Improvement in renal functions with transcatheter aortic valve implantation

BACKGROUND & OBJECTIVES:In recent years, emerging transcatheter aortic valve implantation (TAVI) has become an alternative for surgery。 However, with advanced age, several co-morbid factors together with contrast agent usage can cause deterioration in renal function and increase in the risk of acute kidney injury (AKI) with poor prognosis in patients with AKI。 Therefore, many patients cannot benefit from this treatment。 In this study, we aim to examine the effects of TAVI on renal functions。 METHODS AND RESULTS: Seventy patients, mean age of 77.6 years, underwent TAVI between July 2011 and December 2012。 Estimated glomerular filtration rate (eGFR) was calculated by using the Cockcroft and Gault Formula。 Patients were monitored for 48 h for urine output。 Stage 1 AKI, according to the VARC-2 AKIN system, developed in only five (7.1%) of the patients after the procedure。 There was a statistically significant increase between the mean 1(st) month eGFRs before (68。2 vs。 61.0, P < 0.01) and after (68。2 vs。 63.6, P < 0.05) the TAVI in the cohort。 After TAVI (48。5 mL/min, P < 0.01) and the 1(st) month (52.1 mL/min, P < 0.01), the eGFR of the 36 (51.4%) patients diagnosed with chronic kidney disease before the procedure showed a statistically significant increase in renal functions。 The hospital mortality rate was higher in the group which developed AKI (P < 0.01)。 First month eGFR showed a more statistically significant increase than pre-TAVI eGFR (62.8 and 69.8, P < 0.05, respectively) in AKI developing patients and this difference - though statistically not significant - continued into the sixth month。 CONCLUSIONS: In this study, we showed that the treatment of aortic stenosis through TAVI allows improvement of renal functions, and that AKI rates will be lower with careful patient selection, proper pre-procedural hydration, and careful use of contrast agent。

Background Transcatheter aortic valve implantation （TAVI） is a minimally invasive, emerging therapy in surgically high risk, or in- operable patients. Parameters used for risk classification have some deficiencies in the selection of patients. The objective of this study is to evaluate the impact of TAVI on carbohydrate antigen 125 （CA125） and N-terminal pro brain-type natriuretic peptide （NT-proBNP） as biomarkers that have been used frequently in recent years, and also the relationship of these biomarkers to prognosis. Methods ＆ Results Transcatheter aortic valve implantation was practiced on 31 patients in this study. Then, CA125 and NT-proBNP levels studied in patients prior to and after the TAVI were evaluated. The patients were also grouped in accordance with their left ventrieular ejection fraction （LVEF） and CA125 levels （LVEF 〉 40% and 〈 40%; CA125 ≤ 35 U/L and 〉 35 U/L）. The TAVI operation was successfully performed in all patients. There was no in-hospital mortality and substantial improvement in functional capacity was detected at follow ups. In addition, a statistically significant decrease was detected in post-TAVI CA 125 and NT-proBNP levels of all patients （CA 125 83.8 ± 18.1 U/L vs. 64.3 ±14.2 U/L, P = 0.008; NT-proBNP： 4633.6± 627.6 pg/mL vs. 2866.3±536.8 pg/mL, P 〈 0.001）. In groups divided according to the CA125 levels, there was also statistically significant post-TAVI decline in CA125 levels. Within CA125 〉 35 U/L and LVEF 〈 40% groups, the permanent need for a pacemaker was required in one （3.2%） patient and mortality was observed in two （6.4%） patients after TAVI at follow up. Conclusions The results show that TAVI can be performed effectively and reliably in patients with high baseline levels of CA125 and NT-proBNP. These biomarkers are reduced substantially with TAVI, while high biomarker levels are associated with undesired events, and certainly, these biomarkers can be used for risk classifications in patient selection for TAVI.

Fe_3O_4-PVDF membranes were prepared by blending of magnetic Fe_3O_4 powders with polyvinylidene fluoride to investigate whether those were usable or not in catalytic membrane reactors. Filtration performances and catalytic activity of membranes in microwave conditions were measured in separate processes. Composite Fe_3O_4-PVDF membranes were characterized by TG-DTA, FTIR, XRD, SEM and contact angle *** of α-phases at PVDF was observed with increasing amount of additives from XRD diffraction patterns. Decomposition of polymer fastened due to catalytic effect of Fe_3O_4. Finger-like structures and large number of small pores were observed at the SEM images. Those provided effective transportation of substrate among the active sites of catalyst. At the experiments conducted in batch reactor, 51%, 77%, 66% and 63% benzyl alcohol conversion were recorded for 2%, 4%, 6% and 8% Fe_3O_4-PVDF composite pieces respectively. Catalyst were separated magnetically and reused several times. On the other hand Fe_3O_4 blended PVDF membranes provided improved flux and BSA rejection compared with performance of bare PVDF membrane; 41.6% BSA rejection was obtained with 4% Fe_3O_4-PVDF whereas it was only 6.7% for PVDF. Fe_3O_4-PVDF composites performed high activity for the benzyl alcohol oxidation in batch reactor and also better filtration at filtration cell. These results promise to obtain practical and low cost membrane material for catalytic reactors usable in microwave support to get fast results.

