Recent trends in bone metastasis treatments:A historical comparison using the new Katagiri score system

BACKGROUND Bone metastasis has various negative impacts.Activities of daily living(ADL)and quality of life(QOL)can be significantly decreased,survival may be impacted,and medical expenses may increase.It is estimated that at least 5%cancer patients might be suffering from bone metastases.In 2016,we published the Comprehensive Guidelines for the Diagnosis and Treatment of Bone Metastasis.Since then,the therapeutic outcomes for patients have gradually improved.As life expectancy is a major determinant of surgical intervention,the strategy should be modified if the prolongation of survival is to be achieved.AIM To monitor how bone metastasis treatment has changed before and after launch of our guidelines for bone metastasis.METHODS For advanced cancer patients with bone metastasis who visited the Department of Clinical Oncology at Akita University hospital between 2012 and 2023,parameters including the site and number of bone metastases,laboratory data,and survival time,were extracted from electronic medical records and the Katagiri score was calculated.The association with survival was determined for each factor.RESULTS Data from 136 patients were obtained.The 1-year survival rate for the poor prognosis group with a higher Katagiri score was 20.0%in this study,which was 6%and an apparent improvement from 2014 when the scoring system was developed.Other factors significantly affecting survival included five or more bone metastases than less(P=0.0080),and treatment with chemotherapy(P<0.001),bone modifying agents(P=0.0175)and immune checkpoint inhibitors(P=0.0128).In recent years,advances in various treatment methods have extended the survival period for patients with advanced cancer.It is necessary not only to simply extend survival time,but also to maintain ADL and improve QOL.CONCLUSION Various therapeutic interventions including surgical approach for bone metastasis,which is a disorder of locomotor organs,are increasingly required.Guidelines and scoring system for prognosis need to be revised promptly.

Viral hepatitis represents a major danger to public health,and is a globally leading cause of death.The five liver-specific viruses:Hepatitis A virus,hepatitis B virus,hepatitis C virus,hepatitis D virus,and hepatitis E virus,each have their own unique epidemiology,structural biology,transmission,endemic patterns,risk of liver complications,and response to antiviral therapies.There remain few options for treatment,in spite of the increasing prevalence of viral-hepatitiscaused liver disease.Furthermore,chronic viral hepatitis is a leading worldwide cause of both liver-related morbidity and mortality,even though effective treatments are available that could reduce or prevent most patients’complications.In 2016,the World Health Organization released its plan to eliminate viral hepatitis as a public health threat by the year 2030,along with a discussion of current gaps and prospects for both regional and global eradication of viral hepatitis.Today,treatment is sufficiently able to prevent the disease from reaching advanced phases.However,future therapies must be extremely safe,and should ideally limit the period of treatment necessary.A better understanding of pathogenesis will prove beneficial in the development of potential treatment strategies targeting infections by viral hepatitis.This review aims to summarize the current state of knowledge on each type of viral hepatitis,together with major innovations.

Postoperative functional evaluation of gastrectomy for gastric cancer

To improve the quality of surgical procedures for gastric cancer,it is essential to consider many components comprehensively,including gastric motility,small intestinal absorption,hormones that affect gastric motility and appetite,presence or absence of vagus nerve preservation,esophageal regurgitation on endoscopic findings,in addition to whether or not there is a physiological route for food passage through the duodenum.Furthermore,proper functional evaluation cannot be performed without considering the form and amount of energy in the nutritional supplement to be loaded,and the posture at the time of investigation.The results of functional evaluation vary according to the method selected from many available options,but we believe that use of the most physiologically accurate,appropriate and selectable option will enable us to arrive at the best resection/reconstruction technique.We have reported that it is important to consider the preservation of three elements when performing gastrectomy:1)reduction of the extent of gastrectomy,2)preservation of the pylorus,and 3)preservation of the vagus nerve;among which preservation of the remnant stomach is the most important.Furthermore,the selection of a reconstruction method that maintains secretion of hormones beneficial to gastric motility preserves the energy balance inherent in the human body,and also provides better quality of life.

The COVID-19 pandemic highlighted the urgent need to strengthen public health systems.In response,the United Nations Disaster Risk Reduction(UNDRR) Public Health System Resilience Scorecard(Scorecard) was applied in workshops across multiple countries.The aim of our research was to explore the workshop findings to develop priority strategies for strengthening public health system resilience.We conducted a workshop from 14 to 16 March 2023,at the UNDRR Global Education and Training Institute in Incheon,Republic of Korea.A sequential modified Delphi method was utilized to develop a set of prioritized resilience strategies.These were drawn from 70 strategies identified from 13 distinct workshops in eight countries.After two surveys,23 strategies were finalized.Ten received ratings of "High" or Very High" from89% of participants.These related to the inclusion of public health risks in emergency plans,integrating multidisciplinary teams into public health,enabling local transport mechanisms,and improving the ability to manage an influx of patients.The Scorecard provides an adaptable framework to identify and prioritize strategies for strengthening public health system resilience.By leveraging this methodology,our study demonstrated how resilience strategies could inform disaster risk reduction funding,policies,and actions.

