Celiac disease(CD)affects at least 1%of the Western population but remains largely *** our laboratory,we utilize a novel algorithm to diagnose pediatric CD that offers both high sensitivity and high specificity for diagnosis in an outpatient *** aim of the present study was to challenge this algorithm and to test its performance in children and adults suspected of having *** a three-assay algorithm,screening with the most sensitive tissue transglutaminase(tTG)complexed with deamidated gliadin peptide neoepitope immunoglobulin A(IgA)1IgG assay and confirming with the two specific tTG IgA and tTG IgA1IgG assays,we examined the serological results from 112 children aged 0–17 years old and 60 adults in comparison to their respective biopsy *** algorithm performance was calculated by statistical *** use of the new algorithm enabled us to diagnose CD with 98%sensitivity,93%specificity and 95%accuracy in the pediatric group and 94%sensitivity,92%specificity and 93%accuracy in the total population *** false-negative cases in the adult group were attributed to previous adherence to a gluten-free diet,and the single false-negative result in a young child became a true positive after 6 *** have also monitored three celiac patients before and after diagnosis and found that the algorithm may be suitable for disease *** newly proposed three-assay algorithm for celiac detection is very reliable in both children and *** to the high performance of this assay,the further need for confirmatory intestinal biopsies will be reassessed.

Aim： To report our experience with penile girth augmentation using liquid injectable silicone. Methods： Between August 2003 and July 2006, 324 men （mean age 35 years, range 19-65 years） received a series of liquid silicone subcutaneous injections between the penile skin and the corpora cavernosa on the dorsal and lateral aspects of the penile shaft, under local anesthesia. Digital photographs taken pre- and post-procedure （n = 324）, and penile contour measurements （n = 30） yielded objective results. Subjective results were derived from patient and partner testimony of satisfaction. Follow-up averaged 20 months （range 1-36 months）. Results： Three hundred and twenty-four procedures were primary augmentations. Most men （61%） were married, 7% were accompanied by their partners, and 93% were circumcised. The mean measured penile circumference was 9.5 cm （7.5-11.5 cm） pretreatment and 12.1 cm （10.3-15.3 cm） post-treatment （mean increase of 27% in circumference and 0.84 cm in diameter）. Patient and partner satisfaction was already expressed after the first two treatments. Sexual activity could be resumed after 8 h. Complications （mild bruising） were easily resolved. Conclusion： Penile girth augmentation using liquid injectable silicone yields very satisfactory short-term results with no immediate or short-term complications.

In this paper,we present the experimental results for integrated photonic devices optimized with an energyconstrained inverse design *** this constraint is applied,optimizations are directed to solutions that contain the optical field inside the waveguide core medium,leading to more robust designs with relatively larger minimum feature *** optimize three components:a mode converter(MC),a 1310 nm/1550 nm wavelength duplexer,and a three-channel C-band wavelength demultiplexer for coarse wavelength division multiplexing(CWDM)application with 50 nm channel *** energy constraint leads to nearly binarized structures without applying independent binarization *** also reduces the appearance of small *** the MC,well-binarized design,improved insertion loss,and cross talk are obtained as a ***,the proposed constraint improves the robustness to fabrication imperfections as shown in the duplexer *** energy constraint optimization,the corresponding spectrum shifts for the duplexer under±10 nm dimensional variations are reduced from 105 nm to 55 nm and from 72 nm to 60 nm for the 1310 nm and 1550 nm channel,*** the CWDM demultiplexer,robustness toward±10 nm fabrication error is improved by a factor of *** introduction of the energy constraint into topological optimization demonstrates computational gain with better-performing designs.

