Groundwater Vulnerability and Sensitivity Optimization Using Geographical Information System for Kano Metropolis, North-Western Nigeria

This paper developed an optimization technique for groundwater vulnerability in Kano Metropolis, North-Western Nigeria. A combination of DRASTIC is taken from initial letters of seven parameters namely depth to water table (D), net recharge (R), aquifer media (A), soil media (S), topography (T), impact of vadose zone (V) and hydraulic conductivity (C), while GOD also represents groundwater confinement (G), overlaying strata (O), depth of water (D) and multi-criteria evaluation (MCE) techniques were used in the optimization method by integrating other important and sensitive parameters for groundwater pollution, principally the anthropogenic point source pollution parameters (dump site, petroleum stations, automobile shops and under storage tanks). Geographic Information System was used to perform the sensitivity analysis (SA) using the single parameter and map removal sensitivity methods. Result of sensitivity optimization revealed the depth to groundwater (D), net recharge (N), impact of vadose zone (V) from DRASTIC model, and groundwater conferment (G) from GOD model having significant impact on the groundwater vulnerability, respectively. A combination of these four parameters was used to generate DNVG groundwater vulnerability for the area. This suggests that an integration of other point source pollution parameters can enhance the influence of DRASTIC and GOD model parameters on groundwater vulnerability condition. The paper recommends for the application of the optimization method used in this study in another area with similar geological and anthropogenic point source of pollution with a view to validating or improving on it. In this study, several input data, such as anthropogenic point sources of contamination, are added to the existing DRASTIC and GOD model parameters as part of a sensitivity analysis aiming to optimise the performance of the resultant models.

Effect of Tillage Practices on Hydro-Physical Properties of Soils in a Rice-Grown Environment in North Central Nigeria

This study details soil’s physical and chemical properties in a rice-grown environment in Minna environs Niger State, Nigeria, under different soil and water conservation practices. Soil samples were collected at 0 - 10 cm, 10 - 20 cm and 20 - 30 cm, respectively, using a soil auger to determine their physical and chemical properties such as moisture content, particle size, bulk density, particle density, porosity and organic matter. The textural classification determined the percentage dominance of the soil types in each study site location. The moisture content of site 1 ranged between 14.3% to 13.7%, site 2 ranged between 13.2% to 10.2%, site 3 ranged between 15.4% to 13.2%, site 4 ranged between 15.6% to 13.6% and site 5 ranged between 16.1% to 10.9%. The clay content of sites 3, 4 and 5 is higher than that of sites 1 and 2, while the silt contents of sites 1, 2 and 3 are higher than that of sites 4 and 5. The mean value of soil organic carbon from each study location was 2.37%, 2.03%, 2.43%, 2.07% and 2.17% for sites 1, 2, 3, 4, and 5, respectively. Therefore, site 1 is susceptible to erosion because of poor infiltration rate and well-ploughed or cultivated land by the tractor (harrowed). The particle sizes analysis indicated that the soil type in Site 1 and Site 2 is mainly sandy loam, while Site 3 and Site 5 are mainly loamy and predominantly clay loam in Site 4.

Evaluation of Some Treatment Options Inlate and Neglected Hip Fractures Using the Modified Harris Hip Score

