Does the bile duct angulation affect recurrence of choledocholithiasis?

AIM:To investigate whether bile duct angulation and T-tube choledochostomy influence the recurrence of ***:We conducted a retrospective study including 259 patients who underwent endoscopic sphincterotomy and cholecystectomy for choledocholithiasis between 2000 and *** imaginary line was drawn along the center of the bile duct and each internal angle was measured at the two angulation sites ofthe bile duct *** values of both angles were added *** then tested our hypothesis by examining whether T-tube choledochostomy was performed and stone recurrence occurred by reviewing each subject's medical ***:The overall recurrence rate was 9.3% (24 of 259 patients).The mean value of sums of angles in the recurrence group was 268.3°± 29.6°,while that in the non-recurrence group was 314.8°± 19.9° (P < 0.05).Recurrence rate of the T-tube group was 15.9% (17 of 107),while that of the non T-tube group was 4.6% (7 of 152) (P < 0.05).Mean value of sums of angles after T-tube drainage was 262.5°± 24.6° and that before T-tube drainage was 298.0°± 23.9° in 22 patients (P < 0.05).CONCLUSION:The bile duct angulation and T-tube choledochostomy may be risk factors of recurrence of bile duct stones.

Diffusion-weighted magnetic resonance imaging(DWI)is a well established method for the evaluation of intracranial diseases,such as acute *** for extracranial application is more difficult due to physiological motion artifacts and the heterogeneous composition of the ***,thanks to the newer technical development of DWI,DWI has become increasingly used over the past few years in extracranial organs including the abdomen and *** previous studies of DWI have been limited to the evaluation of diffuse parenchymal abnormalities and focal lesions in abdominal organs,whereas there are few studies about DWI for the evaluation of the biliopancreatic *** further studies are needed to determine its performance in evaluating bile duct,gallbladder and pancreas diseases,DWI has potential in the assessment of the functional information on the biliopancreatic tract concerning the status of tissue cellularity,because increased cellularity is associated with impeded diffusion,as indicated by a reduction in the apparent diffusion *** detection of malignant lesions and their differentiation from benign tumor-like lesions in the biliopancreatic tract could be improved using DWI in conjunction with findings obtained with conventional magnetic resonance ***,DWI can be useful for the assessment of the biliopancreatic tract in patients with renal impairment because contrast-enhanced computed tomography or magnetic resonance scans should be avoided in these patients.

One of the main challenges in stretchable electronics is to achieve high-performance stretchable semiconductors. Here, weintroduce an innovative concept of nanomeshed semiconductor nanomembrane which can be regarded almost as intrinsicallystretchable to conventional microelectronic layouts. By making a silicon film into homogeneous nanomeshes with spring-like nanotraces, we demonstrated a high electron mobility of 50 cm^(2)/V·s, and moderate stretchability with a one-time strain of 25% andcyclic strain of 14% after stretching for 1000 cycles, further improvable with optimized nanomesh designs. A simple analytic modelcovering both fractional material and trace sidewall surfaces well predicted the transport properties of the normally on siliconnanomesh transistors, enabling future design and optimizations. Besides potential applications in stretchable electronics, thissemiconductor nanomesh concept provides a new platform for materials engineering and is expected to yield a new family ofstretchable inorganic materials having tunable electronic and optoelectronic properties with customized nanostructures.

Dear Editor,Angiosarcoma is a rare malignancy,which accounts for approximately 1%-2%of soft-tissue sarcomas[1].It can occur at any site within the human *** research-ers suggested that site distribution may be different between Eastern Asian and Western countries[1].Sev-eral factors,such as age,stage at diagnosis,performance status,site of the disease,histological grade,tumor size,and visceral involvement,were suggested as prognos-tic factors in angiosarcoma[2-5].However,there is still controversy regarding to which prognostic factors are significantly *** there are no randomized clinical trials for angiosarcoma,its treatments are guided based on guidelines of soft-tissue sarcoma or the results of several retrospective *** surgery is the preferred treatment for angiosarcoma[2-4].Some stud-ies suggested that receiving adjuvant chemotherapy(CT)is associated with better outcome[5],while other studies have shown that adjuvant CT has no significant effect[6,7].Chemoradiotherapy has been reported to be effective in angiosarcoma especially for cutaneous angiosarcoma[8].However,evidence for the efficacy and safety of pal-liative cytotoxic CT is still ***,here,we present our analysis based on the clinical features,tumor characteristics,treatment and outcomes of angiosarcoma patients in an attempt to investigate the significant prog-nostic factors and efficient therapeutics in real clinical practice.

