Efficacy and safety of tadalafil taken as needed for the treatment of erectile dysfunction in Asian men： results of an integrated analysis

We evaluated the efficacy and safety of as-needed tadalafil in a diverse clinical population （with varying patient demographics, disease severity, and comorbid medical conditions） of Asian men with erectile dysfunction （ED）. An integrated analysis of five double-blind, placebo-controlled trials （N = 1 046） was performed. Patients were randomly assigned to receive 10 mg tadalafil （N = 185）, 20 mg tadalafil （N = 510）, or placebo （N = 351）. Efficacy assessments included the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP） diary and Global Assessment Question （GAQ）. Patients receiving 10 mg or 20 mg tadalafil showed significant improvement from baseline-to-end point on the Erectile Function domain of the International Index of Erectile Function （IIEF-EF） domain score in all clinical sub-populations analyzed, compared with patients receiving placebo （P 〈 0.001）. The 10-mg and 20-mg tadalafil groups showed a mean success rate of 64.1% and 70.5% for sexual intercourse attempts （SEP3, successful intercourse）, respectively, compared with 33.4% in the placebo group （P 〈 0.001）, and 85.5% and 85.4% reported improved erections at end point GAQ, respectively, versus 43.5% in the placebo group （P 〈 0.001）. Tadalafil was well tolerated across all groups studied. Headache and back pain were the most frequently reported adverse events. Overall, tadalafil was effective and well tolerated across a diverse clinical spectrum of Asian men with ED.

Plasma transforming growth factor-β_1 levels in patients with erectile dysfunction

Aim: To evaluate the plasma TGF-β1 level in erectile dysfunction (ED) patients of various causes. Methods: Sixty-two patients with ED and 26 potent men were subjected to the study. Based on multidisciplinary work-ups, including medical history, physical examinations, blood tests with lipid profile and hormones, penile duplex Doppler ultrasonogram and neurophysiological tests, causes for ED were classified as psychogenic (n=15), neurogenic (n=16) and vasculogenic (n=31). The plasma TGF-β1 level was measured by the ELISA method. Results: The plasma TGF-β1 level was significantly increased in the ED group (6.7 ± 4.9 ng/mL), compared to the control (4.0±2.1 ng/mL) (P king (7.5 ± 4.7 ng/mL) and dyslipidemia groups (7.4 ± 4.4 ng/mL) showed significantly increased plasma TGF-β1 levels, compared to the non-smokers (5.5 ± 2.8 ng/mL), and those without dyslipidemia (4.8 ?2.8 ng/mL) (P<0.05). Conclusion: Vascular risk factors are associated with an elevated plasma TGF-β1 level, which may contribute to cavernous fibrosis and ED.

Aim： To evaluate the efficacy and safety of SK3530, a newly developed type 5 phosphodiesterase inhibitor （PDE5I）, in Korean men with erectile dysfunction （ED）. Methods： A total of 119 patients were randomized at 10 centers in Korea to receive either SK3530 （50, 100, or 150 mg; n = 89） or placebo （n = 30） taken 1 h before anticipated sexual activity for an 8-week period. The patients were evaluated at baseline and 4 and 8 weeks after beginning therapy. Efficacy was assessed using the International Index of Erectile Function （IIEF）, Sexual Encounter Profile （SEP）, and the Global Assessment Question （GAQ）. Safety was analyzed by adverse events, laboratory values and vital signs. Results： At the end of the study, all the primary and secondary efficacy end-points were statistically significantly improved by SK3530 compared with placebo （P 〈 0.05）. Of the 89 patients in the treatment arm, 36 （42.3 %） achieved normal erectile function after treatment, including six patients with severe ED. Treatment-related adverse events occurred in 32 patients. The most common adverse events were flushing, headache, dizziness and eye redness （10.9%, 7.6%, 2.5% and 2.5%, respectively）, and most were mild. Only two patients discontinued treatment during the study period because of adverse events. Conclusion： The results of our phase Ⅱ study have confirmed the efficacy and safety of SK3530 in a broad population of men with ED of various etiologies and severity. The optimal doses in terms of efficacy and safety were determined to be 50 mg and 100 mg, respectively.

