Predictors of the outcomes of acute-on-chronic hepatitis B liver failure

AIM:To identify the risk factors in predicting the outcome of acute-on-chronic hepatitis B liver failure patients.METHODS:We retrospectively divided 113 patients with acute-on-chronic liver failure-hepatitis B virus(ACLF-HBV) and without concurrent hepatitis C or D virus infection and hepatocellular carcinoma into two groups according to their outcomes after anti-HBV therapy.Their demographic,clinical,and biochemical data on the day of diagnosis and after the first week of treatment were analyzed using the Mann-Whitney U test,Fisher's exact test,and a multiple logistic regression analysis.RESULTS:The study included 113 patients(87 men and 26 women) with a mean age of 49.84 years.Fiftytwo patients survived,and 61 patients died.Liver failure(85.2%),sepsis(34.4%),and multiple organ failure(39.3%) were the main causes of death.Multivariate analyses showed that Acute Physiology and Chronic Health Evaluation(APACHE) Ⅱ scores ≥ 12 [odds ratio(OR) = 7.160,95% CI:2.834-18.092,P < 0.001] and positive blood culture(OR = 13.520,95% CI:2.740-66.721,P = 0.001) on the day of diagnosis and model for end-stage liver disease(MELD) scores ≥ 28(OR = 8.182,95% CI:1.884-35.527,P = 0.005) after the first week of treatment were independent predictors of mortality.CONCLUSION:APACHE Ⅱ scores on the day of diagnosis and MELD scores after the first week of anti-HBV therapy are feasible predictors of outcome in ACLFHBV patients.

Complex interactions among insect viruses-insect vector-arboviruses

Insects are the host or vector of diverse viruses including those that infect vertebrates,plants,and fungi.Insect viruses reside inside their insect hosts and are vertically transmitted from parent to offspring.The insect virus-host relationship is intricate,as these viruses can impact various aspects of insect biology,such as development,reproduction,sex ratios,and immunity.Arthropod-borne viruses(arboviruses)that cause substantial global health or agricultural problems can also be vertically transmitted to insect vector progeny.Multiple infections with insect viruses and arboviruses are common in nature.Such coinfections involve complex interactions,including synergism,dependence,and antagonism.Recent studies have shed light on the influence of insect viruses on the competence of insect vectors for arboviruses.In this review,we focus on the biological effects of insect viruses on the transmission of arboviruses by insects.We also discuss the potential mechanisms by which insect viruses affect the ability of hosts to transmit arboviruses,as well as potential strategies for disease control through manipulation of insect viruses.Analyses of the interactions among insect vectors,insect viruses and arboviruses will provide new opportunities for development of innovative strategies to control arbovirus transmission.

γδT cells are a kind of innate immune T cell.They have not attracted sufficient attention because they account for only a small proportion of all immune cells,and many basic factors related to these cells remain unclear.However,in recent years,with the rapid development of tumor immunotherapy,γδT cells have attracted increasing attention because of their ability to exert cytotoxic effects on most tumor cells without major histocompatibility complex(MHC)restriction.An increasing number of basic studies have focused on the development,antigen recognition,activation,and antitumor immune response ofγδT cells.Additionally,γδT cell-based immunotherapeutic strategies are being developed,and the number of clinical trials investigating such strategies is increasing.This review mainly summarizes the progress of basic research and the clinical application ofγδT cells in tumor immunotherapy to provide a theoretical basis for further the development ofγδT cell-based strategies in the future.

Chain Dynamics Heterogeneity in Plasticized Poly(vinyl butyral)(PVB) as Elucidated by Solid-State NMR

