Hydrogeochemical Characterization of Aquifer Systems and Surface Water/Groundwater Relations in the Lower Senegal River Valley

This study assesses the chemical quality of water resources in the Lower Senegal River valley, based on 35 samples collected in November 2022. Major ion concentrations in surface water and groundwater were analyzed using classical geochemical interpretation diagrams (Piper, GIBBS, etc.) and multivariate geostatistical analyses, including hierarchical cluster analysis (HCA) and principal component analysis (PCA). The results revealed three types of facies: Ca-Mg-HCO3-type facies, characteristic of poorly mineralized waters such as surface waters and groundwater from dune formations and the alluvial plain close to the hydraulic axis;Na-Cl type facies associated with well waters located in the alluvial plain that tap Inchirian or Nouakchottian shallow reservoirs and Maastrichtian deep borehole waters;and mixed Ca-Cl and Na-HCO3 type facies observed in certain floodplain and dune reservoirs. The results showed a strong correlation between sodium, chlorides, bromides, and electrical conductivity, indicating a significant contribution of these ions to groundwater mineralization. The various sources of water mineralization include mixing processes between surface water or rainwater, or calcite or dolomite dissolution processes (for weakly mineralized waters), basic exchanges or inverse basic exchanges between the aquifer and the water table (for moderately mineralized waters), and evaporation processes, halite dissolution, and paleosalinity during periods of marine transgression and regression (for highly mineralized waters). The study also highlighted the high vulnerability of the alluvial aquifer to pollution from intensive irrigated agriculture, as significant quantities of sulfates and nitrates were measured in some samples. These results also highlight the importance of water quality management in the Lower Senegal Valley, particularly as concerns the protection of the alluvial aquifer against pollution from irrigated agriculture.

Restoration or Rehabilitation of the Faleme River Affected by Mining Activities: What Methods?

The Faleme River, a West Africa long transboundary stream (625 km) and abundant flow (>1100 million m3) is affected by severe erosion because of mining activities that takes place throughout the riverbed. To preserve this important watercourse and ensure the sustainability of its services, selecting and implementing appropriates restorations techniques is vital. In this context, the purpose of this paper was to present an overview of the actions and techniques that can be implemented for the restoration/rehabilitation of the Faleme. The methodological approach includes field investigation, water sampling, literature review with cases studies and SWOT analysis of the four methods presented: river dredging, constructed wetlands, floating treatment wetlands and chemical precipitation (coagulation and flocculation). The study confirmed the pollution of the river by suspended solids (TSS > 1100 mg/L) and heavy metals such as iron, zinc, aluminium, and arsenic. For the restoration methods, it was illustrated through description of their mode of operation and through some case studies presented, that all the four methods have proven their effectiveness in treating rivers but have differences in their costs, their sustainability (detrimental to living organisms or causing a second pollution) and social acceptance. They also have weaknesses and issues that must be addressed to ensure success of rehabilitation. For the case of the Faleme river, after analysis, floating treatment wetlands are highly recommended for their low cost, good removal efficiency if the vulnerability of the raft and buoyancy to strong waves and flow is under control.

Development and Application of Water Quality Index (WQI) for the Evaluation of the Physico-Chemical Quality of Groundwater in Gold Mining Areas of Southeastern Senegal

Water is the most essential requirement for life. It provides a variety of purposes such as a source of water supply for drinking, domestic and industrial use, irrigated agriculture, livestock, and mining activities. Evaluating the status of water quality from traditional approaches does not guarantee the whole overview of the water quality situation. Therefore, developing a tool that can convert multiple parameters data into information that is understandable by both technical and non-technical personnel is vital. In this context, the purpose of this paper was to develop, calculate, and apply a water quality index for assessing the suitability (for drinking purposes) of groundwater in the gold mining areas in south-eastern Senegal. The development of this index based on WHO water quality guidelines followed the five standards steps i.e., parameters selection, sub-index formation, parameters weighting and sub-index aggregation and evaluation. Finally, the WQI summarized twelve key water quality parameters into 05 simple terms (excellent, good, medium, poor, and very poor) which is more relevant for reporting to managers and the public in a consistent manner. Thus, it was observed in the study area, that the water quality indexes in artisanal and industrial mining areas are either poor or very poor while in the reference stations (where there are no mining activities) WQI are either good or excellent. This situation was attributed to the effects of mining activities in such zones which contribute to the pollution of groundwater with heavy metals, nitrates, and suspended solids.

