Paradoxical carbon dioxide embolism during laparoscopic hepatectomy without intracardiac shunt:A case report

BACKGROUND Laparoscopic hepatectomy has recently become popular because it results in less bleeding than open hepatectomy.However,CO_(2)embolism occurs more frequently.Most CO_(2)embolisms during laparoscopic surgery are self-resolving and non-symptomatic;however,severe CO_(2)embolism may cause hypotension,cyanosis,arrhythmia,and cardiovascular collapse.In particular,paradoxical CO_(2)embolisms are highly likely to cause neurological deficits.We report a case of paradoxical CO_(2)embolism found on transesophageal echocardiography(TEE)during laparoscopic hepatectomy,although the patient had no intracardiac shunt.CASE SUMMARY A 71-year-old man was admitted for laparoscopic left hemihepatectomy.During left hepatic vein ligation,the inferior vena cava was accidentally torn.We observed a sudden drop in oxygen saturation to 85%,decrease in systolic blood pressure(SBP)below 90 mm Hg,and reduction in end-tidal CO_(2)to 24 mm Hg.A“mill-wheel”murmur was auscultated over the precordium.The fraction of inspired oxygen was increased to 100%with 5 cm H2O of positive end-expiratory pressure(PEEP)and hyperventilation was maintained.Norepinephrine infusion was increased to maintain SBP above 90 mm Hg.A TEE probe was inserted,revealing gas bubbles in the right side of the heart,left atrium,left ventricle,and ascending aorta.The surgeon reduced the pneumoperitoneum pressure from 17 to 14 mm Hg and repaired the damaged vessel laparoscopically.Thereafter,the patient’s hemodynamic status stabilized.The patient was transferred to the intensive care unit,recovering well without complications.CONCLUSION TEE monitoring is important to quickly determine the presence and extent of embolism in patients undergoing laparoscopic hepatectomy.

Unveiling anomalous lattice shrinkage induced by Pi-backbonding in Prussian blue analogues

Transition-metal(TM)-based Prussian blue and its analogues(TM-PBAs) have attracted considerable attention as cathode materials owing to their versatile ion storage capability with tunable working voltages. TM-PBAs with different crystal structures, morphologies, and TM combinations can exhibit excellent electrochemical properties because of their unique and robust host frameworks with well-defined ionic diffusion channels. Nonetheless, there is still a lack of understanding regarding the performance dependence of TM-PBAs on structural changes during charging/discharging processes. In this study, in situ X-ray diffraction and X-ray absorption fine structure analyses elucidate the TMdependent structural changes in a series of TM-PBAs during the charging and discharging processes.During the discharging process, the lattice volume of Fe-PBA increased while those of Ni-and Cu-PBAs decreased. This discrepancy is attributed to the extent of size reduction of the cyanometallate complex([Fe(CN)_(6)]) via pi-backbonding from Fe to C due to redox flips of the low-spin Fe^(3+/2+) ion. This study presents a comprehensive understanding of how TM selection affects capacity acquisition and phase transition in TM-PBAs, a promising class of cathode materials.

The demand for new catalysts for renewable energy production has become crucial.As an alternative to metal catalysts for electrocatalysis to produce energy sources,metal-phthalocyanine(MPc)electrocatalysts have shown potential.Their physicochemical and electrochemical properties depend on the chemical structure of the MPc and the central metal atom.Recent reviews of MPcs focused on their electrochemical performance in specific catalytic reactions,such as oxygen reduction reaction and CO_(2)reduction reaction.However,understanding the structure of MPcs in depth is important,since their electrochemical catalytic activity is affected by structural modifications of MPcs.Therefore,this minireview focuses on how the molecular structure of MPcs affects electrochemical catalysis.

Identification and comparison of the local physicochemical structures of transition metal-based layered double hydroxides for high performance electrochemical oxygen evolution reactions

Layered double hydroxides(LDHs) have attracted considerable attention as a cost effective alternative to the precious iridium-and ruthenium-based electrocatalysts for an oxygen evolution reaction(OER),a bottleneck of water electrolysis for sustainable hydrogen production.Despite their excellent OER performance,the structural and electronic properties of LDHs,particularly during the OER process,remain to be poorly understood.In this study,a series of LDH catalysts is investigated through in situ X-ray absorption fine structure analyses and density functional theory(DFT) calculations.Our experimental results reveal that the LDH catalyst with equal amounts of Ni and Fe(NF-LDH) exhibits the highest OER activity and catalytic life span when compared with its counterparts having equal amounts of Ni and Co(NC-LDH)and Ni only(Ni-LDH).The NF-LDH shows a markedly enhanced OER kinetics compared to the NC-LDH and the Ni-LDH,as proven by the lower overpotentials of 180,240,and 310 mV,respectively,and the Tafel slopes of 35.1,43.4,and 62.7 mV dec^(-1),respectively.The DFT calculations demonstrate that the lowest overpotential of the NF-LDH is associated with the active sites located at the edge planes of NF-LDH in contrast to those located at the basal planes of Ni-LDH and NC-LDH.The current study pinpoints the active sites on various LDHs and presents strategies for optimizing the OER performance of the LDH catalysts.

