Evolution of People Living with HiV and AiDS in Medical Consultation at 17 ESOPE Sites in Mali

Despite the efforts made by the Malian government with technical and financial partners in the fight against HiV and AiDS, there are no data on the evolution of people with HiV in medical consultation in antiretroviral treatment sites. The objective of this work is to fill this gap by generating factual data describing the evolution of the number of people with HiV in medical consultation in Mali. To achieve this goal, we conducted a cross-sectional study using data from the Coordination Unit of the Sectoral Committee for AiDS Control of the Ministry of Health and Public Hygiene. Sampling was comprehensive including all people living with HiV. The data collected concerned those of the years 1999 to 2014 by the ESOPE software and analyzed with Stata version 12. A Total of 39,619 people were enrolled in the study. And 26,212 people living with HiV in the 17 ESOPE sites of Mali were female. Patients aged 25-34 and 35-44 years were strongly represented in consultation in the years 2007 and 2011, and those aged 15-24 were the least frequent. HiV1 was the most dominant with 97%. The majority of patients presented for initial consultation were clinical stages 2 and 3 in all years. Cases of lost sites seen in the more common in hospitals are 62.79% at the Hospital of Mopti and 57% at the University Hospital of Point G. The number of cases of people living with HiV in medical consultation has progressively augmented from 1999-2007. Then it dropped from 2008-2010. After this number has increased by 2011, it dropped again in 2012-2014. This study made it possible to note the evolution of the patients in medical consultation in ESOPE antiretroviral treatment sites according to the years and events (safe, subsidized by activities by the global fund). The activities of sensitization and communications, case research should be strengthened by involving the community.

Adsorption of Arsenic by Laterite, Sandstone and Shale in a Fixed-Bed Column

The removal of arsenic from water is essential for the protection of public health. To investigate the adsorption capabilities of laterite, sandstone, and shale for the removal of arsenic from aqueous solutions, column experiments were conducted. in this study, raw materials and heat-treated (calcined) materials were examined. The experiments assessed the influence of various parameters, including initial concentration, bed depth, and the effects of heat treatment. The findings revealed that the breakthrough curves were influenced by the initial concentration of arsenic, the depth of the bed, and the type of material used. For an initial arsenic concentration of 5 mg/L, columns containing 85 cm of calcined laterite, sandstone, and shale produced volumes of 7460 ml (1492 min), 3510 ml (702 min), and 4400 ml (880 min) of water with arsenic levels below 0.01 mg/L, respectively. These calcined materials demonstrate significant potential for the effective removal of arsenic from water.

Mixed integer Program for e-Wallet Security Based on Suspicious Transaction Detection

With the rise of digital transactions, e-Wallets have become prime targets for fraudulent activity. Early detection of suspicious transactions is therefore crucial to protect users and maintain trust in these systems. This article proposes a mathematical model based on a Mixed integer Program (MiP) to identify and block suspicious transactions. This mathematical approach is designed to analyze e-Wallet transactions in real time by combining integer and continuous decision variables, offering greater flexibility in modeling fraud detection constraints. it considers parameters such as transaction cost, geographical location, type of device used for the transaction, iP address and other potential fraud indicators. Assigning suspicion scores to each transaction enables the model to identify risks that become habitual behavior and mark them as suspicious. Tests carried out on 10,000 digital transactions show that using PMNE significantly improves the detection of fraudulent transactions by identifying the most critical anomalies in terms of accuracy, adaptability and operational efficiency. The model also offers greater accuracy, reducing the number of false positives and false negatives, enabling faster intervention to block truly suspicious transactions.

Design and Optimization of Terracotta Tube-Based Direct Evaporative Cooling Exchanger: An Analytical Approach to Heat and Mass Transfer

This study develops an analytical model to evaluate the cooling performance of a porous terracotta tubular direct evaporative heat and mass exchanger. By combining energy and mass balance equations with heat and mass transfer coefficients and air psychrometric correlations, the model provides insights into the impact of design and operational parameters on the exchanger cooling performance. Validated against an established numerical model, it accurately simulates cooling behavior with a Root Mean Square Deviation of 0.43 - 1.18˚C under varying inlet air conditions. The results show that tube geometry, including equivalent diameter, flatness ratio, and length significantly influences cooling outcomes. Smaller diameters enhance wet-bulb effectiveness but reduce cooling capacity, while increased flatness and length improve both. For example, extending the flatness ratio of a 15 mm diameter, 0.6 m long tube from 1 (circular) to 4 raises the exchange surface area from 0.028 to 0.037 m2, increasing wet-bulb effectiveness from 60% to 71%. Recommended diameters range from 5 mm for tubes under 0.5 m to 1 cm for tubes 0.5 to 1 m in length. Optimal air velocities depend on tube length: 1 m/s for tubes under 0.8 m, 1.5 m/s for lengths of 0.8 to 1.2 m, and up to 2 m/s for longer tubes. This model offers a practical alternative to complex numerical and CFD methods, with potential applications in cooling tower optimization for thermal and nuclear power plants and geothermal heat exchangers.

