Systematic review and network meta-analysis of different nonsteroidal anti-inflammatory drugs for juvenile idiopathic arthritis

BACKGROUND Various non-steroidal anti-inflammatory drugs(NSAIDs)have been used for juvenile idiopathic arthritis(JIA).However,the optimal method for JIA has not yet been developed.AIM To perform a systematic review and network meta-analysis to determine the optimal instructions.METHODS We searched for randomized controlled trials(RCTs)from PubMed,EMBASE,Google Scholar,CNKI,and Wanfang without restriction for publication date or language at August,2023.Any RCTs that comparing the effectiveness of NSAIDs with each other or placebo for JIA were included in this network meta-analysis.The surface under the cumulative ranking curve(SUCRA)analysis was used to rank the treatments.P value less than 0.05 was identified as statistically significant.RESULTS We included 8 RCTs(1127 patients)comparing 8 different instructions including meloxicam(0.125 qd and 0.250 qd),Celecoxib(3 mg/kg bid and 6 mg/kg bid),piroxicam,Naproxen(5.0 mg/kg/d,7.5 mg/kg/d and 12.5 mg/kg/d),inuprofen(30-40 mg/kg/d),Aspirin(60-80 mg/kg/d,75 mg/kg/d,and 55 mg/kg/d),Tolmetin(15 mg/kg/d),Rofecoxib,and placebo.There were no significant differences between any two NSAIDs regarding ACR Pedi 30 response.The SUCRA shows that celecoxib(6 mg/kg bid)ranked first(SUCRA,88.9%),rofecoxib ranked second(SUCRA,68.1%),Celecoxib(3 mg/kg bid)ranked third(SUCRA,51.0%).There were no significant differences between any two NSAIDs regarding adverse events.The SUCRA shows that placebo ranked first(SUCRA,88.2%),piroxicam ranked second(SUCRA,60.5%),rofecoxib(0.6 mg/kg qd)ranked third(SUCRA,56.1%),meloxicam(0.125 mg/kg qd)ranked fourth(SUCRA,56.1%),and rofecoxib(0.3 mg/kg qd)ranked fifth(SUCRA,56.1%).CONCLUSION In summary,celecoxib(6 mg/kg bid)was found to be the most effective NSAID for treating JIA.Rofecoxib,piroxicam,and meloxicam may be safer options,but further research is needed to confirm these findings in larger trials with higher quality studies.

What is already known about this topic?Vitamin A deficiency(VAD)is a leading global nutritional concern,ranking among the top four major nutritional deficiencies worldwide.The prevalence of VAD is unevenly distributed across various regions,both within China and globally.What is added by this report?The report adds valuable insights into the vitamin A nutritional status of rural students aged 6–17 years who participated in the Nutrition Improvement Programme for Rural Compulsory Education Students(NIPRCES).Over the decade from 2012 to 2021,there was a modest improvement in vitamin A status.The prevalence of VAD and sub-clinical VAD(SVAD)declined as the students aged.Throughout the majority of the survey years,the incidence of VAD was higher among males and western regions compared to females and central regions,respectively.What are the implications for public health practice?A comprehensive approach,incorporating dietary diversification,nutrition education,and food fortification,should be implemented to prevent VAD and SVAD especially in males,younger children and children in western areas.

MicroRNAs(miRNAs)have received much attention in the past decade as potential key epigenomic regulators of tumors and cancer stem cells(CSCs).The abnormal expression of miRNAs is responsible for different phenotypes of gastric cancer stem cells(GCSCs).Some specific miRNAs could be used as promising biomarkers and therapeutic targets for the identification of GCSCs.This review summarizes the coding process and biological functions of miRNAs and demon-strates their role and efficacy in gastric cancer(GC)metastasis,drug resistance,and apoptosis,especially in the regulatory mechanism of GCSCs.It shows that the overexpression of onco-miRNAs and silencing of tumor-suppressor miRNAs can play a role in promoting or inhibiting tumor metastasis,apart from the initial formation of GC.It also discusses the epigenetic regulation and potential clinical applications of miRNAs as well as the role of CSCs in the pathogenesis of GC.We believe that this review may help in designing novel therapeutic approaches for GC.

Effect of tungsten carbide particles on microstructure and mechanical properties of Cu alloy composite bit matrix

Copper alloy composite bit matrix was prepared by pressureless vacuum infiltration,using at least one of the three kinds of tungsten carbide particles,for example,irregular cast tungsten carbide,monocrystalline tungsten carbide and sintered reduced tungsten carbide particles.The effects of powder particle morphology,particle size and mass fraction of tungsten carbide on the microstructure and mechanical properties of copper alloy composite were investigated by means of scanning electron microscopy,X-ray diffraction and abrasive wear test in detail.The results show that tungsten carbide morphology and particle size have obvious effects on the mechanical properties of copper alloy composites.Cast tungsten carbide partially dissolved in the copper alloy binding phase,and layers of Cu_(0.3)W_(0.5)Ni_(0.1)Mn_(0.1)C phase with a thickness of around 8–15μm were formed on the edge of the cast tungsten carbide.When 45%irregular crushed fine cast tungsten carbide and 15%monocrystalline cast tungsten carbide were used as the skeleton,satisfactory comprehensive performance of the reinforced copper alloy composite bit matrix was obtained,with the bending strength,impact toughness and hardness reaching 1048 MPa,4.95 J/cm^(2) and 43.6 HRC,respectively.The main wear mechanism was that the tungsten carbide particles firstly protruded from the friction surface after the copper alloy matrix was worn,and then peeled off from the matrix when further wear occurred.