Objective:To evaluate the effects of Capsosiphon fulvescens(C.fulvescens)ethanolic extract on inflammation in lipopolysaccharide(LPS)-induced RAW296.7 macrophages.Methods:The protective effects of C.fulvescens ethanolic extract on LPS-induced inflammation in RAW264.7 macrophages were assessed using biochemical analysis,including enzyme-linked immunosorbent assay,quantitative reverse transcription-polymerase chain reaction,and Western blot analysis.To examine reactive oxygen species(ROS)production,flow cytometry analysis,and immunofluorescence staining were used.Furthermore,the modulatory effect of C.fulvescens ethanolic extract on NF-κB activation was investigated.Results:C.fulvescens ethanolic extract significantly attenuated LPS-induced levels of pro-inflammatory cytokines and notably reduced the secretion and mRNA levels of LPS-mediated matrix metalloproteinases.In addition,C.fulvescens ethanolic extract decreased ROS production and suppressed the TLR4/NF-κB signaling pathway.Conclusions:C.fulvescens ethanolic extract alleviates inflammation as well as oxidative stress by modulating the TLR4/NF-κB signaling in LPS-induced RAW264.7 macrophages.C.fulvescens can be used as a potential therapeutic agent to suppress inflammation and oxidative stress-associated diseases.

Plant-derived vesicles(PDVs)are membranous structures that originate from plant cells and are responsible for multiple physiological and pathological functions.In the last decade,PDVs have gained much attention for their involvement in different biological processes,including intercellular communication and defense response,and recent scientific evidence has opened a new avenue for their applications in cancer treatment.Nevertheless,much remains unknown about these vesicles,and current research remains inconsistent.This review aims to provide a comprehensive introduction to PDVs,from their biological characteristics to purification methods,and to summarize the status of their potential development for cancer therapy.

Electrodeposited copper oxides with a suppressed interfacial amorphous phase using mixed-crystalline ITO and their enhanced photoelectrochemical performances

The effectiveness of photoelectrochemical(PEC)water splitting is significantly restricted by insufficient light harvesting,rapid charge recombination,and slow water reduction kinetics.Since the presence of amorphous phases in the interfaces hinders the overcome of these inherent limitations,a photoelectrode must be built strategically.Herein,we artificially controlled the crystallographic orientation of indium tin oxide(ITO)to determine the orientation with the smallest lattice mismatch at the Cu_(2)O interface,thus significantly reducing the amorphous phase in the early stage of electrodeposition nucleation.The[222]/[400]mixed orientation ITO primarily exposed the{400}surface planes and accelerated charge transfer by forming an optimal interface with preferentially grown(111)oriented Cu_(2)O and minimized amorphous region.In addition,the ITO surface energy was calculated using density functional theory to theoretically verify which plane is more active for growing the photoactivation layer.The rationally designed ITO/Cu_(2)O/Al-dope Zn O/TiO_(2)/Rh-P device,with each layer serving a specific purpose,achieved a photocurrent density of 8.23 mA cm^(-2)at 0 VRHEunder AM 1.5 G illumination,providing a standard method for effective solar-to-hydrogen conversion photocathodes.

Real-time Volume Preserving Constraints for Volumetric Model on GPU

This paper presents a parallel method for simulating real-time 3D deformable objects using the volume preservation mass-spring system method on tetrahedron meshes.In general,the conventional mass-spring system is manipulated as a force-driven method because it is fast,simple to implement,and the parameters can be controlled.However,the springs in traditional mass-spring system can be excessively elongated which cause severe stability and robustness issues that lead to shape restoring,simulation blow-up,and huge volume loss of the deformable object.In addition,traditional method that uses a serial process of the central processing unit(CPU)to solve the system in every frame cannot handle the complex structure of deformable object in real-time.Therefore,the first order implicit constraint enforcement for a mass-spring model is utilized to achieve accurate visual realism of deformable objects with tough constraint error.In this paper,we applied the distance constraint and volume conservation constraints for each tetrahedron element to improve the stability of deformable object simulation using the mass-spring system and behave the same as its real-world counterparts.To reduce the computational complexity while ensuring stable simulation,we applied a method that utilizes OpenGL compute shader,a part of OpenGL Shading Language(GLSL)that executes on the graphic processing unit(GPU)to solve the numerical problems effectively.We applied the proposed methods to experimental volumetric models,and volume percentages of all objects are compared.The average volume percentages of all models during the simulation using the mass-spring system,distance constraint,and the volume constraint method were 68.21%,89.64%,and 98.70%,respectively.The proposed approaches are successfully applied to improve the stability of mass-spring system and the performance comparison from our experimental tests also shows that the GPU-based method is faster than CPU-based implementation for all cases.