Background: The choice of the appropriate operative intervention in patients with late and neglected hip fracture continuous to be a huge dilemma for orthopedic surgeons. Purpose: To evaluate the satisfaction or otherwise of some treatment options using the modified Harris-hip score (HHS) in resource poor setting. Materials and Methods: A retrospective cross-sectional study conducted at ATBUTH, Bauchi. Data of 60 patients over the age of 18 years with hip fractures (femoral neck, intertrochanteric and sub trochanteric fractures) who had operative intervention between 1st September 2019 and 31st August 2020 with cannulated screws, Proximal femur lock compression plate (PFLCP), cementlessor cemented bipolar hemiarthroplasty (BHA). Results: The mean age of studied patients was 65.7 ± 16.1 years, with age ranging from 19 - 101 years. M:F ratio was 1.2:1 across all age groups and 1:1.4 amongst those >60 years. 51 patients (85%) presented > 1 week after injury with 24 patients (40%) sustaining hip fractures from low energy trivial indoor fall and 28 patients (46.7%) mostly younger sustaining fractures from high energy motor vehicular accident (MVA). The prevalence rate for femoral neck, intertrochanteric and subtrochanteric fractures were 32 (53.3%), 17 (28.3%) and 11 (18.3%) respectively. From the data retrieved, 21patients (35.0%), 17 patients (28.3%), 20 patients (33.3%) and 2 patients (3.3%) had PFLCP, cemented BHA, cementless BHA and cannulated screw fixation respectively. Most (66.7%) of those who had PFLCP achieved satisfactory radiologic union and there was also 94.1% and 85% satisfactory outcome rate amongst the patients with cemented BHA and cementless BHA respectively. Irrespective of the operative intervention method at 1 year follow-up, there was a statistically significant improvement in post-operative HHS (P value 0.02), with 83.4% having good to excellent results. Antibiotics for eradicating meningococcal carriages:Network meta-analysis and investigation of evidence inconsistency

Apert Syndrome: A Case Report and Review of Literature

Apert syndrome, also known as acrocephalosyndactyly, is one of the causes of craniofacial syndrome or deformity. It is a rare congenital disorder characterized by premature fusion of cranial sutures (craniosynostosis), malformation of skull, hands, face and feet. This congenital deformity has incidence of 1/50,000 to 1/80,000 live births and is an autosomal dominant in inheritance. Apert syndrome, fibroblast growth factor receptor 2 (FGFR2) and the missense substitution mutations occur at adjacent amino acids (i.e. Ser252Trp, Ser 252Phe, Pro253Arg) between the second and third extra cellular immunoglobulin domain of FGFR2, which maps to chromosome bands 10q26. Increased paternal age has been implicated in the development of Apert syndrome. The syndrome has to be thoroughly evaluated as early definitive diagnosis is important in order to distinguish Apert syndrome from other forms of craniosynostosis like Carpenter syndrome, Crouzon disease, Pfeiffer and Saethre-Chotzen syndrome. It is generally accepted that management of Apert syndrome is multidisciplinary in approach, which should compose of neonatologists, neurosurgeons, craniofacial surgeons, plastic surgeons, otolaryngologists, orthodontists, orthopaedic surgeons, ophthalmologists, radiologists, geneticists, clinical psychologists and speech and language pathologists for the effective management of this condition. Early diagnosis and treatment is important because Apert syndrome when treated early has good prognosis in adult life.

This approach yields a calibrated model that is based solely on referenced sources of information about the *** proposed methodology builds on some of the best techniques from the reviewed literature and combines these with an evidence-based approach that uses version control software to track the calibration *** the process produces a single,deterministic model for an inherently stochastic process(building operation),such an approach can produce meaningful conclusions where sub-utilities hourly measured data and significant resources for further investigation and measurement are available.

Contagious bovine Pleuropneumonia (CBPP) is an infectious and highly contagious respiratory disease of cattle and water buffalo, caused by the Mycoplasma mycoides subspecies mycoides. It induces significant economic losses and leads to a severe livestock production problem, negatively influencing people’s livelihoods of affected countries. In Somalia, there is no updated data on the prevalence and distribution of the disease. Hence, a descriptive cross-sectional study was conducted from September 2022 to June 2023 in different villages under the Afgoye District of lower Shabelle region, Somalia. The main purpose of this study is to assess the sero-prevalence and identify the associated risk factors for the occurrence of the disease. In this study, villages, age, sex, breed, and body condition were considered as risk factors. A total of 90 blood samples were collected and tested in the laboratory using the Anti-CBPP Elisa kit test. Out of 90 serum samples from herd cattle, 32 were positive, resulting in an overall prevalence of 35.5%. In addition, we found a statistically significant variation between the prevalence of the disease and factors such as sex, age, body condition and breeds. In summary, the overall prevalence of Contagious bovine pleuropneumonia in this study area is worth to be considered because there is a low quality of health care and less awareness of the Contagious bovine Pleuropneumonia effects on herds, which warrants the official authorities to act and follow appropriate preventive and control measures to reduce the incidence of the disease and generate appropriate controlling and prevention measures in all regions of Somalia.

