Carcinoma in situ arising in a tubulovillous adenoma of the distal common bile duct: A case report

Tubulovillous adenomas are common in the colon and rectum, but are rare in the common bile duct. Biliary adenomas may produce obstructive jaundice, which can be easily confused with a malignant neoplasm or stone. We report a case of a carcinoma in situ arising in a tubulovillous adenoma of the distal common bile duct causing obstructive jaundice. A 55-year-old male presented with a 10-d history of pruritus and progressive jaundice. Abdominal sonography and computed tomography showed a mass in the distal common bile duct. Endoscopic retrograde cholangiopancreatography showed luminal narrowing of the bile duct due to a polypoid mass. Positron emission tomography demonstrated no abnormal uptake. It was thought that this mass was a malignant tumor, thus a pylorus-preserving panceaticoduodenectomy was performed. The final pathology showed a tubulovillous adenoma with carcinoma in situ of the distal common bile duct. At follow-up 8 mo later, endoscopy showed multiple polyps in the rectum, colon and stomach. The polyps were removed by endoscopic mucosal resection and shown to be tubular adenomas with high grade dysplasia. Biliary adenomas require careful follow-up for early detection of recurrence and malignant transformation.

Applications of a novel curve-fitting technique are presented to efficiently predict the motion of the vortex filament, which is trailed from a rigid body such as wings and rotors. The gov- erning equations of the motion, when a Lagrangian approach with the present curve-fitting method is applied, can be transformed into an easily solvable form of the system of nonlinear ordinary dif- ferential equations. The applicability of Bezier curves, B-spline, and Lagrange interpolating polyno- mials is investigated. Local Lagrange interpolating polynomials with a shift operator are proposed as the best selection for applications, since it provides superior system characteristics with minimum computing time, compared to other methods. In addition, the Gauss quadrature formula with local refinement strategy has been developed for an accurate prediction of the induced velocity computed with the line integration of the Biot-Savart law. Rotary-wing problems including a vortex ring problem are analyzed to show the efficiency, accuracy, and flexibility in the applications of the pro- posed method.

The nitrogen changes and the nitrogen mass balance in a free water surface flow constructed wetland （CW） using the four-year monitoring data from 2008 to 2012 were estimated. The CW was composed of six cells in series that include the first settling basin （Cell 1）, aeration pond （Cell 2）, deep marsh （Cell 3）, shallow marsh （Cell 4）, deep marsh （Cell 5） and final settling basin （Cell 6）. Analysis revealed that the NH4＋-N concentration decreased because of ammonification which was then followed by nitrification. The NO4＋-N and NO4＋-N were also further reduced by means of microbial activities and plant uptake during photosynthesis. The average nitrogen concentration at the influent was 37,819 kg/year and approximately 45% of that amount exited the CW in the effluent. The denitrification amounted to 34% of the net nitrogen input, whereas the accretion of sediment was only 7%. The biomass uptake of plants was able to retain only 1% of total nitrogen load. In order to improve the nutrient removal by plant uptake, plant coverage in four cells （i.e., Cells 1, 3, 4 and 5） could be increased.

AIM:To investigate the preventive effects of low-dose proton-pump inhibitors(PPIs) for upper gastrointestinal bleeding(UGIB) in end-stage renal ***:This was a retrospective cohort study that reviewed 544 patients with end-stage renal disease who started dialysis at our center between 2005 and *** examined the incidence of UGIB in 175 patients treated with low-dose PPIs and 369 patients not treated with PPIs(control group).RESULTS:During the study period, 41 patients developed UGIB, a rate of 14.4/1000 *** mean time between the start of dialysis and UGIB events was 26.3 ± 29.6 *** occurred in only two patients in the PPI group(2.5/1000 person-years) and in 39 patients in the control group(19.2/1000 person-years).Kaplan-Meier analysis of cumulative non-bleeding survival showed that the probability of UGIB was significantly lower in the PPI group than in the control group(log-rank test, P < 0.001).Univariate analysis showed that coronary artery disease, PPI use, anti-coagulation, and anti-platelet therapy were associated with *** adjustments for the potential factors influencing risk of UGIB, PPI use was shown to be significantly beneficial in reducing UGIB compared to the control group(HR = 13.7, 95%CI:1.8-101.6; P = 0.011).CONCLUSION:The use of low-dose PPIs in patients with end-stage renal disease is associated with a low frequency of UGIB.