The chain dynamics heterogeneity of the poly(vinyl butyral)(PVB) plasticized by triethylene glycol bis(2-ethylhexa noate)(TEG-EH) was investigated by various solid-state NMR techniques.The plasticized PVB shows two domains in distinct molecular dynamics differences,namely,rigid and soft domains,where the latter is the plasticizer-rich domain.The time domain low field NMR was first used to investigate the dynamics heterogeneity of the plasticized PVB,and the results show the decreasing activated energy of components in the soft domain of plasticized PVB(E_a=20.2 kJ/mol) as compared with that of the pristine one(E_a=24.3 kJ/mol).Detailed dynamics heterogeneity was obtained by high-field NMR with site-specific features.The quadrupole-echo ~2H-NMR was adopted to elucidate the dynamics heterogeneity of the vinyl alcohol(VA) units,where only the hydroxyl group of VA is deuterated.The ~1H-^(13)C WISE NMR spectra show that there is not much difference in the mobility of the VB unit in PVB with and without plasticizer,whereas the glass transition temperature differed by approximately 53℃.This is further supported by Torchia's T_1 relaxation measurements.The origin of such an unusual phenomenon is attributed to the critical role of the remaining VA(~22%) in the soft domain,where the VA units locally aggregate through hydrogen bonding.Also,the existence of a mobility gradient in the VB unit has been demonstrated.Moreover,the mobility difference for VB with different stereo-geometry(meso or racemic conformation) is observed for the first time.This indicates the importance of modulating the ratio of meso over racemic VB for controlling the macroscopic perfo rmance of PVB.

Therapeutic utility of human umbilical cord-derived mesenchymal stem cells-based approaches in pulmonary diseases:Recent advancements and prospects

Pulmonary diseases across all ages threaten millions of people and have emerged as one of the major public health issues worldwide.For diverse disease con-ditions,the currently available approaches are focused on alleviating clinical symptoms and delaying disease progression but have not shown significant therapeutic effects in patients with lung diseases.Human umbilical cord-derived mesenchymal stem cells(UC-MSCs)isolated from the human UC have the capacity for self-renewal and multilineage differentiation.Moreover,in recent years,these cells have been demonstrated to have unique advantages in the treatment of lung diseases.We searched the Public Clinical Trial Database and found 55 clinical trials involving UC-MSC therapy for pulmonary diseases,including coronavirus disease 2019,acute respiratory distress syndrome,bron-chopulmonary dysplasia,chronic obstructive pulmonary disease,and pulmonary fibrosis.In this review,we summarize the characteristics of these registered clinical trials and relevant published results and explore in depth the challenges and opportunitiesfaced in clinical application.Moreover,the underlying mole-cular mechanisms involved in UC-MSC-based therapy for pulmonary diseases are also analyzed in depth.In brief,this comprehensive review and detailed analysis of these clinical trials can be expected to provide a scientific reference for future large-scale clinical application.

Atroposelective Synthesis of 2-Arylindoles via Chiral Phosphoric Acid-Catalyzed Direct Amination of Indoles

Indole-based atropisomers are a very important class of axially chiral compounds.However,the atroposelective synthesis of axially chiral 2-arylindole remains largely unexplored.In this study,we report the successful synthesis of atropisomeric 2-arylindoles using direct amination of indoles with p-quinonediimines in the presence of chiral phosphoric acid as a catalyst.Quinonediimine acts as an aminating reagent through formal polarity inversion of imine.The malonate group on the 2-aryl of 2-indoles was found to be essen-tial for high enantioselectivity of the products.This could be due to the additional interaction between the ester group and the cata-lyst,as well as the intramolecular hydrogen bonding.Our findings provide a new strategy for the asymmetric construction of 2-arylindole atropisomers.

Divergent responses of growth rate and antioxidative system of ten Bacillus strains to acid stresses

Response of growth rate and antioxidative system of ten Bacillus strains to acid stresses was assayed.Strong acid treatment significantly decreased the growth rate of the strains.Acid stresses increased the GPX activity and GSSG content of the tested strains.Divergent changes occurred in ROS and antioxidative system(SOD,CAT,GR,MDA and GSH).Environmental changes including soil acidification exert obvi-ous stresses on soil ecosystems and influence soil microor-ganisms.In this study,ten microbial strains were incubated under different acid treatments to investigate responses of microbial growth and antioxidative system to acid stress.All the strains belong to Bacillus genus,but exhibit distinct ecological functions.We observed that these microbial strains had obviously different pH tolerance threshold,in spite of the close phylogenetic classification among strains.Acid stresses exerted significant effects on microbial antiox-idative system,including superoxide dismutase(SOD),cata-lase(CAT)and glutathione transferring enzymes(GPX and GR)and reactants(GSH and GSSH),but the effects were strain specific.Furthermore,we found acid stress effects on total variances of the investigated microbial antioxidative system along the first two principal components(PCs).Activities of CAT and SOD contributed substantially to PC1 that reflected obvious acid effects on NC7 and ZC4,and closely related to intracellular malondialdehyde content.The GSSG activities and GSH/GSSG contributed greatly to PC2 that unveiled acid stress effects on most of the microbial strains.Our results highlight substantially heterogeneous responses of microbial strains to acid stress and support that phylogenetic closeness does not imply functional similarity of soil microorganisms under environmental changes.

