Efficacy and safety of controlled-release oxycodone/naloxone versus controlled-release oxycodone in Korean patients with cancer-related pain: a randomized controlled trial

Background: Controlled-release oxycodone/naloxone(OXN-CR) maintains the effect of opioid-induced analgesia through oxycodone while reducing the occurrence rate of opioid-induced constipation through naloxone. The present study was designed to assess the non-inferiority of OXN-CR to controlled-release oxycodone(OX-CR) for the control of cancer-related pain in Korean ***: In this randomized, open-labeled, parallel-group, phase IV study, we enrolled patients aged 20 years or older with moderate to severe cancer-related pain [numeric rating scale(NRS) pain score ≥4] from seven Korean oncology/hematology centers. Patients in the intention-to-treat(ITT) population were randomized(1:1) to OXNCR or OX-CR groups. OXN-CR was administered starting at 20 mg/10 mg per day and up-titrated to a maximum of80 mg/40 mg per day for 4 weeks, and OX-CR was administered starting at 20 mg/day and up-titrated to a maximum of 80 mg/day for 4 *** primary efficacy endpoint was the change in NRS pain score from baseline to week4, with non-inferiority margin of-1.5. Secondary endpoints included analgesic rescue medication intake, patientreported change in bowel habits, laxative intake, quality of life(QoL), and safety ***: Of the ITT population comprising 128 patients, 7 with missing primary efficacy data and 4 who violated the eligibility criteria were excluded from the efficacy analysis. At week 4, the mean change in NRS pain scores was not significantly different between the OXN-CR group(n = 58) and the OX-CR group(n = 59)(-1.586 vs.-1.559,P = 0.948). The lower limit of the one-sided 95% confidence interval(-0.776 to 0.830) for the difference exceeded the non-inferiority margin(P < 0.001). Hip External Rotator Strength and Compensatory Movement in Three Different Positions

Measuring intrinsic hip external rotator strength (ER) without compensatory pelvic motion and activation of the sartorius is important for preventing or rehabilitating lower extremity injuries. However, the optimal method for measuring intrinsic hip ER muscle strength while minimizing compensatory pelvic motions and activation of the sartorius is unclear. The purpose of this study is to compare measurements of hip ER strength, compensatory pelvic motion, and sartorius activation in the sitting, prone, and sidelying positions. Thirty-one healthy subjects (16 males and 15 females) were recruited for this study. Hip ER strength, pelvic kinematics, and sartorius muscle activation were measured during maximal isometric contraction of the hip ER in the sitting, prone, and sidelying positions. Hip ER strength was measured using a load-cell-type strength-measurement sensor. Pelvic kinematics was measured using an electromagnetic motion-tracking sensor. Electromyography was used to measure sartorius muscle activity. Data were analyzed using one-way repeated-measures analysis of variance. The result showed that hip ER strength and sartorius muscle activation were significantly lower in the sidelying compared with the sitting and prone positions (p < 0.01). Pelvic anteroposterior tilting was significantly greater in the sitting compared with the prone and sidelying positions (p < 0.01). Pelvic rotation differed significantly among positions (p < 0.01). Pelvic lateral tilting was significantly greater in the prone compared with the sitting position (p < 0.017). Compensatory pelvic motion and sartorius muscle activation were lower when hip ER strength measurements were made in the sidelying position. Therefore, the sidelying position is effective for measuring selective intrinsic hip ER strength.

Low-intensity light detection necessitates high-responsivity *** achieve this,we report In_(0.53)Ga_(0.47)As∕In As∕In_(0.53)Ga_(0.47)As quantum well(InAs QW)photo-field-effect-transistors(photo-FETs)integrated on a Si substrate using direct wafer *** of the In As QW channel was carefully designed to achieve higher effective mobility and a narrower bandgap compared with a bulk In_(0.53)Ga_(0.47)As,while suppressing the generation of defects due to lattice ***-performance 2.6 nm In As QW photo-FETs were successfully demonstrated with a high on/off ratio of 10~5 and a high effective mobility of 2370 cm^(2)∕(V·s).The outstanding transport characteristics in the InAs QW channel result in an optical responsivity 1.8 times greater than InGaAs photo-FETs and the fast rising/falling ***,we experimentally confirmed that the InAs QW photo-FET can detect light in the short-wavelength infrared(SWIR;1.0–2.5μm)near 2μm thanks to bandgap engineering through In As QW *** result suggests that the InAs QW photo-FET is promising for high-responsivity and extended-range SWIR photodetector applications.

