Speckle-tracking echocardiography for detecting myocardial dysfunction in sepsis and septic shock patients:A single emergency department study

BACKGROUND:Septic cardiomyopathy(SCM)occurs in the early stage of sepsis and septic shock,which has implications for treatment strategies and prognosis.Additionally,myocardial involvement in the early stages of sepsis is difficult to identify.Here,we assess subclinical myocardial function using laboratory tests and speckle-tracking echocardiography(STE).METHODS:Emergency department patients diagnosed with sepsis or septic shock were included for analysis.Those with other causes of acute or pre-existing cardiac dysfunction were excluded.Transthoracic echocardiography(TTE),including conventional echocardiography and STE,were performed for all patients three hours after initial resuscitation.Samples for laboratory tests were taken around the time of TTE.RESULTS:Left ventricular functions of 60 patients were analyzed,including 21 septic shock patients and 39 sepsis patients.There was no significant difference in global longitudinal strain(GLS),global circumferential strain(GCS),or global radical strain(GRS)between patients with sepsis and septic shock(all with P>0.05).However,GLS and GCS were significantly less negative in patients with abnormal troponin levels or in patients with abnormal left ventricular ejection fraction(LVEF)values(all with P<0.05).There were also moderate correlations between GLS and levels of cTnI(r=0.40,P=0.002)or N-terminal pro-B-type natriuretic peptide(NT-proBNP)(r=0.44,P=0.001)in sepsis and septic shock patients.CONCLUSION:Myocardial dysfunction,e.g.,lower LVEF or less negative GLS in patients with sepsis or septic shock,is more affected by myocardial injury.GLS could be incorporated into mainstream clinical practice as a supplementary LVEF parameter,especially for those with elevated troponin levels.

Seasonal infl uenza,which is transmitted by droplets and direct contact,is a global public health issue that causes an average of 2.5 excess infl uenza-like illness(ILI)consultations per 1,000 person-years in China.[1]Nonpharmaceutical interventions(NPIs)against droplet and direct contact transmission,including social distancing measures and personal protective measures,are recommended to reduce the spread of disease.Social distancing measures comprised isolating ill persons,quarantining exposed persons,school and workplace closures,and avoiding crowds.Personal protective measures included hygiene,respiratory etiquette,and face masks.[2]In December 2019,coronavirus disease 2019(COVID-19)started to spread,and China introduced NPIs to address the pandemic in January 2020.These NPIs could also help reduce the spread of other respiratory diseases,such as seasonal influenza.[3]In the second half of 2020,the COVID-19 pandemic was gradually brought under control.Production,transport,and schools were returning to normal,while citizens were still required to wear masks in public.Cases of influenza also reappeared.This study compared the ILI percentage(ILI%),laboratory(LAB)-positive rate,incidence rate,and effective reproductive number(Rt)of infl uenzas from 2019 to 2022 to clarify the change in seasonal infl uenza spread after the COVID-19 outbreak.

Photonuclear reactions using a laser Compton scattering(LCS)gamma source provide a new method for producing radioisotopes for medical applications.Compared with the conventional method,this method has the advantages of a high specific activity and less heat.Initiated by the Shanghai Laser Electron Gamma Source(SLEGS),we conducted a survey of potential photonuclear reactions,(γ,n),(γ,p),and(γ,γ')whose cross sections can be measured at SLEGS by summarising the experimental progress.In general,the data are rare and occasionally inconsistent.Therefore,theoretical calculations are often used to evaluate the production of medical radioisotopes.Subsequently,we verified the model uncertainties of the widely used reaction code TALYS-1.96,using the experimental data of the^(100)Mo(γ,n)^(99)Mo,^(65)Cu(γ,n)^(64)Cu,and^(68)Zn(γ,p)^(67)Cu reactions.

BACKGROUND:Many controversies still exist regarding ventilator parameters during cardiopulmonary resuscitation(CPR).This study aimed to investigate the CPR ventilation strategies currently being used among physicians in Chinese tertiary hospitals.METHODS:A survey was conducted among the cardiac arrest team physicians of 500 tertiary hospitals in China in August,2018.Surveyed data included physician and hospital information,and preferred ventilation strategy during CPR.RESULTS:A total of 438(88%)hospitals completed the survey,including hospitals from all 31 Chinese mainland provinces.About 41.1%of respondents chose delayed or no ventilation during CPR,with delayed ventilations all starting within 12 minutes.Of all the respondents who provided ventilation,83.0%chose to strictly follow the 30:2 strategy,while 17.0%chose ventilations concurrently with uninterrupted compressions.Only 38.3%respondents chose to intubate after initiating CPR,while 61.7%chose to intubate immediately when resuscitation began.During bagvalve-mask ventilation,only 51.4%of respondents delivered a frequency of 10 breaths per minute.In terms of ventilator settings,the majority of respondents chose volume control(VC)mode(75.2%),tidal volume of 6–7 mL/kg(72.1%),PEEP of 0–5 cmH2O(69.9%),and an FiO2 of 100%(66.9%).However,62.0%of respondents had mistriggers after setting the ventilator,and 51.8%had high pressure alarms.CONCLUSION:There is a great amount of variability in CPR ventilation strategies among cardiac arrest team physicians in Chinese tertiary hospitals.Guidelines are needed with specific recommendations on ventilation during CPR.

