市政道路生态滤沟雨季积水渗透、旱季保墒是基本功能,而黄土生态滤沟回填土的设计无据可依,常导致景观植被雨季渍害或旱季枯死。为此,通过对重塑黄土进行小尺寸毛细水上升高度试验、大尺寸持水特性模型试验以及非饱和数值分析研究。结果表明:当干密度分别为1.3 g/cm^(3)、1.4 g/cm^(3)时,土柱的最终毛细水上升高度均可达到100 cm。土柱高度超过20 cm后,其含水率基本保持稳定,稳定含水率分别为30.0%、28.0%;在重塑黄土毛细水上升过程中,存在一个暂态饱和区,其中干密度越小,孔隙比越大,大孔隙数量增多,毛细吸力减弱,导致水分迁移速率较慢,同时被湿桥封闭的大气泡数量也较多,因此暂态饱和区的持续时间较长。对于1.3 g/cm^(3)的土柱,其湿润锋时程曲线的模拟值与实测值基本吻合。综合考虑黄土地层稳定的天然平均干密度、毛细水上升高度及其持水特性以及生态滤沟回填土在后期将经历反复的渗透固结与毛细作用,采用1.2 g/cm^(3)作为回填控制干密度,以确保市政道路生态滤沟中的植土在旱季仍能维持植被生长所需的土壤湿度。

Strokes are common around the world and especially in sub-Saharan Africa. They are responsible for severe sequelae and the majority of deaths. In Gabon, no study on stroke mortality has been conducted. Objective: To determine short, medium, long-term mortality and factors associated with long-term stroke mortality. Method: Our study took place in the Neurology Department of the University Hospital Center of Libreville (UHCL). It was a historical cohort study with descriptive and analytical purposes covering the period from June 1 to August 31, 2018 and taking into account hospitalized patients from January 1, 2013 to December 31, 2017. We conducted a comprehensive systematic recruitment of patients with stroke, 18 years old and more, and had been agreed to give information. We included all patients meeting the inclusion criteria. The data was gathered using Epi-Info 7 software. The CHI-2 test was used for the comparison of frequencies and the Student’s test, for comparison of means. Multivariate analysis with logistic regression allowed us to look for factors associated with long-term mortality. A result was statistically significant for a p 0.05 value. Results: At 3 months, 28 patients (18.1%) died, at 6 months thirty-one patients or 20.1% died. At 5 years old, fifty-three patients or 34.4% had died. The factors associated with long-term mortality, if the lost of sight were all alive were tobacco (p = 0.01) and stroke (p = 0.008). If all those who were lost to sight had died, no factor was associated with 5-year mortality. Conclusion: Stroke mortality must not be taken for granted, it can be underestimated because of the large number of lost sight. Measures must be put in place to strengthen post-stroke monitoring. .

Objectives: To determine the epidemiological, therapeutic and evolutionary aspects. Methodology: This was a retrospective study, from January 1, 2016 to December 31, 2018, on the files of patients aged 16 and over, admitted for patella fractures, treated surgically and followed up in our service. Results: The study involved 17 patients including 15 men (88.24%) and 02 women (11.76%) with an average age of 36.65 years. Workers were the most represented (29.42%);AVP were the leading cause of these fractures (52.94%). The fractures were open in 70.58% of cases. Associated lesions were dominated by femur fractures in 29.42%. Duparc type II was the most encountered bone lesion with 47.06% of cases. All our patients benefited from regional anesthesia. Bracing was the most used surgical treatment in 58.83%;the anterior approach was performed in all our patients. Four (04) cases of knee stiffness, two (02) postoperative infections and skin necrosis complicated the short and medium term consequences. After a two-year follow-up, radiologically one case of pseudarthrosis and one case of patellofemoral osteoarthritis were noted. According to the Bosman score, we found 17.65% excellent, 47.06% good and 35.29% poor functional results. Conclusion: Patella fractures are relatively rare. Treatment is primarily surgical for transverse and/or displaced fractures;Bracing is the osteosynthesis technique of choice. However, knee stiffness, pseudarthrosis and patellofemoral osteoarthritis are not uncommon.

The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old male student. The objective of this study was to describe Thrombotic manifestations and therapeutic management during HIV infection and tuberculosis in a 19-year-old student. Observation: It is Mr. S. C., aged 19, admitted for pain and swelling of the right lower limb, physical asthenia and notion of fever. Evolution 5 days, with no particular history of cardiovascular disease, anti-retroviral treatment for 1 year and anti-tuberculosis treatment for 3 months. On clinical examination heart sounds are irregular with a heart rate at 115 bpm, blood pressure at 110/70 mmhg. A febrile red painful swelling at right lower limb with positive Homans sign. 0°C (Temperature 37.7°C), Weight at 58 Kg. The rest of the clinical examination is without particularity. Venous Doppler echo of the lower limbs: shows the presence of acute deep vein thrombosis of the right lateral vein. Mr. S. C. to benefit the following Medical Treatment: lovenox 0.6 UI subcutaneously morning and evening, Sintrom 4 mg 1 comprimed at night, Tramadol 50 mg morning and evening;with a good clinical evolution. CONCLUSION: TB and HIV are chronic infections that result in widespread inflammation predisposing patients to a MTVE (Venous thromboembolic disease) table as well as rifampicin and anti-proteases.

