Efficacy and Safety of Bufei Jiedu Granules in Treating Multidrug-Resistant Pulmonary Tuberculosis:A Multi-center,Double-Blinded and Randomized Controlled Trial

Objective: To assess the efficacy and safety of Bufei Jiedu(BFJD) Granules as adjuvant therapy for patients with multidrug-resistant pulmonary tuberculosis(MDR-PTB). Methods: A large-scale, multi-center, double-blinded, and randomized controlled trial was conducted in 18 sentinel hospitals in China from December 2012 to December 2016. A total of 312 MDR-PTB patients were randomly assigned to BFJD Granules or placebo groups(1:1) using a stratified randomization method, which both received the long-course chemotherapy regimen for 18 months(6 Am-Lfx-P-z-Pto, 12 Lfx-P-z-Pto). Meanwhile, patients in both groups also received BFJD Granules or placebo twice a day for a total of 18 months, respectively. The primary outcome was cure rate. The secondary outcomes included time to sputum-culture conversion, changes in lung cavities and quality of life(QoL) of patients. Adverse reactions were monitored during and after the trial. Results: A total of 216 cases completed the trial, 111 in the BFJD Granules group and 105 in the placebo group. BFJD Granules, as adjuvant treatment, increased the cure rate by 13.6% at the end of treatment, compared with the placebo(58.4% vs. 44.8%, P=0.02), and accelerated the median time to sputum-culture conversion(5 months vs. 11 months). The cavity closure rate of the BFJD Granules group(50.6%, 43/85) was higher than that of the placebo group(32.1%, 26/81;P=0.02) in patients who completed the treatment. At the end of the intensive treatment, according to the 36-item Short Form, the BFJD Granules significantly improved physical functioning, general health, and vitality of patients relative to the placebo group(all P<0.01). Overall, the death rates in the two groups were not significantly different;5.1%(8/156) in the BFJD Granules group and 2.6%(4/156) in the placebo group. We investigate the uniqueness problems of meromorphic functions and their difference operators by using a new *** is proved that if a non-constant meromorphic function f shares a non-zero constant and ∞ counting multiplicities with its difference operators Δcf(z) and Δ_(c)^(2)f(z), thenΔcf(z)≡Δ_(c)^(2)f(z).In particular,we give a difference analogue of a result of *** method has two distinct features:(ⅰ) It converts the relations between functions into the corresponding *** makes it possible to deal with the uniqueness problem by linear algebra and combinatorics.(ⅱ) It circumvents the obstacle of the difference logarithmic derivative lemma for meromorphic functions of infinite order,since this method does not depend on the growth of the ***,the idea in this paper can also be applied to the case for several variables.

RepBoTNet-CESA:An Alzheimer’s Disease Computer Aided Diagnosis Method Using Structural Reparameterization BoTNet and Cubic Embedding Self Attention

Various deep learning models have been proposed for the accurate assisted diagnosis of early-stage Alzheimer’s disease(AD).Most studies predominantly employ Convolutional Neural Networks(CNNs),which focus solely on local features,thus encountering difficulties in handling global *** contrast to natural images,Structural Magnetic Resonance Imaging(sMRI)images exhibit a higher number of channel ***,during the Position Embedding stage ofMulti Head Self Attention(MHSA),the coded information related to the channel dimension is *** tackle these issues,we propose theRepBoTNet-CESA network,an advanced AD-aided diagnostic model that is capable of learning local and global features *** combines the advantages of CNN networks in capturing local information and Transformer networks in integrating global information,reducing computational costs while achieving excellent classification ***,it uses the Cubic Embedding Self Attention(CESA)proposed in this paper to incorporate the channel code information,enhancing the classification performance within the Transformer ***,the RepBoTNet-CESA performs well in various AD-aided diagnosis tasks,with an accuracy of 96.58%,precision of 97.26%,and recall of 96.23%in the AD/NC task;an accuracy of 92.75%,precision of 92.84%,and recall of 93.18%in the EMCI/NC task;and an accuracy of 80.97%,precision of 83.86%,and recall of 80.91%in the AD/EMCI/LMCI/NC *** demonstrates that RepBoTNet-CESA delivers outstanding outcomes in various AD-aided diagnostic ***,our study has shown that MHSA exhibits superior performance compared to conventional attention mechanisms in enhancing ResNet ***,the Deeper RepBoTNet-CESA network fails to make further progress in AD-aided diagnostic tasks.

Ultralow-phase-noise and broadband frequency-hopping coupled optoelectronic oscillator under quiet point operation

Advancements in microwave photonics have yielded novel approaches for generating high-purity microwave sources. Among these, optoelectronic oscillators（OEOs） and coupled optoelectronic oscillators（COEOs） have demonstrated the capability to generate frequency-independent microwaves with exceptionally low phase ***, the tunability of the oscillators is rather limited due to the necessity for narrowband electronic bandpass filters, presenting challenges in achieving both wide and rapid tuning capabilities. Here, we present a COEO featuring ultralow phase noise, flexible tuning capability, and high robustness. This is achieved through a quiet point（QP）-operated harmonic mode-locked fiber laser, which effectively mitigates optical amplifier noise and supermode competition, thus significantly diminishing the necessity for ultra-narrow electronic filters. Due to the liberated tuning ability, we present an oscillator that can be tuned from 2 GHz to 18 GHz, with phase noise as low as-140 d Bc/Hz at 10 k Hz under the QP operation. We then illustrate the practical application of the proposed oscillator in generating frequency-hopping signals with consistent spurious modes less than-85 d Bc,absolute phase noise below-135 d Bc/Hz at 10 k Hz, hopping resolution of 1.25 MHz, and fractional frequency stability below 6.1 × 10-12at 1 s averaging time when locked to a reference. The presented COEO structure emerges as a compelling solution for agile and low-noise microwave sources in advanced wireless communication and radar systems.
