The interaction effects of rocker angle and apex location in rocker shoe design on foot biomechanics and Achilles tendon loading

The influences of rocker shoes on foot biomechanics were controversial because the interaction between two design factors—rocker angle and apex location,was usually *** study investigated the interaction effects of rocker angle and apex location on plantar foot pressure,metatarsophalangeal/ankle angle,and Achilles tendon force during *** participants performed walking trials under six rocker shoe conditions:2 rocker angles(mild and severe)×3 apex locations(distal,standard,and proximal),wherein the plantar foot pressure was measured and the movement data were processed by musculoskeletal modeling to report joint angle and Achilles tendon force.A two-way ANOVA repeated measures was used for *** interaction effects were reported in examinations of forefoot pressure,midfoot pressure,and metatarsophalangeal *** standard apex significantly reduced peak forefoot and midfoot pressures(p=0.008–0.034,Hedges'g=0.75–0.84),which was further decreased by a severe rocker angle(p=0.006,Hedges'g=0.51–0.81).Moving the apex proximally reduced Achilles tendon forces(pchilles tendon *** concluded that apex location was likely the dominant design factor of the rocker sole in influencing foot biomechanics,yet its interactions with rocker angle should be *** configuration of the two features could be varied to possess different therapeutic merits and adapt to specific application purposes.

Different Asian Monsoon Rainfall Responses to Idealized Orography Sensitivity Experiments in the HadGEM3-GA6 and FGOALS-FAMIL Global Climate Models

Recent work has shown the dominance of the Himalaya in supporting the Indian summer monsoon （ISM）, perhaps by surface sensible heating along its southern slope and by mechanical blocking acting to separate moist tropical flow from drier midlatitnde air. Previous studies have also shown that Indian snmmer rainfall is largely unaffected in sensitivity experiments that remove only the Tibetan Plateau. However, given the large biases in simulating the monsoon in CMIP5 models, such results may be model dependent. This study investigates the impact of orographic forcing from the Tibetan Plateau, Himalaya and Iranian Plateau on the ISM and East Asian snmmer monsoon （EASM） in the UK Met Office＇s HadGEM3-GA6 and china＇s Institute of Atmospheric Physics FGOALS-FAMIL global climate models. The models chosen featnre oppositesigned biases in their simulation of the ISM rainfall and circulation climatology. The changes to ISM and EASM circulation across the sensitivity experiments are similar in both models and consistent with previous studies. However, considerable differences exist in the rainfall responses over India and china, and in the detailed aspects such as onset and retreat dates. In particular, the models show opposing changes in Indian monsoon rainfall when the Himalaya and Tibetan Plateau orography are removed. Our results show that a multi-model approach, as suggested in the forthcoming Global Monsoon Model Intercomparison Project （GMMIP） associated with CMIP6, is needed to clarify the impact of orographic forcing on the Asian monsoon and to fully understand the implications of model systematic error.

Primary leiomyosarcoma of the nipple-areola complex:Report of a case and review of literature

Primary leiomyosarcoma of the nipple-areola complex is extremely rare. Less than ten such cases have been reported in English literature so far. Herein we describe a 52-year-old female presenting with a 1.5 cm×1.1 cm×0.7 cm nodular lesion over her left nipple, and leiomyosarcoma was proved by pathological examination of the excised specimen. Positron emitted tomogram (PET) revealed no abnormal signal other than the primary site. Microscopically, this poorly circumscribed tumor was composed of interlacing bundles of smooth muscle cells with bizarre and pleomorphic nuclei, as well as prominent nucleoli. Its mitotic count was up to 7 mitoses per 10 high power fields (HPF). Immunohistochemical study of tumor cells revealed positive stain for α-smooth muscle actin and vimentin; and negative for cytokeratin, CD34 and S-100. Left simple mastectomy was undertaken and no residual mass lesion was noted on the resected specimen. Related literatures about the diagnosis and treatment for breast leiomyosarcoma will be presented here.

Effect of Rorrico, extracted from group of chinese medicines, on influenza A and H1N1 infections

OBJECTIVE: To investigate the therapeutic and preventive effects of Rorrico on influenza, especially influenza A viral infection, including swine flu(H1N1)in ***: Eighty-nine subjects were recruited in Hong Kong and Macao, and divided into treatment group(TG) and prevention group(PG) based on their influenza A and swine flu symptoms. All subjects were prescribed Rorrico or placebo, and monitored by a chinese medicine practitioner. Blood samples were collected before and after 7-day Rorrico or placebo treatment for laboratory ***: After treatment, there were some full recoveries and obvious relief of onset symptoms in the TG. Blood test results showed that Rorrico produced(a) no adverse effects on subjects' renal andliver functions, muscle enzyme and hematological status,(b) no up-regulation of pro-inflammatory cytokines tumour necrosis factor-a and interleukin-18 in both TG and PG,(c) mild yet statistically significant elevation of plasma mannose-binding lectin(MBL) in ***: Rorrico has no up-regulating effect on the participants' immune response, or, equally likely, the immuno-modulatory effects of Rorrico do not non-specifically or unnecessarily promote inflammation when not required. It is possible that oral administration of Rorrico can promote hepatic synthesis of MBL in healthy PG subjects, thereby conferring increased protection against infection.

