Dear Editor,Despite the introduction of novel front-line therapies including rituximab plus high-dose cytarabine followed by consolidative autologous stem cell transplantation(ASCT)and salvage therapy with bendamustine,lenalidomide or bortezomib,mantle cell lymphoma(MCL)is still considered incurable and most patients experience relapse or refractory(RR)disease.

Regeneration of waste SCR catalyst by air lift loop reactor

A solution of 0.1 mol/L to 1.0 mol/L H2SO4 can dissolve alkali metals and alkaline earth metals which weaken an active site of SCR catalyst. The waste catalyst washed with 0.5 mol/L H2SO4 regained the best catalytic activity. When a concentration of the sulfuric acid is less than 0.5 mol/L, sufficient cleaning effects cannot be obtained. In contrast, when the concentration is greater than 1.0 tool/L, the active components, vanadium and tungsten are undesirably eluted. The total BET surface of the catalyst regenerated by air lift loop reactor showed almost the same as that of fresh catalyst due to the removal of insoluble compounds which may be penetrated into pores of catalyst. The addition of a solution of 0.075 mol/L ammonium vanadate （NHnVO3） and 0.075 mol/L ammonium paratungstate （5（NH4）20· 12WO3-5H20） to 0.1 mol/L H2SO4 significantly increases the activity of the waste catalyst.

Current status and challenges in sentinel node navigation surgery for early gastric cancer

Although a number of feasibility studies for sentinel node （SN） concepts in gastric cancer have been conducted since 2000, there remains a debate regarding detailed detection techniques and oncological safety. Two important multicenter phase II clinical trials were performed in Japan that used different methods and reached different conclusions; one confirmed acceptable results with a false-negative rate of 7%, and the other showed an unacceptably high false-negative rate of 46.4%. The Sentinel Node Oriented Tailored Approach （SENORITA） trial is a multicenter randomized controlled phase 1/I trial being performed in Korea. Patient enrollment is now complete and the long-term results are currently awaited. Recently, an image-guided SN mapping technique using infrared ray/fluorescence was introduced. This method might be a promising technology because it allows the clear visualization of SNs. With regard to the primary tumor, the non-exposed endoscopic wall-inversion surgery technique and non-exposure endolaparoscopic full-thickness resection with simple suturing technique have been reported. These methods prevent abdominal infection and tumor seeding and can be good alternatives to conventional laparoscopic gastric wedge resection. For indications, SN navigation surgery can be extended to patients who underwent non-curative endoscopic resection. Although a few studies have been performed on these patients, sentinel concepts may be beneficial to patients as they omit the need for additional gastrectomy. SN navigation surgery can lead to actual organ-preserving surgery and plays a key role in improving the quality of life of patients with early gastric cancer in the future.

Methods to Minimize Optical Noise That Degrade Fluorescence Efficiency of Optical Probe for Near-Infrared Tracking in Surgical Environment

In the medical field, there are growing interests in applied research such as in vivo fluorescence monitoring because of excellent body transmission characteristic of the near-infrared light. However, optical noise by excitation light and illumination equipment for medical applications such as interior light, surgical light decrease efficiency of the fluorescent signal when observers such as surgeons confirm fluorescence signals in medical field. To solve these problems in medical field, we have analyzed external noise factors by effect on image realization, quantification of optical noise generation by external factors, and have suggested methods of minimize the optical noise in this paper. In case of fluorescence imaging in the operating room, it has been confirmed that fluorescent excitation light, interior light and surgical light are factors to generate optical noise. To acquire near-infrared fluorescence images and to compare fluorescence contrast under conditions of darkroom, interior light and surgical light, light emitting diodes (LEDs) sources that have peak wavelength at 740, 760 and 780 nm respectively were used as excitation light sources. In addition, short-pass filter which has transmission edge at 775 nm has been applied to minimize the optical noise in each external noise factor. By comparing contrast of each image before and after use of the short-pass filter, we confirmed that optical noise reduced 49%, 56% and 66% in external noise factors respectively.