BACKGROUND Although single-port laparoscopic cholecystectomy has been performed for over 25 years, it is still not popular. The narrow working space used in this surgery limits the movement of instruments and causes ergonomic challenges. Robotic surgery not only resolves the ergonomic challenges of single-port laparoscopic surgery but is also considered a good option with its additional technical advantages, like a three-dimensional display and not being affected by ***, the extent to which these technical and ergonomic advantages positively affect the surgical outcomes and how safe the single-port robotic surgeries need to be assessed for each particular *** To evaluate the feasibility and safety of single-port robotic cholecystectomy for patients with *** The electronic records of the first 40 consecutive patients with gallbladder lithiasis who underwent single-port robotic cholecystectomy from 2013 to 2021 were analyzed retrospectively. In addition to the demographic characteristics of the patients, we analyzed American Society of Anesthesiologists(ASA) scores and body mass index. Plasma electrolytic oxidation(PEO)coatings were carried out in silicate-based electrolyte embedded with various amounts of carbon nanotube(CNT)additives(0,0.5,1,2 and 4 g/L)on AZ80 magnesium alloy ***,tribological and electrochemical corrosion properties of the coated specimens were *** results demonstrated that the increasing CNT additions into the electrolyte resulted in a gradual increase in the thickness of PEO-coating *** CNT addition to the electrolyte by 0.5 g/L resulted in a slight decrease in the roughness of PEO-coating above which it continually *** resistance of the PEO-coated specimens showed a gradual improvement with increasing CNT-incorporation within the *** electrochemical corrosion tests revealed that the best corrosion resistance was found after the CNT addition into the electrolyte by 0.5 g/L due to the better roughness values,more homogenous coating layers and less pore formation.

Total oxidative stress, paraoxonase and arylesterase levels at patients with pseudoexfoliation syndrome and pseudoexfoliative glaucoma

AIMTo investigate the oxidative stress status of the aqueous humor and serum of patients with pseudoexfoliation (PEX) syndrome and pseudoexfoliative glaucoma (PEG) and to measure paraoxonase (PON) and arylesterase (ARE) *** total of 78 patients were enrolled in the study, with 26 patients in each separate group. The patients were divided into three groups: the first group entailed PEX syndrome patients, while the second group consisted of patients with PEG and the third group involved patients with no additional systemic diseases, other than the diagnosis of cataract as control. Total oxidative stress (TOS), total antioxidant capacity (TAC), PON, and ARE levels in aqueous humor and serum were ***, PON and arylesterase levels in aqueous humor and serum of the PEX syndrome and PEG patients were significantly decreased compared with control group (P<0.05). TOS values were higher in patients with PEX syndrome and PEG than controls (P<0.05). TAC, PON and ARE levels of aqueous humor did not differ significantly between the PEX syndrome and PEG groupsCONCLUSIONThese findings are potentially of significance and add to the growing body of evidence for oxidative stress in PEX syndrome and PEG. Decreased antioxidant defense and increased oxidative stress system may play an important role in the pathogenesis of PEX syndrome and PEG.

Mechanical and Antibacterial Properties of Injection Molded Polypropylene/TiO_2 Nano-Composites:Effects of Surface Modification

Polypropylene （PP）/titanium dioxide （TiO2） nano-composites were prepared by melt compounding with a twin screw extruder. Nanoparticles were modified prior to melt mixing with maleic anhydride grafted styreneethylene-butylene-styrene （SEBS-g-MA） and silane. The composites were injection molded and mechanical tests were applied to obtain tensile strength, elastic modulus and impact strength. Antibacterial efficiency test was applied on the injection molded composite plaques by viable cell counting technique. The results showed that the composites including SEBS-g-MA and silane coated TiO2 gave better mechanical properties than the composites without SEBS-g-MA. Antibacterial efficiency of the composites varied according to the dispersion and the concentration of the particles and it was observed that composites at low content of TiO2 showed higher antibacterial property due to the better photocatalytic activity of the particles during UV exposure.
