Few long-term follow-up studies have compared the changes in renal function according to the type of statin used in Korea.We compared the long-term effects of statin intensity and type on the changes in the glomerular filtration rate(GFR).We extracted data of patients who took statin for the first time.We analyzed whether or not different statins affect the changes in GFR at 3 months after baseline and 4 years after.We included 3678 patients and analyzed the changes in GFR.The GFR decreased by 3.2%±0.4%on average 4 years after the first statin prescription,indicating statistically significant deterioration(from 83.5±0.4 mL/min/1.73 m2 to 79.9±0.4 mL/min/1.73 m2,P<0.001).When comparing the GFR among different statins,significant differences were observed between atorvastatin and fluvastatin(−5.3%±0.7%vs.1.2%±2.2%,P<0.05)and between atorvastatin and simvastatin(−5.3%±0.7%vs.−0.7%±0.8%,P<0.05).In pitavastatin(odds ratio[OR]=0.64,95%confidence interval[CI]=0.46-0.87,P<0.005)and simvastatin(OR=0.69,95%CI=0.53-0.91,P<0.008),the GFR rate that decreased by<60 mL/min/1.73 m2 was significantly lower than that of atorvastatin.Regarding long-term statin intake,GFR changed with the type of statin.This work is the first in Korea to compare each statin in terms of changes in the GFR after the statin prescription.

Background： Evaluating a change in the screening rate for hepatocellular carcinoma （HCC） is critical for understanding screening implementation, and whether targeted population groups are receiving proper screening. This study examined recent nationwide changes in HCC screening use among hepatitis B virus （HBV）-infected populations after the introduction of the Korean National Cancer Screening Program and predictors of screening adherence. Methods： We analyzed 165 and 276 participants 〉40 years of age who were hepatitis B surface antigen-positive fiom 2001 （14,936 participants） to 2010-2011 （9159 participants） Korea National Health and Nutrition Examination Surveys, respectively. Demographic data, socioeconomic factors, and HCC screening use were collected by means of self-reported questionnaires. Results： The rate of HCC screening within the previous 2 years increased significantly from 17.5% in 2001 to 40.3% in 2010- 20I 1 （P〈 0.0001 ）. The rate of HCC screening use increased from 2001 to 2010-2011 in all study populations. Subjects who had a higher income staius and were aware of their infection were more likely to have undergone recent HCC screening. Conclusions： This study showed a substantial increase in HCC screening in high-risk HBV-infected subjects from 2001 to 2010-2011. However, the HCC screening participation rate remained suboptimal despite the introduction of the nationwide screening program. Efforts should be made to identiry high-risk individuals and increase attendance at HCC screening events among high-risk groups.

In this study,the effects of Al addition on the corrosion behavior of pure Mg with controlled impurity contents were systematically analyzed according to the processing history.The results revealed that the corrosion behavior of high-purity Mg-Al alloys is strongly related to changes in the microstructure,including theβphase and Al-Mn or Al-Fe phases,and the protectiveness of the surface film according to the Al content and processing history.In the as-cast alloys,the corrosion rate increased due to the increase ofβphase as the Al content increased,but in the as-extruded alloys,the corrosion rate,which was high due to intermetallic compounds caused by impurities in the low Al alloy,decreased as the Al content increased,and then increased again.This is due to the combined effect of the increase of theβphase and decrease of the impurity effect,and the increase of the dissolved Al content.The results suggest that it is necessary to analyze the effect of alloying elements on the corrosion behavior of pure Mg with information concerning the impurity content and processing history.

Diagnostic delay in inflammatory bowel disease increases the risk of intestinal surgery

AIM To investigate the factors affecting diagnostic delay and outcomes of diagnostic delay in inflammatory bowel disease(IBD) METHODS We retrospectively studied 165 patients with Crohn's disease(CD) and 130 patients with ulcerative colitis(UC) who were diagnosed and had follow up durations > 6 mo at Korea University Ansan Hospital from January 2000 to December 2015. A diagnostic delay was defined as the time interval between the first symptom onset and IBD diagnosis in which the 76^(th) to 100^(th) percentiles of patients were diagnosed.RESULTS The median diagnostic time interval was 6.2 and 2.4 mo in the patients with CD and UC, respectively. Among the initial symptoms, perianal discomfort before diagnosis(OR = 10.2, 95%CI: 1.93-54.3, P = 0.006) was associated with diagnostic delays in patients with CD; however, no clinical factor was associated with diagnostic delays in patients with UC. Diagnostic delays, stricturing type, and penetrating type were associated with increased intestinal surgery risks in CD(OR = 2.54, 95%CI: 1.06-6.09; OR = 4.44, 95%CI: 1.67-11.8; OR = 3.79, 95%CI: 1.14-12.6, respectively). In UC, a diagnostic delay was the only factor associated increased intestinal surgery risks(OR = 6.81, 95%CI: 1.12-41.4).CONCLUSION A diagnostic delay was associated with poor outcomes, such as increased intestinal surgery risks in patients with CD and UC.

