BACKGROUND Endoscopic retrograde cholangiopancreatography(ERCP)is a valuable therapeutic technique for pancreatobiliary diseases,and its application in the elderly is no longer ***,a higher incidence of procedure difficulty and periprocedural adverse events might be expected in elderly patients due to the presence of other medical disorders and the poor general condition of this *** To evaluate the incidence,causes,and management of difficult biliary cannulation during ERCP in elderly patients and the role of difficult cannulation as a risk factor for adverse *** A total of 614 patients who underwent ERCP during the study period were prospectively studied and divided into two groups based on their *** hundred and forty-six patients were aged 80 years or older and 468 patients were aged less than 80 *** primary outcome measurements were cannulation difficulty,cannulation success rate,ERCP procedure time,and related adverse *** There was no difference in the incidence of difficult cannulation among the two groups(32.9%vs 34.4%,P=0.765),as well as in the cannulation success rate(96.6%vs 96.8%,P=0.54).The cannulation techniques were shown to be safe and efficient in achieving successful *** regression analysis showed that patients aged 80 years or older were not associated with increased adverse events;however,difficult cannulation cases[adjusted odds ratio(AOR)=3.478;95%confidence interval(CI):1.877-6.442;P<0.001]and patients with Charlson Comorbidity Index≥2(AOR=1.824;95%CI:0.993-3.349;P=0.045)were more likely to develop adverse *** contrast,other factors including age≤65(AOR=3.460;95%CI:1.511-7.922;P=0.003),female gender(AOR=2.362;95%CI=1.089-5.124;P=0.030),difficult cannulation(AOR=4.527;95%CI:2.078-9.860;P<0.001),and patients with cholangitis(AOR=3.261;95%CI:1.204-8.832;P=0.020)were strongly associated with a higher rate of post-ERCP *** Advanced age has not been prove

Background: The successful treatment of military combat casualties with penetrating injuries is significantly dependent on the time needed to get the patient to an adequate treatment facility. Profound hypothermia induced suspended animation for delayed resuscitation(SADR) is a novel approach for inducing cardiac arrest and buying additional time for such injuries. However, the time used to safely administer circulatory arrest(CA) is controversial. The goal of this study was to evaluate the safety of hypothermia-induced SADR over 90 and 120 min time ***: Sixteen male BAMA minipigs were randomized into two groups: CA90 group(90 min, n =8) and CA120 group(120 min, n =8). Cannulation of the right common carotid arteries and internal jugular veins was performed to establish cardiopulmonary bypass for each animal. Through the perfusion of cold organ preservation solution(OPS), cardioplegia and profound hypothermia(15℃) were induced. After CA, cardiopumonary bypass(CPB) was restarted, and the animals were gradually re-warmed and resuscitated. The animals were assisted with ventilators until spontaneous breathing was achieved. The index of hemodynamic perioperative serum chemistry values [alanine transaminase(ALT), aspartate aminotransferase(AST), creatinine(CR), lactic dehydrogenase(LDH) and troponin T(TnT)] and survival were observed from pre-operation to 7 days ***: Fifteen animals were enrolled in the experiment, while 1 animal in CA120 group died from surgical error. All 8 animals in CA90 group recovered, with only 1 animal displaying mild disability. However, in CA120 group, only 2 animals survived with severe disability, and the other 5 animals died after 2 days post-operation. In CA90 group, the perioperative serum chemistry values increased at 1 day post-operation(ALT 84.43±18.65 U/L; AST 88.99±23.19 U/L; Cr 87.90±24.49μmol/L; LDH 1894.13±322.26 U/L; TnT 0.849±0.135 ng/ml) but decreased to normal or almost normal levels at 7 days p

Cold regions are a special combat environment in which low temperatures have a great impact on human metabolism and other vital functions, including the nervous, motion, cardiovascular, circulatory, respiratory, and urinary systems; consequently, low temperatures often aggravate existing trauma, leading to high mortality rates if rapid and appropriate treatment is not provided. Hypothermia is an independent risk factor of fatality following combat trauma; therefore, proactive preventative measures are needed to reduce the rate of mortality. After summarizing the basic research on battlefield environments and progress in the prevention and treatment of trauma, this article concludes that current treatment and prevention measures for combat trauma in cold regions are inadequate. Future molecular biology studies are needed to elucidate the mechanisms and relevant cell factors underlying bodily injury caused by cold environment, a research goal will also allow further exploration of corresponding treatments.