食物蛋白诱导性小肠结肠炎综合征（food protein-induced enterocolitis, FPIES）是一种非IgE,细胞介导的食物过敏反应,其表现可以很严重,甚至导致休克。虽然该类过敏反应有潜在的严重性,但FPIES 的知晓率却较低；而且有关FPIES 病理生理学,诊断和治疗的高质量研究数据缺乏；其临床预后尚不明确。这篇国际共识由美国变态反应、哮喘和免疫学会（AAAAI）食物不良反应委员会和国际FPIES 协会宣传组召集组织国际工作组总结撰写。这是第一部基于临床证据的FPIES 国际指南,有助于提高FPIES 的临床诊治水平。当然未来还需要更多有关FPIES 患病率,病理生理学,诊断标志物和未来治疗方法的研究,以便为FPIES 患者提供更好的医疗服务。这些指南内容将会在获得更多证据后定期更新。

Tentative New Terminology: Initial Comedonal Phase Preceding Acute Inflammatory Phase of Acne Vulgaris—Importance of Early Treatment

We report a case of a 12-year-old female with closed and open comedo without inflammatory lesions at the first visit. She had been treated with topical adapalene once daily, and closed and open comedo subsided without inflammatory lesions one month after the treatment without any side effects induced by adapalen. The recurrence of comedo had not been observed afterwards. In the Japanese Dermatological Association (JDA) guidelines for the treatment of acne vulgaris, comedonal phase has not been stated. Then we propose the tentative new terminology of a comedonal phase and emphasize its importance in the early initial treatment for comedo to prevent the development of the acute inflammatory phase such as red papules and pustules and formation of acne scars.

Spatio-temporal deposition profile of an experimentally produced turbidity current with a continuous suspension supply

A turbidity current is a turbulent, particle-laden gravity current that is driven by density differences resulting from the presence of suspended sediment particles. The current travels downslope, bearing a large amount of sediment over a great distance, and forms fluvial and submarine bedforms. Knowledge of the spatio-temporal deposition profile of turbidity-deposited sediment is important for a better understanding of sediment transport by turbidity currents. In the current study, the depositional process of experimentally produced quasi-steady turbidity currents in an inclined flume was investigated using an electrical-resistance-based depositmeter. It was found that the amount of sediment deposited along the flume bottom increases linearly with time at a specific rate, and decreases exponentially in the downstream direction. Inspired by this observation, the mass conservation law was modified for the suspension layer and a simple theoretical model was proposed involving two parameters, the initial sedimentation rate and the decay constant. The proposed model reproduces the observed data from the flume experiment with high accuracy, and is consistent with the sediment transport distance of a natural example, the Bengal Fan. It was concluded that the two parameters that govern the sedimentation process are the key to estimating the deposition profile of fluvial systems.

Calcium Signaling in Plant Endosymbiotic Organelles： Mechanism and Role in Physiology

Recent studies have demonstrated that chloroplasts and mitochondria evoke specific Ca2＋ signals in response to biotic and abiotic stresses in a stress-dependent manner. The identification of Ca2＋ transporters and Ca2＋signaling mol- ecules in chloroplasts and mitochondria implies that they play roles in controlling not only intra-organellar functions, but also extra-organellar processes such as plant immunity and stress responses. It appears that organellar Ca2＋ signaling might be more important to plant cell functions than previously thought. This review briefly summarizes what is known about the molecular basis of Ca2＋ signaling in plant mitochondria and chloroplasts.

Gene expression during genital morphogenesis in the ground beetle Carabus maiyasanus

To investigate the developmental genetics of genital formation in the carabid beetle Carabus maiyasanus,we compared gene expression patterns among five stages using transcriptomic RNA sequencing data from abdominal segments and genitalia in the third(last)larval instar(including prepupa)and pupal stages.We identified 18839 genes,of which 10796 were differentially expressed among stages or between sexes.There were relatively few differentially expressed genes(DEGs)between the sexes(3%).The DEGs were clustered into six groups,mainly according to stage-specific expression patterns.Genes in clusters 1-3 showed high expression levels before pupation and low expression levels during the pupal period,whereas genes in clusters 46 showed high expression levels from the prepupal to the pupal stages.Genes related to the initial pupation process and differentiation of genital discs in Drosophila were involved in clusters 4 and 6 and showed low expression levels at early third instar and elevated expression levels from the early prepupal stage,suggesting that the pupation process and genital differentiation started in the prepupal stage.Clusters 4 and 5 included developmental genes related to organ size control,which may be important in the formation of internal genital structures during the pupal stage.

(Arylimido)vanadium(V)-Alkylidene Complexes as Catalysts for Ring-opening Metathesis Polymerization(ROMP) of Cyclic Olefins: Ligand Design for Exhibiting the High Activity

(Imido)vanadium(V)-alkylidene complexes of type V(CHSiMe3)(NR)(OR?)(PMe3)2 [R = Ad, C6H5, 2,6-Me2C6H3, 2,6-Cl2C6H3;R?= 2,6-Me2C6H3, 2,6-i Pr2C6H3, 2,6-F2C6H3, C6F5, C6Cl5] exhibited from moderate to remarkable catalytic activities for ringopening metathesis polymerization(ROMP) of norbornene(NBE). The catalytic activities were affected by the ligand substituents, and V(CHSiMe3)(N-2,6-Cl2C6H3)(OC6X5)(PMe3)2(X = F, Cl) demonstrated the exceptionally high catalytic activities for ROMP of NBE.The complexes polymerized cycloheptene(CHPE) and cis-cyclooctene(COE), and ROMP of COE by the OC6 Cl5 analogue proceeded in a living manner even at 80℃, and the activity increased with increasing the temperature up to 120 ℃. Highly active catalysts for ROMP of cyclic olefins(NBE, cyclopentene, and CHPE) can be generated in situ by premixing isolated V(CHSiMe3)(NC6F5)(O-2,6-iPr2C6H3)(PMe3)2 with 1.0 equiv. of C6F5OH or C6Cl5OH via immediate phenoxy exchange;the activity was affected by the kind of phenol added [TOF in the ROMPs of NBE: 4.62 × 10^4 min^–1(upon addition of C6F5OH) versus 37.3 min^–1(none)].
