing additive,which makes such thermoplastic ternary formulations possible candidates for the insulation of high‐voltage power cables.

Optimal resection of gastric bronchogenic cysts based on anatomical continuity with adherent gastric muscular layer: A case report

BACKGROUND Bronchogenic cysts are congenital lesions requiring radical resection because of malignant ***,a method for the optimal resection of these cysts has not been completely *** SUMMARY Herein,we presented three patients with bronchogenic cysts that were located adjacent to the gastric wall and resected *** cysts were detected incidentally with no symptoms and the preoperative diagnosis was challenging to obtain via radiological *** on laparoscopic findings,the cyst was attached firmly to the gastric wall and the boundary between the gastric and cyst walls was difficult to ***,resection of cysts alone caused cystic wall injury in Patient ***,the cyst was resected completely along with a part of the gastric wall in Patient *** examination revealed the final diagnosis of bronchogenic cyst and revealed that the cyst wall shared the muscular layer with the gastric wall in Patients 1 and *** Patient 3,the cyst was located adjacent to the gastric wall but histopathologically originated from diaphragm rather than *** the patients were free from *** The findings of this study state that a safe and complete resection of bronchogenic cysts required the adherent gastric muscular layer or full-thickness dissection,if bronchogenic cysts are suspected via pre-and/or intraoperative findings.

A survey was carried out with the aim of assessing the level of the total phenolic compounds （TPC）, essential （Na, K, Ca, Mg, Cu, Zn） and toxic （Cd and Pb） elements in 10 Latvian whole grain rye and/or wheat flour bread samples, obtained from the local market. The quantities of the TPC were determined by spectrophotometry. Mineral content was determined by flame photometry, flame atom absorption spectrometry （FAAS） and electrothermal atomic absorption spectrometry （ETAA） after wet digestion in concentrated HNO3. The values were comparable to the literature data. The average content in all the samples was （on a basis of weight of the product） 547 ± 28 mg/100 g for Na; 280 ±20 mg/100 g for K; 32 ± 2 mg/100 g for Ca; 88 ±5 mg/100 g for Mg; 0.49 ± 0.09 mg/100 g for Cu; 1.6 ±0.2 mg/100 g for Zn. Both Pb and Cd were determined below the limit of detection （LOD 5 0.10 mg/100 g）. The LOD values were determined for each element. The TPC content in defatted samples was slightly lower than the non-defatted ones, showing the dependence on the sample preparation. The calculated average levels of the elements were compared with the maximum levels recommended or regulated by the national legislation.
