Precise Three-Dimensional Deformation Retrieval in Large and Complex Deformation Areas via Integration of Offset-Based Unwrapping and Improved Multiple-Aperture SAR Interferometry:Application to the 2016 Kumamoto Earthquake

Change in renal function after sodium phosphate preparation for screening colonoscopy

AIM:To investigate the changes in renal function at 12-24 mo in patients following sodium phosphate(NaP) preparation for screening ***:We carried out a retrospective study on the results from patients who received health check-up services as part of an employer-provided wellness program performed between August 2006 and May 2008 and who were followed up for 12-24 *** to screening colonoscopy,224 patients underwent bowel cleansing with NaP(NaP group) and 113 patients with polyethylene glycol(PEG group).The control group comprised 672 age-matched *** compared the changes in the creatinine levels and the glomerular filtration rates(GFRs) from baseline to 12-24 mo between the NaP,PEG,and control groups using two-way repeated measured analysis of *** addition,multivariate linear regression analysis was performed to assess the risk factors for a decreased ***:The baseline mean serum creatinine level in the NaP,PEG,and control groups was 1.12 ± 0.15,1.12 ± 0.16,and 1.12 ± 0.15 mg/dL,which increased to 1.15 ± 0.15,1.15 ± 0.18,and 1.15 ± 0.15 mg/dL,respectively,after 12-24 *** baseline mean GFR in the NaP,PEG,and control groups was 69.0 ± 7.7,68.9 ± 8.0,and 69.6 ± 6.7 mL/min per 1.73 m2,which decreased to 66.5 ± 7.8,66.5 ± 8.3,and 67.4 ± 6.4 mL/min per 1.73 m2,respectively,after 12-24 *** changes in serum creatinine levels and GFRs were not significantly between the NaP,PEG,and control groups(P = 0.992 and P = 0.233,respectively).Using multivariate linear regression analysis,only the baseline GFR was associated with the change in GFR(P < 0.001).Indeed,the bowel preparations were not associated with the change in GFR(P = 0.297).CONCLUSION:NaP bowel preparation in subjects with normal renal function was not associated with renal injury,and NaP can thus be used safely for screening colonoscopy.

The influence of thermohaline fronts on chlorophyll a concentrations during spring and summer in the southeastern Yellow Sea

A spatial and temporal variation in physiochemical parameters in the southeastern Yellow Sea （YS） is in- vestigated in the spring and summer of 2009 to 2011. Nutrient show a strong negative relationship with chlorophyll a （Chl a） concentration in spring, and the subsurface chlorophyll a maxima （SCM） layer was associated with the nitracline in summer. In summer, the SCM was usually found within or above the pyc- nocline and at the depths of shoals from the open sea to the coastal sea due to tidal and/or topographical fronts in the southernmost study area. High Chl a concentrations were found in the central southern YS, where the YS cold water layer expanded under the pycnocline and encountered water masses during spring and summer. After a typhoon in the summer of2011, Chl a concentration increased, especially in the central southern YS, where cold waters occurred below the pycnocline. The results suggest that the development of thermohaline fronts may play an important role in the growth and accumulation of phytoplankton biomass in the upper layer of the southeastern YS during spring and summer.