Ultrasensitive and Highly Stretchable Multiple‑Crosslinked Ionic Hydrogel Sensors with Long‑Term Stability

Flexible hydrogels are receiving significant attention for their application in wearable sensors.However,most hydrogel materials exhibit weak and one-time adhesion,low sensitivity,ice crystallization,water evaporation,and poor self-recovery,thereby limiting their application as sensors.These issues are only partly addressed in previous studies.Herein,a multiplecrosslinked poly(2-(methacryloyloxy)ethyl)dimethyl-(3-sulfopropyl)ammonium hydroxide-co-acrylamide)(P(SBMA-co-AAm))multifunctional hydrogel is prepared via a one-pot synthesis method to overcome the aforementioned limitations.Specifically,ions,glycerol,and 2-(methacryloyloxy)ethyl)dimethyl-(3-sulfopropyl)ammonium hydroxide are incorporated to reduce the freezing point and improve the moisture retention ability.The proposed hydrogel is superior to existing hydrogels because it exhibits good stretchability(a strain of 2900%),self-healing properties,and transparency through effective energy dissipation in its dynamic crosslinked network.Further,2-(methacryloyloxy)ethyl)dimethyl-(3-sulfopropyl)ammonium hydroxide as a zwitterion monomer results in an excellent gauge factor of 43.4 at strains of 1300-1600%by improving the ion transportability and achieving a strong adhesion of 20.9 kPa owing to the dipole-dipole moment.The proposed hydrogel is promising for next-generation biomedical applications,such as soft robots,and health monitoring.

Incidental accumulation of Technetium-99m pertechnetate in subacute cerebral infarction: A case report

BACKGROUND When interpreting nuclear medicine images,unexpected findings are sometimes encountered.Recognizing these findings and determining the mechanism of their occurrence could have a significant impact on early diagnosis of critical diseases and the appropriate management of patients.CASE SUMMARY A 59-year-old man was admitted to the emergency room due to left hemiparesis,left hemifacial palsy,and mild dysarthria.After 2 wk of hospitalization,the patient complained of dry eyes and mouth.Thus,salivary scintigraphy was performed to evaluate the functional status of his salivary glands.Incidental accumulation in the right frontoparietal area was found on salivary scintigraphy.Fluid-attenuated inversion recovery phase magnetic resonance(FLAIR phase MR)image showed diffuse high signal intensity in the same area.Anterior and posterior horns of the right lateral ventricle were obliterated and the midline was slightly shifted to the left side due to right frontoparietal swelling.On salivary scintigraphy,Tc-99m pertechnetate was incidentally accumulated in a subacute cerebral infarction lesion.Two years after the diagnosis of acute infarction,the second series of salivary scintigraphy showed no abnormal activity in the brain.FLAIR phase MR image also demonstrated markedly decreased high signal intensity in the previous infarction lesion without evidence of swelling indicating chronic cerebral infarction.CONCLUSION This case highlights that Tc-99m pertechnetate could accumulate in a subacute cerebral infarction lesion.The mechanism of an unexpected uptake of Tc-99m pertechnetate in unusual sites should be evaluated and kept in mind for better interpretation.

Recurrent Autoencoder Ensembles for Brake Operating Unit Anomaly Detection on Metro Vehicles

The anomaly detection of the brake operating unit (BOU) in thebrake systems on metro vehicle is critical for the safety and reliability ofthe trains. On the other hand, current periodic inspection and maintenanceare unable to detect anomalies in an early stage. Also, building an accurateand stable system for detecting anomalies is extremely difficult. Therefore,we present an efficient model that use an ensemble of recurrent autoencodersto accurately detect the BOU abnormalities of metro trains. This is the firstproposal to employ an ensemble deep learning technique to detect BOUabnormalities in metro train braking systems. One of the anomalous caseson metro vehicles is the case when the air cylinder (AC) pressures are less thanthe brake cylinder (BC) pressures in certain parts where the brake pressuresincrease before coming to a halt. Hence, in this work, we first extract the dataof BC and AC pressures. Then, the extracted data of BC and AC pressuresare divided into multiple subsequences that are used as an input for bothbi-directional long short-term memory (biLSTM) and bi-directional gatedrecurrent unit (biGRU) autoencoders. The biLSTM and biGRU autoencodersare trained using training dataset that only contains normal subsequences. Fordetecting abnormalities from test dataset which consists of abnormal subsequences, the mean absolute errors (MAEs) between original subsequences andreconstructed subsequences from both biLSTM and biGRU autoencoders arecalculated. As an ensemble step, the total error is calculated by averaging twoMAEs from biLSTM and biGRU autoencoders. The subsequence with totalerror greater than a pre-defined threshold value is considered an abnormality.We carried out the experiments using the BOU dataset on metro vehiclesin South Korea. Experimental results demonstrate that the ensemble modelshows better performance than other autoencoder-based models, which showsthe effectiveness of our ensemble model for detecting BOU anomalies onmetro trains.

