Risk factors for tuberculosis after gastrectomy in gastric cancer

AIM: To examine incidence of tuberculosis(TB) in gastrectomy patients and investigate the risk factors for developing TB after gastrectomy in patients with gastric ***: A retrospective cohort study of gastrectomy patients with gastric cancer was performed at a university-affiliated hospital in Seoul, South Korea between January 2007 and December 2009. We reviewed patient medical records and collected data associated with the risk of TB, surgery, and gastric cancer. Standardized incidence ratios(SIRs) of TB were calculated to compare the incidence of TB in gastrectomy patients with that in the general Korean population, and risk factors for TB after gastrectomies were ***: Among the 1776 gastrectomy patients, 0.9%(16/1776) developed post-gastrectomy TB, with an incidence of 223.7 cases per 100000 patients per year. The overall incidence of TB in gastrectomy patients, adjusted by sex and age, was significantly higher thanthat in the general population(SIR = 2.22, 95%CI: 1.27-3.60). Previous TB infection [odds ratio(OR) = 7.1, P < 0.001], lower body mass index(BMI)(kg/m2; OR = 1.21, P = 0.043) and gastrectomy extent(total gastrectomy vs subtotal gastrectomy)(OR = 3.48, P = 0.017) were significant risk factors for TB after gastrectomy in a multivariate ***: TB incidence after gastrectomy is higher than that in the general population. Previous TB infection, lower BMI, and total gastrectomy are risk factors for TB after gastrectomy in patients with gastric cancer.

The purpose of the present study was to evaluate the effects of a trunk exercise program on the gait and muscle activity in stroke patients. The participants of this pilot study included six hemiplegic stroke patients. The outcomes were surface electromyography (sEMG) and spatiotemporal gait parameters. In analysis of sEMG, no statistically difference was found between pre- and post-training of Maximal Voluntary Isometric Contraction (MVIC) in rectus abdominis and external abdominal oblique muscle, but it tended to increase. However, the gait parameter significantly increased in walking speed, walking cycle, and affected stride length in stroke patients. These results suggest that the trunk exercise program may in part improve the gait of chronic stroke patients.

An Experimental Study on High Temperature and Low Oxygen Air Combustion

High temperature preheated and diluted air combustion has been confirmed as the technology, mainly applied to industrial furnaces and kilns, to realize higher thermal efficiency and lower emissions. The purpose of this study was to investigate fundamental aspects of the above-mentioned combustion experimentally and to compare with those in ordinary hydrocarbon combustion with room temperature air. The test items were exhaust gas components of CO, NOx, flame shape and radical components of CH, OH and C2,which were measured with gas analyser, camera and ICCD(Intensified Charged - Coupled Device) camera. Many Phenomena as results appeared in combustion with the oxidizer, low oxygen concentration and extremely high temperature air, such as expansion of the flammable limits, increased flame propagation speed, it looked so strange as compared with those in existing combustion technology. We confirmed that such extraordinary phenomena were believable through the hot-test experiment.

The recent appearance of mobile application processor now plays an important role in the semiconductor industry. Additionally, there have been endless demands for small form factor, thin profile, outstanding thermal, mechanical properties and electrical performances in the field of IC packages for mobile application processors. MIS (Molded Interconnect Substrate) can provide ideal and this solution in the mobile industry as it contains multiple solutions for the complicated requirement of the IC packages for application processors. Based on the embedded pattern technology, this solution can provide high I/O count, fine pattern for small form factor, and stable flip chip mounting methods. Other advantages of this solution include stable properties required for high-frequency transmission and high thermal dissipation rate as it is only composed of copper and epoxy mold compound materials. These core techniques for MIS technology can be divided into below major concepts. First, Cu pillar electrolytic plating technology electrically connects the inner and the outer layers using photo-lithography method, instead of laser method. The shape of Cu pillar and the plating thickness tolerance control are the key parameters. Second, grinding technology is to precisely grind the plated Cu pillars and applied mold epoxy. And deposition of Cu layer on top of grinded mold surface can construct fine pattern traces.

Gastric cancer surgery in cirrhotic patients: Result of gastrectomy with D2 lymph node dissection