Resource Optimization in Elastic Optical Networks Using Threshold-Based Routing and Fragmentation-Aware Spectrum Allocation

This paper proposes an efficient strategy for resource utilization in Elastic Optical Networks (EONs) to minimize spectrum fragmentation and reduce connection blocking probability during Routing and Spectrum Allocation (RSA). The proposed method, Dynamic Threshold-Based Routing and Spectrum Allocation with Fragmentation Awareness (DT-RSAF), integrates rerouting and spectrum defragmentation as needed. By leveraging Yen’s shortest path algorithm, DT-RSAF enhances resource utilization while ensuring improved service continuity. A dynamic threshold mechanism enables the algorithm to adapt to varying network conditions, while its fragmentation awareness effectively mitigates spectrum fragmentation. Simulation results on NSFNET and COST 239 topologies demonstrate that DT-RSAF significantly outperforms methods such as K-Shortest Path Routing and Spectrum Allocation (KSP-RSA), Load Balanced and Fragmentation-Aware (LBFA), and the invasive Weed Optimization-based RSA (iWO-RSA). Under heavy traffic, DT-RSAF reduces the blocking probability by up to 15% and achieves lower Bandwidth Fragmentation Ratios (BFR), ranging from 74% to 75%, compared to 77% - 80% for KSP-RSA, 75% - 77% for LBFA, and approximately 76% for iWO-RSA. DT-RSAF also demonstrated reasonable computation times compared to KSP-RSA, LBFA, and iWO-RSA. On a small-sized network, its computation time was 8710 times faster than that of integer Linear Programming (iLP) on the same network topology. Additionally, it achieved a similar execution time to LBFA and outperformed iWO-RSA in terms of efficiency. These results highlight DT-RSAF’s capability to maintain large contiguous frequency blocks, making it highly effective for accommodating high-bandwidth requests in EONs while maintaining reasonable execution times.

A Genetic Algorithm Approach for Location-Specific Calibration of Rainfed Maize Cropping in the Context of Smallholder Farming in West Africa

Smallholder farming in West Africa faces various challenges, such as limited access to seeds, fertilizers, modern mechanization, and agricultural climate services. Crop productivity obtained under these conditions varies significantly from one farmer to another, making it challenging to accurately estimate crop production through crop models. This limitation has implications for the reliability of using crop models as agricultural decision-making support tools. To support decision making in agriculture, an approach combining a genetic algorithm (GA) with the crop model AquaCrop is proposed for a location-specific calibration of maize cropping. in this approach, AquaCrop is used to simulate maize crop yield while the GA is used to derive optimal parameters set at grid cell resolution from various combinations of cultivar parameters and crop management in the process of crop and management options calibration. Statistics on pairwise simulated and observed yields indicate that the coefficient of determination varies from 0.20 to 0.65, with a yield deviation ranging from 8% to 36% across Burkina Faso (BF). An analysis of the optimal parameter sets shows that regardless of the climatic zone, a base temperature of 10˚C and an upper temperature of 32˚C is observed in at least 50% of grid cells. The growing season length and the harvest index vary significantly across BF, with the highest values found in the Soudanian zone and the lowest values in the Sahelian zone. Regarding management strategies, the fertility mean rate is approximately 35%, 39%, and 49% for the Sahelian, Soudano-sahelian, and Soudanian zones, respectively. The mean weed cover is around 36%, with the Sahelian and Soudano-sahelian zones showing the highest variability. The proposed approach can be an alternative to the conventional one-size-fits-all approach commonly used for regional crop modeling. A Case Report of Congenital Dermal Sinus Tract with Dermoid Cyst and Lipoma

Congenital dermal sinus tract (CDST) is a rare entity of spinal dysraphism with an incidence of 1 in 2500 live births. The presumed etiology relates to a focal failure of disjunction resulting in a persistent adhesion between the neural and cutaneous ectoderm. CDST is commonly associated with other pathologies such as myelomeningocele, split cord malformation, tethered cord and inclusion tumors. The authors report a case of a 2-year-old girl discharging lumbosacral CDST. The preoperative MRi showed the dermal sinus tract with 2 intradural lesions from L1 to L3. The tract was surgically excised followed by complete excision of the cyst and lipoma. The dermal sinus tract was attached to the medullary cone and was cut there, allowing “en bloc” removal of the dermal sinus tract. There were two distinct types of tumor. One cystic is located posterior to the medullary cone and the other is located laterally to the medullary cone, whose appearance suggests a lipoma. Both tumors were removed completely. The dura was closed in a tight manner. The pathological examination confirmed the lipoma and Dermoid Cyst. in the postoperative course, the child had a motor deficit in the lower limbs, which recovered completely after 3 months of physiotherapy.

