Noninvasive Ventilation interfaces in the Treatment of Acute Respiratory insufficiency: A Critical Review

Background: Noninvasive ventilation (NiV) is an important therapeutic modality for the treatment of acute respiratory failure (ARF). in this review, we critically analyze randomized controlled trials on the most used NiV interfaces in the treatments of ARF. Methods: The searches were conducted in the Medline, Lilacs, PubMed, Cochrane, and Pedro databases from June to November 2021. The inclusion criteria were Randomized clinical trials (RCTs) published from 2016 to 2021 in Portuguese, Spanish, or English and involving adults (aged ≥ 18 years). The eligibility criteria for article selection were based on the PiCO strategy: Population—Adults with ARF;intervention—NiV Therapy;Comparison—Conventional oxygen therapy, high-flow nasal cannula (HFNC) oxygen therapy, or NiV;Outcome—improvement in ARF. The search for articles and the implementation of the inclusion criteria were independently conducted by two researchers. Results: Seven scientific articles involving 574 adults with ARF due to various causes, such as chest trauma, decompensated heart failure, coronavirus disease 2019 (COViD-19), and postoperative period, among others, were included. The interfaces cited in the studies included an oronasal mask, nasal mask, full-face mask, and helmet. in addition, some favorable outcomes related to NiV were reported in the studies, such as a reduction in the rate of orotracheal intubation and shorter length of stay in the iCU. Conclusions: The most cited interfaces in the treatment of ARF were the oronasal mask and the helmet.

Spontaneous infection of Ascites Fluid at the National and University Hospital Hubert Koutoukou Maga in Cotonou: Prevalence and Associated Factors

Background: Spontaneous ascites fluid infection (SAFi) is an extremely serious and frequently encountered complication in cirrhotic patients. We aimed to determine the prevalence of SAFi and the factors associated with it in the largest hospital in Cotonou. Methods: This was a retrospective descriptive and analytical study conducted from January 2013 to July 2019, at the National and University Hospital Hubert Koutoukou Maga (CNHU-HKM) in Cotonou, Benin. All patients followed in the University Clinic of Hepato-Gastroenterology and diagnosed with SAFi were included. Results: Eighty-two patients were included, predominantly males (69.5%), with a mean age of 51.5 ± 14.5 years. Among them, 32 had SAFi, i.e., a prevalence of 39%. Of the 32 cases of SAFi, the culture of ascites fluid was positive in 6 cases (18.7%). The most frequent germ found in SAFi was Escherichia coli (5 patients, 83.3%). The factors associated with SAFi in this study were: abdominal pain (p = 0.004), increased bilirubinemia (p = 0.009), decreased prothrombin level 20 (p = 0.001). Conclusion: SAFi was common in cirrhotic patients in the department. Certain clinical and paraclinical factors were associated with it, as was the severity of cirrhosis. Early diagnosis and aetiological management of cirrhosis could reduce its frequency.

Assessment of the Drafting Quality of Request Forms Submitted to the Malaria and Parasitology Units at the institut Pasteur de Cote d’ivoire

Biological tests provide information on the medical analysis requested by both the patient and the prescriber. it is a communication link between the prescriber and the laboratory staff. The lack of some information on request forms not only affects the drafting quality of the test and patient care, but could also make thousands of data produced by healthcare centers unusable. The aim of this study was to assess the drafting quality of request forms submitted to the Malaria and Parasitology Units at the institut Pasteur de Côte d’ivoire. Methods: it was a descriptive cross-sectional study to assess the drafting quality of request forms of various prescribers received at the institut Pasteur de Côte d’ivoire. This study was conducted at the Malaria and Parasitology Units, department of Parasitology and Mycology (institut Pasteur de Côte d’ivoire), from 6th December 2020 to 6th December 2021. The information on each request forms was recorded on a data collection form designed for this purpose. Each data collection form corresponds to a request forms and each test to a patient. Results: Out of a total of 1990 request forms received, the patient’s age and sex were missing on 18% and 26.8% of the tests respectively. More than half (51.80%) of request forms did not indicate the patient’s place of residence. Clinical information was not provided on 45.90% of the tests. Prescribers omitting their signatures were 51%, stamps were 50.3% and contacts were 71.2%. Only 5.4% of request forms were of good drafting quality. Providing all the required information on the forms could facilitate the use and analysis of data and samples.

Depression and Suicidal Risk in HiV-infected Adults at Brazzaville University Hospital: Prevalence and Associated Factors

