目的探讨基于医联体的基层全科医生技能培训模式,并进行效果评价。方法选取2019—2021年参加自贡市中医适宜技术培训的108名基层全科医生为研究对象,其中沿滩区自贡市中医医院医联体成员单位的51名学员为观察组,非医联体成员单位的57名学员为对照组。观察组采用医联体模式,对照组采用培训班模式。1年后,采用柯氏模型从反应层、学习层、行为层、结果层4个层面评价培训效果。结果在反应层方面,对照组总分为(37.51±3.42)分,观察组为(40.71±3.79)分,观察组高于对照组(t=4.609,P<0.001)。在学习层方面,对照组总分为(67.11±10.28)分,观察组为(72.67±11.69)分,观察组高于对照组(t=2.631,P=0.010)。在行为层方面,对照组总分为(30.63±9.98)分,观察组为(34.16±7.85)分,观察组高于对照组(t=2.024,P=0.045)。在结果层方面,对照组总分为(32.33±4.39)分,观察组为(36.16±8.24)分,观察组高于对照组(t=2.960,P=0.004)。结论基于医联体的技能培训可以提升基层全科医生中医适宜技术水平,促进其学习和医疗行为,并且具有良好的个人效益、组织效益和辐射效益。

Esophageal cancer is one of the leading causes of cancer death in the world,with approximately half of the new cases occurring in China every year.^(1)Esophageal squamous cell carcinoma(ESCC)is the main subtype,accounting for more than 90%,and the five-year survival rate is less than 10%.Using large-scale genome analysis,many driver mutations and key pathways associated with ESCC have been identified.However,these genomic signatures have not improved the clinical management of EscC patients,or established effective targeted therapy.^(2)Esophageal cancer still lacks representative molecular markers.

The E2F family of transcription factors is crucial for cell cycle progression and cell fate decisions.Although E2Fs have been widely studied in mammals,there have been few studies performed in insects.Here,we determined the function of E2F4 in the silkworm,Bombyx mori.We demonstrate that E2F proteins are highly conserved among species from lower animals to higher mammals.Overexpression of the BmE2F4 gene led to cell cycle arrest in the G1 phase,whereas interfering with the BmE2F4 mRNA led to accumulation of cells in the S phase.These results indicate that BmE2F4 is important in cell cycle regulation.We also demonstrate that the BmE2F4 gene is involved in DNA replication of BmN-SWU1 cells and DNA synthesis in the silk gland.Furthermore,we identified a protein called Bm14-3-3ζthat can interact with BmE2F4 and allow it to localize in the nucleus.Overexpression of the Bm14-3-3ζgene led to cell cycle arrest in the G1 phase,while knocking down the gene increased the proportion of cells in S phase.These findings provide important insights into the function of E2F transcription factors and increase our understanding of their involvement in cell cycle regulation.

New classification system for radical rectal cancer surgery based on membrane anatomy

BACKGROUND Total mesorectal excision along the“holy plane”is the only radical surgery for rectal cancer,regardless of tumor size,localization or even tumor stage.However,according to the concept of membrane anatomy,multiple fascial spaces around the rectum could be used as the surgical plane to achieve radical resection.AIM To propose a new membrane anatomical and staging-oriented classification system for tailoring the radicality during rectal cancer surgery.METHODS A three-dimensional template of the member anatomy of the pelvis was established,and the existing anatomical nomenclatures were clarified by cadaveric dissection study and laparoscopic surgical observation.Then,we suggested a new and simple classification system for rectal cancer surgery.For simplification,the classification was based only on the lateral extent of resection.RESULTS The fascia propria of the rectum,urogenital fascia,vesicohypogastric fascia and parietal fascia lie side by side around the rectum and form three spaces(medial,middle and lateral),and blood vessels and nerves are precisely positioned in the fascia or space.Three types of radical surgery for rectal cancer are described,as are a few subtypes that consider nerve preservation.The surgical planes of the proposed radical surgeries(types A,B and C)correspond exactly to the medial,middle,and lateral spaces,respectively.CONCLUSION Three types of radical surgery can be precisely defined based on membrane anatomy,including nerve-sparing procedures.Our classification system may offer an optimal tool for tailoring rectal cancer surgery.