Intrasexual selection occurs in male-male competition over access to females and usually results in the larger male winning.While much research has documented that size matters,little is known about how the larger male wins.Dendroctonus vale ns is an aggregating monogamous bark beetle in which males have large variation in body size and display intense competition over females.Behavioral observation showed two males fight each other within the gallery by pushing/shoving and stridulated more when two males encountered each other.Experiments using two different-sized males synchronously competing showed that larger males won 95%of contests.Reciprocal displacement experiments using muted and intact males of different or equal size were used to simulate male-male competition.Larger males displaced the smaller resident male in 90%of contests,while smaller males prevailed over larger residents in 6.7%of contests.With both males silenced,larger males displaced smaller males in 80%of con tests,while smaller males prevailed in 8%of con tests.Further experiments using equal-sized males showed aggressive soundemitting males displaced muted males in 67%of contests,yet intact males displaced other intact males in only 37.5%of con tests.Sound analysis showed sound pressure level is an honest signal of body size and males chose soft sounds over loud aggressive sounds in assays.Therefore,D.valens males have evolved dual behaviors,fighting and aggressive sounds associated with body size,to assess rivals to compete for a partner,gaining insights in male-male competition for this species and for other animals.

BACKGROUND Splenic rupture associated with Behçet’s syndrome(BS)is extremely rare,and there is no consensus on its management.In this case report,a patient with BSassociated splenic rupture was successfully treated with splenic artery embolization(SAE)and had a good prognosis after the intervention.CASE SUMMARY The patient was admitted for pain in the left upper abdominal quadrant.He was diagnosed with splenic rupture.Multiple oral and genital aphthous ulcers were observed,and acne scars were found on his back.He had a 2-year history of BS diagnosis,with symptoms of oral and genital ulcers.At that time,he was treated with oral corticosteroids for 1 month,but the symptoms did not alleviate.He underwent SAE to treat the rupture.On the first day after SAE,the patient reported a complete resolution of abdominal pain and was discharged 5 d later.Three months after the intervention,a computed tomography examination showed that the splenic hematoma had formed a stable cystic effusion,suggesting a good prognosis.CONCLUSION SAE might be a good choice for BS-associated splenic rupture based on good surgical practice and material selection.

Effect of processing parameters on formability, microstructure, and micro-hardness of a novel laser additive manufactured Ti-6.38Al-3.87V-2.43Mo alloy

A novel Ti-6.38Al-3.87V-2.43Mo alloy was designed with a cluster formula of 12[Al-Ti12](V0.75Mo0.25Ti2)+4[Al-Ti12](Al3)by replacing Ti with Mo/V on the basis of the Ti-Al congruent alloy.The effects of laser power and scanning speed on the molten pool size,surface roughness,relative density,microstructure,and micro-hardness of single-track and bulk Ti-6.38Al-3.87V-2.43Mo samples prepared via laser additive manufacturing(LAM)were investigated.The results show that processing parameters significantly affect the formability,microstructure,and micro-hardness of the alloy.With decreasing laser power from 1,900 W to 1,000 W,the relative density is decreased from 99.86%to 90.91%due to the increase of lack-of-fusion;however,with increasing scanning speed,the relative density does not change significantly,but exceeds 99%.In particular,Ti-6.38Al-3.87V-2.43Mo samples of single-track and bulk exhibit a good formability under an input laser power of 1,900 W and a scanning speed of 8 mm·s_(-1),and display the lowest surface roughness(Ra=13.33μm)and the highest relative density(99.86%).Besides,the microstructure of LAM Ti-6.38Al-3.87V-2.43Mo alloy coarsens with increasing laser power or decreasing scanning speed due to the greater input energy reducing the cooling rate.The coarsening of the microstructure decreases the microhardness of the alloy.

Dear editor,Boerhaave syndrome,firstly described by Hermann Boerhaave in 1724,is a rare and life-threatening condition characterized by spontaneous transmural tear of the oesophagus.[1]The early diagnosis of Boerhaave syndrome is extremely important since a delay in the diagnosis can increase the mortality rate.[2]Here,we reported a Boerhaave syndrome patient combined with tension hydropneumothorax.
