AiM: To report maternal and fetal adverse outcomes, in spite of appropriate treatment and regular follow up, in diabetic pregnant women with proliferative diabetic retinopathy. METHODS: case series of four young pregnant diabetics aged between 20 and 25 years with type 1 diabetes mellitus and proliferative diabetic retrinopathy. RESULTS: The maternal adverse outcomes were abortion in one patient, pre-eclampsia and preterm delivery in one patient, and renal failure requiring dialysis in one patient. The fetal adverse outcomes were neonatal death in one case and premature baby in another case. cONcLUSiON: These cases highlight the fact that diabetic pregnant women should be closely followed up by the obstetricians and physicians when they have proliferative retinopathy. The proliferative diabetic retinopathy should be considered as a part of the assessment when counseling a diabetic woman in antenatal check up and also in the follow up visits during pregnancy.

The goal of this review is to outline some of the important surgical issues surrounding the management of patients with early (T1/T2 and NO), as well as locally advanced (T3/T4 and/or N1) rectal cancer. Surgery for rectal cancer continues to develop towards the ultimate goals of improved local control and overall survival, maintaining quality of life, and preserving sphincter, genitourinary, and sexual function. information concerning the depth of tumor penetration through the rectal wall, lymph node involvement, and presence of distant metastatic disease is of crucial importance when planning a curative rectal cancer resection. Preoperative staging is used to determine the indication for neoadjuvant therapy as well as the indication for local excision versus radical cancer resection. Local excision is likely to be curative in most patients with a primary tumor which is limited to the submucosa (T1N0M0), without high-risk features and in the absence of metastatic disease. in appropriate patients, minimally invasive procedures, such as local excision, TEM, and laparoscopic resection allow for improved patient comfort, shorter hospital stays, and earlier return to preoperative activity level. Once the tumor invades the muscularis propria (T2), radical rectal resection in acceptable operative candidates is recommended. in patients with transmural and/or node positive disease (T3/T4 and/or N1) with no distant metastases, preoperative chemoradiation followed by radical resection according to the principles of TME has become widely accepted. During the planning and conduct of a radical operation for a locally advanced rectal cancer, a number of surgical management issues are considered, including: (1) total mesorectal excision (TME); (2) autonomic nerve preservation (ANP); (3) circumferential resection margin (cRM); (4) distal resection margin; (5) sphincter preservation and options for restoration of bowel continuity; (6) laparoscopic approaches; and (7) postoperative qu

Extensive review of evaporative coolers for the preservation of fresh agricultural produce in some countries was *** of the designs employ mainly direct evaporative cooling methods which are simple but with lower thermal performance,and can only be used for short term storage of agricultural produce with moderate respiratory *** into novel technologies in evaporative cooling systems which can improve the cooling performances,such as membrane air treatments,dew point type and heat pipe type heat exchanger in indirect/direct evaporative cooling application,and their feasibilities in agricultural storage are either absent or *** kinds of materials especially agricultural residues have been used for air water contact in evaporative cooling in different climates,but most of the analyses focused on effect of air flow rate and pad thickness on the cooling effectiveness,and the energy efficiency and evaporation loss of these materials in most cases were not evaluated or *** paper highlights the prospects and constraints of commercialization and marketing of evaporative coolers in some developing countries,and the general weakness in researches and the ways forward in new area of research and development are concluded.

Bioactive glasses(BGs)are promising bone substitute ***,under certain circumstances BGs such as the well-known 45S5 Bioglass®(composition in wt%:45.0 SiO2,24.5 Na2O,24.5 caO,6.0 P2O5)act cytotoxic due to a strong increase in pH caused by a burst release of sodium ions.A potential alternative is a sodiumreduced fluoride-containing BG belonging to the caO–MgO–SiO2 system,namely BG1d-BG(composition in wt%:46.1 SiO2,28.7 caO,8.8 MgO,6.2 P2O5,5.7 caF2,4.5 Na2O),that has already been evaluated in-vitro,in-vivo and in preliminary clinical *** further application,however,BG1d-BG should be compared to the benchmark amongst BGs,the 45S5 Bioglass®composition,to classify its effect on cell viability,proliferation and osteogenic differentiation of human mesenchymal stem cells(MScs).Therefore,in this study,the biocompatibility and osteogenic potential of both BGs were investigated in an indirect and direct culture setting to assess the effect of the ionic dissolution products and the BGs’physical presence on the *** results indicated an advantage of BG1d-BG over 45S5 Bioglass®regarding cell viability and *** BGs induced an earlier onset of osteogenic differentiation and accelerated the expression of late osteoblast marker genes compared to the control *** conclusion,BG1d-BG is an attractive candidate for further experimental *** basic mechanisms behind the different impact on cell behavior should be assessed in further detail,*** further alteration of the BG compositions.

