A 37-year old male presented with an acute abdomen suggestive of an appendiceal *** laparotomy showed a duplicated appendix with one of the lumens involved with appendicitis and a focal periappendicular abscess while the other lumen had a localized appendiceal *** of congenital intestinal duplications in adults is important to avoid serious clinical consequences.

Celiac disease is an immune-mediated disorder that causes severe architectural disturbance in the small intestinal mucosa of genetically-predisposed *** absorption of multiple nutrients results and diarrhea and weight loss *** has accumulated that a strict gluten-free diet can result in resolution of diarrhea,weight gain and normalization of nutrient *** addition,histopathological changes also normalize,but this histopathological res-ponse appears to be time-dependent,sex-dependent and *** to a gluten-free diet is difficult and costly resulting in poor compliance and only a limited clinical *** poses a risk for later long-term complications,including malignancy.A major practical clinical problem is the assessment of compliance to the gluten-free *** symptoms may resolve and serological antibody markers may improve,multiple studies have documented ongoing architectural disturbance and inflammatory change,and with these continued inflammatory changes,a persistent risk for long-term *** immunological studies have suggested that peptides can be detected in both urine and fecal specimens that may be indicative of limited *** the same time,multiple biopsy studies have demonstrated that complete normalization of the mucosa may occur in some patients within 6 mo of initiation of a gluten-free diet,but more often,up to 2 years or more may be required before repeated biopsies eventually show mucosal recovery and mucosal healing.

Collagenous sprue has traditionally been defined as a small intestinal mucosal disorder characterized by persistent diarrhea, severe malabsorption with multiple nutrient def iciencies and progressive weight loss. Pathologically, a severe to variably severe "flattened" mucosal biopsy lesion with distinctive sub-epithelial deposits in the lamina propria region is detected. Histochemical stains and ultrastructural studies have conf irmed that these deposits contain collagens. Often, an initial diagnosis of celiac disease is considered but no continued response to treatment with a gluten-free diet occurs. Recent reports indicate an intimate relationship between collagenous sprue and celiac disease, sometimes with concomitant T-cell enteropathy. In addition, permanent disappearance of these deposits after resection of a localized colon cancer suggested that this disorder could actually represent a paraneoplastic morphologic marker of an occult malignancy. Studies showing either gastric or colonic involvement (or both) with this unusual collagenous inflammatory mucosal process may also reflect a far more extensive and heterogeneous process than previously appreciated.

Testosterone deficiency is common in men with cardiovascular disease （CVD）, and randomized placebo-controlled trials （RCTs） have reported beneficial effects of testosterone therapy on exercise-induced cardiac ischemia in chronic stable angina, functional exercise capacity, maximum oxygen consumption during exercise （VO2m,x） and muscle strength in chronic heart failure （CHF）, shortening of the Q-T interval, and improvement of some cardiovascular risk factors. Testosterone deficiency is associated with an adverse CV risk profile and mortality. Clinical and scientific studies have provided mechanistic evidence to support and explain the findings of the RCTs. Testosterone is a rapid-onset arterial vasodilator within the coronary circulation and other vascular beds including the pulmonary vasculature and can reduce the overall peripheral systemic vascular resistance. Evidence has demonstrated that testosterone mediates this effect on vascular reactivity through calcium channel blockade （L-calcium channel） and stimulates potassium channel opening by direct nongenomic mechanisms. Testosterone also stimulates repolarization of cardiac myocytes by stimulating the ultra-rapid potassium channel-operated current. Testosterone improves cardiac output, functional exercise capacity, VO2m=x and vagally mediated arterial baroreceptor cardiac reflex sensitivity in CHF, and other mechanisms. Independent of the benefit of testosterone on cardiac function, testosterone substitution may also increase skeletal muscle glucose metabolism and enhance muscular strength, both factors that could contribute to the improvement in functional exercise capacity may include improved glucose metabolism and muscle strength. Testosterone improves metabolic CV risk factors including body composition, insulin resistance, and hypercholesterolemia by improving both glucose utilization and lipid metabolism by a combination of genomic and nongenomic actions of glucose uptake and utilization expression of the

Wave height measurement in the Taiwan Strait with a portable high frequency surface wave radar

As an important equipment for sea state remote sensing, high frequency surface wave radar （HFSWR） has received more and more attention. The conventional method for wave height inversion is based on the ratio of the integration of the second-order spectral continuum to that of the first-order region, where the strong external noise and the incorrect delineation of the first- and second-order Doppler spectral regions due to spectral aliasing are two major sources of errors in the wave height. To account for these factors, two more indices are introduced to the wave height estimation, i.e., the ratio of the maximum power of the second-or- der continuum to that of the Bragg spectral region （RSCB） and the ratio of the power of the second harmonic peak to that of the Bragg peak （RSHB）. Both indices also have a strong correlation with the underlying wave height. On the basis of all these indices an empirical model is proposed to estimate the wave height. This method has been used in a three-months long experiment of the ocean state measuring and analyzing ra- dar, type S （OSMAR-S）, which is a portable HFSWR with compact cross-loop/monopole receive antennas developed by Wuhan University since 2006. During the experiment in the Taiwan Strait, the significant wave height varied from 0 to 5 m. The significant wave heights estimated by the OSMAR-S correlate well with the data provided by the Oceanweather Inc. for comparison, with a correlation coefficient of 0.74 and a root mean square error （RMSE） of 0.77 m. The proposed method has made an effective improvement to the wave height estimation and thus a further step toward operational use of the OSMAR-S in the wave height extraction.

The paper by Lu et al. [1] ＂A survey on the status of semen analysis in 118 laboratories in China＂ provides a major review of the current methods of semen analysis in China's Mainland. It involved a 36-item questionnaire completed by technicians in 118 of a possible 145 potential semen laboratories. Recruitment for the study was via contacts developed at Chinese Andrology meetings and training sessions between 2005 and 2007. It is not stated how comprehensive this strategy would be for accessing all semen laboratories in China. About half the participating laboratories were in general hospitals and the others in hospitals with academic affiliations or family planning institutes.

In this article,I present an interpretation in religious terms of what Athenians were doing when they went to Delphi as representatives of their city.I begin by briefly outlining the key moments of Athenian involvement with Delphi in the classical period,which is roughly from 479 to 338 BCE,and the general relationship between city and sanctuary.I then look at the activities of the Athenian delegates to the Delphic Amphiktyony,whose responsibilities included representing the city of Athens at the festival of the Pylaia at Anthela and offering sacrifice at Delphi.I then turn to sacred ambassadors coming to consult the oracle,and show that occasions of consultation were festival occasions and that the experience of the thedroi was profoundly religious.I demonstrate that more than any immediate political concerns,maintaining a good relationship with Apollo was central to these *** will be drawn largely from inscriptions from Delphi and literary sources from Athens,dating to the period under investigation.

A 67-year-old man with celiac disease developed recurrent diarrhea,profound weakness and weight loss, with evidence of marked protein *** clinical course was refractory to a strict gluten-free diet and steroid *** studies led to definition of unrecognized collagenous sprue that caused ulceration and small intestinal *** PCR showed identical monoclonal T-cell populations in antemortem duodenal biopsies and postmortem jejunum,careful pathological evaluation demonstrated no frank ***,overt or even cryptic T-cell lymphoma may complicate collagenous sprue,however, small intestinal ulcers and perforation may also develop *** dramatic findings here may reflect an underlying or early molecular event in the eventual clinical appearance of overt T-cell lymphoma.