Monolithic catalysts with excellent O_(3)catalytic decomposition performance were prepared by in situ loading of Co-doped KMn_(8)O_(16)on the surface of nickel *** triple-layer structure with Co-doped KMn_(8)O_(16)/Ni6MnO_(8)/Ni foam was grown spontaneously on the surface of nickel foam by tuning the molar ratio of KMnO_(4)to Co(NO_(3))_(2)·6H_(2)O ***,the formed Ni6MnO_(8)structure between KMn_(8)O_(16)and nickel foam during in situ synthesis process effectively protected nickel foam from further etching,which significantly enhanced the reaction stability of *** optimum amount of Co doping in KMn_(8)O_(16)was available when the molar ratio of Mn to Co species in the precursor solution was 2:*** the Mn2Co1 catalyst had abundant oxygen vacancies and excellent hydrophobicity,thus creating outstanding O_(3)decomposition *** O_(3)conversion under dry conditions and relative humidity of 65%,90%over a period of 5 hr was 100%,94%and 80%with the space velocity of 28,000 hr^(−1),*** in situ constructed Co-doped KMn_(8)O_(16)/Ni foam catalyst showed the advantages of low price and gradual applicability of the preparation process,which provided an opportunity for the design of monolithic catalyst for O_(3)catalytic decomposition.

Alzheimer’s disease is the most frequent form of dementia characterized by the deposition of amyloid-beta plaques and neurofibrillary tangles consisting of hyperphosphorylated *** amyloid-beta plaques has been a primary direction for developing Alzheimer’s disease treatments in the last ***,existing drugs targeting amyloid-beta plaques have not fully yielded the expected results in the clinic,necessitating the exploration of alternative therapeutic *** evidence unravels that astrocyte morphology and function alter in the brain of Alzheimer’s disease patients,with dysregulated astrocytic purinergic receptors,particularly the P2Y1 receptor,all of which constitute the pathophysiology of Alzheimer’s *** receptors are not only crucial for maintaining normal astrocyte function but are also highly implicated in neuroinflammation in Alzheimer’s *** review delves into recent insights into the association between P2Y1 receptor and Alzheimer’s disease to underscore the potential neuroprotective role of P2Y1 receptor in Alzheimer’s disease by mitigating neuroinflammation,thus offering promising avenues for developing drugs for Alzheimer’s disease and potentially contributing to the development of more effective treatments.

Drug delivery systems(DDS)have recently emerged as a promising approach for the unique advantages of drug protection and targeted ***,the access of nanoparticles/drugs to the central nervous system(CNS)remains a challenge mainly due to the obstruction from brain *** cells infiltrating the CNS in the pathological state have inspired the development of strategies for CNS foundation drug ***,we outline the three major brain barriers in the CNS and the mechanisms by which immune cells migrate across the blood–brain *** subsequently review biomimetic strategies utilizing immune cell-based nanoparticles for the delivery of nanoparticles/drugs to the CNS,as well as recent progress in rationally engineering immune cell-based DDS for CNS ***,we discuss the challenges and opportunities of immune cell-based DDS in CNS diseases to promote their clinical development.

Catalytic oxidation of soot is of great importance for emission control on diesel *** this work,a highly active Cs/Co/Ce-Sn catalyst was investigated for soot oxidation,and it was unexpectedly found that high-temperature calcination greatly improved the activity of the *** the calcination temperature was increased from 500℃ to 750℃,T_(50) decreased from 456.9℃ to 389.8℃ in a NO/O_(2)/H_(2)O/N_(2) *** results revealed that high-temperature calcination can promote the ability to transfer negative charge density from Cs to other metal cations in Cs/Co/Ce-Sn,which will facilitate the production of more oxygen defects and the generation of more surface-active oxygen *** oxygen species are beneficial to the oxidation of NO to NO_(2),leading to the high yield of NO_(2) ***,the Cs/Co/Ce-Sn catalyst calcined at 750℃ demonstrated higher activity than that calcined at 500℃.This work provides a pathway to prepare high efficiency catalysts for the removal of soot and significant insight into the effects of calcination on soot oxidation catalysts.

Simulations of electron zebra stripes in the inner radiation belt using a composite empirical electric field model

The phenomenon termed“zebra stripes”manifests as regular patterns in the energy-space(L shell)spectrum of energetic electrons(ranging from tens to hundreds ke V)within the inner radiation *** structures exhibit drift-periodic behavior and commonly arise from large-scale electric field perturbations near the substorm *** this study,we introduce a composite electric field model and replicate the formation,structure,and evolution of zebra stripes using a bounce-averaged test particle code under this electric field ***-resolution measurements of energetic electrons obtained from the Van Allen Probes and the recently launched Macao Science Satellites-1 are used as initial conditions and served to validate our test particle *** analyses between observed data and simulations demonstrate our test particle method's efficacy in capturing zebra stripes'general ***,the composite model proves capable of reproducing realistic variations in the electric field within the inner radiation belt to a certain ***,subtle differences emerge in the flux strength and the positions of *** disparities primarily stem from limitations inherent in the electric field model and the initial conditions of the *** that the model represents an average case,it is conceivable that real-world scenarios may deviate from the average,thereby introducing variations in the observed phenomena.
