Objective: To investigate the clinical features of the multiple cutaneous and uterine leiomyomatosis (mCUL) syndrome, including the hereditary leiomyomatosis and renal cell cancer syndrome. Design: A case series of patients with multiple skin leiomyomas solicited via a circular letter to dermatologists. Setting: Research institute. patients: A total of 108 affected individuals, including 46 probands and 62 affected relatives. main Outcome measures: The proportion of probands with underlying fumarate hydratase (FH) mutations, the penetrance of FH mutations, and clinicopathologic features of mCUL. Results: Forty-one (89% ) of 46 probands with multiple skin leiomyomas had evidence of germline FH mutations, which were highly penetrant. All 26 male mutation carriers had skin leiomyomas. Of 67 women with FH mutations, 46 (69% ) had both skin and uterine leiomyomas; 10 (15% ) had only skin leiomyomas;5(7% )had only uterine leiomyomas;and 6(9% )were clinically unaffected. patients presented with skin leiomyomas at a mean age of 24 years and had a mean of 25 lesions. Forty-one individuals (89% ) reported painful lesions, particularly in response to cold or trauma. Fibroids were histologically unremarkable, highly symptomatic, and associated with a high risk of early hysterectomy. One individual had a very aggressive collecting duct renal cancer. The G354R FH mutation predisposed patients to uterine fibroids without skin leiomyomas (p=.03). many patients with skin leiomyomas had not previously presented for medical attention. Fibroids were rarely recognized as cases of mCUL. Conclusions: Highly penetrant FH mutations underlie mCUL. increased clinical awareness is important because of the associated risk of severe uterine fibroids and, in some cases, aggressive renal cancer.

Functional trait variation of plant species includes both inter-and intraspecific variation;however,traitbased plant ecology generally considers only interspecific variation while ignoring intraspecific *** reason for this neglect is that intraspecific variation may be negligible when compared to interspecific variation;however,direct comparisons between inter-and intraspecific variation of plant species are lacking,especially in tropical *** we investigated intraspecific leaf trait variation(leaf area,specific leaf area,leaf thickness,leaf density,leaf chlorophyll content) of pittosporopsis kerrii Craib(icacinaceae),the most abundant tree species in the Xishuangbanna tropical seasonal rainforest in southwestern China,along an elevational gradient(703-824 m).We found a substantial range of intraspecific variation in *** that was never less than 22.1% of range of the interspecific variation among 462 tree species reported before in the same ***,with increased elevation,both leaf thickness and density increased and specific leaf area decreased *** could be more important for the individuals of *** to produce thicker and denser leaves to tolerate environmental stress(e.g,soil water availability) rather than having high growth rates at the places with higher elevation in the Xishuangbanna tropical seasonal rainforest.

Suicide ideation, depression and HiV among pregnant women in rural South Africa

Background: Despite public perception to the contrary, pregnancy does not offer a protective effect for suicide ideation. in low and middle income countries (LmiC) multiple concurrent risk factors including unplanned pregnancies, testing for HiV, intimate partner violence and depression may increase suicide risk among pregnant women. We examine suicide ideation among women attending routine antenatal care in rural South Africa. method: A consecutive series of 109 women in the second half of pregnancy were enrolled in a cross-sectional study at a large primary health care facility in a rural area of KwaZulu-Natal with high HiV prevalence. Using a mixed-methods approach, women were assessed using the major Depression Section of the Structured Clinical interview for Depression (SCiD) for DSm-iV diagnosis;qualitative interview data were collected on women’s experiences of suicide ideation and plans. Women were assessed two weeks after routine HiV testing as part of prevention of mother-to-Child Transmission (pmTCT) screening. Results: Both HiV-positive (49/109) and HiV-negative (60/109) women were assessed. Depression was high (51/109, 46.7%) and equally common amongst HiV-positive and negative women. Suicide ideation was high (30/109, 27.5%) with thoughts of self harm closely linked to suicide plans. Age, previous history of depression and current depression were significantly associated with suicide ideation. Close to equal numbers of HiV-positive and negative women were suicidal. Suicide methods were frequently violent and most women cited family and partnership conflict related to an unplanned pregnancy or their newly HiV-positive status as triggers to their suicide ideation. Conclusions: Rates of suicide ideation among pregnant women testing for HiV are high. Testing HiV-positive causes considerable distress, and among HiV-negative women unplanned and unwanted pregnancies are important risk factors for suicide ideation. pregnancy brings women into increased contact

目的探讨黄芪总黄酮(TFA)对高脂饲料诱导家兔动脉粥样硬化(AS)的防护作用。方法 60只新西兰白兔被随机分为5组,分别给正常饲料(对照组,n=10)、高脂饲料(造模组,n=10)、高脂饲料加维生素E 400 iU·d^(-1)(VitE组,n=10)、高脂饲料加TFA 60 mg·d^(-1)(TFA 1组,n=15)和高脂饲料加TFA 120 mg·d^(-1)(TFA 2组,n=15)处理12 wk,动态监测(0、8和12 wk)血脂的变化,实验结束前,所有动物用CO_2处死,分别检测主动脉弓的病理变化和胆固醇的含量。结果实验数据表明所有接受高脂饲料的动物血浆中的总胆固醇、甘油三脂、主动脉弓的粥样硬化斑块面积、胆固醇含量和动脉壁内膜与中膜平均厚度比值均高于正常饲料组。TFA能降低血浆总胆固醇(pmg·d^(-1))能分别降低43.1％和63.0％的AS斑块面积;维生素E可降低动脉壁中的胆固醇沉积和动脉壁内膜与中膜比率(p<0.05),减低43.7％的AS斑块面积。在本实验中TFA和维生素E对甘油三脂代谢无影响。结论 TFA对高脂饲料诱导家兔AS形成有一定的防护作用。

Owls have the potential to be keystone species for conservation in fragmented landscapes, as the absence of these predators could profoundly change community structure. Yet few studies have examined how whole communities of owls respond to fragmentation, especially in the tropics. When evaluating the effect of factors related to fragmentation, such as fragment area and distance to the edge, on these birds, it is also important in heterogeneous landscapes to ask how ＇location factors＇ such as the topography, vegetation and soil of the fragment predict their persistence. in Xishuangbanna, southwest China, we established 43 transects （200 mx60 m） within 20 forest fragments to sample nocturnal birds, both visually and aurally. We used a multimodel inference approach to identify the factors that influence owl species richness, and generalized linear mixed models to predict the occurrence probabilities of each species. We found that fragmentation factors dominated location factors, with larger fragments having more species, and four of eight species were significantly more likely to occur in large fragments. Given the potential importance of these birds on regulating small mammal and other animal populations, and thus indirectly affecting seed dispersal, we suggest further protection of large f ragments and programs to increase their connectivity to the remaining smaller fragments.
