In this study,twelve machine learning(ML)techniques are used to accurately estimate the safety factor of rock slopes(SFRS).The dataset used for developing these models consists of 344 rock slopes from various open-pit mines around Iran,evenly distributed between the training(80%)and testing(20%)*** models are evaluated for accuracy using Janbu's limit equilibrium method(LEM)and commercial tool GeoStudio *** assessment metrics show that the random forest model is the most accurate in estimating the SFRS(MSE=0.0182,R2=0.8319)and shows high agreement with the results from the LEM *** results from the long-short-term memory(LSTM)model are the least accurate(MSE=0.037,R2=0.6618)of all the models ***,only the null space support vector regression(NuSVR)model performs accurately compared to the practice mode by altering the value of one parameter while maintaining the other parameters *** is suggested that this model would be the best one to use to calculate the SFRS.A graphical user interface for the proposed models is developed to further assist in the calculation of the SFRS for engineering *** this study,we attempt to bridge the gap between modern slope stability evaluation techniques and more conventional analysis methods.

AIM： To compare the peripapillary retinal nerve fiber layer（RNFL） thickness measured via optical coherence tomography（OCT） between different groups of myopia severity and controls. METHODS： This was a prospective cross-sectional study. All subjects underwent a full ophthalmic examination, refraction, visual field analysis and A-scan biometry. Myopic patients were classified as low myopia（LM） [spherical equivalent（SE） from greater than-0.5 D, up to-3.0 D], moderate myopia（MM; SE greater than-3.0 D, up to-6.0 D） and high myopia（HM; SE greater than-6.0 D）. The control group consisted of emmetropic（EM） patients（SE from ＋0.5 D to-0.5 D）. A Zeiss Cirrus HD-OCT machine was used to measure the peripapillary RNFL thickness of both eyes of each subject. The mean peripapillary RNFL thickness between groups was compared using both analysis of variance and analysis of ***： A total of 403 eyes of 403 subjects were included in this study. The mean age was 31.48±10.23 y. There were 180（44.7%） eyes with EM, 124（30.8%） with LM, 73（18.1%） with MM and 26（6.5%） with HM. All groups of myopia severity had a thinner average RNFL than the EM group, but after controlling for gender, age, and axial eye length, only the HM group differed significantly from the EM group（P=0.017）. Likewise, the superior, inferior and nasal RNFL was thinner in all myopia groups compared to controls, but after controlling for confounders, only the inferior quadrant RNFL was significantly thinner in the HM group, when compared to the EM group（P=0.017）. CONCLUSION： The average and inferior quadrant RNFL is thinner in highly myopic eyes compared to emmetropic eyes. Refractive status must be taken into consideration when interpreting the OCT of myopic patients, as RNFL thickness varies with the degree of myopia.

Orbital apex syndrome after tooth extraction in an immunocompromised patient

A 60 year-old man presented with acute on set of left eye proptosis and ptosis. It was associated with poor vision, eye pain and restriction of eye movement of the same eye after 5 hours post left upper molar tooth extraction. The visual acuity in the left eye was hand movement. There was severe ptosis and proptosis of the left eye. The conjunctiva was chemotic with quite anterior chamber. The pupil was mid dilated and sluggished to light. The ocular movement was restricted in all directions. Fundoscopy of the left eye revealed features of central retinal artery occlusion with hyperaemic disc and subretinal exudates at posterior pole. The right eye appeared normal. Urgent MRI brain and orbit revealed severe left paranasal sinusitis with anterior displacement of the left globe and presence orbital abscess. Patient was managed with Otorhinolaryngology and Neurosurgery teams. He underwent emergency transnasal drainage of abscess. Histopathological examination of unhealthy sinus mucosa showed evidence of fungal infection. However, the culture and sensitivity result was inconclusive. Patient was treated with amphotericin B, ceftriaxone, amoxicillin clavulanate and metronidazole. Patient was detected to have high blood sugar level and was managed accordingly. The proptosis improved with treatment. However, his vision, ptosis and ophthalmoplegia remained static. Assessing the immunocompromised status is important for the management of patient presented as acute orbital apex syndrome to avoid fatal outcome.

21岁和11岁两位患者患严重的持续性盾形溃疡,包括局部激素、肥大细胞稳定剂及抗组胺药的药物治疗均无效。两者均接受溃疡的外科清创,上睑板下注射曲安奈德,局部用0.5g/L环孢霉素A和0.25g/L富马酸酮替芬治疗。3wk后溃疡愈合。2mo后两位患者停用局部环孢霉素A。患者症状缓解,6mo没有复发。联合溃疡的外科清创,上睑板下注射曲安奈德,局部用0.5g/L环孢霉素A和0.25g/L富马酸酮替芬是治疗重度春季盾形溃疡的有效方案。

Primary non Hodgkin's lymphoma of the lacrimal sac is uncommon but potentially delay in diagnosis as it may mimic the presentation of primary post saccal nasolacrimal duct obstruction. In this article, we reported a case of primary non Hodgkin's lymphoma of the lacrimal sac presented with recurrent acute dacryocystitis in a young lady. A 28 years old Malay lady had history of persistent epiphora on left eye for 4 years. Prior to presentation to our clinic, it was preceded by progressive recurrent painful medial canthal swelling for 6 months duration. Clinical diagnosis of post saccal naso lacrimal duct obstruction was made and otorhinolaryngology assessment revealed intranasal mass. Endoscopic excision was done showed diffuse large B cell lymphoma on histology. The patient was started on 6 cycles of chemotherapy which subsequently result in no recurrence of the tumour post chemotherapy. Any case of post saccal nasolacrimal duct obstruction should be investigated thoroughly as it may become as a presentation of other sinister pathology.

Inverted papilloma is an uncommon histopathological finding of maxillary sinus tumor. We reported here a rare case of inverted papilloma of maxillary sinus presented initially with ipsilateral proptosis. A middle-aged lady with history of having allergic rhinitis presented with progressive left eye proptosis. Assessment revealed presence of orbital mass arising from paranasal sinus, in which the histopathological examination was initially inconclusive, and later it turned up to be inverted papilloma, partly with malignant transformation. High index of suspicion is necessary especially in those patients with recurrent non-specific sinonasal symptoms, which may need repeated biopsy and histopathological examination. Ocular manifestation signifies a more advance disease.

Background: Intraorbital foreign bodies can result in various complications if not appropriately managed. Aim: To report a case of lower lid entropion secondary to a missed intraorbital foreign body. Case Presentation: A 7-year-old boy presented with inward turning of the left lower lid for three days prior to admission. A week before, the child was in the garden when part of the roof of their home suddenly broke off, with pieces of the debris falling near him. He complained of left eye irritation, but a visit to a general practitioner found no eye abnormalities. Three days after that, the mother sought an ophthalmology opinion as she noticed inversion of the left lower lid. On examination, visual acuity in both eyes was 6/6. There was left lower lid entropion, and a deeply embedded foreign body seen in the lower lid. No other eye abnormalities noted. Computed tomography scan of the orbits revealed a welldefined foreign object in the lower orbit, fracturing the antero-superior wall of the left maxillary sinus. The left eye foreign body was removed uneventfully via a transconjunctival approach. Conclusion: A thorough examination is mandatory in any case of ocular trauma especially in young children, who are usually unable to provide a clear history of the injury.
