Volumetric fluid analysis of fixed monthly anti-VEGF treatment in patients with neovascular age-related macular degeneration

·AIM:To evaluate visual outcomes and changes in fluid after administering monthly anti-vascular endothelial growth factor(VEGF)injections to treat neovascular agerelated macular degeneration(n AMD)with subretinal fluid(SRF)and pigment epithelial detachment(PED).·METHODS:This prospective study included eyes with n AMD previously treated with as-needed anti-VEGF injections.The patients were treated with six monthly intravitreal injections of ranibizumab.Quantitative volumetric segmentation analyses of the SRF and PED were performed.The main outcome measures included best-corrected visual acuity(BCVA),and SRF and PED volumes.·RESULTS:Twenty eyes of 20 patients were included in this study.At the 6-month follow-up,BCVA and PED volume did not change significantly(P=0.110 and 0.999,respectively)but the mean SRF volume decreased from 0.53±0.82 mm3 at baseline to 0.08±0.23 mm3(P=0.002).The absorption rate of the SRF volume was negatively correlated with the duration of previous antiVEGF treatment(P=0.029).Seven of the 20 eyes(35%)showed a fluid-free macula and significant improvement in BCVA(P=0.036)by month 6.·CONCLUSION:Quantifying the SRF can precisely determine the patient’s responsiveness to anti-VEGF treatment of n AMD.

Phase engineering is an efficient strategy for enhancing the kinetics of electrocatalytic reactions.Herein,phase engineering was employed to prepare high‐performance phosphorous‐doped biphase(1T/2H)MoS_(2)(P‐BMS)nanoflakes for hydrogen evolution reaction(HER).The doping of MoS_(2)with P atoms modifies its electronic structure and optimizes its electrocatalytic reaction kinetics,which significantly enhances its electrical conductivity and structural stability,which are verified by various characterization tools,including X‐ray photoelectron spectroscopy,high‐resolution transmission electron microscopy,X‐ray absorption near‐edge spectroscopy,and extended X‐ray absorption fine structure.Moreover,the hierarchically formed flakes of P‐BMS provide numerous catalytic surface‐active sites,which remarkably enhance its HER activity.The optimized P‐BMS electrocatalysts exhibit low overpotentials(60 and 72 mV at 10 mA cm^(−2))in H_(2)SO_(4)(0.5 M)and KOH(1.0 M),respectively.The mechanism of improving the HER activity of the material was systematically studied using density functional theory calculations and various electrochemical characterization techniques.This study has shown that phase engineering is a promising strategy for enhancing the H*adsorption of metal sulfides.

Visual and morphological outcomes of vitreomacular traction syndrome in retinitis pigmentosa treated by vitrectomy

Retrospective review of 13 eyes that had undergone pars plana vitrectomy（PPV） for vitreomacular traction（VMT） syndrome in retinitis pigmentosa（RP） and had a postoperative follow-up period ≥9 mo in order to evaluate whether it is necessary to treat VMT in RP by vitrectomy. The 13 RP eyes suffering from VMT were evaluated by means of best corrected visual acuity（BCVA）, anterior and posterior binocular examination, spectral-domain optical coherence tomography（SD-OCT） before and after operation. We detected that although vitrectomy could improve the macular morphology in some RP patients with VMT, visual outcomes might be limited and unpredicted most likely because of the long-standing retinal dysfunction, but the vitrectomy is necessary in order to remain or improve the central vision in some cases.

Traumatic eye injury is the leading cause of unilateral blindness worldwide,especially in developing countries.Open globe injuries,which have the tendency to lead to severe visual impairment,account for about 10%of these injuries(1).The risk for open globe injury was highest in young adults(2)who were actively engaged in economic activities;therefore,open globe injuries not only affect the quality of life and economic condition of patients,but also cause a great deal of cost for society as a whole(3).

Disaster forensic approaches aim to identify the causes of disasters to support disaster risk management.However,few studies have conducted a systematic literature review of scientific articles that labeled themselves as a forensic approach to disasters.This article provides a qualitative analysis of these forensic studies,focusing on five main issues:(1)the methodologies applied;(2)the forensic approaches used in the disaster risk management phases;(3)the hazards addressed;(4)if the methodologies involve social participation,and using what types of participation;and(5)if there are references to urban planning in the scientific studies analyzed.Our results showed a predominance of the Forensic Investigations of Disasters(FORIN)and Post-Event Review Capability(PERC)methodologies used in isolation or combination.There is a need for methodologies that engage people in participatory FORIN,fostering the co-production of knowledge and action research approaches.

