Acute and Mid-Term Results after Pulmonary Veins Isolation Using a Novel Circular Irrigated Multielectrode Mapping and Ablation Catheter (nMARQ)

Background: Pulmonary vein isolation (PVI) is the cornerstone of atrial fibrillation (AF) ablation in both paroxysmal and persistent AF. However, this procedure is still challenging and time consuming. Objective: The aim of this study was to present our approach for PVI using a novel circular irrigated multielectrode mapping and ablation catheter (nMARQTM) and to present acute and mid-term results. Methods: The study included 31 consecutive patients with symptomatic AF (4 had persistent and 27 had paroxysmal AF) who underwent PVI using the nMARQTM catheter. Circular ablation was guided by CT image integrated into fast anatomical map and by intra cardiac echo. Isolation of pulmonary veins was identified using the nMARQTM catheter if it was possible to advance it into the veins, otherwise Lasso catheter was used. Patients were followed up to 20 months. Results: PVI was achieved in 119 (98%) out of 121 pulmonary veins identified, and final PVI was obtained in 30 (97%) out of 31 patients. Lasso catheter was needed for PVI confirmation in 16 (52%) patients. Touch up ablation using standard catheter guided by Lasso catheter was needed in 4 (13%) patients. Pericardial tamponade occurred in 1 patient who was treated with pericardiocentesis. No other major complications were detected. During follow-up (mean 15.9 ± 3.6 months, range 9 - 20 months), 4 (13%) patients had recurrence of atrial tachyarrhythmia. Conclusion: PVI using the novel nMARQTM catheter is safe with good acute and mid-term efficacy. Long term follow up trials are needed.

Impact of computed tomography image and contact force technology on catheter ablation for atrial fibrillation

AIM:To investigate the impact of using computed tomography(CT) and contact force(CF) technology on recurrence of atrial tachyarrhythmia after atrial fibrillation(AF) ***: This non-randomized study included 2 groups of patients. All patients had symptomatic recurrent paroxysmal or persistent AF and were treated with at least 1 anti arrhythmic medication or intolerant to medication. The first group included 33 patients who underwent circumferential pulmonary veins isolation(PVI) for AF during 2012 and 2013 guided by CT image integration(Cartomerge, Biosense Webster, Diamond Bar, CA, United States) of left atrium and pulmonary veins into an electroanatomic mapping(EAM) system(CT group) using standard irrigated radiofrequency catheter(Thermo Cool, Carto, Biosense Webster, Diamond Bar, CA, United States) or irrigated catheter with integrated CF sensor(Smart Touch, Carto, Biosense Webster, Diamond Bar, CA, United States). The second group included immediately preceding 32 patients who had circumferential PVI by standard irrigated catheter(Thermo Cool) using only EAM(Carto) system(EAM group). Linear lesions were performed according to the discretion of operator. RESULTS: Sex, age, and persistent AF were not different between groups. PVI was achieved in all patients in both groups. Linear ablations including cavo-tricuspid isthmus and or roof line ablation were not different between groups. Free of atrial tachyarrhythmia during follow-up of 24 mo was significantly higher among CT group compared to EAM group(81% vs 55%; respectively; P = 0.027). When 11 patients from CT group who had ablation using Smart Touch catheter were excluded, the difference between CT group and EAM became non significant(73% vs 55%; respectively; P = 0.16). Sub analysis of CT group showed that patients who had ablation using Smart Touch catheter tend to be more free of atrial tachyarrhythmia compared to patients who had ablation using standard irrigated catheter during follow-up(100% vs

Improvement in MR and in the dyssynchrony between the mid segments predict super responders in patients undergoing CRT

Background: Twenty to thirty percent of patients with left heart failure in randomized trials do not respond clinically to cardiac resynchronization therapy (CRT). The objectives of our work were to estimate the effec-tiveness of CRT in our routine practice, and to find predictors of favorable response to CRT. Methods: Retrospective analysis of consecutive patients who underwent CRT from 2003-2007. Results: Fifty six consecutive patients (73.2% men, age 66.3 ± 10.2 years) with left heart failure (LHF) (ischemic cardiomyopathy 60.7%) received CRT and were followed for a mean of 27 months. After the implantation: Mean NYHA improved by one class (p < 0.001), LHF admissions were reduced by 79% (p < 0.0001) and mean ejection fruction improved by 31% (19.5% vs 25.6%, p < 0.002). Significant improvement in severity of mitral regurgitation (MR) was an important predictor of high responsiveness (p = 0.004). Improvement of MR was associated with complete left bundle branch block (CLBBB) (p = 0.04) and lower total mortality (p = 0.005). Improvement in synchronization between the mid lateral and mid septal segments, as was calculated with longitudinal strain using echocardiography (105 ± 38 vs 20 ± 23 msec, p = 0.008) was predictor of super responsiveness and reduction in MR among patients with moderate to severe MR prior to implantation. Worsening the delay between the mid posterior and mid anterior septal segments (21 ± 27 vs 120 ± 0 msec, p = 0.006) was associated with absence of response and lack of improvement in MR. Conclusions: Patients who improve mid segments dyssynchrony in four chamber view, have more chance to improve MR and to become super responders.