自适应发汗（Self—Contained Adaptive Transpiration—SCAT）鼻锥研制计划的总目的是,确定SCAT冷却方案用于人们所关心的高级再入弹头鼻锥的可能性。本合同的具体目的是:（1）为合适的论证试验进行初步的SCAT鼻锥设计;（2）对特定的SCAT鼻锥现象、制造技术和材料性能进行认证实验,用以支持论证试验和设计工作。设计工作又为初步选择冷却剂和驱动剂以及要求的材料性能,如多孔端头的渗透性,打下基础。初步选择的冷却剂、驱动剂和材料在认证试验中都已经使用。认证实验的结果曾用来检验目前鼻锥特性模型采用的模型化技术,而且一经证实,便可用来修正特性模型。设计工作包括为SCAT方案论证确定合适的飞行试验弹和能胜任指定再入弹道的“有限应用”的SCAT鼻锥。为HEART-2试验弹（弹道系数β=2,400磅/呎2,在250千呎高处再入速度V=17.8千呎/秒,再入角γ=-35度）拟定了一个重量为23磅的SCAT鼻锥方案。这个鼻锥是F结构的改型,F结构是以前为了能用于多种再入条件而设计的。改型设计缩短了鼻锥长度,并且使用了钨丝制造的单向流动多孔端头,这样就减轻了重量。F结构原来的重量为34磅。对于指定的弹道（弹道系数β=2,000磅/呎2,在400千呎高处再入速度V=23.3千呎/秒,再入角γ=20度）利用一个头部半径为0.75吋,半锥角为8.5度的鼻锥结构进行了有限的设计研究。为了在驻点区增加冷却剂的供应量,这个“有限应用”的SCAT鼻锥也使用了单向流动多孔端头。这个SCAT鼻锥的重量约为23磅,而且,通过最佳设计还有可能把重量进一步减少4～10磅。证实实验和认证试验在以下这些方面得到了有价值的结果:单向钨丝多孔端头的制造,多孔材料与密实钨的焊接。多孔结构的性能测定,冷却剂和驱动剂之间相互作用的测定,以及SCAT鼻锥底部密封的设计、制造和鉴定。用热等静压技术成功地制造了单向钨丝多孔端头,利用高温钎焊技术把多孔结构和密实钨连成一体。利用液态铊测定了多孔结构的液态金属高温高压渗透性,从而实现了对多孔结构的表征。为了确定冷却剂与驱动剂相互作用对最后产生的压力可能带来的影响,在有铊存在的情况下确定了可能用的驱动剂（例如水和苯酚）的蒸汽压—温度特性。在这些冷却剂和驱动剂之间没有发现相互作用。此外,为了说明在鼻锥特性模型中所使用的假设是正确的,进行了第一轮分析。这些假设是:冷却剂和驱动剂的换位是瞬时发生的,冷却剂和驱动剂在再入期间不发生持久的混合。在对泄漏速率进行试验测量的基础上,确定了由于底部密封泄漏引起的SCAT鼻锥压力降低的速率。发现,实际的泄漏速率和压力损失对于鼻锥冷却剂流率没有明显影响。本报告建议继续研制SCAT方案。这些建议的目的是希望在合适的飞行试验中验证SCAT方案。还建议进行系统的应用研究与设计以及制造研究,以便上述主要建议得以实现。

Background. Ischemic steal syndrome is a complication of arteriovenous access creation for hemodialysis, and is little known among dermatologists despite the fact is fairly common and has a cutaneous presentation. Herein we report the case of a man presenting with serious ischemic injuries to the fingers associated with steal syndrome. Observation. A 49-year-old diabetic man presented with painful acrosyndrome of the left hand, with a necrotic ulcer on the third finger appearing 5 months after the creation of an arteriovenous access. Clinical examination showed marked pain relief after manual compression of the fistula. Arteriography showed an over functional fistula, causing decreased distal perfusion pressure, and this was consistent with the diagnosis of steal syndrome. Banding of the anastomosis was performed but was insufficient to spare the finger from amputation. Discussion. The aim of our case-report is to increase awareness among dermatologists of this fairly frequent complication of arteriovenous accesses. We discuss the physiopathology, risk factors, clinical features and therapeutic management of this condition. Since diagnosis of steal syndrome is fairly straightforward, early detection is essential. Only adequate surgical management can avoid gangrenous manifestations leading to more or less widespread amputation.