AIM： To explore the feasibility of performing gastrectomy with D2 lymphadenectomy in gastric cancer patients with liver cirrhosis. METHODS： A total of 7 178 patients were admitted with a diagnosis of liver cirrhosis from January 1993 to December 2003. We reviewed the records of 142 patients who were diagnosed with liver cirrhosis and gastric adenocarcinoma during the same period. Gastrectomy with D2 lymph node dissection for carcinoma of the stomach was performed in 94 patients with histologically proven hepatic cirrhosis. RESULTS： All but 12 patients were dassified as Child＇s class A. Only 35 patients （37.2%） were diagnosed with cirrhosis before operation. Seventy-three patients underwent a subtotal gastrectomy （77.7%） and 21 patients （22.3%） underwent a total gastrectomy, each with D2 or more lymph node dissection. Two patients （3.8%） who had prophylactic intra-operative drain placement, died of postoperative complications from hepatorenal failure with intractable ascites. Thirty-seven patients （39.4%） experienced postoperative complications. The extent of gastric resection did not influence the morbidity whereas serum aspartate aminotransferase level （P = 0.011） and transfusion did （P = 0.008）. The most common postoperative complication was ascites （13.9%） followed by wound infection （10.6%）. CONCLUSION： We concluded that the presence of compensated cirrhosis, i.e. Child class A, is not a contraindication against gastrectomy with D2 or more lymph node dissection, when curative resection for gastric cancer is possible. Hepatic reserve and meticulous hemostasis are the likely determinants of operative prognosis.

Influence of a microscopic positive proximal margin in the treatment of gastric adenocarcinoma of the cardia

AIM： To investigate the influence of a positive proximal margin in total gastrectorny patients with gastric adenocarcinorna of the cardia. METHODS： Medical records of 191 patients with total gastrectornies for adenocarcinorna of the cardia between 1995 and 2000 were reviewed. The clinicopathologic features associated with a positive margin were determined, and the predictors for survival were analyzed. RESULTS： The incidence of positive proximal margin was 8.4% （16/191）. The positive margins were associated with advanced diseases. The tumor size and the depth of tumor invasion were independent risk factors for a positive margin. The mean survival in the positive margin group was 33.9 mo as *** 62.4 mo in the negative group （P 〈 0.001）. However, the difference in survival lost significance in subgroup analysis according to stage. Multivariate analysis identified that a positive margin was not an independent prognostic factor for survival. CONCLUSION： A positive margin is more of an indication of advanced disease in patients with gastric adenocarcinoma of the cardia rather than an independent prognostic factor for survival.

Biodiesel Resistance of Thin Resin Cr-Free Steel Sheets for Fuel Tank

The content of biodiesel mixed with diesel fuel were compared to inspect the fuel resistance of thin resin Cr-free steel sheets, which are widely used as steel sheets of automobile fuel tank. Some additives which can be presented during the process of biodiesel preparation were added for CCT (Cyclic Corrosion Test). These additives can accelerate the occurrence of corrosion. The corrosion was appeared on the coating and painting layer and in serious cases even substrate material was corroded. For methanol, mixing with blended fuel showed the reduction in corroded area as the additive concentration was reduced in the mixed fuel. Especially the peroxide hydrogen showed the strongest corrosiveness. It is known that formic acid has a tendency of weaker corrosiveness than peroxide hydrogen, but the corrosion is occurred throughout the specimen. Water is not mixed well with fuel, and does not seem to impact on corrosion significantly. However, water is easily mixed with other additives and is considered to facilitate the corrosion by other additives.

Background： Glehnia littoralis has been used for traditional Asian medicine, which has diverse therapeutic activities. However, studies regarding neurogenic effects of G. littoralis have not yet been considered. Therefore, in this study, we examined effects of G. littoralis extract on cell proliferation, neuroblast differentiation, and the maturation of newborn neurons in the hippocampus of adult mice. Methods： A total of 39 male ICR mice （12 weeks old） were randomly assigned to vehicle-treated and 100 and 200 mg/kg G. littoralis extract-treated groups （n = 13 in each group）. Vehicle and G. littoralis extract were orally administrated for 28 days. To examine neurogenic effects ofG. litmralis extract, we performed immunohistochemistry tbr 5-bromo-2-deoxyuridine （BrdU, an indicator for cell proliferation） and doublecortin （DCX, an immature neuronal marker） and double immunofluorescence staining for BrdU and neuronal nuclear antigen （NeuN, a mature neuronal marker）. In addition, we examined expressional changes of brain-derived neurotrophic factor （BDNF） and its major receptor tropomyosin-related kinase B （TrkB） using Western blotting analysis. Results： Treatment with 200 mg/kg, not 100 mg/kg, significantly increased number of BrdU-immunoreactive （＋） and DCX＋ cells （48.0 ±3.1and 72.0 ± 3.8 cells/section, respectively） in the subgranular zone （SGZ） of the dentate gyrus （DG） and BrdU＊/NeuN＋ cells （17.0 ±1.5 cells/section） in the granule cell layer as well as in the SGZ. In addition, protein levels of BDNF and YrkB （about 232% and 244% of the vehicle-treated group, respectively） were significantly increased in the DG of the mice treated with 200 mg/kg ofG. littoralis extract. Conclusion： G. littoralis extract promots cell proliferation, neuroblast differentiation, and neuronal maturation in the hippocampal DG, and neurogenic effects might be closely related to increases ofBDN F and TrkB proteins by G. littoralis extract treatment.
