Minimally invasive surgery has become common in the surgical resection of gastrointestinal submucosal tumors(SMTs). The purpose of this article is to review recent trends in minimally invasive surgery for gastric SMTs. Although laparoscopic resection has been main stream of minimally invasive surgery for gastrointestinal SMTs, recent advances in endoscopic procedures now provide various treatment modalities for gastric SMTs. Moreover, investigators have developed several hybrid techniques that include the advantages of both laparoscopic and endoscopic procedure. In addition, several types of reduced port surgeries, modification of conventional laparoscopic procedures, have been recently applied to the surgical resection of SMTs. Meanwhile, robotic surgery for SMTs requires further evidence and improvement.

Minimally invasive surgery for gastric cancer has rapidly gained popularity due to the early detection of early gastric cancer. As advances in instruments and the accumulation of laparoscopic experience increase, laparoscopic techniques are being used for less invasive but highly technical procedures. Recent evidence suggests that the short- and long-term outcomes of minimally invasive surgery for early gastric cancer and advanced gastric cancer are comparable to those of conventional open surgery. however, these results should be confirmed by large-scale multicenter prospective randomized controlled clinical trials.

AIM: To evaluate the nature of the 'learning curve' for laparoscopy-assisted distal gastrectomy (LADG) with systemic lymphadenectomy for early gastric cancer. METhoDS: The data of 90 consecutive patients with early gastric cancer who underwent LADG with systemic lymphadenectomy between April 2003 and November 2004 were reviewed. The 90 patients were divided into 9 sequential groups of 10 cases in each group and the average operative time of these 9 groups were determined. Other learning indicators, such as transfusion requirements, conversion rates to open surgery, postoperative complication, time to first flatus, and postoperative hospital stay, were evaluated. RESULTS: After the first 10 LADGs, the operative time reached its first plateau (230-240 min/operation) and then reached a second plateau (＜200 min/operation) for the final 30 cases. Although a significant improvement in the operative time was noted after the first 50 cases, there were no significant differences in transfusion requirements, conversion rates to open surgery, postoperative complications, time to first flatus, or postoperative hospital stay between the groups. CONCLUSION: Based on operative time analysis, this study show that experience of 50 cases of LADG with systemic lymphadenectomy for early gastric cancer is required to achieve optimum proficiency.

Objective:The laparoscopic approach for locally advanced gastric cancer has recently been adopted based on the results of several randomized controlled trials(RCTs).however,findings from RCTs have not been examined at the national *** study aimed to investigate the external validity of the Korean Laparoscopic Gastrointestinal Surgery Study-02(KLASS-02)trial involving 13 tertiary hospitals,using data from the Korean Gastric Cancer Association(KGCA)-led nationwide survey involving 68 tertiary or general ***:Data on patients who underwent laparoscopic or open distal gastrectomy for pathological stageⅠB-ⅢC gastric cancer under the same conditions were collected from the KLASS-02 trial and the KGCA nationwide survey *** outcomes were assessed for each dataset and multivariable analyses were performed to examine the effect of the laparoscopic approach on surgical ***:The laparoscopic group had a lower overall complication rate than the open group in both KLASS-02and KGCA datasets(16.1%vs.23.5%for the KLASS-02 and 12.6%vs.19.6%for the KGCA).Moreover,the laparoscopic group had fewer wound problems,and fewer gradeⅡ,Ⅲa,andⅣcomplications than the open group in the KGCA data(0.8%vs.3.4%,5.8%vs.10.4%,2.3%vs.3.7%,and 0.5%vs.1.4%,respectively),which were not observed in the KLASS-02 *** analyses revealed that the laparoscopic approach was not associated with overall complications,but reduced wound problems and more harvested lymph nodes in the KGCA survey data(adjusted odds ratios,0.19 for wound problems,adjustedβcoefficient 4.39 for number of harvested lymph nodes),which were not shown in the KLASS-02 ***:The safety and feasibility of the laparoscopic approach for locally advanced gastric cancer were validated at a national *** laparoscopic approach for locally advanced gastric cancer can be implemented in the Republic of Korea.

We present herein a case report of sigmoidorectal intussusception as an unusual case of sigmoid adenomatous polyp. The patient was a 56-year-old man who suffered from rectal bleeding for one day. He initially visited his general practitioner and was diagnosed as having an intraluminal mass of 15 cm from the anal verge. Several hours after admission to our coloproctology clinic, he suddenly presented with lower abdominal cramping pain with rectal bleeding during his bowel preparation using polyethylene glycol electrolyte solution. An emergency colonoscopy revealed that the invaginated colon with polypoid mass was protruded to the lower rectum. Gastrograffin enema showed that the invaginated bowel segment was 3 cm from the anal verge. CT scan showed the typical finding of intussusception. We performed laparoscopic anterior resection and anastomosis after the sponge-on-the-stickassisted manual reduction. The permanent pathologic finding showed villotubular adenoma of the sigmoid colon.

To emulate the functionality of the human retina and achieve a neuromorphic visual system,the development of a photonic synapse capable of multispectral color discrimination is of paramount ***,attaining robust color discrimination across a wide intensity range,even irrespective of medium limitations in the channel layer,poses a significant ***,we propose an approach that can bestow the color-discriminating synaptic functionality upon a three-terminal transistor flash memory even with enhanced discriminating *** incorporating the strong induced dipole moment effect at the excitation,modulated by the wavelength of the incident light,into the floating gate,we achieve outstanding RGB color-discriminating synaptic functionality within a remarkable intensity range spanning from 0.05 to 40 mW cm^(-2).This approach is not restricted to a specific medium in the channel layer,thereby enhancing its *** effectiveness of this color-discriminating synaptic functionality is demonstrated through visual pre-processing of a photonic synapse array,involving the differentiation of RGB channels and the enhancement of image contrast with noise ***,a convolutional neural network can achieve an impressive inference accuracy of over 94%for Canadian-Institute-For-Advanced-Research-10 colorful image recognition task after the *** proposed approach offers a promising solution for achieving robust and versatile RGB color discrimination in photonic synapses,enabling significant advancements in artificial visual systems.