Carbon and nitrogen storage in an age-sequence of Pinus densiflora stands in Korea

The carbon(C) and nitrogen(N) storage capabilities of Pinus densiflora in six different stand ages(10,27,30,32,44,and 71 years old) were investigated in *** sample trees were destructively harvested and 12 were *** from the above and belowground tree components,coarse woody debris(CWD),forest floor,and mineral soil(0-30 cm) were *** biomass was highest in the 71-year-old stand(202.8 t ha-1) and lowest in the 10-year-old stand(18.4 t ha-1).C and N storage in the mineral soil was higher in the 71-year-old stand than in the other stands,mainly due to higher soil C and N ***,the total ecosystem C and N storage(tree+forest floor+CWD+soil) was positively correlated with stand age:increasing from a minimum in the 10 year old stand(18.8 t C ha-1 and 1.3 t N ha-1) to a maximum in the 71-year-old stand(201.4 t C ha-1 and 8.5 t N ha-1).The total ecosystem C storage showed a similar sigmoidal pattern to that of tree C storage as a function of the age-sequence,while N storage in the CWD,forest floor and mineral soil showed no significant temporal *** results provide important insights that will increase our understanding of C and N storage in *** stands and our ability to predict changes according to stand age in the region.

Clinicopathologic and molecular features associated with patient age in gastric cancer

AIM: To compare characteristics and prognosis of gastric cancer based on ***: A retrospective study was conducted on clinical and molecular data from patients(n =1658) with confirmed cases of gastric cancer in seoul National University Bundang Hospital(seoul, South Korea) from 2003 to 2010 after exclusion of patients diagnosed with lymphoma, gastrointestinal stromal tumor, and metastatic cancer in the stomach. DNA was isolated from tumor and adjacent normal tissue,and a set of five markers was amplified by polymerase chain reaction to assess microsatellite instability(MSI). MSI was categorized as high, low, or stable if ≥ 2, 1, or 0 markers, respectively, had *** was performed on tissue sections to detect levels of expression of p53, human epidermal growth factor receptor(HER)-2, and epidermal growth factor receptor. Statistical analysis of clinical and molecular data was performed to assess prognosis based on the stratification of patients by age(≤ 45 and> 45 years).RESULTS: Among the 1658 gastric cancer patients, the number of patients with an age ≤ 45 years was 202(12.2%; 38.9 ± 0.4 years) and the number of patients> 45 years was 1456(87.8%; 64.1 ± 0.3 years).Analyses revealed that females were predominant inthe younger group(P < 0.001). Gastric cancers in the younger patients exhibited more aggressive features and were at a more advanced stage than those in older patients. Precancerous lesions, such as atrophic gastritis and intestinal metaplasia, were observed less frequently in the older than in the younger group(P < 0.001). Molecular characteristics, including overexpression of p53(P < 0.001), overexpression of HER-2(P = 0.006), and MSI(P = 0.006), were less frequent in gastric cancer of younger patients. Prognostic factors in patients with node-negative gastric carcinoma:A comparison with node-positive gastric carcinoma

