Clinical and Bacteriological Profile of Infections in Sickle Cell Children in Two Referral Hospitals in Niamey, Niger

Introduction: Infections are significant causes of mortality in sickle cell children in resource-limited countries. This study aimed to determine the clinical profile and bacterial ecology of infections in children with sickle-cell disease in two referral hospitals in Niamey. Patients and methods: A retrospective descriptive study was conducted from January 2018 to July 2020 in two referral hospitals in Niamey. All children aged one (1) to 15 years with sickle cell disease admitted for suspected infection, including at least one bacterial culture, were studied. Bacteriological analysis was performed using the appropriate culture media, using BactAlert (Reference 4700003 BTA3D60 BioMérieux). Results: Over 36-months, 350 children with a mean age of 10.9 months were admitted. The sex ratio was 1.2. The SS electrophoretic profile was the most common (93.4%). Immunization status was up to date in 66% of patients. Fever was the common reason for consultation (55.1%). Infection was confirmed in 62 patients (17.7%). The primary diagnoses were bacterial gastroenteritis (24.2%) and urinary tract infection (19.4%). Blood cultures were isolated from Salmonella typhi (13.0%) and Escherichia coli (8.7%). Klebsiella spp (7.1%) and Escherichia coli (5.0%) were detected in cytobacteriological examination of urine. Salmonella typhi (23.5%) and Escherichia coli (5.9%) were isolated on coproculture. Conclusion: Bacterial ecology appears not different from that usually observed in sickle-cell children. Salmonella and Escherichia coli were predominant.

Evaluation of Some Treatment Options Inlate and Neglected Hip Fractures Using the Modified Harris Hip Score

Background: The choice of the appropriate operative intervention in patients with late and neglected hip fracture continuous to be a huge dilemma for orthopedic surgeons. Purpose: To evaluate the satisfaction or otherwise of some treatment options using the modified Harris-hip score (HHS) in resource poor setting. Materials and Methods: A retrospective cross-sectional study conducted at ATBUTH, Bauchi. Data of 60 patients over the age of 18 years with hip fractures (femoral neck, intertrochanteric and sub trochanteric fractures) who had operative intervention between 1st September 2019 and 31st August 2020 with cannulated screws, Proximal femur lock compression plate (PFLCP), cementlessor cemented bipolar hemiarthroplasty (BHA). Results: The mean age of studied patients was 65.7 ± 16.1 years, with age ranging from 19 - 101 years. M:F ratio was 1.2:1 across all age groups and 1:1.4 amongst those >60 years. 51 patients (85%) presented > 1 week after injury with 24 patients (40%) sustaining hip fractures from low energy trivial indoor fall and 28 patients (46.7%) mostly younger sustaining fractures from high energy motor vehicular accident (MVA). The prevalence rate for femoral neck, intertrochanteric and subtrochanteric fractures were 32 (53.3%), 17 (28.3%) and 11 (18.3%) respectively. From the data retrieved, 21patients (35.0%), 17 patients (28.3%), 20 patients (33.3%) and 2 patients (3.3%) had PFLCP, cemented BHA, cementless BHA and cannulated screw fixation respectively. Most (66.7%) of those who had PFLCP achieved satisfactory radiologic union and there was also 94.1% and 85% satisfactory outcome rate amongst the patients with cemented BHA and cementless BHA respectively. Irrespective of the operative intervention method at 1 year follow-up, there was a statistically significant improvement in post-operative HHS (P value 0.02), with 83.4% having good to excellent results. Profile of Factors of Arduous Work among Healthcare Professionals in Healthcare Facilities in Northern Benin in 2021

Introduction: Work environments create the basis for arduous nursing work. This is what led us to research the factors of arduousness in the exercise of the health profession in North Benin, through this study. Methods: This is a cross-sectional and descriptive study which focused on health professionals in the health zones of Parakou-N’dali and Tchaourou. The sampling was an exhaustive census. All health workers who had given their free and informed consent were included. The data was collected using a questionnaire. The variables studied were the factors of arduous work and socio-professional characteristics. The data collected was processed and analyzed with the Epi info 7.***.0.1 software. Results: A total of 692 were surveyed, the participation rate was 85.11% and the sex ratio (M/F) equal to 0.7. The average age was 38 years old. Nurses made up 32.66% of the sample. They were 86.42% subject to night work and 82.66% to alternating work. Overall, 70.23% worked more than 40 hours per week. Between 78.12% and 96.46% of non-specialist doctors, nurses, midwives and biomedical analysis technicians were required to work night shifts. Nurses, midwives, radiology and laboratory technicians were between 89.53% and 97.35%, organized in alternating work. In the sample, 55.20% complained about the insufficient number of work materials, 26.29% handled harmful chemical substances. Among those surveyed, 58.14% had been attacked. Conclusion: All professional categories of caregivers are subject to arduous work. Measures are needed to reverse the situation.

