Ventriculo-atrial shunt with occlusion of the internal jugular vein: operative experience and surgical technique

Background Ventricular shunts are one of the most frequent techniques used for the management of hydrocephalus. The ventriculoperitoneal shunt (VPS) is the most commonly performed procedure, and the ventriculoatrial shunt (VAS) is the second option in most medical centers. The main objective of this study is to introduce and describe a surgical approach for VAS outlining our experience and comparing it with traditional shunting *** in this retrospective cohort comparison study, we included patients with hydrocephalus treated with a surgical procedure between January 2010 and February 2021 at a single academic institution. We categorized the procedures into two groups: patients with VPS and conventional VAS grouped together into the conventional technique (CT) group, and the second group was patients with whom we performed VAS with complete internal jugular vein occlusion (iJVOT). We compared the surgical time, postoperative complications, and occurrence of shunt failure among the groups by performing univariate analysis using the Fisher exact *** Out of the 106 patients included in the analysis, iJVOT was performed in 66 patients, and CT in 40 patients. The median surgical time was 60 min (iQR 60-90) for iJVOT versus 100 min (iQR 60-120) for CT (p in the follow-up a month after the procedure, 83.3% of patients with iJVOT and 62.5% of patients with CT did not require shunt removal or shunt revision (p ision rates were 12.5% and 1.5% for CT while 1.5% and 2.5% for iJVOT at 1 and 6 months after the *** Our findings demonstrate that VAS with iJVOT is a safe method that exhibited shorter surgical times and outcomes comparable to CT. However, since the present study represents the first cohort evaluating iJVOT, it is imperative to conduct larger prospective studies, along with clinical trials, to fully explore and establish efficacy, long-term outcomes, and an in-depth comparison among shunting techniques

The present study aimed to evaluate multi-element concentrations(Al,As,Ca,Cd,Co,Cr,Cu,Fe,Mg,Mn,Mo,Ni,Pb,Sr)in suspended particulate material(SPM)collected from Ensenada de La Paz(ELP)lagoon,Baja California Sur,Mexico in two different periods(September and May)to comprehend their origin,geochemical behavior,mass transfer and associated ecotoxicological *** 24 hr variation coefficient of volumetric SPM levels were found to be 51.7%in September and 40.5%in May,signifying the effects of oceanic *** calculated enrichment factor(EF)values for all the studied elements were of higher magnitude because of the high surface area and oxide nature of SPM,and in this study,Mo had the highest EF of 46.77 probably due to its origin from continental *** the ecotoxicological perspective,the integrated toxic risk index revealed low toxic risk to the benthic ***,the mean-ERM-Quotient calculated using the particulate concentrations of As,Cd,Cr,Cu,Ni,Pb indicated 9%probability of toxicity to *** comprehensive geochemical and ecotoxicological assessment of particulate metal concentrations in the ELP lagoon signify low to moderate contamination.

Phage lytic proteins: a natural approach to agro-food safety

Sustainability is a leading trend in the context of food production. Additionally， consumers increasingly demand safer and less-processed products. Among the different technologies used to maintain the quality and extend the shelf-life of fresh and minimally-processed food， natural antimicrobial agents offer a promising strategy to replace conventional compounds. in this regard， phage lytic proteins or lysins， such as endolysins and virion-associated peptidoglycan hydrolases (VAPGHs)， have been proposed as a viable option for the avoidance and elimination of undesirable bacteria within the food production chain. Even when applied exogenously， these proteins can degrade the bacterial cell wall maintaining their lytic activity. This feature， alongside their modular structure， which can be exploited for bioengineering， provides significant biotechnological potential. However， despite the promising properties of lysins， the main obstacle for their commercialization is the limited legal information regulating their use. This challenge underscores the need to navigate complex regulatory pathways. The primary objective of this review is to address this crucial gap and summarize the many prospective applications of endolysins during the different stages of food production. By doing so， we aim to provide clarity and insight into the regulatory challenges that must be overcome for the successful utilization of lysins.

Correlative species distribution models(SDMs)are important tools to estimate species’geographic distribution across space and time,but their reliability heavily relies on the availability and quality of occurrence *** can be biased when occurrences do not fully represent the environmental requirement of a *** tested to what extent species’physiological knowledge might influence SDM *** on the Japanese sea cucumber Apostichopus japonicus within the coastal ocean of East Asia,we compiled a comprehensive dataset of occurrence *** then explored the importance of incorporating physiological knowledge into SDMs by calibrating two types of correlative SDMs:a naïve model that solely depends on environmental correlates,and a physiologically informed model that further incorporates physiological information as *** further tested the models’sensitivity to calibration area choices by fitting them with different buffered areas around known *** with naïve models,the physiologically informed models successfully captured the negative influence of high temperature on *** and were less sensitive to the choice of calibration *** naïve models resulted in more optimistic prediction of the changes of potential distributions under climate change(i.e.,larger range expansion and less contraction)than the physiologically informed *** findings highlight benefits from incorporating physiological information into correlative SDMs,namely mitigating the uncertainties associated with the choice of calibration *** these promising features,we encourage future SDM studies to consider species physi-ological information where available.

in order to find optimum design parameters in earthquake engineering, an objective function is optimized. This function comprises the initial cost of a structure and the cost due to the damage of earthquakes. intangible losses may be included in the latter, such as how much society is willing to invest to preserve a human life. in this paper, the expression of the objective function is developed in terms of the seismic design coefficient, and the aforementioned intangible loss is calculated from both the individual point of view and that of society. The calculation of the intangible is based on utility curves. Finally, optimum seismic design coefficients are calculated for a firm ground site.