背景强迫症是一种较为常见的慢性、致残的疾病，患者出现无法自控的强迫思维及强迫行为。10％以上的强迫症病人对药物和行为治疗无效。对一些常规治疗无效的难治性强迫症患者，神经外科已经开展一些手术治疗，例如：扣带前回毁损、脑白质切断术、尾状核下束切断、内囊前支毁损。扣带前回毁损相对来说比较安全同时没有明显的副作用，但是也有术后存在认知障碍的报道。为判断立体定向双侧扣带前回毁损是否是一种适合难治性强迫症的治疗，我们评价了立体定向双侧扣带前回毁损术的长期疗效及其副作用。方法患者筛选：在1999年3月至2003年4月间，17例难治性强迫症患者（男10例，女7例）接受了双侧扣带前回毁损术，并且接受了超过2年的随访。患者的选择标准如下：患者符合美国精神障碍诊断与统计手册第4版（DSM-Ⅳ）强迫症的诊断标准，病程超过3年，疾病导致社会心理功能降低。每位患者皆由两位精神科医师依照DSM-Ⅳ的AxisⅠ和AxisⅡ的疾病诊断标准行临床诊断。如伴有精神病性症状，就先用一线药物彻底治疗。手术方法：立体定向的靶点通过与MRI配套的Leksell适配器，在GESigma1．5 T的机器上得以确认。局部麻醉下，在两侧的冠状缝中线外20mm颅骨钻孔。将尖端直径．1．8mm，长度10mm的电极插至靶点，用85℃，90S进行射频毁损。在双侧扣带前回，每侧各4个射频毁损灶，2个针道。第一个射频毁损灶在两侧侧脑室前角后缘15mm，侧脑室顶部上2mm，旁开中线7mm处。射频电极接着回撤8mm，制作第二个射频毁损灶。第三个射频毁损处在两侧侧脑室前角后缘22mm，侧脑室顶部上2mm，旁开中线7mm处。再往上8mm，制作第四个射频毁损灶。结果是出现类似椭圆柱形的射频毁损灶，大约高18mm、前后径13mm，横径6mm。术前和术后随访评估：两位精神科医师用Yale-Brown强迫症量表（Y-BOCS）、临床总体印象量表（CGI）、汉密顿抑郁量表（HAMD）、汉密顿焦虑量表（HAMA），分别在手术前1个月、术后12个月、术后24个月对患者进行评估。评估的精神科医师接受了培训，Kappa值超过0．75，以确保评估的准确性。在我们研究中，患者的Y-BOCS得分改善35％或更高、CGI评分为1（极其显著改善）或者2（显著改善）；由专业的临床精神科医生对患者进行一套固定的神经心理测试，测试分别在术前1个月、术后12个月、24个月进行。这些测试包括：K—WAIS测验，视觉空间功能（RCFT），Hopkins语言学习测验（HVLT），威斯康星卡片分类测验（WCST），口语流畅测验（COWAT）。K-WAIS评分由韦氏成人智力量表（WAIS—R）改编而来，以产生总智商（IQ）、言语IQ和行为IQ来测量智力水平。RCFT评分用于测试空间构建能力。记忆力也包括回忆的精确性、瞬时回忆、延迟回忆和识别指数等的得分。HVLT用于测量口头记忆能力和学习能力。使用WCST和COWAT评分来检查认知的灵活性和执行功能。在WCST中，正确、错误反应的总数，完成的项目数，持续的反应，持续的错误数都用于评分。COWAT评分采用字母翻译和目录翻译的单词数目，字母翻译用来检查语音的流利，目录翻译用来评价语义流利性。使用Wileoxon符号秩检验对术前的Y—BOCS，HAMD，HAMA评分和术后6、12个月的评分进行比较，使用Wileoxon符号秩检验对手术前和手术后12个月的K—WAIS，RCFT，HVLT，WCST，COWAT评分进行比较。统计软件采用SPSS10。结果扣带回毁损术后强迫、抑郁、焦虑症状均改善：Y-BOCS，CGI、HAMD，HAMA评分显示了扣带回毁损术的有效性，在12个月的随访中，与术前Y-BOCS评分相比，改善率为36％，在24个月的随访中，改善率升至48％。术前、术后认知功能和记忆的改变：手术前和手术后24个月的K-WAIS，RCFT，HVLT，COWAT评分没有显著性差异。在WCST评分中，与术前基线相比，术后的错误总数、持续错误数和反应数明显减少。术后的错误总数、持续错误数和反应数的改变说明在扣带回毁损术后执行功能的提高。在术后2—3个月，3例患者有记忆障碍，但持续时间少于2个月。结论强迫症的扣带回毁损术术后24个月的随访结果显示症状明显改善，但没有认知功能的改变，包括智商、言语、视觉空间记忆和执行功能等均没有改变。

How to improve patient satisfaction during midazolam sedation for gastrointestinal endoscopy?

To determine the procedure-related factors that affect sedation satisfaction and to make a suggestion to improve *** prospectively enrolled a total of 456 patients who underwent outpatient endoscopy procedures with midazolam sedation between March 2014 and August 2014. All patients completed both pre- and post-endoscopy questionnaires about sedation expectations and *** study cohort included 167 (36.6%) patients who underwent esophagogastroduodenoscopy (EGD), 167 (36.6%) who underwent colonoscopy, and 122 (26.8%) who underwent a combined procedure (EGD and colonoscopy). Over 80% of all patients were satisfied with sedation using midazolam. In univariate and multivariate analyses, total procedure time in the EGD group, younger age (≤ 50 years), and longer colonoscopy withdrawal time in the colonoscopy group were related to decreased satisfaction with sedation. However, in active monitoring and intervention group, there was no decrease in grade of satisfaction despite longer procedure time due to more procedures during colonoscopy. Younger age (≤ 50 years), longer inter-procedure time gap, and colonoscopy withdrawal time were related to decreased satisfaction in the combined EGD and colonoscopy *** is still a safe and effective sedative for gastrointestinal endoscopy. Satisfaction with sedation depends on several factors including age (≤ 50 years) and procedure time duration. To improve patient satisfaction with sedation, active monitoring of sedation status by the endoscopist should be considered for patients who require long procedure time.