Optical Probe for Near-Infrared (NIR) Fluorescence Signal Detection with High Optical Performance and Thermal Stability

We propose a new optical probe for near-infrared (NIR) fluorescence signal detection with high optical performance and thermal stability. The optical probe is composed of an optical source part for efficient excitation of NIR fluorescence signal, a heat dissipation part for stable operation of the NIR fluorescence probe, and an optical detection part for efficient detection of NIR fluorescence signal. From a simulation by use of an optical simulation tool, Light ToolsTM, we could confirm that the optical probe has optical propagation efficiency of 79.6% in case of using a circular detector with 20 cm in diameter located at 20 cm in distance from the optical source. From a measurement of temperature variation of the optical probe, we could also confirm that the optical probe has thermal stability with a standard deviation of 2.19°C under room temperature condition. Finally, from an evaluation of fluorescence image quality, we could confirm that an optical noise which can bring on by overlapped band between optical spectrum of the optical source for fluorescence excitation and optical spectrum of the emitted fluorescence signal decreased effectively in the optical probe.

Novel imaging system for positioning of the indocyanine green (ICG) target;visible projection of the near-infrared fluorescence image

Background: Even though NIR fluorescence imaging has many advantages in SLN mapping and cancer detection, NIR fluorescence imaging shows a serious drawback that NIR cannot be detected by the naked eye without any detectors. This limitation further disturbs accurate SLN detection and adequate tumor resection resulting in the presence of cancerous cells near the boundaries of surgically removed tissues. Materials and methods: To overcome the drawback of the conventional NIR imaging method, we suggest a novel NIR imaging system which can make the NIR fluorescence image visible to the naked eye as NIR fluorescence image detected by a video camera is processed by a computer and then projected back onto the NIR fluorescence excitation position with a projector using conspicuous color light. Image processing techniques were used for projection onto the exact position of the NIR fluorescence image. Also, we implemented a phantom experiment to evaluate the performance of the developed NIR fluorescence projection system by use of the ICG. Results: The developed NIR fluorescence projection system was applied in normal mouse model to confirm the usefulness of the system in the clinical field. A BALB/c nude mouse was prepared to be applied in normal mouse model and 0.25 mg/ml stock solution of the ICG was injected through a tail vein of the mouse. From the application in normal mouse model, we could confirm that the injected ICG stayed in the liver of the mouse and verify that the projection system projected the ICG fluorescence image at the exact location of the ICG by performing laparotomy of the mouse. Conclusions: From the application in normal mouse model, we could verify that the ICG fluorescence image was precisely projected back on the site where ICG fluorescence generated. It can be demonstrated that the NIR fluorescence projection system can make it possible to visualize the invisible NIR fluorescence image and to realize that SLN mapping and cancer detection in clinic

Objective:The laparoscopic approach for locally advanced gastric cancer has recently been adopted based on the results of several randomized controlled trials(RCTs).However,findings from RCTs have not been examined at the national *** study aimed to investigate the external validity of the Korean Laparoscopic Gastrointestinal Surgery Study-02(KLASS-02)trial involving 13 tertiary hospitals,using data from the Korean Gastric Cancer Association(KGCA)-led nationwide survey involving 68 tertiary or general ***:Data on patients who underwent laparoscopic or open distal gastrectomy for pathological stageⅠB-ⅢC gastric cancer under the same conditions were collected from the KLASS-02 trial and the KGCA nationwide survey *** outcomes were assessed for each dataset and multivariable analyses were performed to examine the effect of the laparoscopic approach on surgical ***:The laparoscopic group had a lower overall complication rate than the open group in both KLASS-02and KGCA datasets(16.1%vs.23.5%for the KLASS-02 and 12.6%vs.19.6%for the KGCA).Moreover,the laparoscopic group had fewer wound problems,and fewer gradeⅡ,Ⅲa,andⅣcomplications than the open group in the KGCA data(0.8%vs.3.4%,5.8%vs.10.4%,2.3%vs.3.7%,and 0.5%vs.1.4%,respectively),which were not observed in the KLASS-02 *** analyses revealed that the laparoscopic approach was not associated with overall complications,but reduced wound problems and more harvested lymph nodes in the KGCA survey data(adjusted odds ratios,0.19 for wound problems,adjustedβcoefficient 4.39 for number of harvested lymph nodes),which were not shown in the KLASS-02 ***:The safety and feasibility of the laparoscopic approach for locally advanced gastric cancer were validated at a national *** laparoscopic approach for locally advanced gastric cancer can be implemented in the Republic of Korea.