Dear Editor Hepatocellular carcinoma(HCC)is among the most com-mon cancers worldwide,causing about 600,000 deaths annully[1].In HCC,stem cell-like characteristics,which drive early recurrence and therapy resistance,are major contributors to poor prognosis[2].In this current study,we integrated and analyzed gene expression data from human fetal liver cells and primary HCC tumors(n=1231)and.uncovered two clinically and biologically distinct hepatic stem cell(HS)subtypes,potential biomarkers associated with these subtypes,and a potential new therapeutic inter-vention for these subtypes.

Aim:To investigate the immunoregulatory functions of water extracts of Hericiumerinaceum (WEHE) focusing on natural killer (NK) cell-based anticancer activities.Methods:Mouse splenocytes or purely isolated NK ceils were stimulated with1-100 mg/L WEHE for 24 h followed by co-culture with51Cr-labled Yac-1 cells for4 h,then NK cell-derived cytolytic activity was measured using a radio-releaseassay.Neutralizing antibodies against mouse interleukin-12 (IL-12) were addedinto the WEHE-stimulated splenocytes,thereafter,cytotoxicity was measured toexamine the involvement of IL-12.RT-PCR and ELISA analyses were performed toconfirm the induction of transcription and the translation of IL-12 and interferon-gamma (IFN-gamma) in the WEHE-treated splenocytes.Results:WEHE enhancedthe cytolytic activity of total splenocytes towards Yac-1 ceils in a dose-dependentmanner.However,this activation was not observed when the NK cells isolatedfrom the splenocytes were treated with WEHE.Furthermore,the treatment withantibodies against IL-12 abolished the effect of WEHE on splenocyte-derivedcytolytic activity.RT-PCR and ELISA analyses showed the induction of IL-12 andIFN-gamma in the WEHE-treated splenocytes.Conclusion:WEHE indirectlyactivates the cytolytic ability of NK cells via the induction of IL-12 in totalsplenocytes,and possibly via other immuno-mediators or cellular components.

AIM: The genes were divided into seven categories according to biological function; apoptosis-reiated, immune response-related, signal transduction-related, cell cyclerelated, cell growth-related, stress response-related and transcription-related genes.METHODS: We compared the gene expression profiles of SNU-C4 cells between amygdalin-treated (5 mg/mL,24 h) and non-treated groups using cDNA microarray analysis. We selected genes downregulated in cDNA microarray and investigated mRNA levels of the genes by RT-PCR. RESULTS: Microarray showed that amygdalin downregulated especially genes belonging to cell cycle category: exonuclease 1 (EXO1), ATP-binding cassette, sub-family F, member 2 (ABCF2), MRE11 meiotic recombination 11 homolog A (MRE114), topoisomerase (DNA) I (TOP1), and FK506 binding protein 12-rapamycin-associated protein 1 (FRAP1). RT-PCR analysis revealed that mRNA levels of these genes were also decreased by amygdalin treatment in SNU-C4 human colon cancer cells.CONCLUSION: These results suggest that amygdalin have an anticancer effect via downregulation of cell cycle-related genes in SNU-C4 human colon cancer cells,and might be used for therapeutic anticancer drug.

AIM: To evaluate the long-term efficacy adefovir(ADV)-based combination therapies in entecavir(ETV)-resistant chronic hepatitis B(CHB) patients. METHODS: F i fty CHB pat ient s wi t h genotypic resistance to ETV at 13 medical centers in South Korea were included for the analysis. All the patients received rescue therapy with the combination of ADV plus ETV(ADV/ETV,n = 23) or ADV plus lamivudine(LMV)(ADV/LMV,n = 27) for more than 12 mo. Patients were monitored at least every 3-4 mo during ADV-based combination therapy by clinical examination as well as biochemical and virological assessments. Hepatitis B virus(HBV) DNA levels were measured by realtime PCR and logarithmically transformed for analysis. Cumulative rates of virologic response(VR; HBV DNA < 20 IU/m L) were calculated using the Kaplan-Meier method,and the difference was determined by a logrank test. Multivariate logistic regression and Cox proportional hazards models were used to identify independent risk factors significantly associated with short-term and long-term VR,respectively.RESULTS: Baseline median HBV DNA levels were 5.53(2.81-7.63) log10 IU/m L. The most commonly observed ETV genotypic mutation sites were rt184 and rt202. Patients were treated for a median of 27(12-45) mo. Overall,cumulative VR rates at 6,12,24,and 36 mo were 26%,36%,45%,and 68%,respectively. Patients treated with the ADV/ETV combination showed higher cumulative VR rates(35%,43%,65%,and 76%,respectively) than those with the ADV/LAM combination(18%,30%,30%,and 62%,respectively; P = 0.048). In the multivariate analysis,low baseline HBV DNA levels(< 5.2 log10 IU/m L) and initial virologic response at 3 mo(IVR-3; HBV DNA < 3.3 log10 IU/m L after 3 mo) were independent predictive factors for VR. Patients with favorable predictors achieved cumulative VR rates up to 90% at 36 mo. During the same period,the cumulative incidence of virologic breakthrough was as low as 6% in patients with the both favorable predictors.CONCLUSION: If tenofovir is not available,ADV/ETV combination could