Evaluation of Water Resources Quality in Sabodala Gold Mining Region and Its Surrounding Area (Senegal)

Geochemical and Geostatistical tools were used to assess: 1) the chemical quality and, geochemical processes in crystalline rock aquifers in Sabodala (Eastern Senegal) and its surroundings and 2) the impact of mining activities on their quality. A total of 26 water samples collected at boreholes, dug wells and stream, were analyzed to determine major and trace elements concentration focused on elements that represent more threats on human health. Boxplots define chemical characteristics of water for each aquifer formation compared to surface waters. Geostatistical analysis show two sources of water mineralization with regard to major elements: a first natural source characterized by Ca-Mg-HCO3 water type from boreholes and unpolluted surface water and a second group characterized by polluted water by nitrates with Na-NO3-Cl type mainly observed in upper weathered aquifers. However, considering trace element, geostatistical analysis showed three water groups: water with very low trace element concentrations encountered in boreholes and unpolluted surface waters, and waters with relatively high trace element concentrations such as Al observed in areas affected by gold mining activities and finally, polluted waters by Ni, Co, Mn and Cr observed at Sabodala. Results show that in eastern Senegal well waters are vulnerable and often affected by pollution.

Clinical and Epidemiological Aspects of Hepatocellular Carcinoma at the Internal Medicine Department of Point “G” Teaching Hospital in Mali

Liver cancer is the malignant transformation of liver cells. It develops in 90% of cases of cirrhosis, more rarely on chronic non-cirrhotic liver disease, and exceptionally in a healthy liver. This study aimed to investigate the clinical aspects of Hepatocellular Carcinoma (HCC). It was a retrospective descriptive study covering 10 years, focusing on HCC cases seen in outpatient and inpatient settings at the Internal Medicine Department. We recorded 153 cases out of 7021 patient records, resulting in a hospital frequency of 2.17%. The male-to-female ratio was 3.5. The mean age was 52.37 ± 14.34 years. The most common presenting complaint was pain in 16.3% of cases. A history of jaundice was found in 25.5% of cases. Alcohol consumption was observed in 15.38% of cases. The main physical sign found was hepatomegaly in 76% of cases. HBsAg was positive in 33.3% of cases. Alpha-fetoprotein levels were above 400 IU/ml in 50.81% of cases. Patients classified as CHILD PUGH A represented 39.72% of cases. Abdominal ultrasound revealed portal thrombosis associated with heterogeneous multinodular hepatomegaly in 11% of cases. Cytology confirmed HCC in four out of six patients who underwent the examination. We recorded 63 deaths out of 111 hospitalized patients. Complications included encephalopathy, hematemesis, and ascites in 48 patients. Hepatocellular carcinoma remains a significant public health issue. Its predominance in men and its occurrence in adults with factors such as viral infections and ethylism mean that prevention of this pathology could greatly reduce its incidence.

Introduction: Myasthenia is a rare and disabling autoimmune disease. Few studies were devoted to this pathology. We report a clinical case of myasthenia in the Department of Internal Medicine at the Point G University Teaching Hospital. Observation: This is a 41-year-old patient of Malian origin, a trader with a history of hemorrhoidectomy in 2011 and familial hypertension, was admitted on August 19, 2014, for muscle weakness, dysphonia, and dysphagia. The disease started 1 year before admission at the department with progressive muscle weakness of the upper limbs aggravated by repetitive movements and spreading to the head “drooping head” and inferior limbs, associated with general fatigue, dysphonia and selective dysphagia for solid foods. The diagnosis of myasthenia was maintained based on the clinical signs such as ptosis, bilateral diplopia, and weakness with lower limbs muscle strength rated at 3/5, and confirmed with confirmatory exams;a positive anti-acetylcholine receptor antibody (RIA), the post-synaptic neuromuscular conduction block at ENMG and the positive neostigmine pharmacological test. The patient received Neostigmine (prostagmine) 0.5 mg one ampoule in IM/day and Prednisone at a dosage of 1 mg/kg/day. The evolution was marked by a moderate improvement of the symptomatology afterward the patient was evacuated to Tunisia on family request where he received a course of immunoglobulin 2 g/kg in 2 days. The evolution in Tunisia was favorable. The patient returned in Mali and death occurred after 2 months in a context of respiratory distress. Conclusion: Myasthenia is a rare but serious disease requiring careful management and monitoring to reduce respiratory complications.