A novel device for endoscopic submucosal dissection,the Fork knife

AIM： To introduce and evaluate the efficacy and technical aspects of endoscopic submucosal dissection （ESD） using a novel device, the Fork knife. METHODS： From March 2004 to April 2008, ESD was performed on 265 gastric lesions using a Fork knife （Endo FS）（group A） and on 72 gastric lesions using a Flexknife （group B） at a single tertiary referral center. We retrospectively compared the endoscopic characteristics of the tumors, pathological findings, and sizes of the resected specimens. We also compared the en b/oc resection rate, complete resection rate, complications, and procedure time between the two groups. RESULTS： The mean size of the resected specimens was 4.27 ± 1.26 cm in group A and 4.29 ± 1.48 cm in group B. The en b/oc resection rate was 95.8% （254/265 lesions） in group A and 93.1% （67/72） in group B. Complete ESD without tumor cell invasion of the resected margin was obtained in 81.1% （215/265） of group A and in 73.6% （53/72） of group B. The perforation rate was 0.8% （2/265） in group A and 1.4% （1/72） in group B. The mean procedure time was 59.63 ± 56.12 min in group A and 76.65 ± 70.75 min in group B （P 〈 0.05）. CONCLUSION： The Fork knife （Endo FS） is useful for clinical practice and has the advantage of reducing the procedure time.

A nomogram to preoperatively predict 1-year disease-specific survival in resected pancreatic cancer following neoadjuvant chemoradiation therapy

Objective: This study aimed to develop a nomogram to predict the 1-year survival of patients with pancreatic cancer who underwent pancreatectomy following neoadjuvant treatment with preoperatively detectable clinical parameters. Extended pancreatectomy is necessary to achieve complete tumor removal in borderline resectable and locally advanced pancreatic cancer. However, it increases postoperative morbidity and mortality rates, and should be balanced with potential benefit of long-term ***: The medical records of patients who underwent pancreatectomy following neoadjuvant treatment from January 2005 to December 2016 at Severance Hospital were retrospectively reviewed. Medical records were collected from five international institutions from Japan and Singapore for external ***: A total of 113 patients were enrolled. The nomogram for predicting 1-year disease-specific survival was created based on 5 clinically detectable preoperative parameters as follows: age(year), symptom(no/yes), tumor size at initial diagnostic stage(cm), preoperative serum carbohydrate antigen(CA) 19-9 level after neoadjuvant treatment(<34/≥34 U/m L), and planned surgery [pancreaticoduodenectomy(PD)(pylorus-preserving PD)/distal pancreatectomy(DP)/total pancreatectomy]. Model performance was assessed for discrimination and *** calibration plot showed good agreement between actual and predicted survival probabilities;the the Greenwood-Nam-D’Agostino(GND) goodness-of-fit test showed that the model was well calibrated(χ~2=8.24,P=0.5099). A total of 84 patients were used for external validation. We aimed to evaluate the current nationwide trend,efficacy,safety,and quality of life(QoL)profiles of hormone treatment in real-world practice settings for prostate cancer(PCa)patients in Korea.A total of 292 men with any biopsy-proven PCa(TanyNanyMany)from 12 institutions in Korea were included in this multi-institutional,observational study of prospectively collected *** luteinizing hormone-releasing hormone(LHRH)agonists were allowed to be investigational *** was defined as(1)the rate of castration(serum testosterone≤50 ng dl^-1)at 4-week visit and(2)breakthrough(serum testosterone>50 ng dl^-1 after castration).Safety assessments included routine examinations for potential adverse events,laboratory tests,blood pressure,body weight,and bone min era I density(BMD,at baseli ne and at the last follow-up visit).QoL was assessed using the Expa nded Prostate Cancer Index Composite-26(EPIC-26).The most comm on initial therapeutic regimen was LHRH agonist with an ti-androge n(78.0%),and the most commonly used LHRH agonist for combination and mono therapy was leuprolide(64.0%for combi natio n and 58.0%for monotherapy).The castration and breakthrough rates were 78.4%and 6.6%,*** laboratory results related to dyslipidemia worsened after 4 weeks of hormone *** addition,the mean BMD T-score was significantly lower at the last follow-up(mean:-1.950)compared to baseline(mean:-0.195).The mean total EPIC-26 score decreased from 84.8(standard deviation[s.d.]:12.2)to 78.3(s.d.:8.1),with significant deterioration only in the urinary domain(mean:23.5 at baseli ne and 21.9 at the 4-week visit).These findi ngs dem on strate the n ationwide trend of current practice setti ngs in hormone treatment for PCa in Korea.
