Chronic hepatitis C virus (HCV) infection affects approximately 3 million people in the United States and places tremendous demands on the health care system. As many observers have predicted, the disease burden continues to grow as the infected population ages. In this study, we analyzed inpatient data from the Health care Cost and Utilization Project, outpatient data from the National Ambulatory Medical Care Survey, and drug data from the Verispan Source Prescription Audit. We examined recent growth in the use of health care resources among HCV patients by age group and found average annual increases of 25% to 30% for hospitalizations, charges, hospital days, and physician visits. Corresponding time-trend coefficients were positive (P < .001). From 1994 to 2001, the HCV burden increased among patients aged 40 to 60 years, reflecting the natural history of disease progression. In sensitivity analysis, HCV outcome growth rates remained significant, unless more than 3 out of 4 cases were initially underreported. Also, patients co-infected with HIV and HCV in 2001 constituted 7.5 times as many hospitalizations and incurred 2.9 times the charges in 1994, relative to all HIV hospitalizations and charges. Our findings highlight the urgency concerning HCV outcomes. In conclusion, as patients continue to age and disease burden progresses, suboptimal decisions regarding HCV treatments will bring increasing opportunity costs for the health care system and society.

目的描述加拿大重症甲型H1N1流感患儿病例特点,治疗过程和预后情况。设计对2009年4月16日至8月15日收入加拿大9所PICU的重症甲型H1N1流感患儿进行观察性研究。测量指标和主要结果对重症甲型H1N1流感患儿的病例特点进行记录,将确诊病例与包含所有甲型H1N1流感住院患儿资料的全国监测数据库进行比较。应用Cox比例风险模型进行风险因素分析。PICU病例队列与全国监测数据进行比较和卡方检验。57例患儿因社区获得性甲型 H1 N1流感住院。70.2%的患儿合并至少1种慢性基础疾病,24.6%的患儿为土著居民。68%的患儿接受机械通气治疗,20例(35.1%)患儿入院第1天发生急性肺损伤,机械通气时间中位数为6d(0～67 d),PICU病死率为7%(4/57)。与非ICU住院患儿比较,PICU患儿合并慢性基础性疾病者更常见(相对风险度1.73)。土著居民并非ICU住院的风险因素。结论因人群对甲型H1N1这种新型流感病毒普遍易感,在疾病首次暴发时,重症病例多见于合并慢性基础性疾病的儿童和土著儿童。重症甲型H1N1流感病例多因严重呼吸衰竭发生缺氧和机械通气时间延长。与季节性流感相比,甲型H1N1流感病例呼吸系统病变发生率较高,程度较重,并未导致病死率增高。

Population responses to envir on mental extremes often dictate the bounds to species' distributions. However, population dynamics at, or near, those range limits may also be affected by sublethal effects. We exposed late instars and pupae of an invasive leafroller, Epiphyaspostvittana (Walker)(Lepidoptera: Tortricidae), to cold temperatures and measured the effects of exposure on subsequent survivorship, development, and reproduction. Cold temperature was applied as acute exposure to -10 °C (a low, but not immediately lethal temperature for this species) or the onset of freezing (the peak of the supercooling point exotherm). Survival was defined as the ability to successfully eclose as an adult. We measured immature development times, pupal mass, and adult longevity as proxies of fitness in survivors. Additionally, surviving insects were mated with individuals that had not been exposed to cold to measure fertility. There was no difference between the proportion of larvae or pupae that survived acute exposure to -10 °C and those exposed to the control temperature. Approximately 17% of larvae and 8% of pupae survived brief periods with internal ice formation and continued development to become reproductively viable adults. Importantly, surviving the onset of freezing came with significant fitness costs but not to exposure to -10 °C;most insects that survived partial freezing had lower fertility and shorter adult lifespans than either the -10 °C or control group. These results are discussed within the context of forecasting invasive in sect distributions.

CHONDRULES are the major constituent of chondrites, but the nature of their origin remains elusive. Models based on nebular settings have attracted much support since Wood suggested that they formed there ascondensates. Subsequently, the melting of dust in the nebula has been widely invoked. Recently,however, models suggesting that chondrules formed in parent body regoliths have been revived, couldhelp to explain the coexistence of chondrules with chemically differentiated clasts The Boxian meteorite is a fall and was originally described as L4. The widely quoted classification of the chondrite is LL4. In this study, 362 chondrules were separated from the Boxian meteorite,and were measured for their grain sizes, masses and morphologies. 101 of these chondrules were successfully split and parts of them made into polished blocks; the remaining segments were individually set asidefor future oxygen isotopic analyses. Repeatability and Variability of the 3-Min All-Out Test at the Subject Level

Purpose The constant work-rate to exhaustion tests must be repeated several times at each work-rate to estimate subject-level trial-to-trial variance(intra-individual variability,IIV)of critical power(CP)and work capacity(W').Alternatively,these parameters and their variance can be estimated by repeating the 3-min all-out test(3MT)fewer *** purpose of this study was to propose a method to determine subject-level repeatability of the 3MT and demonstrate the need to repeat the test multiple times to estimate *** Seven cyclists performed a ramp test and four 3MTs on a *** parameters CP,W',peak power(Pp),and total work(TW)were compared across trials using repeated measures ANOVA,Bland-Altman analysis,Intraclass Cor-relation Coefficients(ICC),Typical Error(TE)of measurement,and Coefficient of Variation(CV).Results For the group,average CP and W'were 284±58 W and 10.214±3.143 *** reliability statistics,CP(ICC=0.97,TE=8 W,CV=2.94%)and W'(ICC=0.88,TE=1.11 kj,CV=10.87%),indicated strong ***-level repeatability was determined by comparing time-to-peak power(TPp),absolute difference in Pp(δPp),and TW(δTW)for pairs of *** average IIVs estimated by the 95%confidence intervals were±15 W for CP and±1.68 kj for W'.Conclusions Thresholds are proposed for TPp(7 s),δPp(10%),andδTW(3%)to determine subject-level repeatability of the 3MT before computing the IIV of CP and W'.It is suggested that the 3MT is repeated at least three times to estimate the IIV,which aids in personalized measurement of training improvements and performance optimization。

Blastomyces dermatitidis, the causative agent of blastomycosis, a potentially lethal dimorphic fungal disease of humans and animals has been difficult to diagnose in the clinical laboratory. We are attempting to develop and improve immunodiagnostic assays by producing novel yeast lysate reagents for the detection of antibodies in blastomycosis. The objective of this study was to use lysate antigens prepared from four B. dermatitidis antigens isolated from dogs infected with blastomycosis from two different endemic areas (Wisconsin and Tennessee) testing for the detection of antibodies in serum specimens from immunized rabbits and infected dogs using the indirect ELISA. In the dog sera, absorbance values ranged from 0.774 to 1.350, while the rabbit sera values ranged from 0.533 to 1.191. Antigen T-58 appeared to lack any geographical specificity in antibody detection, which could prove useful in future immunodiagnostic detection of blastomycosis infections.
