Compensating the impurities on the Cu surface by MOFs for enhanced hydrocarbon production in the electrochemical reduction of carbon dioxide

Copper(Cu)provides a cost-effective means of producing value-added fuels through the electrochemical reduction of carbon dioxide(CO_(2)RR).However,we observed the production of hydrocarbons via CO_(2)RR on commercial Cu films is less efficient because of the surface impurities,i.e.,Fe.Carbon monoxide(CO),a reaction intermediate of CO_(2)RR to hydrocarbons,binds strongly to the Fe sites and interrupts the production of hydrocarbons,resulting in an active hydrogen evolution reaction(HER).Herein,we report a method of blocking the effect of Fe impurities on the Cu surface through the preferential growth of nano-sized metal-organic frameworks(MOFs)on Fe site.When zirconium(Zr)-based MOFs(UiO-66)forms a compensating layer on Cu film via the terephthalic acid(TPA)-Fe coordination bond,the Ui O-66 coated Cu film(UiO-66@Cu)presents significantly improved hydrocarbon Faradaic efficiency(FE)of 37.59%compared to 14.68%FE on commercial Cu film(99.9%purity)by suppressing HER.According to X-ray photoelectron spectroscopy(XPS)analysis,the UiO-66 ligand binds to entire metallic Fe site on the Cu surface,while metallic Cu is retained.Thus,UiO-66@Cu provides active sites of Cu for CO_(2)RR and leads to highly efficient and selective production of hydrocarbons.

Antiphospholipid syndrome with renal and splenic infarction after blunt trauma:A case report

BACKGROUND In trauma patients,bleeding is an immediate major concern.At the same time,there are few cases of acute vascular occlusion after blunt trauma,and it is unclear what assessment and diagnosis should be considered for these cases.Herein,we describe a patient diagnosed with antiphospholipid syndrome after a hypercoagulable workup for acute renal and splenic vascular occlusion due to blunt trauma.CASE SUMMARY A 20-year-old man was admitted to the emergency department with abdominal pain after hitting a tree while riding a sled 10 h ago.He had no medical history.Radiological investigations revealed occlusion of the left renal artery with global infarction of the left kidney and occlusion of branches of the splenic artery with infarction of the central portion of the spleen.Attempted revascularization of the left renal artery occlusion through percutaneous transluminal angioplasty failed due to difficulty in passing the wire through the total occlusion.Considering the presence of acute multivascular occlusions in a young man with low cardiovascular risk,additional laboratory tests were performed to evaluate hypercoagulability.The results suggested a high possibility of antiphospholipid syndrome.Treatment with a subcutaneous injection of enoxaparin was started and changed to oral warfarin after two weeks.The diagnosis was confirmed,and he continued to visit the rheumatology outpatient clinic while taking warfarin.CONCLUSION A hypercoagulable workup can be considered in trauma patients with acute multivascular occlusion,especially in young patients with low cardiovascular risk.

Most commercially available herbicides contain surfactants as co-formulants to increase adhesion and absorption by plant leaves.Ethoxylated amines,one of the most used surfactants,are non-ionic and derived from animal fats.They represent a class of surfactants with similar structural features,including polyethoxylated tallow amine(POEA).POEA is widely used in glyphosate,glufosinate-containing herbicides.In 2015,the European Food Safety Society(EFSA)concluded that POEA was more toxic than glyphosate when tested in glyphosate-based formulations.[1]They also attributed the poisoning following ingestion by humans to the presence of POEA.

BACKGROUND Omental infarction(OI)is a surgical abdominal disease that is not common in adults and is very rare in children.Similar to various acute abdominal pain diseases including appendicitis,diagnosis was previously achieved by diagnostic laparotomy but more recently,ultrasonography or computed tomography(CT)examination has been used.CASE SUMMARY A 6-year-old healthy boy with no specific medical history visited the emergency room with right lower abdominal pain.He underwent abdominal ultrasonography by a radiologist to rule out acute appendicitis.He was discharged with no significant sonographic finding and symptom relief.However,the symptoms persisted for 2 more days and an outpatient visit was made.An outpatient abdominal CT was used to make a diagnosis of OI.After laparoscopic operation,his symptoms resolved.CONCLUSION In children’s acute abdominal pain,imaging studies should be performed for appendicitis and OI.

Coronary sinus endocarditis in a hemodialysis patient:A case report and review of literature

BACKGROUND Infective endocarditis is more common in hemodialysis patients than in the general population and is sometimes difficult to diagnose.Isolated coronary sinus(CS)vegetation is extremely rare and has a good prognosis,but complicated CS vegetation may have a poorer clinical course.We report a case of CS vegetation accidentally found via echocardiography in a hemodialysis patient with undifferentiated shock.The CS vegetation may have been caused by endocardial denudation due to tricuspid regurgitant jet and subsequent bacteremia.CASE SUMMARY A 91-year-old man with dyspnea and hypotension was transferred from a nursing hospital.He was on regular hemodialysis and had a history of severe grade of tricuspid regurgitation.There was no leukocytosis or fever upon admission.Repetitive and sequential blood cultures revealed absence of microorganism growth.Chest computed tomography showed lung consolidation and a large pleural effusion.A mobile band-like mass on the CS,suggestive of vegetation,was observed on echocardiography.We diagnosed him with infective endocarditis involving the CS,pneumonia,and septic shock based on echocardiographic,radiographic,and clinical findings.Infusion of broad-spectrum antibiotics,fluid resuscitation,inotropic support,and ventilator care were performed.However,the patient died from uncontrolled infection and septic shock.CONCLUSION CS vegetation can be fatal in hemodialysis patients with impaired immune systems,especially when it delays the diagnosis.

