AiM： To evaluate the diagnostic accuracy and clinical utility of a new ELiSA （URiNELiSA） test for detecting Helicobacter pylori （H pylorl） antibody in the urine of Taiwan Residents population. METHODS： in this prospective study, 317 consecutive dyspeptic patients （171 men, 146 women; mean age, 51.0 years） were included. They underwent gastroendoscopy for evaluation. invasive tests, including culture, histology, and rapid urease test （RUT）, and non-invasive ^13C-urea breath test were preformed. At the same time, urine specimens were collected for URiNELiSA. The status of H pylori infection was considered as positive when either culture was positive, or when two of the other, RUT, histology or 13C-UBT, were positive. RESULTS： The sensitivity, specificity, positive predictive value, and negative predictive value of URiNELiSA are 91.7% （211/230）, 90.8% （79/87）, 96.3% （211/219）, and 80.6% （79/98） respectively. CONCLUSiON： This URiNELiSA test is reliable, inexpensive and easy-to-use. The high diagnostic accuracy warrants the use of URiNELiSA as a first-line screening tool for diagnosis of Hpyloriinfection in untreated patients.

AiM:To investigate the etiology of esophageal cancer among Taiwan Residents ***:This is a multi-center,hospital-based,case-control *** patients consisted of women who were newly diagnosed and pathology-proven to have esophageal squamous cell carcinoma(ESCC) from three large medical centers(one from Northern and two from Southern Taiwan,respectively)between August 2000 and December *** ESCC patient was matched with 4 healthy women based on age(within 3 years)and hospital of origin,from the Department of Preventive Medicine in each hospital.A total of 51 case patients and 204 controls,all women,were ***:Frequencies of smokers and drinkers among ESCC patients were 19.6%and 21.6%,respectively,which were significantly higher than smokers(4.4%) and drinkers(4.4%)among controls(OR=4.07,95%Ci:1.36-12.16,P=0.01;OR=3.55,95%Ci:1.03-12.27,P=0.04).Women who drank an amount of alcohol more than 158 g per week had a 20.58-fold greater risk(95%Ci:1.72-245.62,P=0.02)of ESCC than those who never drank alcohol after adjusting for other covariates,although the sample size was ***:Cigarette smoking and alcohol drinking,especially heavy drinking,are the major risks for developing ESCC in Taiwan Residents women.

AiM Capsaicin, a pungent ingredient found in red pepper, has long been used in spices, food additives, and drugs. Cell death induced by the binding of capsaidn was examined in a human gastric adenocarcinoma cell line （AGS cells）. METHODS： By using XTT-based cytotoxicity assay, flow cytometry using the TUNEL method, and quantitation of DNA fragmentation, both cell death and DNA fragmentaUon were detected in AGS cells treated with capsaicin. By using Western blotting methods, capsaicin reduced the expression of Bcl-2, the antiapoptotic protein, in AGS cells in a concentration-dependent manner. RESULTS- After incubation of AGS cells with capsaicin for 24 h, cell viability decreased significantly in a dose-dependent manner. After incubation of AGS cells with capsaicin for 24 h, apoptotic bodies also significantly increased, and were again correlated with the dose of capsaicin. When the concentration of capsaicin was 1 mmol/L, the amount of DNA fragments also increased. Similar results were also in the lower traces. CONCLUSiON： These results suggest that capsaicin- induced cell death might be via a Bcl-2 sensitive apoptotic pathway. Therefore, capsaicin might induce protection from gastric cancer.

AiM： To explore the role of Helicobacter pylori （Hpylori） infection on the risk of digestive tract cancers. METHODS： in total, 199 oral squamous-cell carcinoma （SCC）, 317 esophagea! SCC, 196 gastric cardia and non-cardia adenocarcinoma and 240 colon adenocarcinoma patients were recruited for serum tests of Hpylori infection. Two hospital- and one community-based control groups were used for the comparisons. Hpylori seropositivity was determined by an enzyme linked immunosorbent assay method against Hpylori igG. RESULTS： Presence of H pylori infection was significantly inversely associated with esophageal SCC [adjusted odds ratio （AOR）： 0.315-0.472, all P-value 〈 0.05] but positively associated with gastric adenocarcinoma （both cardia and non-cardia）（AOR： 1.636-3.060, all P-value 〈 0.05） in comparison to the three control groups. Similar results were not found in cancers of the oral cavity and colon. CONCLUSiON： Our findings support the finding that H pylori seropositivity is inversely associated with esophageal SCC risk, but increases the risk of gastric cardia adenocarcinoma.