due to the easy coarsening caused by poor thermal stability,the verified annealing-induced hardening in nanograined metals can only maintain at a relatively low-temperature range.In this study,a nanolam-inated(CrCoNi)_(97.4)Al_(0.8)Ti_(1.8)medium-entropy alloy with an average lamellae thickness of∼20 nm embedded by thinner nanotwins was fabricated by severe cold rolling to achieve superior thermal stability.Compared with the conventional nanotwinned CrCoNi with nanotwins inside ultra-fined grains,the hier-archical nanolaminated-nanotwinned(CrCoNi)_(97.4)Al_(0.8)Ti_(1.8) exhibits a significant annealing-induced hard-ening effect,i.e.,hardness increasing from∼250 HV in the original specimen to∼500 HV in the cold-rolled status and finally∼630 HV after annealing at 600℃for 1 h.Detailed microstructure characterizations reveal that the reduced dislocation density and formation of L1_(2)ordered domain are mainly responsible for such hardening effect,which is facilitated by the effectively suppressed coarsening with annealing temperature,i.e.,slow detwinning process and well-retained low-angle nanolamellar structure.The coarsening mechanisms from the cold-rolled nanolamellae to the fully recrystallized micro-equiaxed structures under the annealing temperatures ranging from 400 to 800℃ were also elucidated by atomic observations.

Coal is one of the world’s main energy resources,accounting for approximately 68%of China’s current total power generation.However,several studies have demonstrated that dust,exhaust fumes,and other harmful factors in coal mines increase the risk of cardiovascular disease(CVD)among miners^([1]).Arterial stiffness(AS)is an independent risk factor of CVD,and epidemiological studies have shown that AS plays a vital role in assessing the risk of CVD^([2]).

Grain boundary diffusion process(GBDP)has been developed as an effective approach to increase the coercivity of sintered Nd-Fe-B magnets by regulating the compositions and phase distributions near grain boundaries.This work aims to explore how to select the optimum annealing temperature after GBDP.In this work GBDP was performed on a sintered Nd-Fe-B magnet using Dy_(70)Cu_(30) alloy.After GBDP the low eutectic temperature of the grain boundary phases decreases from the initial 492 to 451℃.The magnetic property dependent on different annealing temperatures near the low eutectic temperature was studied.The magnetic properties,especially the squareness factor of demagnetization curve show a strong dependence on the annealing temperature.After GBDP the optimal magnetic properties can be obtained after annealing just above the low eutectic temperature of the grain boundary phases.The mechanism is discussed based on the microstructure analysis.

Fat is one of the three macronutrients and a significant energy source for piglets. It plays a positive role in maintaining intestinal health and improving production performance. during the weaning period,physiological, stress and diet-related factors influence the absorption of fat in piglets, leading to damage to the intestinal barrier, diarrhea and even death. Signaling pathways, such as fatty acid translocase(CD36), pregnane X receptor(PXR), and AMP-dependent protein kinase(AMPK), are responsible for regulating intestinal fat uptake and maintaining intestinal barrier function. Therefore, this review mainly elaborates on the reasons for diarrhea induced by insufficient fat absorption and related signaling pathways in weaned-piglets, with an emphasis on the intestinal fat absorption disorder. Moreover, we focus on introducing nutritional strategies that can promote intestinal fat absorption in piglets with insufficient fat absorption-related diarrhea, such as lipase, amino acids, and probiotics.

Background Clonorchiasis and opisthorchiasis, caused by the liver flukesClonorchis sinensis andOpisthorchis viverrini respectively, represent significant neglected tropical diseases (NTDs) in Asia. The co-existence of these pathogens in overlapping regions complicates effective disease control strategies. This study aimed to clarify the distribution and interaction of these diseases within Southeast Asia.Methods We systematically collated occurrence records of human clonorchiasis (n = 1809) and opisthorchiasis (n = 731) across the Southeast Asia countries. Utilizing species distribution models incorporating environmental and climatic data, coupled machine learning algorithms with boosted regression trees, we predicted and distinguished endemic areas for each fluke species. Machine learning techniques, including geospatial analysis, were employed to delineate the boundaries between these flukes.Results Our analysis revealed that the endemic range ofC. sinensis andO. viverrini in Southeast Asia primarily spans across part of China, Vietnam, Thailand, Laos, and Cambodia. during the period from 2000 to 2018, we identifiedC. sinensis infections in 84 distinct locations, predominantly in southern China (Guangxi Zhuang Autonomous Region) and northern Vietnam. In a stark contrast,O. viverrini was more widely distributed, with infections documented in 721 locations across Thailand, Laos, Cambodia, and Vietnam. Critical environmental determinants were quantitatively analyzed, revealing annual mean temperatures ranging between 14 and 20 °C in clonorchiasis-endemic areas and 24–30 °C in opisthorchiasis regions (P < 0.05). The machine learning model effectively mapped a distinct demarcation zone, demonstrating a clear separation between the endemic areas of these two liver flukes with AUC from 0.9 to1. The study in Vietnam delineates the coexistence and geographical boundaries ofC. sinensis andO. viverrini, revealing distinct endemic zones and a transitional area where both liver fluke species overlap.Conclusions Our findings