Parallel Cloth Simulation Using OpenGL Shading Language

The primary goal of cloth simulation is to express object behavior in a realistic manner and achieve real-time performance by following the fundamental concept of physic.In general,the mass–spring system is applied to real-time cloth simulation with three types of springs.However,hard spring cloth simulation using the mass–spring system requires a small integration time-step in order to use a large stiffness coefficient.Furthermore,to obtain stable behavior,constraint enforcement is used instead of maintenance of the force of each spring.Constraint force computation involves a large sparse linear solving operation.Due to the large computation,we implement a cloth simulation using adaptive constraint activation and deactivation techniques that involve the mass-spring system and constraint enforcement method to prevent excessive elongation of cloth.At the same time,when the length of the spring is stretched or compressed over a defined threshold,adaptive constraint activation and deactivation method deactivates the spring and generate the implicit constraint.Traditional method that uses a serial process of the Central Processing Unit(CPU)to solve the system in every frame cannot handle the complex structure of cloth model in real-time.Our simulation utilizes the Graphic Processing Unit(GPU)parallel processing with compute shader in OpenGL Shading Language(GLSL)to solve the system effectively.In this paper,we design and implement parallel method for cloth simulation,and experiment on the performance and behavior comparison of the mass-spring system,constraint enforcement,and adaptive constraint activation and deactivation techniques the using GPU-based parallel method.

Sarcopenia in pancreatic cancer:Effect on patient outcomes

Pancreatic cancer is a challenging disease with an increasing incidence and extremely poor prognosis.The clinical outcomes of pancreatic cancer depend on tumor biology,responses to treatments,and malnutrition or cachexia.Sarcopenia represents a severe catabolic condition defined by the age-related loss of muscle mass and strength and affects as much as 70%of malnourished pancreatic cancer patients.The lumbar skeletal muscle index,defined as the total abdominal muscle area at the L3 vertebral level adjusted by the square of the height,is widely used for assessing sarcopenia in patients with pancreatic cancer.Several studies have suggested that sarcopenia may be a risk factor for perioperative complications and decreased recurrence-free or overall survival in patients with pancreatic cancer undergoing surgery.Sarcopenia could also intensify chemotherapyinduced toxicities and worsen the quality of life and survival in the neoadjuvant or palliative chemotherapy setting.Sarcopenia,not only at the time of diagnosis but also during treatment,decreases survival in patients with pancreatic cancer.Theoretically,multimodal interventions may improve sarcopenia and clinical outcomes;however,no study has reported positive results.Further prospective studies are needed to confirm the prognostic role of sarcopenia and the effects of multimodal interventions in patients with pancreatic cancer.

Crystal Facet Engineering of TiO_(2) Nanostructures for Enhancing Photoelectrochemical Water Splitting with BiVO_(4) Nanodots

Although bismuth vanadate(BiVO4)has been promising as photoanode material for photoelectrochemical water splitting,its charge recombination issue by short charge diffusion length has led to various studies about heterostructure photoanodes.As a hole blocking layer of BiVO4,titanium dioxide(TiO_(2)) has been considered unsuitable because of its relatively positive valence band edge and low electrical conductivity.Herein,a crystal facet engineering of TiO_(2) nanostructures is proposed to control band structures for the hole blocking layer of BiVO4 nanodots.We design two types of TiO_(2) nanostructures,which are nanorods(NRs)and nanoflowers(NFs)with different(001)and(110)crystal facets,respectively,and fabricate BiVO4/TiO_(2) heterostructure photoanodes.The BiVO4/TiO_(2) NFs showed 4.8 times higher photocurrent density than the BiVO4/TiO_(2) NRs.Transient decay time analysis and time-resolved photoluminescence reveal the enhancement is attributed to the reduced charge recombination,which is originated from the formation of type II band alignment between BiVO4 nanodots and TiO_(2) NFs.This work provides not only new insights into the interplay between crystal facets and band structures but also important steps for the design of highly efficient photoelectrodes.
