To the Editor:Hereditary severe insulin resistance syndrome(H-SIRS)shows a wide and variable clinical spectrum and results in severe complications in the endocrinological and cardiovascular *** treatments are recommended to control hyperglycemia in H-SIRS,including dietary intervention,insulin therapy,insulin sensitization,and recombinant human insulin-like growth factor-1(rhIGF-1)administration.[1]However,the heterogeneous etiology of H-SIRS leads to poor glycemic control in patients with different mutations,requiring multiple antihyperglycemic *** study described two cases diagnosed with H-SIRS and the treatment effect of multi-drug therapy and sodium-glucose cotransporter 2 inhibitor(SGLT2i)monotherapy.

创伤性脑损伤(traumatic brain injury,TBI)是指脑组织遭受碰撞、打击后,出现功能紊乱的病理过程。全球范围内,每年约有超过1000万人因TBI住院或死亡[1],它对社会、经济和医疗保健领域有重大负面影响。近年来,对于TBI的病理生理机制及重症患者监护等方面研究皆有进展,但TBI患者死亡率及不良预后率仍较高[2]。目前,临床上诊断TBI主要使用格拉斯哥昏迷评分(Glasgow coma scale,GCS)和影像学检查,例如头部CT、核磁共振成像(magnetic resonance imaging,MRI)等。

根肿病是由芸薹根肿菌(Plasmodiophora brassicae Woronin)引起的全球性土传病害,对十字花科芸薹属作物的产量和质量构成了严重的威胁。根肿菌为专性活体寄生菌,具有强烈的生理小种分化现象,因此,对不同地区的生理小种类型进行评估,有利于芸薹属作物的抗性育种及作物栽培的整体规划。精确评估土壤中芸薹根肿菌休眠孢子的数量,有助于准确预测田块中根肿病发生的风险水平,可为后续防治方法的选择和防治效果的评估奠定基础。本文对芸薹根肿菌生理小种的鉴定以及土壤中根肿菌休眠孢子的检测和定量方法进行了简要综述,同时提出了一种有望应用于根肿菌生理小种研究的单细胞测序技术,并对重组酶聚合酶扩增技术(recombinase polymerase amplification,RPA)在根肿菌检测与定量方面的可行性进行了分析。本文旨在为根肿病的高效防控提供理论依据和方法参考,同时为根肿菌鉴定与检测方法的研究提供新的研究思路和解决方案。

A consensus meeting of national experts from all major national hepatobiliary centres in the country was held on May 26,2023,at the Pakistan Kidney and Liver Institute&Research Centre(PKLI&RC)after initial consultations with the *** Pakistan Society for the Study of Liver Diseases(PSSLD)and PKLI&RC jointly organised this *** effort was based on a comprehensive literature review to establish national practice guidelines for hilar cholangiocarcinoma(hCCA).The consensus was that hCCA is a complex disease and requires a multidisciplinary team approach to best manage these *** coordinated effort can minimise delays and give patients a chance for curative treatment and effective *** diagnostic and staging workup includes high-quality computed tomography,magnetic resonance imaging,and magnetic resonance *** cytology or biopsy utilizing endoscopic retrograde cholangiopancreatography is a mainstay for ***,histopathologic confirmation is not always required before *** ultrasound with fine needle aspiration of regional lymph nodes and positron emission tomography scan are valuable adjuncts for *** only curative treatment is the surgical resection of the biliary tree based on the Bismuth-Corlette *** patients with unresectable hCCA can be considered for liver *** chemotherapy should be offered to patients with a high risk of *** use of preoperative biliary drainage and the need for portal vein embolisation should be based on local multidisciplinary *** with acute cholangitis can be drained with endoscopic or percutaneous biliary *** chemotherapy with cisplatin and gemcitabine has shown improved survival in patients with irresectable and recurrent hCCA.

BACKGROUND Coagulopathy and thromboembolic events are associated with poor outcomes in coronavirus disease 2019(COVID-19)*** is conflicting evidence on the effects of chronic anticoagulation on mortality and severity of COVID-19 *** To summarize the body of evidence on the effects of pre-hospital anticoagulation on outcomes in COVID-19 *** A Literature search was performed on LitCovid PubMed,WHO,and Scopus databases from inception(December 2019)till June 2023 for original studies reporting an association between prior use of anticoagulants and patient outcomes in adults with *** primary outcome was the risk of thromboembolic events in COVID-19 patients taking *** outcomes included COVID-19 disease severity,in terms of intensive care unit admission or invasive mechanical ventilation/intubation requirement in patients hospitalized with COVID-19 infection,and *** random effects models were used to calculate crude and adjusted odds ratios(aORs)with 95%confidence intervals(95%CIs).RESULTS Forty-six observational studies met our inclusion *** unadjusted analysis found no association between prior anticoagulation and thromboembolic event risk[n=43851,9 studies,odds ratio(OR)=0.67(0.22,2.07);P=0.49;I2=95%].The association between prior anticoagulation and disease severity was non-significant[n=186782;22 studies,OR=1.08(0.78,1.49);P=0.64;I2=89%].However,pre-hospital anticoagulation significantly increased all-cause mortality risk[n=207292;35 studies,OR=1.72(1.37,2.17);P<0.00001;I2=93%].Pooling adjusted estimates revealed a statistically non-significant association between pre-hospital anticoagulation and thromboembolic event risk[aOR=0.87(0.42,1.80);P=0.71],mortality[aOR=0.94(0.84,1.05);P=0.31],and disease severity[aOR=0.96(0.72,1.26);P=0.76].CONCLUSION Prehospital anticoagulation was not significantly associated with reduced risk of thromboembolic events,improved survival,and lower disease seve

The electrochemical skeleton for the production of hydrogen production from water splitting consists of electrocatalysts,electrolytes,substrates,and standard *** selenides have been considered efficient electrocatalysts due to their outstanding ***,electrolytes and employed substrates can significantly influence the electrocatalytic performance of the metal selenides for hydrogen evolution reaction(HER).Several studies of metal selenides for HERs have been carried out and summarized,but there are some severe limitations regarding the synchronization of the employed substrates,electrolytes,and *** experiments suggest that metal selenides deposited on carbon-based substrates inject and transport the charge carriers effectively in an acidic medium and boost the electrocatalytic ***,the metal selenides deposited on materials other than carbon-based substrates inject and make the facile transportation of the charge carriers in an alkaline medium and promote the electrocatalytic ***,the fundamental characteristics,critical factors,and electrocatalytic performance of metal selenides deposited on carbon-based substrates in acidic electrolytes have been effectively *** electrocatalytic performance of the metal selenides in an alkaline electrolyte and substrates like nickel foam have also been summarized to provide direction,challenges,and feedback about the electrocatalytic performance of metal selenides.