introduction: Highly active antiretroviral tritherapies have improved the quality of life of people living with HiV (PVViH) and extended their life expectancy. However, the nervous system is faced with a neurotropic virus that evolves chronically, sometimes creating neurological disorders directly or indirectly linked to stigmatization or therapeutic effects. Objective: to determine the prevalence of depression associated with suicidal risk (SR) and identify associated factors. Patients and method: This was a prospective descriptive and analytical study from January 1 to November 30, 2022, including all HiV-positive patients receiving or not receiving antiretroviral therapy. The study took place in the infectious diseases department of Brazzaville University Hospital. This department has an inpatient capacity of 36 beds, and an outpatient capacity of 25 patients per day, three times a week. This is the largest center for PVViH in Brazzaville. The Patient Heath Questionnaire 9 (PHQ-9) was used to assess the degree of depression and suicidal risk. Therapeutic adherence was assessed using the Morisky questionnaire. Data were analyzed using SPSS version 23 software. Qualitative variables were expressed as numbers and percentages, and quantitative variables as means and standard deviations. Statistical tests were used according to their applicability criteria. For all tests, the significance threshold was set at 0.05. Results: A total of 150 patients were consulted, constituting the sample size. The prevalence of depression and suicidal risk was 32%. These patients had an average age of 42.83 ± 10.24 years, were female (n = 101;67.3%), Christian (n = 97;64%), single (n = 68;45.3%). Stigmatization (n = 75;50%) was the reason for attempted suicide. HiV was perceived as treatable (n = 103;68.7%), unlucky (n = 62;41.3%). Patients were WHO stage 1 (n = 105;70%). RS was present in 47 cases (31.3%). Evaluation of a cost effective technique for treating aquaculture water discharge using Lolium perenne Lam as a biofilter

Wastewater stabilization ponds generate low cost by-products that are useful for agriculture. The utilization of these by-products for soil amendment and as a source of nutrients for plants requires a high level of sanitation and stabilization of the organic matter, to maintain acceptable levels of soil, water and air quality. in this study, two aquaculture wastewater treatment systems; recirculating system and a floating plant bed system were designed to improve the quality of irrigation water in local communities with low income. in both systems the grass species Lolium perenne Lam was used as a plant biofilter while vegetable specie Amaranthus viridis was used to evaluate the performance of the system and the suitability of the phyto-treated water for irrigation. it was found that the harmful material removal rate for recirculating system was 88.9% for TAN （total ammonia nitrogen）, 90% for NO2--N, 64.8% for NO3--N while for floating plant bed system 82.7% for TAN, 82% for NO2--N and 60.5% for NO3--N. Comparative analysis of the efficiency of waste element removal between the two systems revealed that both systems performed well, however, plant growth was not robust for floating plant bed system while recirculating system is energy consuming. Although both systems did not attain sufficient levels of TN （total nitrogen） and TP （total phosphorus） load reduction, the treatment with L. perenne remarkably improved the irrigation water quality. A. viridis plants irrigated with the phyto-treated discharge water had lesser concentrations of heavy metals in their tissues compared to those irrigated with untreated discharge. The control plants irrigated with untreated discharge were also found to be highly lignified with few stems and small leaves.

Vegetation recovery after fire in mountain grasslands of Argentina

Fire is a natural disturbance occurring every few years in many grasslands ecosystems. However, since European colonization, fire has been highly reduced or even suppressed in Argentinean grasslands, fostering ignitable material accumulation. This has led to occasional catastrophic controldemanding fire events, extended for larger areas. The aims of this work are to study vegetation recovery and change after a non-natural fire event in mountain grasslands. The study area is located in the Ventania mountain system, mid-eastern Argentina. We studied vegetation recovery after fire(January 2014) in two different communities: grass-steppes(grasslands) and shrub-steppes(open low shrublands). We measured vegetation cover, species richness and bare ground percentage in burned and unburned areas 1, 4, 8, 11 and 23 months after fire. Vegetation surveys were also performed at the end of the growing season(December) 11 and 23 months after fire. Data were analyzed using regression analysis, ANOVA and multivariate analysis(NMS, PERMANOVA). Both communities increased their vegetation cover at the same rate, without differences between burned and unburned areas after two years. Species richness was higher in shrublands and their recovery was alsofaster than in grasslands. Considering functional composition, besides transient changes during the first year after fire, there were no differences in abundance of different functional vegetation groups two years after fire. At the same time, shrublands showed no differences in species composition, while grasslands had a different species composition in burned and unburned plots. Also, burned grassland showed a higher species richness than unburned grassland. Data shown mountain vegetation in Pampas grassland is adapted to fire, recovering cover and richness rapidly after fire and thus reducing soil erosion risks. Vegetation in mountain Pampas seems to be well adapted to fire, but in grasslands species composition has changed due to fire.

Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)recently emerged as a highly virulent respiratory pathogen that is known as the causative agent of coronavirus disease 2019(COViD-19).Diarrhea is a common early symptom in a significant proportion of patients with SARS-CoV-2 ***-CoV-2 can infect and replicate in esophageal cells and enterocytes,leading to direct damage to the intestinal *** infection decreases the level of angiotensinconverting enzyme 2 receptors,thereby altering the composition of the gut ***-CoV-2 elicits a cytokine storm,which contributes to gastrointestinal *** direct cytopathic effects of SARS-CoV-2,gut dysbiosis,and aberrant immune response result in increased intestinal permeability,which may exacerbate existing symptoms and worsen the *** exploring the elements of pathogenesis,several therapeutic options have emerged for the treatment of COViD-19 patients,such as biologics and biotherapeutic ***,the presence of SARS-CoV-2 in the feces may facilitate the spread of COViD-19 through fecal-oral transmission and contaminate the *** gastrointestinal SARS-CoV-2 infection has important epidemiological *** development of new therapeutic and preventive options is necessary to treat and restrict the spread of this severe and widespread infection more ***,we summarize the key elements involved in the pathogenesis and the epidemiology of COViD-19-associated diarrhea.