Human Body Tracking and Pose Estimation Using Modified Camshift Algorithm

In this paper, we propose multiple CAMShift Algorithm based on Kalman filter and weighted search windows that extracts skin color area and tracks several human body parts for real-time human tracking system. The CAMShift Algorithm we propose searches the skin color region by detecting the skin color area from background model. Kalman filter stabilizes the floated search area of CAMShift Algorithm. Each occlusion areas are avoided by using weighted window of non-search areas and main-search area. And shadows are eliminated from background model and intensity of shadow. The proposed modified Camshaft algorithm can estimate human pose in real-time and achieves 96.82% accuracy even in the case of occlusions.

Oenanthe javanica is an aquatic perennial herb that belongs to theOenanthe genus in Apiaceae family, and it displays well-known medicinal properties such as protective effects against glu-tamate-induced neurotoxicity. However, few studies regarding effects ofOenanthe javanica on neurogenesis in the brain have been reported. In this study, we examined the effects of a normal diet and a diet containing ethanol extract ofOenanthe javanica on cell proliferation and neu-roblast differentiation in the subgranular zone of the hippocampal dentate gyrus of adolescent rats using Ki-67 （an endogenous marker for cell proliferation） and doublecortin （a marker for neuroblast）. Our results showed thatOenanthe javanica extract signiifcantly increased the number of Ki-67-immunoreactive cells and doublecortin-immunoreactive neuroblasts in the subgranular zone of the dentate gyrus in the adolescent rats. In addition, the immunoreactivity of brain-derived neurotrophic factor was signiifcantly increased in the dentate gyrus of the Oenanthe javanica extract-treated group compared with the control group. However, we did not ifnd that vascular endothelial growth factor expression was increased in theOenanthe javanica extract-treated group compared with the control group. These results indicate thatOenanthe javanica extract improves cell proliferation and neuroblast differentiation by increasing brain-de-rived neurotrophic factor immunoreactivity in the rat dentate gyrus.

Ischemic preconditioning（IPC） is a condition of sublethal transient global ischemia and exhibits neuroprotective effects against subsequent lethal ischemic ***,in this study,examined the neuroprotective effects of IPC and its effects on immunoreactive changes of antioxidant enzymes including superoxide dismutase（SOD） 1 and SOD2,catalase（CAT） and glutathione peroxidase（GPX） in the gerbil hippocampal CA1 region after transient forebrain *** neurons of the stratum pyramidale（SP） in the hippocampal CA1 region of animals died 5 days after lethal transient ischemia without IPC（8.6%（ratio of remanent neurons） of the sham-operated group）;however,IPC prevented the pyramidal neurons from subsequent lethal ischemic injury（92.3%（ratio of remanent neurons） of the sham-operated group）.SOD1,SOD2,CAT and GPX immunoreactivities in the sham-operated animals were easily detected in pyramidal neurons in the stratum pyramidale（SP） of the hippocampal CA1 region,while all of these immunoreactivities were rarely detected in the stratum pyramidale at 5 days after lethal transient ischemia without ***,their immunoreactivities in the sham-operated animals with IPC were similar to（SOD1,SOD2 and CAT） or higher（GPX） than those in the sham-operated animals without ***,their immunoreactivities in the stratum pyramidale of the ischemia-operated animals with IPC were steadily maintained after lethal ischemia/*** of western blot analysis for SOD1,SOD2,CAT and GPX were similar to immunohistochemical *** conclusion,IPC maintained or increased the expression of antioxidant enzymes in the stratum pyramidale of the hippocampal CA1 region after subsequent lethal transient forebrain ischemia and IPC exhibited neuroprotective effects in the hippocampal CA1 region against transient forebrain ischemia.