Anatomical resection of hepatocellular carcinoma: A critical review of the procedure and its benefits on survival

Hepatocellular carcinoma(HCC) is the sixth most common type of cancer and the third most frequent cause of cancer-related death. Advances in preoperative assessment of HCC(e.g., imaging studies and liver function tests), surgical techniques, and postoperative care have improved the surgical outcomes and survival of patients who undergo hepatic resection for HCC. However, in the last 20 years, the long-term survival after hepatectomy has remained unsatisfactory owing to the high rates of local recurrence and multicentric occurrence. Anatomical liver resection(AR) was introduced in the 1980 s. Although several studies have revealed tangible benefits of AR for HCC, these benefits are still debated. Because most HCCs occur in patients with liver cirrhosis and poor hepatic function, there are many factors that affect survival, including the surgical method. Nevertheless, many studies have documented the perioperative and long-term benefits of AR in various conditions. In this article, we review the results of several recently published, well-designed comparative studies of AR, to investigate whether AR provides real benefits on survival outcomes. We also discuss the potential pitfalls associated with this approach.

Pathological findings following radical prostatectomy in patients who are candidates for active surveillance： impact of varying PSA levels

Active surveillance is an acceptable treatment option in men with a low-risk prostate cancer. In the present study, we have retrospectively reviewed the outcomes of 509 men who fit the criteria for active surveillance but selected radical prostatectomy. Then, the impact of varying prostate-specific antigen （PSA） levels on the risk of upstaging and upgrading in these patients was assessed. Pathological characteristics of patients who fulfilled the inclusion criteria under three active surveillance criteria--those of the University of California-San Francisco, the National Cancer Institute and the European Association of Urology--were examined. The proportion of men who were deemed candidates for active surveillance but were subsequently upstaged or upgraded was determined. Of 509 patients, 186 （36.5%）, 132 （25.9%） and 88 （17.3%） men fulfilled the active surveillance criteria, respectively. Upgrading （Gleason scores 7-10） ranged from 32.8% to 38.6%, while upstaging （≥ pT3） ranged from 10.2% to 12.5%, depending on the three active surveillance criteria. After a median follow-up of 24 months, three patients developed a biochemical recurrence. When the impact of varying PSA levels was examined using a test for trend analysis in the context of PSA for each protocol, rates of upstaging were lower in men with PSA 〈4 ng m1-1. However, there was no impact of varying PSA levels on upgrading. In conclusion, commonly used active surveillance protocols carry the risks of upgrading and upstaging. More reliable and accurate markers are needed to better stratify the risks of men who are appropriate candidates for active surveillance.

Usage patterns of emergency medical services in Korea: analysis of patient flow

Background:This study used the National Emergency Department Information System (NEDIS) data to analyze the flow of emergency and critical emergency patients and to identify the patterns of emergency medical service usage in Korea.Methods:The relevance index (RI) and commitment index (CI) were calculated from the 2016 NEDIS data. In this study, the number of clusters was determined using NbClust, and cluster analysis was used to analyze the usage patterns of emergency and critical emergency patients.Results:The RI and CI were calculated using 8,389,766 cases of 214 districts. The results of the RI and CI suggested that there were 3 types of clusters among the emergency patients. In Cluster 1, 54 districts (25.2%) had low RI and high CI, and it was of outflow type. Cluster 2 was categorized as the influx-type in 58 districts (27.1%) irrespective of RI and low CI. Cluster 3 was categorized as the self-sufficient type found in 102 districts (47.7%), with high RI and high CI. The cluster analysis of the critical emergency patients was divided into 2 types. Cluster 1 was categorized as outflow type with high CI found in 129 districts (60.3%), while Cluster 2 was categorized as inflow type with low CI found in 85 districts (39.7%).Conclusions:This study elucidates the regional status of usage patterns of emergency and critical emergency patients in Korea. This study might serve as a basis for the establishment and selection of emergency medical service areas and vulnerable emergency medical service areas.

Differences in respiration components and their dominant regulating factors across three alpine grasslands on the Qinghai−Tibet Plateau

Greenhouse gases(GHGs)emissions from high-cold terrestrial ecosystems underlain by permafrost on the Qinghai–Tibet Plateau(QTP)have received widespread attention.However,the dominant factors regulating ecosystem respiration(Re)and its components(soil respiration Rs and heterotrophic respiration Rh)and how the differences in carbon emissions from different ecotypes and seasons remain are still unclear.We conducted a 2-year field investigation(August 2018 to October 2020)and applied the structural equation model(SEM)to clarify the changes in the factors controlling the respiration components during different seasons.The results indicate that the R_(e)and its controlling factors in three alpine grassland ecosystems(alpine steppe,alpine meadow,and swamp meadow)vary with seasons.Furthermore,autotrophic respiration(Ra)contributes the most to the seasonal changes in R_(e).The R_(e)gradually increases in the early growing season and rapidly decreases in the late growing season.Rh remains relatively stable during the year.Under these seasonal variations in the respiration components,the dominant factors controlling R_(e)in the nongrowing season are the temperature of the atmosphere–soil interface(heat flux,atmospheric temperature,and soil temperature at 5 cm depth)and microbial activity(microbial carbon and pH)with the variable importance projections>1.5.During the growing season,the dominant factors regulating R_(e),Rs,and Rh are the soil temperature with a standardized direct effect(SDE)of 0.424,soil nutrient conditions(total nitrogen and pH)with SDEs of 0.570–0.614,and microbial activity(microbial carbon)with a SDE of 0.591,respectively.In addition,meteorological conditions have an important impact on the respiration components during the growing season.Specifically,the atmospheric vapor pressure is the dominant factor regulating the three respiration components(standardized total effects=0.44−0.53,p<0.001).The optimal soil water contents during the growing season(water content at which R_(e)reaches the maximum)are 10%