Hydrogeochemical Characterization of Aquifer Systems and Surface Water/Groundwater Relations in the Lower Senegal River Valley

This study assesses the chemical quality of water resources in the Lower Senegal River valley, based on 35 samples collected in November 2022. Major ion concentrations in surface water and groundwater were analyzed using classical geochemical interpretation diagrams (Piper, GIBBS, etc.) and multivariate geostatistical analyses, including hierarchical cluster analysis (HCA) and principal component analysis (PCA). The results revealed three types of facies: Ca-Mg-HCO3-type facies, characteristic of poorly mineralized waters such as surface waters and groundwater from dune formations and the alluvial plain close to the hydraulic axis;Na-Cl type facies associated with well waters located in the alluvial plain that tap Inchirian or Nouakchottian shallow reservoirs and Maastrichtian deep borehole waters;and mixed Ca-Cl and Na-HCO3 type facies observed in certain floodplain and dune reservoirs. The results showed a strong correlation between sodium, chlorides, bromides, and electrical conductivity, indicating a significant contribution of these ions to groundwater mineralization. The various sources of water mineralization include mixing processes between surface water or rainwater, or calcite or dolomite dissolution processes (for weakly mineralized waters), basic exchanges or inverse basic exchanges between the aquifer and the water table (for moderately mineralized waters), and evaporation processes, halite dissolution, and paleosalinity during periods of marine transgression and regression (for highly mineralized waters). The study also highlighted the high vulnerability of the alluvial aquifer to pollution from intensive irrigated agriculture, as significant quantities of sulfates and nitrates were measured in some samples. These results also highlight the importance of water quality management in the Lower Senegal Valley, particularly as concerns the protection of the alluvial aquifer against pollution from irrigated agriculture.

Thyroid Disease at the University Hospital of Conakry, Guinea

Objectives: To describe the epidemiological, clinical and therapeutic characteristics of thyroid disease at the University Hospital of Conakry, Guinea. Methods: This was a descriptive study with prospective data collection, carried out over the period of December 2016 to April 2019 at the endocrinology outpatient consultation at the University Hospital of Conakry. All the epidemiological, clinical and therapeutic data of the patients followed-up for thyroid disease were collected, analyzed and classified according to the epidemiological, clinical and therapeutic profile. The functional, morphological and autoimmune aspects of the thyroid gland were studied. Results: Out of a total of 3.517 endocrinology consultations during the study period, 204 patients were diagnosed with thyroid disease (180 women and 24 men, F/H ratio: 7.5). The average age was 47 ± 16 years. A total of 90 patients (44.33%) had hyperthyroidism, including 52 cases of Graves-Basedow disease (57.6%) and 24 cases of toxic multi-nodular goiter (32.6%). Hypothyroidism was confirmed in 25 patients (11%);about half (48%) of them had a complete thyroid surgery. The treatment of hyperthyroidism was exclusively done by synthetic antithyroid drugs, except 5 cases of Hashimoto disease with transient initial hyperthyroidism phase. Hypothyroidism was treated by hormone replacement therapy. Thyroidectomy, partial (n = 5) or total (n = 12), was performed for aesthetic discomfort (n = 6), cervical compression (n = 8) and suspicion of thyroid cancer (n = 3). Follow-up was considered regular in 40 cases (44%). Conclusion: Thyroid disease was frequent in this study conducted in Conakry and its clinical features were very diverse, dominated by hyperthyroidism mainly due to Grave disease. However, diagnostic and therapeutic strategies were hindered by the limitation in availability of biological and morphological explorations.

This study analyses change in rainfall and temperature indices by 2035 and 2050 in Senegal, with a focus on the Fatick region. These parameters are crucial for understanding the impacts of anthropogenic climate change on some vital socio-economic sectors such as agriculture and water resources in this region. To this end, a multi model ensemble mean of 21 bias-adjusted global climate models participating in CMIP5 has been used. We considered two Representative Concentration Pathways (RCP4.5 and RCP8.5). The results indicate an increase of 0.7˚C for maximum and minimum temperature by 2035 compared to the reference period (1976 - 2005). By 2050, an increase of 1.4˚C (2˚C) is projected for RCP4.5 (RCP8.5). These increases in temperature are statistically significant at the 90% confidence level. Conversely, the mean rainy season length decreases from 95 to 85 days by 2035 and less than 80 days by 2050. These decreases in rainy season length are mainly due to a delayed rainy season onset by 2035 and 2050, with the ensemble mean projecting an onset in the second half of July by 2050 instead of around the middle of June. The changes in both the onset and the length of the rainy season are significant at the 90% confidence level. Our results show a slight decrease in seasonal cumulated total rainfall by 2035 and 2050. However, we note a slight increase in seasonal cumulated extreme rainfall. These future changes in climate indices could induce yield reduction and water resources availability. To reduce yield losses, it would be interesting to adopt longer season varieties and also diversify income-generating activities. Concerning water resources, many actions could be done such as carrying out water retention works, treatment and reuse of non-conforming water for agriculture and livestock to reduce pressure on the resource.