Objective Risk factor modification is key to preventing subsequent cardiac events after a heart attack. This study was designed to investigate the disparity between preventive guidelines and clinical practice among smoking patients. Methods The study was carried out in smokers admitted with myocardial infarction (MI). Atotal of 275 patients who had been regularly followed for over one year after MI were randomly selected and enrolled in this *** investigated changes in smoking behavior and the adherence rate to ACC/AHA Guidelines for secondary prevention in patients with coronary artery disease at the time of, and one year after, the index event. Results The study population consisted of 275 patients (97.1% males) with a mean age of 57.0 ±11.2 years. Achievement of target goals at one year was as follows: smoking cessation, 52.3%; blood pressure, 83.9%; HbA1c, 32.7%; lipid profile, 65.5%; and body mass index (BMI), 50.6%. Over one year, 80% of the patients attempted to quit smoking; 27% of them re-started smoking within one month after discharge while 65% succeeded in cessation of smoking. At one year, only 52% of the patients overall had stopped smoking. From the multivariate logistic analysis including smoking patterns and clinical characteristics, the severity of coronary artery disease was the only independent predictor for smoking cessation (Relative risk (RR): 1.230; P = 0.022). Conclusions Only a small percentage of MI patients adhere to guidelines for secondary prevention and a sizable proportion fail to stop smoking. These findings underscore the need for an effective patient education system.

AIM：To investigate the incidence and general characteristics of sports-related eye injuries in patients visiting the Emergency Department. Background: Leptomeningeal seeding （LMS） is a devastating complication of malignancy. Among several pathogenesis, surgical spillage has been one of the iatrogenic factors for the development of LMS. We retrospectively analyzed various surgical factors affecting the development of LMS. Patients and Methods: We identified 294 patients with brain metastasis who had been treated using surgery between 2001 and 2009. We classified the tumor location into subcortical, pial, epipial, juxtaventricular and ventricular. The description of surgical resection was divided into piecemeal and en-bloc and the risk of using cavitron ultrasonic aspirator （CUSA） were investigated. Results: A total 44 of the 294 patients （15%） developed LMS after undergoing index procedure. The median time to LMS development was 6months （range, 1.0-42.0）. Among the 294 surgical patients, seven out of 101 en-bloc removal patients （6.9%） developed LMS, whereas 37 out of 193 piecemeal removal patients （19.2%） developed LMS, and the incidence of LMS was significantly different between these two surgical resection modes （p = 0.005）.The adjusted hazard ratio （HR） for patients whose tumors were resected by piecemeal compared to en-bloc was 3.12 （95% CI, 1.4-7.0;p = 0.006）. The incidence of LMS was significantly higher （p ＜ 0.001） in patients whose tumors were resected by CUSA, and the HR of using CUSA was 4.18 （95% CI, 1.9-9.4;p = 0.001）. The location of tumor at high-risk of CSF exposure （epipial and ventricular） conferred increased risk of LMS after the treatment compared to low-risk group with the HR 2.52 （95% CI, 1.4-4.7;p = 0.004）. On the other hand, the location of tumor at low-risk of CSF exposure （subcortical） showed significantly reduced risk of LSM compared to the other locations （HR=0.27;95% CI 0.12-0.65;p-value = 0.003）. Conclusion: The piecemeal resection mode and the use of CUSA were found to increase risk of LMS in this retrospective study. Therefore, the piecemeal resection using CUSA should be

A Case Report of a Sarcomatoid Carcinoma Arising in the Renal Pelvis with Exuberant Osteosarcomatous Element

Sarcomatoid carcinoma is a rare malignant tumor that has both malignant epithelial and mesenchymal components. We describe a sarcomatoid carcinoma arising in the right renal pelvis of a 68-year-old man. The dominant component of the tumor was osteosarcomatous, but there were also focal carcinomatous areas. The sarcomatous tumor cells produced abundant osteoid matrix surrounded by osteoblastic cells. The carcinomatous tumor cells consisted of papillary urothelial carcinoma. Immunohistochemical assay showed that the sarcomatous tumor cells were positive for vimentin and negative for cytokeratin. The papillary urothelial carcinoma was positive for cytokeratin and negative for vimentin. After surgery, the patient underwent adjuvant chemotherapy. Four months later, he presented with recurrence in the right subphrenic area and metastasis in the right middle lobe of the lung.