Ballistic resistance and energy dissipation of woven-fabric composite targets:Insights on the effects of projectile shape and obliquity angle

In the present study,the effect of aging heat treatment at 650,750,and 850？C on the impact toughness of 316L austenitic stainless steel,2205 duplex stainless steel and their weldments has been *** process was conducted using the TIG（tungsten inert gas） welding *** impact testing,at room temperature,was employed to determine the effect of aging treatment on the impact properties of investigated *** treatment resulted in degradation in the impact toughness as demonstrated by the reduction in the impact fracture energy and deformation parameters（strain hardening capacity,fracture deffection,and crack initiation and propagation energy）.The degree of embrittlement was more noticeable in duplex stainless steel parent and weld-metal than in 316L stainless steel and became greater with the increase of aging *** degradation in impact toughness was discussed in relation to the observed precipitation of the intermetallic sigma phase in the microstructure of the stainless steel weldments and the corresponding fracture surface morphology.

Wear Behaviour of Plasma Paste Boronized of AISI 8620 Steel with Borax and B_2O_3 Paste Mixtures

In the present study, AlSl 8620 steel was plasma paste boronized （PPB） by using paste mixtures with different borax and B2O3 percentages. The PPB process was carried out in a DC plasma system at 973, 1023 and 1073 K for 5 h in a gas mixture with 70% H2-30% Ar under a constant pressure of 103 Pa （10 mbar）. The thickness of boride layers ranged from 15.9 to 53.6 tim depending on the temperature and process time. The hardness of the borides formed on the surface of the steel substrate and unborided steel substrate were 1648-1924 HV0.05 and 226 HV0.05, respectively. X-ray diffraction analysis of the boride layers on the surface of the samples were obtained for FeB and Fe2B phases. The wear tests on the steel were performed by using a ball-on-disc test device at room temperature with applied load of 10 N and with a shear velocity of 0.2 m/s. Wear test results showed that the highest value of wear rate was observed in the unborided sample while the lowest wear rate was observed in the PPB samples with 30% borax ＋ 70% B4C paste at 1073 K for 5 h.

Recent advances on flexible electrodes for Na-ion batteries and Li–S batteries

Flexible energy storage devices are essential for emerging flexible electronics. The existing state-of-the-art Li-ion batteries are slowly reaching their limitation in terms of cost and energy density. Hence, flexible Na-ion batteries (SIBs) with abundanee Na resources and Li-S batteries with high energy density become the alternative for the Li-ion batteries in future. This review summarizes the recent advances in the development of flexible electrode materials for SIBs with metallic matrix and carb on aceous matrix such as carb on nano-tubes, carbon nano-fiber, graphene, carbon cloth, carbon fiber cloth, and cotton textiles. Then, the potential prototype flexible full SIBs are discussed. Further, the recent progress in the development of flexible electrode materials for Li-S batteries based on carb on nano-fiber, carb on nano-tubes, graphene, and cotton textiles is reviewed. Moreover, the design strategies of suitable interlayer, separator, electrolyte, and electrodes to prevent the dissolution and shuttle effect of polysulfides in flexible Li-S batteries are provided. Finally some prospective investigation trends towards future research of flexible SIBs and Li-S batteries are also proposed and discussed. The scientific and engineering knowledge gained on flexible SIBs and Li-S batteries provides conceivable development for practical application in near future.

Super Gaussian enhancers in the Schwinger mechanism

We study the Schwinger mechanism in the presence of an additional uniformly oriented,weak super Gaussian of integer order 4 N+*** the worldline approach,we determine the relevant critical points to compute the leading order exponential factor *** show that increasing the parameter N gives rise to a strong dynamical *** N=2,this effect turns out to be larger compared to a weak contribution of the Sauter *** higher orders,specifically,for the rectangular barrier limit,i.e.N→∞,we approach the Lorentzian case as an upper *** the mentioned backgrounds significantly differ in Minkowski spacetime,we show that the found coincidence applies due to identical reflection points in the Euclidean instanton *** addition,we also treat the background in perturbation theory following recent *** doing so,we show that the parameter N determines whether the weak contribution behaves perturbatively or nonperturbatively with respect to the field strength ratio,and,hence,reveals an interesting dependence on the background *** particular,we show that for backgrounds,for which higher orders in the field strength ratio turn out to be relevant,a proposed integral condition is not *** view of these findings,the latter may serve as an indicator for the necessity for higher-order contributions.

Generalized thermoelastic interaction in functional graded material with fractional order three-phase lag heat transfer

The present work is concerned with the solution of a problem on thermoelastic interactions in a functional graded material due to thermal shock in the context of the fractional order three-phase lag model. The governing equations of fractional order generalized thermoelasticity with three-phase lag model for functionally graded materials(FGM)(i.e., material with spatially varying material properties) are established. The analytical solution in the transform domain is obtained by using the eigenvalue *** inversion of Laplace transform is done numerically. The graphical results indicate that the fractional parameter has significant effects on all the physical quantities. Thus, we can consider the theory of fractional order generalized thermoelasticity an improvement on studying elastic materials.
