AIM: To describe the intensive care unit(ICU) outcomes of critically ill cancer patients with Acinetobacter baumannii(AB) ***: This was an observational study that included 23 consecutive cancer patients who acquired AB infections during their stay at ICU of the National Cancer Institute of Mexico(INCan), located in Mexico City. Data collection took place between January 2011, and december 2012. Patients who had AB infections before ICU admission, and infections that occurred during the first 2 d of ICU stay were excluded. Data were obtained by reviewing the electronic health record of each patient. This investigation was approved by the Scientific and Ethics Committees at INCan. Because of its observational nature, informed consent of the patients was not ***: Throughout the study period, a total of 494 critically ill patients with cancer were admitted to the ICU of the INCan, 23(4.6%) of whom developed AB infections. Sixteen(60.9%) of these patients had hematologic malignancies. Most frequent reasons for ICU admission were severe sepsis or septic shock(56.2%) and postoperative care(21.7%). The respiratory tract was the most frequent site of AB infection(91.3%). The most common organ dysfunction observed in our group of patients were the respiratory(100%), cardiovascular(100%), hepatic(73.9%) and renal dysfunction(65.2%). The ICU mortality of patients with 3 or less organ system dysfunctions was 11.7%(2/17) compared with 66.6%(4/6) for the group of patients with 4 or more organ system dysfunctions(P = 0.021). Multivariate analysis identified blood lactate levels(BLL) as the only variable independently associated with inICU death(OR = 2.59, 95%CI: 1.04-6.43, P = 0.040). ICU and hospital mortality rates were 26.1% and 43.5%, ***: The mortality rate in critically ill patients with both HM, and AB infections who are admitted to the ICU is high. Aim: To investigate the role of IL- 7 in HIV- infected children on highly a ctive antiretroviral therapy (HAART) and its association with laboratory paramet ers related to disease progression. Patients and methods: A cross- sectional study in 31 vertically HIV- infected children (median age 8.4 y) t reated with HAART, and a longitudinal study in four of those same children was c arried out. In both studies, viral load, CD4+ T- cell counts, thymic producti on of T cells by TCR rearrangement excision circles (TRECs), IL- 7 plasma level s and viral phenotype were determined. Results: IL- 7 levels were higher in HIV - infected children than in age- matched, uninfected controls. In addition, HI V children with CD4+ T cells between 200 and 500 T cells/mm3 had higher IL- 7 levels and lower TREC values than HIV- infected children with CD4+ T cells > 500 T cells/mm3. IL- 7 levels were higher in children with syncytium- inducin g (SI) phenotype than in those with nonsyncytium- inducing (NSI) variants. Duri ng the follow- up of four HIV children, the decrease in viral load after HAART was always associated with a recovery of CD4+ T cells and TRECs, which was fol lowed by a decrease in IL- 7 returning to the levels present prior to the drop in CD4+ Tcells. The four HIV- infected children had SI/X4 isolates in PBMC be fore HAART, and the viral phenotype switched to NSI/R5 after HAART. Conclusion: Our data suggest that IL- 7 plays a key role in the maintenance of T- cell hom eostasis in HIV- infected children on HAART, both through peripheral expansion and through a thymus- dependent mechanism.

We reported here the first known case of natural infection of a lion(Panthera leo-Linnaeus,1738) with Leishmania(Leithmania) chagasi(***) in *** specimen was created by a circus handler in the state of Mato Grosso and was donated to the zoological park of the Federal University of Mato *** by *** was detected using a PCR-RFLP *** was known that the domestic felids can act as reservoir of infection of *** in endemic areas, making it important that studies demonstrate their participation in the epidemiological chain. We demonstrate in this work that wild animals can have an important role in the epidemiological chain and must be considered in order to plan methods of control of this zoonosis.

[背景]流行病学文献表明,空气污染物暴露与胎儿的发育相关联。[目的]探讨产妇在妊娠2-8周期间的空气污染物暴露及其与先天性心脏缺陷的关联。[方法]此项出生缺陷预防研究是一项由9个州参加的病例对照研究。给参加这项研究的妊娠妇女分配1周和7周里一氧化碳、二氧化氮、臭氧、二氧化硫的每日最高浓度平均值,以及细颗粒物和粗颗粒物的24 h测量值,采用的方法是在妊娠早期收集这些妇女住所50 km内最接近的空气监测点的测量值。根据不同的污染物,最多4 632例活产对照和3 328例活产、死胎或流产病例有空气污染暴露的数据。应用分层回归模型,并以产妇的人口统计学、烟草和酒精使用情况进行调整。对于多污染物共同暴露的情况,则采用主成分分析来评估这些关联。[结果]二氧化氮暴露与主动脉缩窄及肺动脉瓣狭窄之间存在正性关联。细颗粒物暴露与左室发育不良心脏综合征呈正性相关,但与房间隔缺损呈负性相关。检测个体的暴露周数表明,使用7周的平均水平未观察到污染物与缺陷之间的关联。多污染物分析的结果支持左室流出道阻塞与二氧化氮之间、左室发育不良心脏综合征与颗粒物之间存在关联,尽管在最高的暴露水平时估计值是下降的。[结论]采用每日最高污染物水平探讨个体暴露周数揭示了某些污染物与心脏缺陷之间存在一些正向关联,并且提示在妊娠期间存在潜在的敏感性窗口期。

随机对照试验结果表明,抑制胆固醇酯转运蛋白（cholesteryl ester transfer protein,CETP）的疗法并不能降低冠状动脉性心脏病（coronary heart disease,CHD）的发生风险。研究失败的可能原因包括靶目标无效、靶目标外小分子的不良反应和随机对照设计因素影响等。在编码药物靶点的基因中具有天然存在的遗传变异,以此为基础,人类研究可以深入了解针对基因产物的治疗的潜在功效和安全性。