Extensive contact tracing and screening to control the spread of vancomycin-resistant Enterococcus faecium ST414 in Hong Kong

Background Proactive infection control management is crucial in preventing the introduction of multiple drug resistant organisms in the healthcare setting. In Hong Kong, where vancomycin-resistant enterococci （VRE） endemicity is not yet established, contact tracing and screening, together with other infection control measures are essential in limiting intra- and inter-hospital transmission. The objective of this study was to illustrate the control measures used to eradicate a VRE outbreak in a hospital network in Hong Kong. Methods We described an outbreak of VRE in a healthcare region in Hong Kong, involving a University affiliated hospital and a convalescent hospital of 1600 and 550 beds respectively. Computer-assisted analysis was utilized to facilitate contact tracing, followed by VRE screening using chromogenic agar. Multi-locus sequence typing （MLST） was performed to assess the clonality of the VRE strains isolated. A case-control study was conducted to identify the risk factors for nosocomial acquisition of VRE. Results Between November 26 and December 17, 2011, 11 patients （1 exogenous case and 10 secondary cases） in two hospitals with VRE colonization were detected during our outbreak investigation and screening for 361 contact patients, resulting in a clinical attack rate of 2.8% （10/361）. There were 8 males and 3 females with a median age of 78 years （range, 40-87 years）. MLST confirmed sequence type ST414 in all isolates. Case-control analysis demonstrated that VRE positive cases had a significantly longer cumulative length of stay （P 〈0.001）, a higher proportion with chronic cerebral and cardiopulmonary conditions （P=0.001）, underlying malignancies （P 〈0.001）, and presence of urinary catheter （P 〈0.001）, wound or ulcer （P 〈0.001）, and a greater proportion of these patients were receiving β-lactam/ β-1actamase inhibitors （P=0.009）, carbapenem group （P 〈0.001）, fluoroquinolones （P=0.003）, or vancomycin （P=0.001） when compared with the controls. Postoperative tunnel enlargement has been frequently reported after anterior cruciate ligament（ACL）*** screw,as a surgical implant in ACL reconstruction,may influence natural loading transmission and contribute to tunnel *** aims of this study are（1）to quantify the alteration of strain energy density（SED）distribution after the anatomic single-bundle ACL reconstruction;and（2）to characterize the influence of screw length and diameter on the degree of the SED alteration.A validated finite element model of human knee joint was *** screw length ranging from 20 to 30 mm with screw diameter ranging from 7 to 9 mm were *** the post-operative knee,the SED increased steeply at the extra-articular tunnel aperture under compressive and complex loadings,whereas the SED decreased beneath the screw shaft and nearby the intra-articular tunnel *** the screw length could lower the SED deprivation in the proximal part of the bone tunnel;whereas increasing either screw length or diameter could aggravate the SED deprivation in the distal part of the bone *** the elastic modulus of the screw could lower the bone SED deprivation around the *** consideration of both graft stability and SED alteration,a biodegradable interference screw with a long length is recommended,which could provide a beneficial mechanical environment at the distal part of the tunnel,and meanwhile decrease the bone-graft motion and synovial fluid propagation at the proximal part of the *** findings together with the clinical and histological factors could help to improve surgical outcome,and serve as a preliminary knowledge for the following study of biodegradable interference screw.

Sleep posture monitoring is an essential assessment for obstructive sleep apnea(OSA)*** objective of this study is to develop a machine learning-based sleep posture recognition system using a dual ultra-wideband radar *** collected radiofrequency data from two radars positioned over and at the side of the bed for 16 patients performing four sleep postures(supine,left and right lateral,and prone).We proposed and evaluated deep learning approaches that streamlined feature extraction and classification,and the traditional machine learning approaches that involved different combinations of feature extractors and *** results showed that the dual radar system performed better than either single *** statistical features with random forest classifier yielded the best accuracy(0.887),which could be further improved via an ablation study(0.938).Deep learning approach using transformer yielded accuracy of 0.713.

BACKGROUND There remains a dearth of Asian epidemiological literature for paediatric inflammatory bowel disease(PIBD).AIM To describe the presenting features of PIBD from 7 Asia-Pacific pediatric gastroenterology centers via a central standardised electronic data *** Clinical,endoscopic and radiologic data at diagnosis from the registry were extracted between 1^(st) January 1995 to 31^(st) December *** phenotypic characteristics were classified as per the Paris classification *** There was a distinct rise in new PIBD cases:Nearly half(48.6%)of the cohort was diagnosed in the most recent 5 years(2015-2019).The ratio of Crohn’s disease(CD):Ulcerative colitis(UC):IBDUnclassified was 55.9%:38.3%:5.8%.The mean age was 9.07 years with a high proportion of very early onset IBD(VEO-IBD)(29.3%)and EO-IBD(52.7%).An over-representation of the Indian/South Asian ethnic group was observed which accounted for 37.0% of the overall Singapore/Malaysia subcohort(6.8%-9.0%Indians in census).Indian/South Asian CD patients were also most likely to present with symptomatic perianal disease(P=0.003).CD patients presented with significantly more constitutional symptoms(fever,anorexia,malaise/fatigue and muscle-wasting)than UC and higher inflammatory indices(higher C-reactive protein and lower albumin levels).CONCLUSION We observed a high incidence of VEO-IBD and an over-representation of the Indian *** Asian CD patients were more likely to have symptomatic perianal disease.