Upconverting nanoparticle-containing erythrocyte-sized hemoglobin microgels that generate heat,oxygen and reactive oxygen species for suppressing hypoxic tumors

Inspired by erythrocytes that contain oxygen-carrying hemoglobin(Hb)and that exhibit photo-driven activity,we introduce homogenous-sized erythrocyte-like Hb microgel(μGel)systems(5-6μm)that can(i)emit heat,(ii)supply oxygen,and(iii)generate reactive oxygen species(ROS;1O2)in response to near-infrared(NIR)laser irradiation.Hb μGels consist of Hb,bovine serum albumin(BSA),chlorin e6(Ce6)and erbium@lutetium upconverting nanoparticles(UCNPs;~35 nm)that effectively convert 808 nm NIR light to 660 nm visible light.These Hb μGels are capable of releasing oxygen to help generate sufficient reactive oxygen species(^(1)O_(2))from UCNPs/Ce6 under severely hypoxic condition upon NIR stimulation for efficient photodynamic activity.Moreover,the Hb μGels emit heat and increase surface temperature due to NIR light absorption by heme(iron protoporphyrin IX)and display photothermal activity.By changing the Hb/UCNP/Ce6 ratio and controlling the amount of NIR laser irradiation,it is possible to formulate bespoke Hb μGels with either photothermal or photodynamic activity or both in the context of combined therapeutic effect.These Hb μGels effectively suppress highly hypoxic 4T1 cell spheroid growth and xenograft mice tumors in vivo.

Assessment of disease activity by fecal immunochemical test in ulcerative colitis

AIM To evaluate the efficacy of quantitative fecal immunochemical test(FIT) as biomarker of disease activity in ulcerative colitis(UC).METHODS Between February 2013 and November 2014, a total of 82 FIT results, obtained in conjunction with colonoscopies, were retrospectivelyevaluated for 63 patients with UC. The efficacy of FIT for evaluation of disease activity was compared to colonoscopic findings. Quantitative fecal blood with automated equipment examined from collected feces. Endoscopic disease severity were assessed using the Mayo endoscopic subscore(MES) classification. The extent of disease were classified by proctitis(E1), left sided colitis(E2), and extensive colitis(E3). Clinical activity were subgrouped by remission or active.RESULTS All of 21 patients with MES 0 had negative FIT( 134.89 ng/m L. The sensitivity, specificity, positive predictive value(PPV), negative predictive value(NPV) and accuracy of negative FIT about mucosal healing were 73.33%, 81.82%, 91.49%, 51.43% and 73.17%, respectively. The sensitivity, specificity, PPV, NPV and accuracy of predictive value of positive FIT(cutoff value > 100 ng/mL) about active disease status were 45.45%, 93.33%, 71.43%, 82.35%and 26.83%, respectively. Among patients with clinical remission, FIT was negative in 31(81.6%) of 38 cases, with a mean fecal hemoglobin concentration of 6.12 ng/mL(range, negative to 80.9 ng/mL) for this group of patients. Among patients with clinical active disease, FIT was negative in 16(36.4%) out of 44 cases, with a mean fecal hemoglobin concentration > 167.4 ng/mL for this group of patients. FIT was positively correlated with endoscopic activity(r = 0.626, P < 0.01) and clinical activity(r = 0.496, P < 0.01). But, FIT did not correlate with the extent of disease(r =-0.047, P = 0.676)CONCLUSION Quantitative FIT can be a non-invasive and effective biomarker for evaluation of clinical and endoscopic activity in UC, but not predict the extent of disease.