This study is concerned with the three-dimensional(3D)stagnation-point for the mixed convection flow past a vertical surface considering the first-order and secondorder velocity *** the authors’knowledge,this is the first study presenting this very interesting *** partial differential equations for the flow problem are transformed into nonlinear ordinary differential equations(Odes)by using appropriate similarity *** Odes with the corresponding boundary conditions are numerically solved by utilizing the bvp4c solver in MATLAB programming *** effects of the governing parameters on the non-dimensional velocity profiles,temperature profiles,skin friction coefficients,and the local Nusselt number are presented in detail through a series of graphs and ***,it is reported that the reduced skin friction coefficient decreases for the assisting flow situation and increases for the opposing flow *** numerical computations of the present work are compared with those from other research available in specific situations,and an excellent consensus is *** exciting feature for this work is the existence of dual *** important remark is that the dual solutions exist for both assisting and opposing flows.A linear stability analysis is performed showing that one solution is stable and the other solution is not *** notice that the mixed convection and velocity slip parameters have strong effects on the flow *** effects are depicted in graphs and discussed in this *** obtained results show that the first-order and second-order slip parameters have a considerable effect on the flow,as well as on the heat transfer characteristics.

BAcKGROUND Autologous fat injection in facial reconstruction is a common cosmetic *** cerebral fat embolism(cFE)as a complication is rare,it carries serious health *** SUMMARY We present a case of a 29-year-old female patient who developed acute cFE following facial fat filling *** the surgery,the patient experienced symptoms including headache,nausea,vomiting,and difficulty breathing,which was followed by neurological symptoms such as slurred speech and left-sided *** physical examination and auxiliary investigations,including blood tests,head and neck computed tomography angiography,and cranial magnetic resonance diffusion-weighted imaging,were performed upon *** clinical diagnosis was acute cerebral embolism following facial fat filling *** included measures to improve cerebral circulation,dehydration for intracranial pressure reduction,nutritional support,and rehabilitation therapy for left limb *** patient showed a significant improvement in symptoms after 2 weeks of *** recovered left limb muscle strength to grade 5,had clear speech,and experienced complete relief of *** Our case highlights the potential occurrence of severe complications in patients undergoing fat injection in facial *** prevent these complications,plastic surgeons should enhance their professional knowledge and skills.

Monthly and seasonal streamflow forecasting of large dryland catchments in Brazil

Streamflow forecasting in drylands is *** are scarce,catchments are highly humanmodified and streamflow exhibits strong nonlinear responses to *** goal of this study was to evaluate the monthly and seasonal streamflow forecasting in two large catchments in the Jaguaribe River Basin in the Brazilian semi-arid *** adopted four different lead times:one month ahead for monthly scale and two,three and four months ahead for seasonal *** gaps of the historic streamflow series were filled up by using rainfall-runoff ***,time series model techniques were applied,i.e.,the locally constant,the locally averaged,the k-nearest-neighbours algorithm(k-NN)and the autoregressive(AR)*** criterion of reliability of the validation results is that the forecast is more skillful than streamflow *** approach outperformed the streamflow climatology for all monthly *** average,the former was 25%better than the *** seasonal streamflow forecasting(SSF)was also reliable(on average,20%better than the climatology),failing slightly only for the high flow season of one catchment(6%worse than the climatology).considering an uncertainty envelope(probabilistic forecasting),which was considerably narrower than the data standard deviation,the streamflow forecasting performance increased by about 50%at both *** forecast errors were mainly driven by the streamflow intra-seasonality at monthly scale,while they were by the forecast lead time at seasonal *** best-fit and worst-fit time series model were the k-NN approach and the AR model,*** rainfall-runoff modelling outputs played an important role in improving streamflow forecasting for one streamgauge that showed 35%of data *** developed data-driven approach is mathematical and computationally very simple,demands few resources to accomplish its operational implementation and is applicable to other dryland *** findings may be part of drou

The levered-dual response(LEDAR)coulomb-damped system attains near resonant vibration isolation by differential preloads/offsets in linear *** takes the advantages of both the preloads/offsets in linear springs and the guiderail friction for realizing different levels of vibration *** isolation capacities are investigated on the strategies with both the horizontal and vertical guiderails,with the horizontal rail only,and without *** compressive preloads generally result in the consumption of most of the initial excitation energy so as to overcome the potential *** isolation onsets at the frequency ratio of 1∓0.095 on the left-hand side(LHS)and the right-hand side(RHS)of the lever are relative to the load plate *** observed near resonant isolation thus makes the LEDAR system a candidate for the isolation of the mechanical systems about resonance while opening a path for simultaneous harvesterisolation functions and passive functions at extreme frequencies.