AIM： To identify the clinicopathological characteristics of lymph node-negative gastric carcinoma, and also to evaluate outcome indicators in the lymph node-negative patients.METHODS： Of 2848 gastric carcinoma patients, 1524 （53.5%） were lymph nede-negative. A statistical analysis was performed using the Cox model to estimate outcome indicators. RESULTS： There was a significant difference in the recurrence rate between lymph nede-negative and lymph node-positive patients （14.4% vs 41.0%, P〈0.001）. The 5-year survival rate was significantly lower in lymph node-positive than in lymph node-negative patients （31.1% vs 77.4%, P〈0.001）. Univariate analysis revealed that the following factors influenced the 5-year survival rate： patient age, tumor size, depth of invasion, tumor location, operative type, and tumor stage at initial diagnosis. The Cox proportional hazard regression model revealed that tumor size, serosal invasion, and curability were independent, statistically significant, prognostic indicators of lymph node-negative gastric carcinoma. CONCLUSION： Lymph node-negative patients have a favorable outcome attributable to high curability, but the patients with relatively large tumors and serosal invasion have a poor prognosis. Curability is one of the most reliable predictors of long-term survival for lymph nodenegative gastric carcinoma patients.

Revascularization of immature necrotic teeth is a reliable treatment alternative to conventional apexogenesis or apexification. In case 1, a 12-year-old boy had his necrotic, immature mandibular left second premolar treated with a revascularization technique. At a 24-month follow-up, periapical radiolucency had disappeared and thickening of the root wall was observed. In cases 2 and 3, a lO-year-old boy had his necrotic, immature, bilateral mandibular second premolars treated with the same modality. At 48-month （in case 2） and 42-month （in case 3） follow-ups, loss of periapical radiolucencies and increases in the root wall thickness were also observed.

Removal of press-through-packs impacted in the upper esophagus using an overtube

Foreign bodies in the upper esophagus should be re-moved as soon as possible to avoid serious complica-tions. However, removals of foreign bodies in the upper esophagus are very difficult, especially if they have sharp edges, such as press-through-packs (PTPs). We expe-rienced four cases of the impacted PTPs in the upper esophagus which was successfully extracted endoscopi-cally with the overtube. Because two edges of PTPs were so firmly impacted in the esophageal wall in all cases, the PTPs were not movable in the upper esophagus. However, after insertion of the overtube, PTPs became movable and were successfully extracted and no serious complications occurred after extraction of PTPs. In one case, insertion of the overtube rapidly expanded the up-per esophagus and PTP progressed to the gastric cavity and it could be extracted with the endoscopic protector hood. The endoscopic removal with the overtube was a simple, safe and effective technique for the removal of the impacted PTPs in upper esophagus.

Randomized clinical trial comparing fixed-time split dosing and split dosing of oral Picosulfate regimen for bowel preparation

AIM To compare the efficacy of fixed-time split dose and split dose of an oral sodium picosulfate for bowel *** This is study was prospective, randomized controlled study performed at a single Institution(2013-058). A total of 204 subjects were assigned to receive one of two sodium picosulfate regimens(i.e., fixed-time split or split) prior to colonoscopy. Main outcome measurements were bowel preparation quality and subject *** There was no statistical difference between the fixedtime split dose regimen group and the split dose regimen group(Ottawa score mean 2.57 ± 1.91 vs 2.80 ± 2.51, P = 0.457). Cecal intubation time and physician's satisfaction of inspection were not significantly different between the two groups(P = 0.428, P = 0.489). On subgroup analysis, for afternoon procedures, the fixed-time split dose regimen was equally effective as compared with the split dose regimen(Ottawa score mean 2.56 ± 1.78 vs 2.59 ± 2.27, P = 0.932). There was no difference in tolerability or compliance between the two groups. Nausea was 21.2% in the fixed-time split dose group and 14.3% in the split dose group(P = 0.136). Vomiting was 7.1% and 2.9%(P = 0.164), abdominal discomfort 7.1% and 4.8%(P = 0.484), dizziness 1% and 4.8%(P = 0.113), cold sweating 1% and 0%(P = 0.302) and palpitation 0% and 1%(P = 0.330), respectively. Sleep disturbance was two(2%) patients in the fixed-time split dose group and zero(0%) patient in the split dose preparation(P = 0.143) *** A fixed-time split dose regimen with sodium picosulfate is not inferior to a split dose regimen for bowel preparation and equally effective for afternoon colonoscopy.