超声骺软骨厚度与骨龄的相关性分析

目的结合长骨关节端的声像表现,初步探索采用超声定量评估骺软骨的发育情况,并分析其与骨龄的相关性。方法对2023年3—6月就读于济宁市某体校的青少年进行研究,记录年龄、身高等信息,于1周内行X线骨龄检查,并由1名内分泌科医师使用Greulich-Pyle图谱法评估骨龄。由2名超声医学科医师针对非优势侧手及腕部、膝关节共7个部位(包括第三掌骨头部背侧、尺骨茎突、桡骨茎突、股骨内上髁、股骨外上髁、胫骨背侧、胫骨内侧髁)进行超声检查,测量超声骺软骨厚度(ultrasonic epiphysis cartilage thickness,UECT),并分析其与骨龄的相关性。采用组内相关系数(intraclass correlation coefficient,ICC)评估UECT测量的信度与可重复性。结果共纳入141名青少年,其中男性80名,女性61名。中位年龄为13(12,14)岁,男性中位骨龄为15(13,17)岁,女性中位骨龄16(14,17)岁,男、女骨龄与年龄差值>1岁者分别占比60%、67%,男性身高显著高于女性。男、女7个部位UECT均随骨龄增长而减小,各部位UECT与骨龄均呈负相关。男性单部位UECT与骨龄均呈高度负相关(|r|≥0.80),女性则表现出中高度负相关(0.65≤|r|≤0.75)。男、女7个部位UECT总和与骨龄的负相关性均达到较高水平(|r|=0.93,|r|=0.80),单独选取手和腕部3个部位或膝关节4个部位时,女性UECT总和与骨龄呈高度负相关(|r|≥0.76),而男性则表现出非常高度负相关性(|r|≥0.90)。7个部位UECT的测量一致性均较好(ICC≥0.75)。结论超声可观察描述长骨关节端的生长发育变化,UECT与骨龄表现出中至高度负相关,具有用于骨成熟度定量评估的潜在价值。

健脾益肾消溃方治疗溃疡性结肠炎脾肾阳虚证的研究

目的:观察健脾益肾消溃方治疗溃疡性结肠炎脾肾阳虚证的疗效及对白细胞介素(interleukin,IL)-6、IL-10、肿瘤坏死因子(tumor necrosis factor,TNF)-α水平和炎症性肠病问卷(inflammatory bowel disease questionnaire,IBDQ)评分的影响。方法:将64例溃疡性结肠炎脾肾阳虚证患者采用随机数字表法分为治疗组和对照组,每组32例。对照组给予美沙拉秦缓释颗粒,0.5 g/次,3次/d,口服。治疗组给予健脾益肾消溃方(药物组成:山药、淫羊藿、薏苡仁、炒白芍、党参、陈皮、茯苓、白及、地榆炭、五味子、吴茱萸、甘草片)治疗,1剂/d,水煎至400 mL,200 mL/次,早晚2次温服。两组均治疗4周判定疗效。结果:治疗组治愈19例,有效5例,好转6例,无效2例,有效率为93.75%(30/32);对照组治愈12例,有效6例,好转5例,无效9例,有效率为71.88%(23/32)。两组疗效对比,差异有统计学意义(P<0.05)。治疗后,两组临床症状评分、中医证候积分、IL-6和TNF-α较治疗前下降(P<0.01),且治疗组低于对照组(P<0.01);IL-10和IBDQ评分较治疗前升高(P<0.01),且治疗组高于对照组(P<0.01)。结论:健脾益肾消溃方能较好改善溃疡性结肠炎患者临床症状、中医证候、炎症因子水平和IBDQ评分。

Effects of Acupotomy on Immobilization-Induced Gastrocnemius Contracture and Fibrosis in Rats via Wnt/β-Catenin Signaling

