To the Editor:Chloroma is a rare solid tumor that was formed sometimes from malignant myeloid blast cells outside the hematopoietic system;these are known as chloroma or granulocytic sarcoma[1]Chloroma can be diagnosed anywhere outside of the hematopoietic system,such as the head and neck,skeletal system,breast,uterus,and central nervous system.[2]This tumor is common in leukemic patients.[3]Chloroma presents as an epidural mass resulting in spinal cord compression with neurologic symptoms.[4]Spinal lesions occur most frequently at the thoracic level;however,the most common primary symptoms are low back pain and radiating pain in the legs[5].

The ganglion impar block was first introduced in 1990 by Plancarte et al.[1]Since then,several modified versions have been reported by Wemm and Saberski,[2]Nebab and Florence,[3]and Foye.[4]They tried to reduce pain effectively by considering the anatomy of the ganglion impar and coccyx.There were differences in the strengths of each individual’s technique.However,owing to the anatomical diversity of the structure itself,[5]it was difficult to determine which technique would be more effective.Radiofrequency ablation(RFA)developed in the ganglion impar block also faced this controversy.In this clinical background,we present a new technique to implement ganglion impar RFA.Written informed consent was obtained from the patient before the initiation of the study.

Nearly 21% of pediatric surgical operations are accompanied by adverse events,12.8% of which are respiratory complications including bronchospasm （1.2%） and pulmonary edema （11%）. Among them,aspiration pneumonia and negative-pressure pulmonary edema are rare pulmonary complications in pediatrics.However,they are difficult to predict and can be fatal if not appropriately treated;careful monitoring is required.We present a case of pulmonary complication secondary to aggravation of bronchial spasm during intubation of a pediatric patient.

To the Editor:A first infection of varicella-zoster virus(VZV)during pregnancy can cause serious systemic disease to both mother and the fetus.Despite this maternal herpes zoster(HZ)does not increase fetal mortality,and the transmission of varicella-zoster virus to the fetus rarely occurs.Although HZ infection has a minimal effect on the fetus,maternal HZ can cause significant complications for the mother.

To the Editor： A lumbar epidural block is used to treat patients with a herniated intervertebral disc, spinal stenosis, or lumbosacral radiculopathy. This approach reduces radicular pain by decreasing inflammation of nerve roots through epidural corticosteroid injection. Patients with moderate-to-severe spinal stenosis who taking various anticoagulant agents have more risk for epidural hematoma.

All T cells require cytokine signaling for their development and maintenance, and CD8αα^(+) IEL T cells are noted for their requirement for IL-15 [8]. This IL-15 dependency was demonstrated by the paucity of CD8αα^(+) T cells among SI IELs in Il15–/– mice (Fig. 1d and Fig. S1a). However, the cytokine requirements of CD8αα^(+)CD4^(+) T cells are currently unclear. Because of the shared phenotype of CD8αα expression with CD8αα^(+) IELs, we postulated that CD4^(+)CD8αα^(+) IELs would also depend on IL-15.

Background:Pancreatic cysts are common.However,most studies are based on data collected from individual centers.The present study aimed to evaluate the changes of management patterns for pancreatic cystic lesions(PCLs)by analyzing large epidemiologic data.Methods:Between January 2007 and December 2018,information regarding pancreatic cystic lesions was acquired from the nationwide Health Insurance Review and Assessment Service database in Korea.Results:The final number of patients with pancreatic cysts was 165277 among the total claims for reimbursement of 855983 associated with PCLs over 12 years.The total number of claims were increased from 19453 in 2007 to 155842 in 2018 and the prevalence increased from 0.04%to 0.23%.For 12 years,2874(1.7%)had pancreatic cancer and 8212(5.0%)underwent surgery,and 36 had surgery for twice(total 8248 pancreatectomy).After ruling out claims from the first 3 years of washout period,the incidence increased from 9891 to 24651 and the crude incidence rate of PCLs expanded from 19.96 per 100000 to 47.77 per 100000.Compared to specific neoplasm codes(D136 or D377),the use of pancreatic cyst code(K862)has been remarkably increased and the most common since 2010.The annual number of pancreatectomies increased from 518 to 861 between 2007 and 2012,and decreased to 596 until 2018.The percentage of pancreatic cancer in patients who received pancreatectomy increased from 5.6%in 2007 to 11.7%in 2018.Conclusions:The incidence of PCLs is rapidly increasing.Among PCLs,indeterminate cyst is increasing outstandingly.A trend of decreasing in the number of resections and increasing cancer rates among resected cysts may be attributed to the updated international guidelines.

International guidelines for post-cardiac arrest care recommend using multi-modal strategies to avoid the withdrawal of life-sustaining therapy(WLST)in patients with the potential for neurological recovery.[1]However,a clear methodology for multi-modal approaches has yet to be developed.Neuron-specific enolase(NSE)is currently the only recommended biomarker,and the European Resuscitation Council(ERC)and the European SocietyofIntensiveCareMedicine(ESICM)have proposed a cutoff value of 60μg/L at 48 and/or 72 h after the return of spontaneous circulation(ROSC)as a multimodal prognostic tool for predicting poor neurological outcomes.

Ca_(3-x)(PO_(4))_(2):xTb^(3+)(0.2≤x≤0.4),Ca_(2.3)(PO_(4))_(2):0.35 Tb^(3+),0.35 A^(+)(A=Li,Na,K),and Ca_(2.3)(PO_(4))_(2):0.35 Tb^(3+),yLi^(+)(0.35≤y≤0.455)phosphors were prepared by solid-state reaction.All the prepared phosphors formed a rhombohedral unit cell with the R3c space group.To improve the photoluminescence(PL)properties of the Ca_(2.65)(PO_(4))_(2):0.35 Tb^(3+)phosphor,monovalent charge compensators such as Li^(+),NA^(+),and K^(+)were added to the Ca_(2.65)(PO_(4))_(2):0.35 Tb^(3+)phosphor.The charge compensators acted as fluxes,so they improved the crystallinity.The excitation and emission properties were significantly improved through the incorporation of charge compensators.In particular,among the charge compensators,Li^(+)ion substantially enhanced the emission intensity and color purity.Furthermore,considering the evaporation of Li_(2)CO_(3)during the annealing process,we optimized the concentration of Li^(+)charge compensator to enhance its PL performance.Impressively,the green emission intensity of the Ca_(2.3)(PO_(4))_(2):0.35 Tb^(3+),0.385 Li^(+)phosphor was 260%higher than that of the Ca_(2.65)(PO_(4))_(2):0.35 Tb^(3+)phosphor.We believe that the effect of charge compensators on the PL properties and the optimum concentration of Li^(+)cha rge compensator are useful for the design of phosphors in light-emitting diodes.