Soil Fertility improvement by Using Crotalaria Species Combined to Organic Amendment

in Burkina Faso, agricultural land degradation, including soil fertility decline, has prompted the implementation of agricultural innovations to address this challenge. Therefore, this study aimed to evaluate the contribution of four Papilionaceous ecotypes in enhancing the fertility of degraded soils. Experiments were conducted in Noumousso and Arbollé from July to December 2024, using a randomized split-plot design combining ecotypes and organic matter and/or Burkina Phosphate amendments. Amendments were applied in four treatments: T0 (control), T1 (Organic Matter), T2 (OM + Burkina Phosphate), and T3 (BP). Determined physico-chemical properties were: particle size distribution, pH, carbon (C), nitrogen (Nt), total Phosphorus (Pt), available P (P_Bray), potassium (Kt), exchangeable bases (EB), cation exchange capacity (CEC), and saturation rate (SR). The evaluated agronomic parameter was total biomass. Results revealed that the soils at both sites were characterized by acidic sandy-clay texture and low levels of C, N, P, K, EB, and CEC. Cultivating papilionaceous and applying organo-mineral amendments contributed to increased levels of C, available P, and Mg2+, as well as improved SR. Acidity decreased at Arbollé, while showing a slight increase at Noumousso. interaction between the two factors helped reduce acidity at both sites and significantly increased C, N, and available P contents. The organo-mineral amendment also enhanced biomass production across all ecotypes. At Noumousso, compared to T0, OM, OM + BP, and BP treatments resulted in biomass increases of 64.8%, 73.1%, and 38.6%, respectively. At Arbollé, the corresponding increases were 33.5%, 47%, and 23.4%. On both sites, OM + BP amendment had the greatest impact on improving biomass production, followed by organic amendment. The Outcomes of Pregnancy in Patients with Cirrhosis in a Low-income Country

Pregnancy in cirrhotic-woman is a rare event [1] [2]. it causes particular risks related to liver disease and its treatments, with maternal and fetal consequences. The largest reported series to date is by Welton’s with 13 cases [3]. in Africa, few cases were reported. The purpose of this work was to report the hepatic and obstetric complications in pregnant women in Burkina Faso, a low-income country. it was a descriptive cross-sectional study with retrospective data collection covering the period from January 1, 2013 to December 31, 2023. All the women diagnosed with cirrhosis through clinical basis, biological and imaging arguments associated with a pregnancy confirmed by ultrasound were included. A total of ten women were included in the study. Among them, two were known cirrhotics and were being followed. in the other eight cases, cirrhosis was discovered during pregnancy. Eight women were HBsAg positive and two of them had only the contact marker of hepatitis B virus (anti- HBc antibody). The evolution was, ascitic decompensation in nine cases, digestive hemorrhage due to rupture of esophageal varices in two cases, nine cases of anemia, two cases of ascitic fluid infection and one case of conjunctival jaundice. Two cases of probable hepatocellular carcinoma were noted with one case of extensive portal thrombosis. The evolution was without maternal complications in one case. Regarding fetal and neonatal complications, there were three cases of hypotrophy, one case of prematurity, one case of acute neonatal distress, two cases of fetal death in utero and one case of fetal tachycardia. Four pregnancies had a favorable evolution without any complications. The association of cirrhosis and pregnancy is an uncommon event, and maternal and fetal complications can occur, making this pregnancy a high-risk pregnancy.

Numerical analysis and geophysical monitoring for stability assessment of the Northwest tailings dam at Westwood Mine

The Westwood Mine aims to reuse the tailings storage facility #1(TSF #1) for solid waste storage, but,downstream of the Northwest dike is considered critical in terms of stability. This paper uses numerical modeling along with geophysical monitoring for assessing the Northwest dike stability during the restoration phase. The impact of waste rock deposition in the upstream TSF #1 is considered. The geophysical monitoring is based on electrical resistivity methods and was used to investigate the internal structure of the dike embankment in different deposition stages. The numerical simulations were performed with SLOPE/W code. The results show a factor of safety well above the minimum recommended value of 1.5. Geophysical monitoring revealed a vertical variation in the electrical resistivity across the dike, which indicates a multilayer structure of the embankment. Without any current in situ data, the geophysical monitoring helped estimating the nature of the materials used and the internal structure of the embankment. These interpretations were validated by geological observation of geotechnical log of the embankment. Based on this study, it is recommended that the water polishing pond be partly filled before waste rock is deposited in TSF #1. in addition, to ensure the stability of the dike, the piezometric head monitoring prior to and during waste rock deposition is recommended.