Objective: To determine whether acupotomy ameliorates immobilization-induced muscle contracture and fibrosis via Wnt/β-catenin signaling pathway. Methods: Thirty Wistar rats were randomly divided into 5 groups(n=6) by a random number table, including control, immobilization, passive stretching, acupotomy, and acupotomy 3 weeks(3-w) groups. The rat model of gastrocnemius contracture was established by immobilizing the right hind limb in plantar flexion for 4 weeks. Rats in the passive stretching group received passive stretching at gastrocnemius, a daily series of 10 repetitions for 30 s each at 30-s intervals for 10 consecutive days. Rats in the acupotomy and acupotomy 3-w groups received acupotomy once and combined with passive stretching at gastrocnemius a daily series of 10 repetitions for 30 s each at 30-s intervals for 10 consecutive days. Additionally, rats in the acupotomy 3-w group were allowed to walk freely for 3 weeks after 10-day therapy. After treatment, range of motion(ROM), gait analysis [i.e., paw area, stance/swing and maximum ratio of paw area to paw area duration(Max dA/dT)], gastrocnemius wet weight and the ratio of muscle wet weight to body weight(MWW/BW) were tested. Gastrocnemius morphometric and muscle fiber cross-sectional area(CSA) were assessed by hematoxylin-eosin staining. Fibrosis-related mRNA expressions(i.e., Wnt 1, β-catenin, axin-2, α-smooth muscle actin, fibronectin, and types Ⅰ and Ⅲ collagen) were measured using real-time quantitative polymerase chain reactions. Wnt 1, β-catenin and fibronectin concentrations were measured by enzyme-linked immunosorbent assay. Types Ⅰ and Ⅲ collagen in the perimysium and endomysium were analyzed using immunofluorescence. Results: Compared with the control group, ROM, gait function, muscle weight, MWW/BW and CSA were significantly decreased in the immobilization group(all P <0.01), while protein levels of types Ⅰ and Ⅲ collagen, Wnt 1, β-catenin, fibronectin and mRNA levels of fibrosis-related genes were obviously increased(all P<0.01). Restenosis is a common complication following coronary angioplasty.The traditional use of seaweeds for health benefits has increasingly been explored,however few studies exist reporting its protective effects on the development of restenosis and gut dysbiosis.The aim of this study was to investigate the potential of seaweed extracts(SE) of Ascophyllum nodosum and Fucus vesiculosus in inhibiting intimal hyperplasia in a rat model of restenosis and its underlying mechanisms in macrophages and vascular smooth muscle cells(vSMCs).16S rRNA sequencing was done to investigate the regulatory effect of SE on the gut microbiome of injured rats.As indicated by the results,SE significantly inhibited the progression of intimal hyperplasia in vivo,attenuated inflammation in macrophages and could inhibit the proliferation,dedifferentiation and migration of vSMCs.It was observed through immunoblotting assays that treatment with SE significantly upregulated PTEN expression in macrophages and inhibited the upregulation of PI3K and AKT expression in vSMCs.Meanwhile,according to the 16S rRNA gene sequencing analysis,supplementation with SE modulated gut microbiota composition in injured rats.In conclusion,SE could ameliorate intimal hyperplasia by inhibiting inflammation and vSMCs proliferation through the regulation of the PTEN/PI3K/AKT pathway and modulating the gut microbiome.

Multi-scale data joint inversion of minerals and porosity in altered igneous reservoirs—A case study in the South China Sea

There are abundant igneous gas reservoirs in the South China Sea with significant value of research,and lithology classification,mineral analysis and porosity inversion are important links in reservoir evaluation.However,affected by the diverse lithology,complicated mineral and widespread alteration,conventional logging lithology classification and mineral inversion become considerably difficult.At the same time,owing to the limitation of the wireline log response equation,the quantity and accuracy of minerals can hardly meet the exploration requirements of igneous formations.To overcome those issues,this study takes the South China Sea as an example,and combines multi-scale data such as micro rock slices,petrophysical experiments,wireline log and element cutting log to establish a set of joint inversion methods for minerals and porosity of altered igneous rocks.Specifically,we define the lithology and mineral characteristics through core slices and mineral data,and establish an igneous multi-mineral volumetric model.Then we determine element cutting log correction method based on core element data,and combine wireline log and corrected element cutting log to perform the lithology classification and joint inversion of minerals and porosity.However,it is always difficult to determine the elemental eigenvalues of different minerals in inversion.This paper uses multiple linear regression methods to solve this problem.Finally,an integrated inversion technique for altered igneous formations was developed.The results show that the corrected element cutting log are in good agreement with the core element data,and the mineral and porosity results obtained from the joint inversion based on the wireline log and corrected element cutting log are also in good agreement with the core data from X-ray diffraction.The results demonstrate that the inversion technique is applicable and this study provides a new direction for the mineral inversion research of altered igneous formations.
