Background： No data on the incidence ofpleural effusion （PE） in Chinese patients with pulmonary embolism are available to date. The aim of the current study was to investigate the frequency of PE in a Chinese population of patients with pulmonary embolism. Methods： This was a retrospective observational single-center study. All data of computed tomography pulmonary angiography （CTPA） performed over 6-year period on adult patients with clinically suspected pulmonary embolism were analyzed. Results： From January 2008 until December 2013, PE was identified in 423 of 3141 patients （13.5%） with clinically suspected pulmonary embolism who underwent CTPA, The incidence of PE in patients with pulmonary embolism （19.9%） was significantly higher than in those without embolism （9.4%）（P 〈 0.001 ）. Majority of PEs in pulmonary embolism patients were small to moderate and were unilateral. The locations of emboli and the numbers of arteries involved, CT pulmonary obstruction index, and parenchymal abnormalities at CT were not associated with the development of PE, Conclusions： PEs are present in about one fifth of a Chinese population of patients with pulmonary embolism, which are usually small, unilateral, and unsuitable for diagnostic thoracentesis.

Emergency exit is the most common form of engineering structure in railway tunnel disaster prevention and rescue *** order to clarify the safety guarantee rate of personnel evacuation through emergency exit after fire in railway tunnel,with the help of fire dynamics simulation（FDS） and building-EXODUS,this paper has established the fire combustion model and personnel evacuation model of the 25-type passenger trains in the 3 D ***,combined with the personnel safety evacuation criteria,the safety probability of evacuation of personnel in different scenarios at the emergency exit of the railway tunnel fire mode is *** conclusion shows that the larger the width of the running surface,the smaller the total number of evacuation,the higher the evacuation safety *** evacuation person number is 2145（the hard seat is overloaded 60%）,and the width of the running surface is increased from 1.5 m（standardized value） to 3.5 m,the evacuation safety rate increased from 86.4% to 87.7%.The width of the running surface was 1.5 m,and the number of evacuation person was 1531（not overloaded）,the evacuation safety rate reached 100%.Therefore,for the 25-type passenger train,if the personnel are not overloaded,they can choose to evacuate the fire at the emergency exit.

随着中国科技的不断发展进步,工业体系已经日趋完善,石化领域更是首当其冲,对国民经济和社会发展起着不可或缺的重要作用。近年来,石油化工储罐火灾、爆炸等安全事故频繁发生,造成巨大损失和人员伤亡,国家、企业越发关注石化储罐的消防安全。当下,国内储罐的消防灭火方式多采用传统的吸入式泡沫灭火系统,这类系统国内使用技术较成熟,但从长期的工程实践来看,系统存在一定的安全隐患。随着消防技术的发展和实际生产的需求,一些新型技术也开始被广泛应用,文章主要就压缩空气泡沫灭火系统在石油化工储罐应用问题进行分析和探讨。

Background Active suppression by CD4^＋CD25^＋ regulatory T lymphocytes plays an important role in the down-regulation of T cell responses to foreign and self-antigens. This study was conducted to analyze whether the CD4^＋CD25^＋ regulatory T cells exist and function normally in tuberculous pleural effusion. Methods The percentages of CD4^＋CD25^＋ T cells in pleural effusion and peripheral blood from patients with tuberculous pleurisy and peripheral blood from healthy control subjects were determined by flow cytometry. The expression of forkhead transcription factor Foxp3 was also examined. CD4^＋CD25^＋ and CD4^＋CD25^-T cells from pleural effusion and blood were isolated, and were cultured to observe the effects of CD4^＋CD25^＋ T cells on proliferation response of CD4^＋CD25^- T cells in vitro. Results There were increased numbers of CD4^＋CD25^＋ T cells in tuberculous pleural effusion compared with peripheral blood from both patients with tuberculous pleurisy and normal subjects, and these cells demonstrated a constitutive high-level expression of Foxp3. Moreover, CD4^＋CD25^＋ T cells mediated potent inhibition of proliferation response of CD4^＋CD25^- T cells. Conclusion The increased CD4^＋CD25^＋ T cells in tuberculous pleural effusion express a high level of Foxp3 transcription factor, while potently suppressing the proliferation of CD4^＋CD25^- T cells.

基于数据挖掘分析国医大师治疗水肿验案的组方特点及核心方,并利用网络药理学阐释核心方治疗水肿的作用机制。通过中国知网(CNKI)收集并整理3届国医大师治疗水肿有效病案的期刊报道,利用中医传承计算平台3.0构建数据库,并通过关联规则和k均值聚类等方法分析病案处方,利用TCMSP和TCMID数据库检索成分并筛选靶点,在DrugBank和GeneCards数据库查找水肿(edema)相关的疾病靶点,通过STRING 11.0构建蛋白质-蛋白质相互作用(PPI)网络筛选出核心靶点,利用FunRich 3.1.3软件富集核心方的表达部位,利用Metascape数据库对交集靶点进行京都基因与基因组百科全书(KEGG)富集分析,运用Cytoscape 3.6.0软件进行"中药-活性成分-核心靶点-通路"网络可视化展示。结果发现治疗水肿的国医大师验案315则,关联规则和聚类分析获得5个核心方。核心方1:茯苓、白术、黄芪、泽泻、甘草、党参,包含166种化学成分,涉及1 125个靶点。核心方2:黄芪、丹参、茯苓、川芎、赤芍、当归,包括化学成分138种,涉及1 112个靶点。核心方3:茯苓、丹参、黄芪、白术、泽泻、薏苡仁,涉及化学成分126种,1 121个靶点。核心方4:茯苓、连翘、白术、白茅根、蝉蜕、薏苡仁,涉及化学成分58种,820个靶点。核心方5:茯苓、白术、黄芪、泽泻、鳖甲、山药,涉及化学成分68种,919个靶点。5个核心方的核心靶点均涉及AKT1、ALB、CASP3、MAPK3、EGFR、SRC、MAPK1、TNF,且5个核心方治疗水肿潜在靶点大多在胃、膀胱、肺、肾中较高表达。KEGG通路富集于炎症和细胞周期等相关通路,其中炎症相关通路涉及最多,推测其可能通过健脾、利水、活血和益气等作用发挥消炎、调节免疫系统等治疗水肿,该研究为国医大师经验总结和新药研发提供参考。

2,5-呋喃二甲酸(2,5-FDCA)是一种重要的生物质基聚酯单体。针对己糖二酸脱水环合制备2,5-FDCA中存在反应时间长的问题,在传统加热与微波加热的对比实验基础上,开展了HBr-MgBr_(2)催化下微波强化己糖二酸脱水环合制备2,5-FDCA的工艺研究。实验结果证实了微波的引入可在保证2,5-FDCA高收率的前提下显著提高己糖二酸脱水环合制备2,5-FDCA的反应速率,大大缩短反应时间。通过工艺优化,得到较佳的工艺条件:温度为110℃、催化剂质量分数w(HBr)=3%、w(MgBr_(2))=3%、微波强化反应15 min,2,5-FDCA收率可达75.0%。进一步考察了不同己糖二酸原料对微波强化脱水环合制备2,5-FDCA的影响,2,5-FDCA收率从高到低的次序为:葡萄糖二酸单钾盐>葡萄糖二酸钙>半乳糖二酸。研究工作将为己糖二酸路线制备2,5-FDCA的工业化进程起到一定的推动作用。