Microscale and nanoscale cilia are ubiquitous in natural systems where they serve diverse biological functions.Bioinspired artificial magnetic cilia have emerged as a highly promising technology with vast potential applications,ranging from soft robotics to highly precise sensors.In this review,we comprehensively discuss the roles of cilia in nature and the various types of magnetic particles utilized in magnetic cilia;additionally,we explore the top-down and bottom-up fabrication techniques employed for their production.Furthermore,we examine the various applications of magnetic cilia,including their use in soft robotics,droplet and particle control systems,fluidics,optical devices,and sensors.Finally,we present our conclusions and the future outlook for magnetic cilia research and development,including the challenges that need to be overcome and the potential for further integration with emerging technologies.

Association between Occupational Change Trajectories and Mental Health: Results from the Korean Longitudinal Study of Aging

Objectives:This study was to longitudinally investigate the association between occupational change trajectories and mental health in the Korean population aged 45 years and older from the Korean Longitudinal Study of Aging(KLo-SA).Methods:After excluding missing values,the data of 6,224 participants from thefirst to eighth waves of the KLo-SA were analyzed using t-test,Analysis of variance(ANOVA),Group-based Trajectory Model(GBTM)from 1–5th KLoSA and Time-Lagged Generalized estimating equation(GEE)model from 5–8th KLoSA to analyze the association between occupational change trajectories and mental health in the Korean population aged 45 years and older.Results:An analysis of the association between occupational change trajectories and mental health demonstrated that,among all age group,the“sustained in unemployment”group had a worse MMSE and CESD score compared to“sustained WC”group.unlike the under-54 age group,those aged≥55 years demonstrated reductions in the MMSE and CESD scores in the sustained unemployment and the changed working condition from Standard BC to Unemployment groups,whereas those who underwent a change from the Standard WC to unemployment group only demonstrated a reduction in MMSE scores.Conclusions:There was a close relationship between occupational change trajectories and mental health.Therefore,this study can serve as the foundation for policies and institutional measures to manage the deteriorate mental health in the late middle-aged and older population.

Single-cell RNA sequencing of mitoticarrested prospermatogonia with DAZL::GFP chickens and revealing unique epigenetic reprogramming of chickens

Background:Germ cell mitotic arrest is conserved in many vertebrates,including birds,although the time of entry or exit into quiescence phase differs.Mitotic arrest is essential for the normal differentiation of male germ cells into spermatogonia and accompanies epigenetic reprogramming and meiosis inhibition from embryonic development to post-hatch.However,mitotic arrest was not well studied in chickens because of the difficulty in obtaining pure germ cells from relevant developmental stage.Results:We performed single-cell RNA sequencing to investigate transcriptional dynamics of male germ cells during mitotic arrest in DAZL::GFP chickens.Using differentially expressed gene analysis and K-means clustering to analyze cells at different developmental stages(E12,E16,and hatch),we found that metabolic and signaling pathways were regulated,and that the epigenome was reprogrammed during mitotic arrest.In particular,we found that histone H3K9 and H3K14 acetylation(by HDAC2)and DNA demethylation(by DNMT3B and HELLS)led to a transcriptionally permissive chromatin state.Furthermore,we found that global DNA demethylation occurred gradually after the onset of mitotic arrest,indicating that the epigenetic-reprogramming schedule of the chicken genome differs from that of the mammalian genome.DNA hypomethylation persisted after hatching,and methylation was slowly re-established 3 weeks later.Conclusions:We found a unique epigenetic-reprogramming schedule of mitotic-arrested chicken prospermatogonia and prolonged hypomethylation after hatching.This will provide a foundation for understanding the process of germ-cell epigenetic regulation in several species for which this process is not clearly described.Our findings on the biological processes related to sex-specific differentiation of prospermatogonia could help studying germline development in vitro more elaborately.