Preliminary Study of Chemical Elements Distribution in Petroleum Source Rocks Donga and Yogou Formations of the Termit Sedimentary Basin (Est-Niger)

XRF and EDX analyses were carried out on 18 batches of representative raw samples to determine the distribution of major chemical elements in the petroleum source rocks of Donga and Yogou formations of Termit sedimentary basin. The chemical composition of these formations is dominated by silicon (Si), aluminum (Al) and iron (Fe). This is consistent with the oxide composition, which is also dominated by silicon oxide (SiO2), aluminum oxide (Al2O3) and iron monoxide (FeO). No less important chemical elements are calcium (Ca), potassium (K), sulfur (S), titanium (Ti), magnesium (Mg), manganese (Mn) and barium (Ba), as well as some of their oxides. All these major chemical elements are carried by silicate detrital minerals associated with pyrite and goethite and/or clay minerals such as kaolinite and interstratified illite, smectite and chlorite. This trend is illustrated by the values of the Si/Al and SiO2/Al2O3 ratios.

Study of the Constraints of Millet Production (Pennisetum glaucum (L.) R. Br.) and the Peasant Perception of Biological Control in the Tahoua Region

Millet (Pennisetum glaucum (L.) R. Br.) is the Sahelian crop par excellence due to its adaptation to the particular production conditions in this region. Unfortunately, in recent years this crop has been threatened by very strong parasitic pressure and drought during the production period. The objective of this study is to analyze the main constraints of millet production and the solutions known to producers. A survey was carried out in November 2022 with a sample of 298 producers in five municipalities in the Tahoua region. The main constraints are drought and pressure from crop pests (locust, millet ear miner, floricultural insects) according to 57.9% of respondents. The millet ear miner is the most formidable pest according to 55% of respondents. Thus, the average yield obtained in a year of good production without the leafminer is 194 kg/ha and that obtained in a year of millet ear leafminer is around 27 kg to 43 kg/ha depending on the municipality. The yield obtained this last campaign after the attack of this leafminer varies from 64 to 77 kg/ha depending on the municipalities compared to a potential yield of over 1000 kg/ha. More than half of producers (58.1%) are unaware of the existence of biological control compared to only 12.5% who are aware of this alternative method. Work to popularize this technology is necessary in the five municipalities and the entire region in general.

Exoscopy of Detrital Zircons from Niamey Neoproterozoic Sandstones (Eastern Edge of the West African Craton, Southwestern Niger): Interpretation of Detrital Sediments Provenance

This study focuses on exoscopic analyses of detrital zircon grains of Archean and Paleoproterozoic age, contained in the sediments from the Niamey Neoproterozoic sandstones (Niamey region), with a view to confirming the sediments sources already proposed (Kénéma-Man domain for Archean-age zircons and Baoulé Mossi domain for Paleoproterozoic-age zircons). Exoscopic analysis reveals that Archean zircon grains are more corroded, with rounded to sub-rounded shapes, while Paleoproterozoic zircon grains are less corroded and mostly angular in shape. The strong corrosion of Archean zircon grains, implying long-distance transport, is consistent with the remoteness of the Kenema-Man domain which are the source these sediments. The fact that the Paleoproterozoic zircon grains show little or no wear implies a proximal source of sediments, corresponding to the Baoulé Mossi domain.

Fournier’s Gangrene: 13-Year Experience in a Tertiary Center, North Eastern Nigeria

Background: Fournier’s gangrene is a fulminant infection of the genitourinary tract characterized by progressive necrosis of the skin and subcutaneous tissue of the external genitalia. Initially mainly seen involving the scrotum of elderly men with different moods of occurrence each unpleasantly lethal. With varying types of presentation only one thing has remained constant;the poor prognosis of this condition. Purpose/Aim: Thus, it’s important to study the trends of the presentation of this condition laying emphasis on the challenges in the management of these patients;both the negative and positive prognostic factors. Materials and Methods: Urology ward record books, clinic record books and operating theater records were used to identify patients managed for Fournier gangrene in ATBUTH Bauchi. A retrospective study of the medical files of all the patients managed from January 2011 to January 2024 was done. Data was analyzed using SPSS version 29. Results: Data from 47 patients seen during the period of study were carefully collected and analyzed. The age range is 2 weeks to 97 years. There were two neonates, one at 2nd week of life and the other at 3rd week. Mortality rate is 36%. The average time duration before presentation for patients that died was two weeks, a minimum of 9 days prior to presentation and a maximum of 21 days, about 10 of which came in septic shock and the remaining presented with fever and very extensive necrotizing fasciitis of the perineum. All the patients that died had diabetes mellitus as a comorbidity except the neonate. All the patients that survived were much younger, all were below 60 years of age (2 weeks - 53 years). Conclusion: Here, we share our experience managing patients with Fournier’s gangrene in our facility in the past 13-year period from January 2011 to January 2024. .