FAIR Guidelines and Data Regulatory Framework for Digital Health in Nigeria

Adopting the FAIR Guidelines—that data should be Findable, Accessible, Interoperable and Reusable(FAIR)—in the health data system in Nigeria will help protect data against use by unauthorised parties, while also making data more accessible to legitimate users. However, little is known about the FAIR Guidelines and their compatibility with data and health laws and policies in Nigeria. This study assesses the governance framework for digital and health/e Health policies in Nigeria and explores the possibility of a policy window opening for the FAIR Guidelines to be adopted and implemented in Nigeria’s e Health sector. Ten Nigerian policy documents were examined for mention of the FAIR Guidelines(or FAIR Equivalent terminology) and the 15 sub-criteria or facets. The analysis found that although the FAIR Guidelines are not explicitly mentioned, 70% of the documents contain FAIR Equivalent terminology. The Nigeria Data Protection Regulation contained the most FAIR Equivalent principles(73%) and some of the remaining nine documents also contained some FAIR Equivalent principles(between 0–60%). Accordingly, it can be concluded that a policy window is open for the FAIR Guidelines to be adopted and implemented in Nigeria’s e Health sector.

Fournier’s Gangrene: 13-Year Experience in a Tertiary Center, North Eastern Nigeria

Background: Fournier’s gangrene is a fulminant infection of the genitourinary tract characterized by progressive necrosis of the skin and subcutaneous tissue of the external genitalia. Initially mainly seen involving the scrotum of elderly men with different moods of occurrence each unpleasantly lethal. With varying types of presentation only one thing has remained constant;the poor prognosis of this condition. Purpose/Aim: Thus, it’s important to study the trends of the presentation of this condition laying emphasis on the challenges in the management of these patients;both the negative and positive prognostic factors. Materials and Methods: Urology ward record books, clinic record books and operating theater records were used to identify patients managed for Fournier gangrene in ATBUTH Bauchi. A retrospective study of the medical files of all the patients managed from January 2011 to January 2024 was done. Data was analyzed using SPSS version 29. Results: Data from 47 patients seen during the period of study were carefully collected and analyzed. The age range is 2 weeks to 97 years. There were two neonates, one at 2nd week of life and the other at 3rd week. Mortality rate is 36%. The average time duration before presentation for patients that died was two weeks, a minimum of 9 days prior to presentation and a maximum of 21 days, about 10 of which came in septic shock and the remaining presented with fever and very extensive necrotizing fasciitis of the perineum. All the patients that died had diabetes mellitus as a comorbidity except the neonate. All the patients that survived were much younger, all were below 60 years of age (2 weeks - 53 years). Conclusion: Here, we share our experience managing patients with Fournier’s gangrene in our facility in the past 13-year period from January 2011 to January 2024. .

Hailey-Hailey disease also known as familial benign chronic pemphigus is a rare bullous genodermatosis that affects intertriginous area symmetrically. It presents with flaccid blisters, erosions and maceration resulting in increased morbidity, reduced quality of life for affected patients. It is rare in occurrence with an incidence of rate of 1 in 50,000. It is diagnosed with a combination of clinical and histopathological findings. While there is no known cure, its relapsing remitting course can be managed with medication. This case describes an unusual presentation of familial benign chronic pemphigus with a late age of onset of symptoms, atypical distribution and resistant to multiple therapies.
