Prophylaxis of hepatitis B recurrence in post-liver transplantation patients with lamivudine-resistant YMDD mutant

Background The most frequently used therapy for post-transplantation recurrence of hepatitis B virus （HBV） infection is lamivudine, but this drug is associated with a high resistance rate due to YMDD mutant. In preliminary reports, adefovir dipivoxil （ADV） has been shown to have activity against lamivudine-resistant strains of HBV. However, clinical experience in treatment of HBV infection after liver transplantation （LT） is still not entirely clear. This study was aimed to evaluate the prophylactic efficacy of ADV plus hepatitis B immunoglobulin （HBIG） in patients with YMDD mutant before LT. Methods From March 2004 to March 2006, 16 patients with chronic hepatitis B had lamivudine-resistant YMDD mutants detected prior to liver transplantation and received treatment with ADV plus additional intramuscular HBIG after LT as prophylaxis against graft reinfection. Tests for liver function, serum HBsAg, anti-HBs （HBIG）, HBeAg, anti-HBc, anti-HBe, HBV-DNA, and creatinine were assessed pre- or post-liver transplantation. Results The median follow-up of these patients post-liver transplantation was 19.4 months. Fifteen patients survived and one patient died of recurrence of hepatocellular carcinoma （HCC）. There was significant difference （10.98% vs. 2.26%, P〈0.05） in YMDD mutant rate between the patients with HBV-DNA over 106 copies/ml and those with HBV-DNA less than 106 copies/mi. Fifteen patients （93.8%） had undetectable HBV-DNA at 4 weeks and 1 （6.3%） at 6 months after LT. No hepatitis B recurrence was detected by persistent testing of HBsAg, HBeAg, and HBV-DNA and no increase of serum creatinine level associated with ADV was observed in any of the patients. Conclusion ADV combined with intramuscular HBIG can effectively prevent patients with pre-transplantation YMDD mutant from HBV recurrence after LT.

Application of a medical image processing system in liver transplantation

BACKGROUND: At present, imaging is used not only to show the form of images, but also to make three-dimensional (3D) reconstructions and visual simulations based on original data to guide clinical surgery. This study aimed to assess the use of a medical image-processing system in liver transplantation surgery. METHODS: The data of abdominal 64-slice spiral CT scan were collected from 200 healthy volunteers and 37 liver cancer patients in terms of hepatic arterial phase, portal phase, and hepatic venous phase. A 3D model of abdominal blood vessels including the abdominal aorta system, portal vein system, and inferior vena cava system was reconstructed by an abdominal image processing system to identify vascular variations. Then, a 3D model of the liver was reconstructed in terms of hepatic segmentation and liver volume was calculated. The Free Form modeling system with a PHANTOM force feedback device was used to simulate the real liver transplantation environment, in which the total process of liver transplantation was completed. RESULTS: The reconstructed model of the abdominal blood vessels and the liver was clearly demonstrated to be three-dimensionally consistent with the anatomy of the liver, in which the variations of abdominal blood vessels were identified and liver segmentation was performed digitally. In the model, liver transplantation was simulated subsequently, and different modus operandi were selected successfully. CONCLUSION: The digitized medical image processing system may be valuable for liver transplantation.

Epidemiology of hepatitis B virus infection among young adults in Taiwan,china after public vaccination program

Background The public vaccination program of hepatitis B virus （HBV） was launched during 1984 in Taiwan, china. However, the long-lasting protective efficacy of HBV vaccination among adolescents older than 15 years of age was seldom recorded. Methods A seroepidemiological survey was conducted among 4575 first-year university students in Taiwan, china during 2000 to 2003, including the serological data of HBV by testing HBV surface antigen （HBsAg）, surface antibody （anti-HBs）, HBV core antibody （anti-HBc） and demographic information. Results HBsAg carrier rate among male university students born before the initiation of the HBV vaccination program decreased from 12.8% to 4.8% among those born after the vaccination program （P〈0.001, Х^2 test for linear trend）. Similarly, HBsAg carrier rate among female university students born before the initiation of the HBV vaccination program decreased from 8.1% to 2.7% among those born after the vaccination program （P〈0.001, Х^2 test for linear trend）. Both male and female students in eastern Taiwan had the highest HBsAg carrier rate compared with the other places. Using multiple logistic regression analysis, compared with students born after July 1984, the adjusted OR of HBsAg carrier rate decreased from 3.10 for students born before June 1981 to 1.56 for students born from July 1983 to June 1984 （95% CI 1.96-4.91, P〈0.001; 95% CI1.06-2.28, P=0.024; respectively）. Conclusions Public vaccination provides long-lasting protection again HBV infection among the university students in Taiwan, china older than 18 years of age. There is a geographic variation of HBV infection among young adults in Taiwan china.

Impacts of Land Cover Changes on Runoff and Sediment in the Cedar Creek Watershed,*** River,Indiana,United States

The relation between runoff and sediment and land cover is investigated in the Cedar Creek Watershed (CCW), located in Northeastern Indiana, United States. The major land cover types in this watershed are cultivated land, woodland and pasture /Conservation Reserve Program (CRP), which account for approximate 90 % of the total area in the region. Moreover, land use was changed tremendously from 2000 to 2004, even without regarding the effect of the crop rotation system (corn & soybean). At least 49 % of land cover types were changed into other types in this period. The land cover types, ranking by changing area from high to low series, are rye, soybean, corn, woodland and pasture/CRP. The CCW is divided into 21 sub- watersheds, and soil and water loss in each sub-watershed is computed by using Soil and Water Assessment Tool (SWAT). The results indicate that the variations in runoff and sediment have positive relation to the area of crops (especially corn and soybean); sediment is more sensitive to land cover changes than runoff; more heavy rainfall does not always mean more runoff because the combination of different land cover types always modify runoff coefficient; and rye, soybean and corn are the key land cover types, which affected the variation in runoff and sediment in the CCW.

针对车用质子交换膜燃料电池(PEMFC)系统最佳过氧比(OER)控制问题,基于60kW PEMFC系统,构建了面向控制的三阶非线性空气系统模型,分别设计了基于稳态工作点近似线性化模型的动态前馈+PI控制器和基于全局线性化模型的前馈/反馈线性化控制器。仿真结果表明:前馈/反馈线性化方法解决了基于近似线性化模型控制方法由于模型误差而使OER响应存在稳态误差的问题,并且通过引入非线性前馈环节消除了负载电流变化对OER响应的影响,能在不同工况负载下跟踪最佳OER,有效提高了PEMFC系统效率。

针对传统基于鲁棒主成分分析(Robust Principal Component Analysis,RPCA)的扩展模型在进行运动目标检测时速度较慢的问题,提出了一种基于RPCA和支持向量机(Support Vector Machine,SVM)的快速运动目标检测算法。算法先使用RPCA对图像序列进行分解得到稀疏前景,同时对图像序列进行超像素分割;然后分别提取前景所对应超像素区域的特征,并使用支持向量机进行训练得到模型。在进行运动目标检测时,只要使用已知模型对超像素的特征进行判断操作便可知此超像素是否属于前景。实验结果表明,所提方法在保证F-measure值较高的同时,检测的速度得到大幅提升,是已有RPCA扩展模型的50倍左右。