Theoretical understanding of correlation between magnetic phase transition and the superconducting dome in high-Tc cuprates

Many issues concerning the origin of high-temperature superconductivity(HTS)are still under debate.For example,how the magnetic order varies with doping and its relationship with the superconducting temperature(Tc);and why Tcalways peaks near the quantum critical point.In this paper,taking hole-doped La_(2)CuO_(4)as a classical example,we employ the first-principles band structure and total energy calculations with Monte Carlo simulations to explore how the symmetry-breaking magnetic ground state evolves with hole doping and the origin of a dome-shaped superconductivity region in the phase diagram.We demonstrate that the local antiferromagnetic order and doping play key roles in determining the electron-phonon coupling,thus Tc.Initially,the La_(2)CuO_(4)possesses a checkerboard local antiferromagnetic ground state.As the hole doping increases,Tcincreases with the enhanced electron-phonon coupling strength.But as the doping increases further,the strength of the antiferromagnetic interaction weakens and spin fluctuation increases.At the critical doping level,a magnetic phase transition occurs that reduces the local antiferromagnetism-assisted electron-phonon coupling,thus diminishing the Tc.The superconductivity disappears in the heavily overdoped region when the ferromagnetic order dominates.These observations could account for why cuprates have a dome-shaped superconductivity region in the phase diagram.Our study,thus,contributes to a fundamental understanding of the correlation between doping,local magnetic order,and superconductivity of HTS.

Degradation of o-dichlorobenzene by DBD-NTP co-modified titanium gel catalyst

In order to study the degradation process of dioxins in industrial flue gas,the decomposition of o-dichlorobenzene(o-DCB)in a DBD plasma catalytic reactor was investigated.The results showed that an NTP-catalyzed system,especially using the Cu Mn Ti Oxcatalyst,had better o-DCB degradation performance compared to plasma alone.The combination of the Cu Mn Ti Oxcatalyst with NTP can achieve a degradation efficiency of up to 97.2%for o-DCB;the selectivity of CO and CO_(2)and the carbon balance were 40%,45%,and 85%,respectively.The dielectric constant and electrical property results indicated that the surface discharge capacity of the catalysts played a major role in the degradation of o-DCB,and a higher dielectric constant could suppress the plasma expansion and enhance the duration of the plasma discharge per discharge cycle.According to the O1s XPS and O_(2)-TPD results,the conversion of CO to CO_(2)follows the M-v-K mechanism;thus,the active species on the catalyst surface play an important role.Moreover,the Cu Mn Ti Oxand NTP mixed system exhibited excellent stability,which is probably because Cu doping improved the lifetime of the catalyst.This work can provide an experimental and theoretical basis for research in the degradation of o-DCB by plasma catalyst systems.

Investigation on cavitating turbulent flow for the twisted NACA66 hydrofoil using a PANS model with helicity modification

In the present paper,the unsteady cavitating turbulent flow over the twisted NACA66 hydrofoil is investigated based on an modified shear stress transfer k-ωpartially averaged Navier-Stokes(MSST PANS)model,i.e.,new MSST PANS(NMSST PANS)model,where the production term of kinetic energy in the turbulence model is modified with helicity.Compared with the experimental data,cavitation evolution and its characteristic frequency are satisfactorily predicted by the proposed NMSST PANS model.It is revealed that the interaction among the main flow,the reentrant jets,and sheet cavitation causes the formation of the primary shedding cavity near the mid-span and the secondary shedding cavity at each side of the twisted hydrofoil,and further induces the remarkable pressure gradient around shedding cavities.Along with the development of the primary and the secondary shedding cavities,the great pressure gradient associated with large cavity volume variation promotes the vortical flow generation and the spatial deformation of vortex structure during cavitation evolution,and results in the primary and the secondary U-type vortices.Further,dynamic mode decomposition(DMD)analysis is utilized to confirm the interaction among the main flow,the main reentrant jet and two side reentrant jets,and cavitation.These results indicate that the proposed NMSST PANS model is suitable to simulate the complicated cavitating turbulent flow for various engineering applications.