AiM： Prior Helicobacterpylori （Hpylori infection has often been underestimated. These underestimations have misled physicians attempting to determine the significance between Hpyloriand certain gastrointestinal lesions such as intestinal metaplasia, atrophic gastritis, and gastric cancer. Our study endeavored to debit past Hppylorinfections accurately, easily, and rapidly with the newly developed irnmunoblot kit, Helico Blot 2.1. METHODS： Thirty-three patients, including 25 H pylori infected and 8 uninfected cases, were enrolled in our study. All patients received consecutive gastroendoscopic examinations and ^3C-urea breath test （UBT） tests at 6- or 12-mo intervals for up to 4 years. Serum samples were obtained from each patient at the same time. intragastric H pylori infection was confirmed in accordance with the gold standard. Twenty-five H pylori-infected patients received triple therapies after initial bacterial confirmation, and were successful in eradicating their infections. Serially obtained sera were tested by means of Helico Blot 2.1. RESULTS： Current infection marker detected by Helico Blot 2.1 was unreliable for representing ongoing Hpylori infection. Only 35 and 37 ku antibodies of H pylorihad significant seroconversion rates 1 year after having been cured. The seroposiUve rates of 116 ku （cytotoxin-associated antigen [CagA]） and Helico Blot 2.1 were nearly 100% during 4-year follow-up period. Both CagA antigen and Helico blot 2.1 could serve as indicators of long-term H pylori infection. CONCLUSiON： Helico Blot 2.1 can detect past H pylori infections for up to 4 years, and is the best method to date for detecting previous long-term H pylori infection. 2005 The WJG Press and Elsevier inc. AiM： To evaluate the optimal intervention point of a selective cyclooxygenase-2 （COX-2） inhibitor, Celecoxib, for inhibiting Helicobacter pylori （H pylori-associated gastric carcinogenesis in Mongolian gerbils （MGs）.METHODS： One hundred and twelve MGs were divided into six groups （A-F）. One hundred gerbils were inoculated with H pylor/（groups A-E）. Twelve gerbils were inoculated with vehicle broth only （group F）. After 4 wk, they were given N＇-methyl-N＇ -nitro-N-nitroso-guanidine （MNNG）（50 μg/mL） in the drinking water for 20 wk. in groups B-E, the animals were given the stock Celecoxib （10 mg/kg per day） diet from the 21st, 31st, 21st and 41st week respectively. The periods of administering Celecoxib were 30, 20, 20, and 15 wk respectively. On the 51st week, the animals were sacrificed for histological examination. Local PCNA expression was examined by the immunohistochemistry method. The expression of COX-2 protein was assessed by Western Blot. Analysis used the 2 test. The difference was regarded as significant when P value was less than 0.05. RESULTS： Seventeen percent （17/100） of H pyloriinfected MGs developed gastric cancer. All of these lesions were well-differentiated adenocarcinoma. The incidence rates of adenocarcinoma in groups A-F were 40%, 0%, 0%, 20%, 25%, and 0% respectively. The inflammatory scores were higher in group B than in other groups. There was no inflammatory response noted in group F. Celecoxib treatment resulted in a significant reduction in the proliferation of H pyloriinfected mucosal cells （groups B, C and D）（P 〈 0.01）. The expression of COX-2 protein was significantly attenuated in the groups which were Celecoxib-treated for more than 20 wk （groups B, C, D）. The groups treated with Celecoxib had a significantly lower rate of advanced gastric cancer （34% vs 75%, P 〈 0.001） There were no sudden deaths in any of the groups. Porphyry copper deposits（PCDs） supply the majority of the world's copper（Cu） as well as significant amounts of molybdenum（Mo） and gold（Au）. The formation of these deposits is controlled by a variety of geological factors, among which the magmatic oxidation is one of the focuses of current research. This study investigates the redox characteristics of arc magmas by analyzing the ratio of vanadium（V） to scandium（Sc） and its correlation with the oxygen fugacity （fO2） of magmas from arcs of various crustal thicknesses globally. We compiled an extensive dataset of published V/Sc values and studied the partition coefficients of Vand Sc between minerals and silicate melts across various magmatic environments. The results demonstrate that fractional crystallization of magnetite markedly reduces V/Sc values of the residual melt. Furthermore, magnesium number（Mg#） at the onset of magnetite fractionation is strongly positively correlated with fO2of the equilibrium magma. Based on this,the oxidation of arc magmas from different crustal thicknesses can be evaluated. The result suggests that magmas generated in crusts thicker than 20 km tend to be more oxidized with increasing crust thickness. This trend is attributed to enhanced intracrustal differentiation processes involving garnet and amphibole, particularly in mature island and continental arcs. in contrast,magmas in thinner arc crusts（ich are significantly influenced by subducted slab materials, display higher oxidation states than most of their thicker crust counterparts. These findings suggest that both subduction dynamics and crustal thickness play a vital role in determining the oxidation state of arc magmas and, consequently, their capacity to generate PCDs. Highly oxidized, primitive arc magmas are less favorable for PCD formation due to that they tend to experience magnetite fractionation and thus sulfide saturation in the early stage of magma evolution, a process leading to copper depletion.