Clinical Versus Doppler Based Assessment in Determining Amputation Level in Diabetic Foot Gangrene;A Prospective Cross-Sectional Study at Atbuth, Bauchi

Background: Amputation is defined as the surgical removal of a limb or part of a limb through the bone. If the amputation is done above or below the knee, they are termed major while minor amputations involve the partial removal of foot including forefoot resections, ray amputation of the digits or parts of the digits. Significant number of patients with diabetic foot ulcers end with amputations. In the past the amputation level was decided by clinical assessment alone, such as physical examination using color, temperature, peripheral pulses and wound bleeding during surgical procedure. The use of Doppler ultrasound to measure arterial blood pressure at the proposed amputation site has been advocated as a predictor of amputation success. An optimal choice of the level of amputation can reduce amputation complications. Methodology: A Prospective comparative randomized cross-sectional study carried out between 1st January 2022 and 1st January 2024 in ATBUTH, Bauchi amongst patients with diabetic foot Wagener stage IV and V scheduled for amputation. Outcome measures of wound break down, flap necrosis and re-amputation were assessed amongst the clinical based level assessment group and the doppler based level assessment group. Results: A total of 171 patients were recruited into the study. Males 103 and 68 are females, giving a male to female ratio of 1.5:1. mean age 47 years. There were 84 patients in the clinical based level assessment group and 87 patients in the doppler based level assessment. Conclusion: Diabetes mellitus foot disease is a significant risk factor for non-traumatic lower limb amputation and doppler level assessment is superior to clinical level in determining amputation level among diabetic patients scheduled for amputation. P-value 0.003.

One of the major causes of road accidents is sleepy *** accidents typically result in fatalities and financial losses and disadvantage other road *** studies have been conducted to identify the driver’s sleepiness and integrate it into a warning *** studies have examined how the mouth and eyelids ***,this limits the system’s ability to identify drowsiness ***,this study designed an Accident Detection Framework(RPK)that could be used to reduce road accidents due to sleepiness and detect the location of *** drowsiness detectionmodel used three facial parameters:Yawning,closed eyes(blinking),and an upright head *** model used a Convolutional Neural Network(CNN)consisting of two *** initial phase involves video processing and facial landmark coordinate *** second phase involves developing the extraction of frame-based features using normalization *** these phases used OpenCV and *** dataset contained 5017 images with 874 open eyes images,850 closed eyes images,723 open-mouth images,725 closed-mouth images,761 sleepy-head images,and 1084 non-sleepy head *** dataset of 5017 images was divided into the training set with 4505 images and the testing set with 512 images,with a ratio of 90:*** results showed that the RPK design could detect sleepiness by using deep learning techniques with high accuracy on all three parameters;namely 98%for eye blinking,96%for mouth yawning,and 97%for head ***,the test results have provided an overview of how the developed RPK prototype can accurately identify drowsy *** findings will have a significant impact on the improvement of road users’safety and mobility.

BACKGROUND Oral candidiasis(OC)is an oral health disease that could influence patients’oral health quality of *** To estimate prevalence of OC among disabled and non-disabled individuals and its potential risk factors in the Al-Baha region,Saudi *** An observational cross-sectional study was carried out among 148 disabled and non-disabled *** technique of concentrated oral rinse employing the Sabouraud Dextrose Agar medium accompanied with 0.05%chloramphenicol was conducted to assess and isolate *** examination using the World Health Organization guidelines was conducted to examine participants’oral hea-lth status.A pre-designed questionnaire was also used to evaluate sociodemo-graphic,medical history,and oral hygiene habits of the studied *** Out of 148 participants(n=57,38%)had colonized *** of the studied population had visible Candida ***,Candida was found in the oral rinses without the subject presenting any lesions or issues caused by Candida(asymptomatic colonization).The most common prevalent OC among participants were Candida albicans,Candida glabrata,Candida dubliniensis,Candida krusei,Candida tropicalis,and Candida parapsilosis(n=35,61%;n=8,14%;n=6,10%;n=5,9%;n=2,4%;and n=1,2%)***,smoking,poor plaque,and gingival status were key potential risk factors that significantly associated with candida’s density and presence(P=0.001,P=0.001,P=0.01,and P=0.01)*** status had no statistically significant effect on presence and density of *** The prevalence of OC is almost third of the studied population;thus,may provoke a need to develop preventive strategies to reduce the OC rate and establish solid treatment plans.
