Steroids in Maxillofacial Space Infection: A Retrospective Cohort Study

Maxillofacial space infection (MSI) is one of the most common conditions encountered in oral and maxillofacial surgery clinics. Early recognition and proper management of MSI could prevent a life-threatening event. Objectives: To report a series of MSI managed with antibiotics, surgical intervention and exogenous steroids as an adjunct, highlighting functional improvement following steroid administration. Methods: A retrospective cohort study was carried out from December 2013 to September 2016, involving 30 patients (n = 30, 22 males, 8 females) diagnosed with MSI. All patients were initially managed with intravenous empirical antibiotics, analgesics and removal of potential source of infection. A course of 3 doses of IV Dexamethasone 8 mg at an interval of 8 hours was started during the first day of hospital admission. Results: This series reports 30 patients presenting with MSI, who received prompt antibiotics and 3 doses of steroids as inpatients. Significant clinical improvement was noted in the form of amelioration of pain, rapid reduction in edema, and improved trismus, shortening hospital stay to an average of 3.5 days, and omission of surgical intervention in 50% of our cases. No adverse effects or drug reaction was noted. Conclusion: In conclusion, the value of synergistic use of corticosteroids with antibiotics in management of MSI is significant. Despite these promising findings, there is scarce evidence in the literature to fully support the use of corticosteroids in abscess management. The role of corticosteroids in treatment of MSI should be explored further.

Wisdom Tooth Surgery Complications—Local Anaesthesia versus General Anaesthesia

Purpose: To compare the complication rates arising from surgical removal of lower third molars (L3M) under general anaesthesia (GA) versus local anaesthesia (LA) in the Oral and Maxillofacial Surgery (OMFS) Department, Hospital Sultanah Nora Ismail (HSNI), Batu Pahat, Johor, Malaysia. Materials and methods: This is a retrospective clinical audit of patients who underwent L3M removal under LA or GA from 1/1/2013 to 31/3/2018, with recorded complications such as surgical site infection (SSI), wound breakdown, severe pain, trismus, retained tooth structure, nerve injury and dry socket evaluated at different time intervals. Results: A total of 313 patients with 375 L3M were included in this study (male: 160, 51.1%;female: 153, 48.9%) with an age range of 18 to 40 years (mean = 27.43 years). 79 L3M were removed under GA (34.2% were classified as mild;65.8% were moderate), whereas 296 L3M were removed under LA, (31.4% were mild impaction, 63.9% moderate and 4.7% severe. Preoperative antibiotics were given before all L3M removal under GA, while only 23 out of 296 L3M removal under LA had antibiotics. 15.2% of L3M removal under GA and 16.6% of those done under LA were associated with complications. Generally, patient with GA had lesser complications;however only SSI outcome was significant (p = 0.034). Conclusion: L3M removal under LA may have a higher risk of SSI when compared to GA. This raises the possibility that a single prophylactic antibiotic dose may prevent SSI in LA procedures.

BACKGROUND Ankylosing spondylitis(AS)is strongly associated with the human leukocyte antigen(HLA)B27 *** regions where conventional polymerase chain reaction for HLA typing is available for antigens such as HLA B27 or HLA B51,it is common to perform the HLA B27 test for evaluation of *** HLA B27-associated clustered occurrences of AS have been reported in families,we report the first case series of HLA B51-related occurrences of AS in a *** SUMMARY A father and his daughters were diagnosed with AS and did not have the HLA B27 *** they were positive for HLA B51,they exhibited no signs of Behçet’s disease(BD).Of the five daughters,one had AS,and three,including the daughter with AS,were positive for HLA *** two daughters with the HLA B51 haplotype(excluding the daughter with AS)exhibited bilateral grade 1 sacroiliitis,whereas the daughters without the HLA B51 haplotype did not have ***,this Korean family exhibited a strong association with the HLA B51 haplotype and clinical sacroiliitis,irrespective of the symptoms of *** It is advisable to check for HLA B51 positivity in patients with AS/spondyloarthropathy who test negative for HLA B27.

The genus Lactarius Pers.(Russulales)is a cosmopolitan group of Basidiomycota that forms ectomycorrhizal relationships primarily with both deciduous and coniferous *** the genus has been well-studied in Europe and North America,only fragmentary researches have been carried out on Asian *** particular,the distribution of Lactarius species in South Korea is poorly understood due to insufficient morphological descriptions and a lack of DNA sequence *** addition,the misuse of European and North American names has added to confusion regarding the taxonomy of Asian Lactarius *** this study,the diversity of Lactarius in South Korea was evaluated by employing both morphological and phylogenetic approaches.A multi-locus phylogenetic analysis of 729 Lactarius specimens collected between 1960 and 2017 was performed using the internal transcribed spacer(ITS)region,partial nuclear ribosomal large subunit(nrLSU),partial second largest subunit of RNA polymerase II(rpb2),and minichromosome maintenance complex component 7(mcm7).49 Lactarius species were identified in three Lactarius subgenera:***(17 spp.),***(22 spp.),and ***(10 spp.).Among them,28 Lactarius species were identified as new to science,while just 17 were previously described Lactarius *** of the taxa remain un-named due to paucity of materials.A key to Korean Lactarius species,molecular phylogenies,a summary of diversity,and detailed description are provided.

Intravitreal aflibercept in neovascular age-related macular degeneration previously treated with ranibizumab

AIM：To report the change in visual acuity and central macular thickness（CMT）following treatment with intravitreal aflibercept injections in patients with neovascular agerelated macular degeneration（n AMD）with suboptimum response to ***：This was a retrospective study. The inclusion criteria were patients with n AMD who responded poorly to ranibizumab. Patients then received either 3 consecutive aflibercept injections followed by pro re nata（PRN）treatment or PRN alone. Primary endpoints were mean change in bestcorrected visual acuity（BCVA）and CMT at 12 mo. Secondary endpoints were number of injections and adverse ***：Forty-nine eyes from 49 patients met the inclusion criteria and completed 12-month follow up on aflibercept. Thirty-eight eyes received 3 consecutive aflibercept injections followed by PRN treatment and 11 eyes received PRN injections alone. At 12 mo,mean BCVA improved by one letters（log MAR 0.56±0.31 to 0.54±0.34）and mean CMT decreased from 303.9±82.1 to 259.2±108.3 μ*** percent of eyes gained 15 letters or more,6% lost more than 15 letters and the remaining 90% had stable BCVA. Accurate photovoltaic(PV)power prediction has been a subject of ongoing study in order to address grid stability concerns caused by PV output unpredictability and *** paper proposes an ultra-short-term hybrid photovoltaic power forecasting method based on a dendritic neural model(DNM)in this *** model is trained using improved biogeography-based optimization(IBBO),a technique that incorporates a domestication operation to increase the performance of classical biogeography-based optimization(BBO).To be more precise,a similar day selection(SDS)technique is presented for selecting the training set,and wavelet packet transform(WPT)is used to divide the input data into many *** is then used to train DNM weights and thresholds for each component ***,each component’s prediction results are stacked and *** suggested hybrid model is used to forecast PV power under various weather conditions using data from the Desert Knowledge Australia Solar Centre(DKASC)in Alice *** results indicate the proposed hybrid SDS and WPT-IBBO-DNM model has the lowest error of any of the benchmark models and hence has the potential to considerably enhance the accuracy of solar power forecasting(PVPF).