Efficacy and safety of limited endoscopic sphincterotomy before self-expandable metal stent insertion for malignant biliary obstruction

AIM To evaluate the safety and efficacy of limited endoscopic sphincterotomy (ES) before placement of selfexpandable metal stent(SEMS).METHODS This was a retrospective analysis of 244 consecutive patients with unresectable malignant biliary obstruction, who underwent placement of SEMSs following limited ES from December 2008 to February 2015. The diagnosis of malignant biliary obstruction and assessment of patient eligibility for the study was established by a combination of clinical findings, laboratory investigations, imaging and pathological results. All patients were monitored in the hospital for at least 24 h following endoscopic retrograde cholangio pancreatography(ERCP). The incidence of immediate or early post-ERCP complications such as post-ERCP pancreatitis(PEP) and bleeding related to limited ES were considered as primary outcomes. Also, characteristics and complications according to the cancer type were classified.RESULTS Among the 244 patients included, the underlying diagnosis was cholangiocarcinoma in 118 patients,pancreatic cancer in 79, and non-pancreatic or nonbiliary malignancies in the remaining 47 patients. Early post-ERCP complications occurred in 9 patients(3.7%), with PEP in 7 patients (2.9%; mild, 6; moderate, 1) and mild bleeding in 2 patients (0.8%). There was no significant association between the incidence of post-ERCP complications and the type of malignancy(cholangiocarcinoma vs pancreatic cancer vs others, P = 0.696) or the type of SEMS used (uncovered vs covered, P = 1.000). Patients who had more than one SEMS placed at the first instance were at a significantly higher risk of post-ERCP complications (one SEMS vs two SEMS, P = 0.031). No other factors were predictive of post-ERCP complications.CONCLUSION Limited ES is feasible and safe, and effectively facilitates the placement of SEMS, without any significant risk of PEP or severe bleeding.

Influence of dynamic power dissipation on Si MRM modulation characteristics

We experimentally observe that Si micro-ring modulator（MRM） modulation characteristics are strongly influenced by the modulation data rate and the data pattern and determine this influence is due to the temperature increase caused by dynamic power dissipation within the Si MRM device. We also quantitatively determine the amount of Si MRM resonance wavelength shift due to different modulation data rates, data patterns, and modulation voltages. Our results should be of great help for achieving reliable and optimal modulation characteristics for Si MRMs.

Isochrone Fitting of Galactic Globular Clusters—Ⅵ. High-latitude Clusters NGC 5024 (M53), NGC 5053, NGC 5272 (M3), NGC 5466, and NGC7099 (M30)

We fit various color–magnitude diagrams(CMDs) of the high-latitude Galactic globular clusters NGC 5024(M53),NGC 5053, NGC 5272(M3), NGC 5466, and NGC 7099(M30) by isochrones from the Dartmouth Stellar Evolution Database and Bag of Stellar Tracks and Isochrones for α–enrichment [α/Fe] = +0.4. For the CMDs, we use data sets from Hubble Space Telescope, Gaia, and other sources utilizing, at least, 25 photometric filters for each cluster. We obtain the following characteristics with their statistical uncertainties for NGC 5024, NGC 5053,NGC 5272, NGC 5466, and NGC 7099, respectively: metallicities [Fe/H] =-1.93 ± 0.02,-2.08 ± 0.03,-1.60 ± 0.02,-1.95 ± 0.02, and-2.07 ± 0.04 dex with their systematic uncertainty 0.1 dex; ages 13.00 ± 0.11,12.70 ± 0.11, 11.63 ± 0.07, 12.15 ± 0.11, and 12.80 ± 0.17 Gyr with their systematic uncertainty 0.8 Gyr;distances(systematic uncertainty added) 18.22 ± 0.06 ± 0.60, 16.99 ± 0.06 ± 0.56, 10.08 ± 0.04 ± 0.33, 15.59 ±0.03 ± 0.51, and 8.29 ± 0.03 ± 0.27 kpc; reddenings E(B-V) = 0.023 ± 0.004, 0.017 ± 0.004, 0.023 ± 0.004,0.023 ± 0.003, and 0.045 ± 0.002 mag with their systematic uncertainty 0.01 mag; extinctions AV= 0.08 ± 0.01,0.06 ± 0.01, 0.08 ± 0.01, 0.08 ± 0.01, and 0.16 ± 0.01 mag with their systematic uncertainty 0.03 mag, which suggest the total Galactic extinction AV= 0.08 across the whole Galactic dust to extragalactic objects at the North Galactic Pole. The horizontal branch morphology difference of these clusters is explained by their different metallicity, age, mass-loss efficiency, and loss of low-mass members in the evolution of the core-collapse cluster NGC 7099 and loose clusters NGC 5053 and NGC 5466.