Introduction: The metabolic syndrome according to the IDF (International Diabetes Federation) is at the origin of the double global epidemic of type 2 diabetes and cardiovascular diseases. This work aims to study the metabolic syndrome in the internal medicine department of CHU Point G. Methodology: This was a descriptive study of patients who presented a metabolic syndrome according to the definition of the IDF definition, hospitalized in the internal medicine department of the CHU du point G for the period from January 1 2010 to December 31, 2019. Results: During the study period, 4189 patients were hospitalized, including 60 with metabolic syndrome, representing a hospital frequency of 1.43%. The sex ratio was 0.36. The age group of 50 and 60 years accounted for 28.3%. Forty-six point six percent (46.6%) of our patients were diabetic, 45% obese, 60% hypertensive, 70% sedentary and 10% smokers. Our patients had in 53.3% of cases a blood pressure figure ≥ 130/85 mmHg, abdominal obesity in 100%, hypertriglyceridemia in 33.3%, HDL-C less than 0.40 g/l in 62.5% of men and less than 0.50 g/l in 77.27% of men, blood sugar ≥ 1 g/l in 88.3%. Conclusion: The metabolic syndrome, in view of the entities that compose it, is a real cardiovascular risk factor and therefore a major public health issue.

Introduction: The severity of Sars-Cov-2 infection is associated with the development of acute respiratory distress syndrome (ARDS). The progression to ARDS appears to be driven by a major inflammatory mechanism potentially sensitive to corticosteroids. Observation: This article describes the case of an elderly patient was admitted to emergency departments for intense asthenia, accompanied by motor diarrhoea, dyspnoea with desaturation in ambient air, in a context of strong suspicion of infection linked to COVID-19. The article also reviews the existing literature on the diagnosis and treatment of this severe form of the disease. Conclusion: Corticosteroids, and in particular dexamethasone, have been shown to be effective in the management of patients with COVID-19, an oxygen-releasing disease.

Screening of Foot at Risk in Diabetic Patients

Introduction: The term “diabetic foot” refers to all conditions that affect the foot and are directly related to the impact of diabetes. Objective: Screen the foot at risk in diabetic patients at the hospital of Mali. Methods: It was a cross-sectional study from January 1st, 2016 to June 30, 2016, at the Department of Internal Medicine and endocrinology of the Hospital of Mali. It was focused on all hospitalized diabetic patients. Results: Thirty-two (32) patients had a foot at risk among 76 diabetic patients during the study period representing 42.10%. The sex ratio was 0.52. Type 2 diabetes accounted for 82%. A glycemic imbalance (HBA1C > 7%) was observed in 88.15%. Eighteen percent (18%) of patients had a history of ulceration or amputation;33% were walking barefoot;78.9% had tingles in the foot;31.6% had intermittent claudication;64.5% had foot cleanliness;8% claw toes;42% had abolition or reduction of superficial tenderness to monofilament and 21% had mixed foot (neuropathy + arteriopathy). In our study, 58.9% of patients had no risk of podiatry. Conclusion: Screening of foot at risk is essential in the management of diabetes because it determines the podiatric risk enabling to minimize future functional disabilities.

Introduction: Diabetes, a chronic pathology, is the source of many acute and chronic complications whose treatment requires many daily constraints that can have a serious impact on the quality of life of the patient and his family. The diabetic foot is a real public health problem which is still dominated by a very high lower limb amputation rate even in countries with high socioeconomic status. The diabetic foot classically results from the association of three entangled mechanisms which are neuropathy, arteriopathy and infection. Objectives: To determine the frequency of arterial disease, neuropathy, and other aggravating factors that influence the podiatry risk, and to proceed with the gradation of the podiatry risk. Methodology: This was a cross-sectional study concerning the period from July 1 to October 31, 2017, covering all diabetic patients received in consultation and hospitalization in the Internal Medicine Department of the G-University Hospital Center. Results: Our study included 50 patients (35 women for 15 men) out of 95 diabetic patients receiving a frequency of 52.6%, with a sex ratio of 0.42. The average age of our patients was 54.38 ± 13.98 years. Overweight/obesity found in 44% of patients. The average Body Mass Index (BMI) of our patients was 27.16 kg/m2. Patients with diabetes less than 5 years old represented 56% with an average duration of 5.39 ± 5.34 years. The mode of discovery of diabetes was polyuria-polydipsia syndrome in 56% of cases. Type 2 diabetes represented 74% of patients. Among the patients who had performed the HbA1C test, 80.77% were not well balanced. The antecedent of macrosomia in women accounted for 77.1%. Patients who did not know foot hygiene recommendations accounted for 82%. Among the identified podiatry risk factors, diabetic neuropathy accounted for 73.8%, and arterial disease was 32.3%.
