Clinical utility of hepatitis B surface antigen kinetics in treatment-na?ve chronic hepatitis B patients during longterm entecavir therapy

AIM To investigate the utility of hepatitis B surface antigen(HBsAg) kinetics in chronic hepatitis B patients during long-term entecavir *** This retrospective study included treatment-na?ve chronic hepatitis B patients who received at least 2 years of consecutive entecavir treatment. Patients were followed up at three to six month intervals with liver biochemistry, hepatitis B virus DNA, and abdominal sonography. In hepatitis B e antigen(HBeAg)-positive patients, HBeAg levels were assessed every three to six month until results became negative. Serum HBsAg levels were determined at the baseline, oneyear and five-year time points. Liver cirrhosis was diagnosed through liver biopsy, imaging examinations, or clinical findings of portal hypertension. Hepatocellular carcinoma was diagnosed by histological examination or dynamic image *** A total of 211 patients were enrolled. The median treatment time was 5.24(2.00-9.62) years. Multivariate analysis showed that lower baseline HBsAg levels were associated with an earlier virological response, earlier hepatitis B e antigen(HBeAg) seroconversion, and earlier biochemical response in HBeAg-positive patients(cut-off value: 4 log IU/mL) and an earlier virological response in HBeAg-negative non-cirrhotic patients(cut-off value: 2.4 log IU/mL). Although HBsAg levels decreased slowly during long-term entecavir treatment, higher HBsAg decrease rates were found in the first year for HBeAg-positive non-cirrhotic patients, and patients with higher baseline HBsAg levels. More favorable clinical outcomes were not observed by a rapid HBsAg decline per se, but depended on lower baseline HBsAg *** Baseline HBsAg can be used to predict treatment responses. HBsAg levels and decrease rates should be considered together according to disease status while interpreting HBsAg changes.

Numerical Implementation of a Unified Viscoplastic Model for Considering Solder Joint Response under Board-Level Temperature Cycling

An implicit integration scheme was developed for simulating the viscoplastic constitutive behavior ofSn3.0Ag0.5Cu solder and programmed into a user material subroutine of the finite element software ANSYS. Thenumerical procedure first solves the essential state variables by using a three-level iterative procedure, and updatesthe remaining stress and state variables accordingly. The numerical implementation was applied to consider theresponses of solder joints in an electronic assembly under temperature cycling condition. The viscoplastic strainenergy density accumulation over one temperature cycle was identified as a feasible parameter for evaluating thethermomechanical reliability of the solder joints.

Distribution and contamination status of chromium in surface sediments of northern Kaohsiung Harbor, Taiwan

Thedistribution, enrichment, accumulation, and potential ecological risk of chromium （Cr） in the surface sediments of northern Kaohsiung Harbor, Taiwan, Chinawere investigated. Sediment samples from ten locations located between the river mouths and harbor entrance of northern Kaohsiung Harborwere collected quarterly in 2011 and characterized for Cr, aluminum,water content, organic matter, total nitrogen, total phosphorous, total grease, and grain size. Results showed that the Cr concentrations varied from 27.0 to361.9 mg/kgwith an average of （113.5 ± 87.0） mg/kg. High Cr concentrationwas observed near the Jen-Gen River mouth. The mean Cr concentrationwas high at 255.5 mg/kg,whichwas at least 2 to 7 times than that of other sites. This might imply significant Cr contribution from upstream receiving tannerieswastewater into the Jen-Gen River. The spatialdistribution of Cr reveals relatively high in the river mouth region, especially in Jen-Gen River, and graduallydiminishes toward the harbor entrance region. This indicates that the major sources of Cr pollution from upstream industrial and municipalwastewatersdischarged along the river bank; and Cr maydriftwith sea current and bedispersed into open sea. Moreover, Cr concentrations correlated closely to the physical-chemical properties of the sediments,which suggested the influence of industrial and municipalwastewatersdischarged from the neighboring industrial parks and river basins. Results from the enrichment factor andgeo-accumulation index analyses imply that the Jen-Gen River sediments can be characterized as moderate enrichment and none to medium accumulation of Cr, respectively. However, results of potential ecological risk index indicate that the sediment has low ecological potential risk. The results can provide valuable information todeveloping future strategies for the management of river mouth and harbor.

Terminal ileum gangrene secondary to a type Ⅳ paraesophageal hernia

Type Ⅳ paraesophageal hernia(PEH) is very rare, and is characterized by the intrathoracic herniation of the abdominal viscera other than the stomach into the chest. We describe a 78-year-old woman who presented at our emergency department because of epigastric pain that she had experienced over the past 24 h. On the day after admission, her pain became severe and was accompanied by right chest pain and dyspnea. Chest radiography revealed an intrathoracic intestinal gas bubble occupying the right lower lung field. Emergency explorative laparotomy identified a type Ⅳ PEH with herniation of only the terminal ileum through a hiatal defect into the right thoracic cavity. In this report, we also present a review of similar cases in the literature published between 1980 and 2015 in Pub Med. There were four published cases of small bowel herniation into the thoracic cavity during this period. Our patient represents a rare case of an individual diagnosed with type Ⅳ PEH with incarceration of only the terminal ileum.

Efficient password authentication schemes based on a geometric approach for a multi-server environment

Password authentication schemes based on a geometric approach are proposed. Based on geometrics such as a plane or space, the proposed schemes can be used to fulfill the various requirements for security and achieve more efficient results. They allow a user to register with many servers once without remembering different passwords for each, and also allow a user to change the password freely and update it offline. In addition, the proposed schemes do not need to maintain a verification table and are also computationally efficient. Moreover, we propose a specific access right (AR) to verify the legitimate user who has different authorization levels on a server in a multi-server environment.

BACKGROUND Pneumocephalus is a rare complication presenting in the postoperative period of a thoracoscopic *** report a case in which tension pneumocephalus occurred after thoracoscopic resection as well as the subsequent approach of surgical *** SUMMARY A 66-year-old man who received thoracoscopic resection to remove an intrathoracic,posterior mediastinal,dumbbell-shaped,pathology-proven neurogenic *** patient then reported experiencing progressively severe headaches,especially when in an upright position.A brain computed tomography scan at a local hospital disclosed extensive *** surgery for resection of the pseudomeningocele and repair of the cerebrospinal fluid leakage was thus arranged for the *** the operation,we traced the cerebrospinal fluid leakage and found that it might have derived from incomplete endoscopic clipping around the tumor stump near the dural sac at the T3 *** that,we wrapped and sealed all the possible origins of the leakage with autologous fat,tissue glue,gelfoam,and duraseal layer by *** patient recovered well,and the computed tomography images showed resolution of the *** This report and literature review indicated that the risk of developing a tension pneumocephalus cannot be ignored and should be monitored carefully after thoracoscopic tumor resection.

Method of Inequalities-based Multiobjective Genetic Algorithm for Optimizing a Cart-double-pendulum System

This article presents a multiobjective approach to the design of the controller for the swing-up and handstand control of a general cart-double-pendulum system （CDPS）. The designed controller, which is based on the human-simulated intelligent control （HSIC） method, builds up different control modes to monitor and control the CDPS during four kinetic phases consisting of an initial oscillation phase, a swing-up phase, a posture adjustment phase, and a balance control phase. For the approach, the original method of inequalities-based （MoI） multiobjective genetic algorithm （MMGA） is extended and applied to the case study which uses a set of performance indices that includes the cart displacement over the rail boundary, the number of swings, the settling time, the overshoot of the total energy, and the control effort. The simulation results show good responses of the CDPS with the controllers obtained by the proposed approach.
